The Clinical Report on 1 Case of Failed Back Surgery Syndrome Who were Diagnosed as the Cauda Equina Syndrome (마미 증후군 FBSS 증례보고)

  • Objectives : The purpose of this study was to evaluate the oriental conservative treatment for FBSS patient who were diagnosed as the Cauda equina syndrome. Methods : The patient were treated by the oriental medical conservative treatment including herbal medication, dry needle acupuncture, direct moxibustion and indirect moxibustion, cupping therapy and, physical therapy. We estimated by Visual Analog Scale and Oswestry disability index for evaluate the effect of Oriental conservative treatment. Results : After treatment, patient's visual analogue scale score and Oswestry disability index score were generally decreased. Conclusions : The oriental medical conservative treatment might be an effective method to treat the FBSS patient who were diagnosed as the Cauda equina syndrome.

Comparative Study of Acupuncture, Bee Venom Acupuncture, and Bee Venom Pharmacopuncture on the Treatment of Herniation of Nucleus Pulpous (요추 추간판 탈출증 환자의 침 치료와 봉독침, 봉약침 병행치료에 대한 비교연구)

  • Objectives : Herniation of Nucleus Pulpous (HNP) of Lumbar is one of the most common causes of low back pain. The aim of this study is to investigate the effectiveness of acupuncture, bee venom acupuncture (BVA), and bee venom Pharmacopuncture (BVP) therapy for HNP. Methods : We separated 35 patients with HNP of L-spine into three groups; Acupuncture treated group, BVA treated group, and BVP treated group, and monitored for 30 days. 4 grades of recovery degree, score of Visual Analog Scale (VAS), and Oswestry Disability Index (ODI) were examined for comparison. Results : 1. Visual Analogue Scale of low back pain and radiating pain showed significant decrease in BVA and BVP groups compared to the Acupuncture group at VAS0_2, VAS0_3 evaluations. 2. ODI of disability of daily activities showed significant decrease in BVA and BVP groups compared to the Acupuncture group at VAS0_1, VAS0_2, and VAS0_3 evaluations. 3. Compared to the Acupuncture group, BVA and BVP groups were improved in S.L.R.T and R.O.M of L-spine as confirmed by the physical examination. 4. Treatment efficacy was more apparent in acute cases as ODI was significantly lower than chronic cases. Conclusion : BVA and BVP therapies can be used in addition to the acupuncture therapy for highly effective treatment of HNP. Further clinical studies are required to verify these findings.

  • Objectives : The purpose of this study is to investigate the effect of chuna treatment on cervical sprain caused by traffic accident in early stage. Methods : This study carried out on 20 patients who have received hospital treatment in Daejeon Univ. cheonan Oriental Hospital. Non-chuna group got acupunture-therapy, herbal medication, physical therapy and Chuna group got chuna treatment besides. We measured Heart Rate Variability(HRV) and Visual Analog Scale(VAS) on 2nd, 4th day. Results : After being treated by our methods, Chuna Group showed the inclination to balance the sympathetic and parasympathetic nerve. In chuna group, an autonomic nerve activity showed the inclination to increase. But there were no significant difference between both groups. Chuna group's VAS were significantly decreased(p=0.043). Conclusions : The results suggest that Chuna treatment help traffic accident patients in early stage to reduce pain. Refer to autonomic nerve system, chuna treatment seem to do positive effect but Further long tenn study in a large scale is needed.

Clinical Evaluation of Herniation of nucleus purposus patients treated by Bee venom therapy (봉약침으로 치료한 요추간판탈출증 환자의 임상적 평가)

  • Objective : he purposes of this study are to evaluate the efficacy of Bee Venom therapy(BV) on HNP(Herniation of Nucleus Purposus) of Lumbar spine by use of Visual Analog Scale(VAS), Pain Rating Scale(PRS) and Digital Infrared Thermographic Imaging(DITI), and to investigate their correlation. Methods : We researched 20 patients who were diagnosed by MRI as having a HNP, and treated them by Oriental medical therapy(including BV) for 4 weeks. The evaluation was peformed twice(admission day and after treatment for 4 weeks), and we compared the results. Results : 1. VAS, PRS and ${\Delta}t$(by DITI) were decreased after BV for 4 weeks significantly(p<0.01). 2. There was significant correlation between VAS and PRS(p<0.05). 3. There was significant correlation between PRS and ${\Delta}t$(p<0.05). 4. There was no significant correlation between VAS and ${\Delta}t$. Conclusions : BV improved HNP subjectively and objectively, and correlation was found between VAS and PRS and between PRS and ${\Delta}t$. Further study is needed for investigating their correlation.

Factors Affecting the Upper Limb Function in Stroke Patients (뇌졸증 환자의 상지기능에 영향을 미치는 요인)

  • This study was carried out to identify the factors which correlated with upper limb function after stroke and to analyze the effect of related factors on upper limb function. The 100 stroke patients(MMSE-K>24) were participated. The upper limb function according to gender, hand dominance, stroke type, affected location, site of paralysis, speech disorder showed no significant difference, and show significant difference according to shoulder subluxation. The upper limb muscle strength(Manual Muscle Testing), proprioception, muscle tone(Modified Ashworth Scale), grip strength(Dynamometer), paint (Visual Analog Scale) showed significant correlations with upper limb function. These predictors explained 77.6% of the upper limb function and the most significant affecting factor of upper limb function was upper limb muscle strength. In conclusion, the upper limb muscle strengthening will be effective strategy to improving the upper limb function and considering the proprioception, muscle tone, grip strength, pain, subluxation will be helpful to develop the strategies.

Effect of Extracorporeal Shock Wave Therapy on Pain and Function in Patients with Rotator Cuff Tendinitis (체외충격파치료가 회전근개 건염 환자의 통증 및 기능에 미치는 영향)

  • The purpose of this study was to evaluate whether extracorporeal shock wave (ESWT) is an effective treatment for rotator cuff tendinitis. Study subjects included 32 patients (16 in the experimental group and 16 in the control group). The patients were evaluated by assessing pain and function using visual analog scale (VAS), pressure pain threshold (PPT), Constant and Murley Scale (CMS), and simple shoulder test (SST). The results of this study indicated that ESWT after the subjects in the experimental group experienced significant changes in pain, range of motion, muscle strength, and function when compared to the control group. These results indicate that ESWT could be considered as an effective and efficient treatment for rotator cuff tendinitis.

Comparing the Effects of Stability Exercise, ESWT, and Taping for Patients with Myofascial Pain Syndrome of Upper Trapezius (안정화 운동, 체외충격파, 테이핑이 상승모근 근막통증 증후군에 미치는 효과 비교)

  • Purpose: In this study, the effects of stability exercise, extracorporeal shock wave therapy, and taping on pain and function in patients with myofascial pain syndrome of upper trapezius were compared. Methods: The subjects were divided into the stability exercise, ESWT and the taping treatment group and the clinical outcomes were evaluated by visual analog scale (VAS), pressure pain threshold (PPT) and a constant-murley scale (CMS) at pre-treatment and post-treatment. Paired t-test and ANOVA was used for statistical analysis. Results: All groups were statistical significance in the change in visual analog scale (p<0.05). The difference between the ESWT group and taping group was statistical significance in the change in pressure pain threshold (p<0.05) except for the taping group. Using the constant-murley scale, the stability exercise group showed a significant decrease in pain, and a significant increase in ROM, ADL, strength, total score of shoulder (p<0.05); however, the ESWT group showed no difference on ADL. In addition, there was no difference in strength for the taping group. The comparison of the effect between the stability exercise group, ESWT group and taping group in CMS showed a statistical significant difference in pain, ADL and ROM (p<0.05). Conclusion: These results indicate that stability exercise, ESWT and taping could be considered an effective and efficient treatment modality for myofascial pain syndrome of upper trapezius.

Efficacy and Safety of Cheong-A-Won Gagambang (JCE003) on Knee Osteoarthritis: Randomized Controlled Pilot Trial

  • Background: The aim of this study was to evaluate the effectiveness and safety of Cheong-A-Won Gagambang (JCE003) treatment for degenerative knee osteoarthritis. Methods: This was a single-center, randomized, double-blind, placebo-controlled pilot clinical trial. There were 36 adults with degenerative knee osteoarthritis who were randomly allocated into JCE003 1,000 mg, JCE003 2,000 mg, or the placebo group (in a 1:1:1 ratio). The participants received 12 weeks of treatment and had scheduled tests every 6 weeks. The primary outcomes were measured using the Korean Western Ontario and McMaster Universities scale, and the secondary outcomes were measured using the visual analog scale, European quality of life-5-dimensions, patient global impression of change, C-reactive protein, and erythrocyte sedimentation rate. Changes between baseline scores and scores following study completion were analyzed. Results: There were 29 participants whose data were analyzed in this study. The change of Korean Western Ontario and McMaster Universities, visual analog scale, European quality of life-5-dimensions scores showed significant improvement in the JCE003 1,000 mg group. The change of patient global impression of change was significantly improved in the placebo group. There were 14 adverse events, but there was no clinically significant relationship with the intake of JCE003 compared with the placebo. Conclusion: Taking JCE003 may be effective at improving knee pain in patients with degenerative knee osteoarthritis and appears to be safe. Based on this study, the concentration and feasibility of the test group may be used when conducting a large-scale clinical trial of degenerative knee osteoarthritis in the future.

  • Pain is subjective, while statistics related to pain research are objective. This review was written to help researchers involved in pain research make statistical decisions. The main issues are related with the level of scales that are often used in pain research, the choice of statistical methods between parametric or nonparametric statistics, and problems which arise from repeated measurements. In the field of pain research, parametric statistics used to be applied in an erroneous way. This is closely related with the scales of data and repeated measurements. The level of scales includes nominal, ordinal, interval, and ratio scales. The level of scales affects the choice of statistics between parametric or non-parametric methods. In the field of pain research, the most frequently used pain assessment scale is the ordinal scale, which would include the visual analogue scale (VAS). There used to be another view, however, which considered the VAS to be an interval or ratio scale, so that the usage of parametric statistics would be accepted practically in some cases. Repeated measurements of the same subjects always complicates statistics. It means that measurements inevitably have correlations between each other, and would preclude the application of one-way ANOVA in which independence between the measurements is necessary. Repeated measures of ANOVA (RMANOVA), however, would permit the comparison between the correlated measurements as long as the condition of sphericity assumption is satisfied. Conclusively, parametric statistical methods should be used only when the assumptions of parametric statistics, such as normality and sphericity, are established.
