The Effect of Case Management for Clients with Hypertension in Home Visiting Health Services (일 보건소 방문건강관리사업 고혈압대상자의 사례관리 효과)

  • Purpose: This study was conducted in order to present the effects of case management of a home visiting health service for clients with hypertension. Method: One-group pretest-posttest design was used. The subjects were 280 clients who received case management among the first and second registered group. The data were eight-week-case management results from January to December, 2010. In order to evaluate the effect of the visiting nursing service, biological indexes(blood pressure, total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein, BMI) and self-care abilities(knowledge/attitude of hypertension, management of medication, nutritional care, confidence of self care) were measured. Result: Biological indexes and self-care abilities were improved, except total cholesterol, low density lipoprotein. Conclusion: The case management of home visiting health service by public health centers is considered to be an effective nursing service. Therefore, greater effort is needed for better maintenance of case management, and more research is needed in order to examine a variety of biological indexes.

End of life Nursing Care Through a Visiting Nurse in Long-Term Care Insurance: A Case Report using the OMAHA System (방문간호사의 생애말기 환자 간호사례: 오마하시스템을 활용하여)

  • Purpose: This case report was attempted to present the process of the end of life nursing care provided by the visiting nurse. Methods: The subject was a person who was decided the long-term care Grade 1 and received a visiting nursing service, and the service was terminated on the death, and then was selected as a case with the consent of his family. The data were collected through long-term care benefit provision records and interviews with the visiting nurse. The nursing process was presented by applying the Omaha System. Results: The subject had digestion-hydration problems and respiration problems in the physiological domain, and the problems of role change, caretaking/parenting, spirituality, and grief in the psychosocial domain were identified. Depending on the problem, the end of life nursing care was provided to the subject and family members through activities on physical symptoms/signs, dietary management, end-life care, and coping skills. Conclusion: We expect that if the visiting nurse provides anticipatory guidance on the death process, the subject will be able to prepare for death comfortably with the family at home instead of vague fear of death.

An Estimation on the Need and Supply for Visiting Nursing Services of Health Center in Seoul (서울시 보건소 방문간호 수요.공급 추계)

  • Purpose: The purpose of this study was to estimate the demand and supply of visiting nursing services provided by health centers in urban area, aiming at strengthening infrastructure, which may improved the quality of life and health status of vulnerable population in the community. Methods: This study was conducted through nominal group discussion, focus group study. The demand and supply of visiting nursing were estimated by health economists based on the secondary analysis data from 25 health centers in Seoul. Result: Primary targets for the visiting nursing must be people who are homebound in the community. They can be classified into: a group of Level I: chronic patients who need visiting nursing care at least once a week: and a group of Level II: vulnerable families that need management periodically e. g. twice a month. Based on the estimation of demand for visiting nursing services in the community, the estimated supply required was $651{\sim}770$ visiting nurses including home health nurses in visiting nursing programs based on health centers in Seoul. Conclusions: The estimated demand and supply of visiting nursing are expected to provide basic data for establishing alternative policies on visiting nursing infrastructure that might be accomplished through demand-based visiting nursing programs by districts.

A study on the Status and Policy Issues of Visiting Nursing Programs at Public Health Centers in Urban Areas (대도시 보건소 방문보건관리 현황과 정책 과제)

  • Purpose: The purpose of this study was to identify the current management status and the policy issues of visiting nursing programs at public health center in urban area. Method: A questionnaire surveys was conducted on all public health nurses in 25 health centers in Seoul. Result: The major results were as follows: The characteristics of the visiting nurse showed that the average age was 41.1 years, and total nursing experience was 122 months. Their visiting nursing experience was only 30 months. The recipients of visiting nursing services consisted of 60.1% females, and 32.5% of the recipients were over 65 years. The major health problems of the recipients were hypertension, diabetes mellitus, arthritis and psychiatric problems. The visiting frequency was an average of 5.4 times per day, and 357 families were managed per visiting nurse. The major problems of visiting nursing programs were the shortages of visiting nurse manpower. The most important related policy issue was being in charge of a workload, which was not related to the visiting nursing services. Conclusion: By analyzing work for visiting nurses and identifying weak points, this research can present recommendations to be practically used as baseline data for establishing policy in relation to vitalizing visiting nursing programs.

A Study on the Activity of Occupational Health Nurses in Occupational Group Health Service (보건관리대행기관 산업보건간호사의 업무에 관한 조사)

  • This study was aimed to investigate the actual condition of the performance of the occupational health nursing services, and to analyze a main cause which affects on the execution of its duties for occupational health nurses in occupational group health service. It had been surveyed by responsed data from the 105 respondents out of 180 persons. The results were as follows : 1. In performance of occupational health nursing services by the number of assigned plant, participation rate of health examination tended to significantly increase with decreasing the number of assigned and it was observed that the visiting rate of every month was also significantly higher on touring inspection of work plant, recommendation of facility improvement for work environment, understanding the modification of manufacturing process, and inspection of welfare and sanitation facilities. 2. While they executed their nursing services according to duration of service, the more, their duration of service was, the higher, participation rate of health examination, health check in returning to work for injured workers, talk with other health managers on promoting welfare, service evaluation for a year plan, record of health statistic, and experience of first aid service were. And it was found out that the visiting rate of every month was also significantly higher on touring inspection of work plant. 3. In regard to occupational health nursing services by age, the participation rate of health examination and the experience of first aid service were significantly higher as the age increased. And also the visiting rate of every month was significantly higher on touring inspection of work plant, compared with young ones. 4. In performance of occupational health nursing services by the time required visiting plant, The shorter, the time required visiting plant was, the higher, the visiting rate of every month was, on time of health consultation for diseased workers, and inspection of welfare and sanitation facilities. 5. The subjects of health education were mostly liver disease and occupational hearing loss and video tape was used mostly in health education, monthly health education was the highest in 1-3 times(74.5%) and its duration was the highest in about 60 minutes(33.3%). 6. Vaccination of hepatitis among the specific health services was the highest accounting for 64.7%.

Development of an Urban Community Based Comprehensive Service Model for Bed-Ridden Elderly (재가와상노인 서비스제공 모델 개발을 위한 연구)

  • Purpose: This study aims to identify the ADL and IADL of bed-ridden elderly. Also it is used to show fuctional status, and to investigate the content and the level of nursing services provided. Method: The subjects were 191 elderly who received visiting nurse service through public health centers in the Seoul Metopolitan and Chungnam Province. Data collection was conducted by public health center nurses during four months in 2000. Result: As for daily living activities, 100% of subjects had at least one difficulty in ADL and IADL. Among them, only 0.5% had moderate disabilities and 99.5% had severe disabilities by HFS, 27.9% were in a semi bed-ridden state and 72.1% were completely bed-ridden by JABC. The major service provided was a visiting nurse service which was preferable to the social welfare service. In the visiting nurse service, there was no significant difference according to the elderlys' functional status. In addition there was no standadization about the qualification of the visiting nurse, and single entry point for the nursing service. Conclusion: The researchers urgently suggest that a community based comprehensive service model has to be developed to respond to the needs of the elderly in Korea.

  • This study was done to identify a status of home visiting project as a community health nursing system, that was the organization. personal who have age, educational background, marital status, position, experience of the home visiting in the public sectors in part of Seoul. Kyonggi, Kang-won area, It was done to provide basis data for the development of effective visiting nurses project in the health sectors, where was Health Centers in urban and rural. Branch of Health Center in rural and Health posts. The question airs were distributed 352 public health workers who working place was 118 health workers in 12 health centers in Seoul. 56 public health workers among 39 health center and other public health sectors in Kyonggi and 178 public health workers among health center and health care sectors. Data collected from October to December. 2000. The analysis by SAS system with F test, percentage and frequency. The major result were as follows. The general characteristics of the respondent show that most of them were graduates from community college and RN-BS with broadcast that they had not completed CPHN course but only two health workers have trained for the visiting nurses project. As for their grade in the position, the most of health workers have seventh level and the other CHP were above sixth level in the health care post that in the government structure. This indicates that workers do not have great authority in decision making, the most period of works in the position was one and two years indicating that they change jobs frequently. On an average their clinical experience was 4.11 years which is ideal for the total service. As for preparation of staff for home visiting workers education on visiting nurses program have to receive short term or longer term training course for strong emphasis. The analysis showed that public health visiting workers responds about active job performance that based on an area, approach of acting by districts, education and position are shown statistically significant difference between acceptance of the visiting nursing job show the same as well as visiting nurses project. Special concerns for visiting Nursing care spread came to burden, many of activity carry out main solution is covered the health problem connective support system needs of quality and quantity which out health problem. As 71.1% of visiting health service held on the poor population was under the guardianship of the law, but people who health insurance wide application under law shown a tendency to increase gradually. The general characteristics of the patients showed 56.2% of female on average of age was 66.1 years old, they have health problem was the most of 47.6% of high blood pressure and stroke, the other and as a problem that economics, which is complex welfare with out health problem. Community health care service should be combined health and social work program. The form of delivery of visiting health care given the most guide and education with counselling and support. (33.6%) Among the six category of visiting care service shown statistically significant difference and next is fundamental care, remedy care with priority.

Evaluation of Visiting Nursing Services for the Vulnerable Family in Urban Community (도시지역 취약가구의 방문간호서비스 평가)

  • Objectives: The purpose of this study was to verify a change for family nursing phenomena and satisfaction of clients of vulnerable families in an urban community. Methods: The study subjects were 711 families, randomly selected, who had chronic diseases(arthritis diabetes, stroke, hypertension, mental disease, cancer, dementia) with basic social welfare services from the nation and had disabled persons in an urban community, South Korea. The instruments used were the family nursing phenomena in Korea by ICNP and client satisfaction. Client satisfaction consisted of client satisfaction on home visiting nurses(4 Likert scales) and home visiting services(3 Likert scales). Results: The average visiting number is 3.82. The service number of education and counseling is 3.16, patient and symptom management 3.08, assessment and diagnosis 3.08, test 2.02, medication service 1.71회, dressing 1.01, referral to social welfare institute 1.00회, referral to medical service institute 0.21. In both, pre home visiting and post home visiting, the highest rated phenomenon was the 'lack of social support system' and following that 'deficit of financial management skill and support'. 'lack of family interaction in community', and 'social isolation' 'unhealthy life style' and 'inadequate care management of sick member', in that order. The percentages of phenomena besides 'deficit of financial management skill and support' decreased. The satisfaction level of clients towards the nurses was 3.27 points on a scale of 4, and the nursing services was 2.70 points on a scale of 3. Conclusion: Home visiting nursing services should continue to provide comprehensive healthcare services and support for vulnerable families, in urban communities.

Current Status of Home Visit Programs: Activities and Barriers of Home Care Nursing Services (재가간호서비스 제공자의 업무 수행 현황과 장애요인)

  • Purpose: The purpose of this study was to examine the current status of home care nursing services provided by community health nurses and to identify barriers to the services. Methods: A cross-sectional survey was conducted with three types of community health care nurses. Participants were 257 nurses, 46 of whom were hospital based home care nurses, 176 were community based visiting nurses, and 35 were long term care insurance based visiting nurses. A structured questionnaire on 7 domains of home care nursing services with a 4-point Likert scale was used to measure activities and barriers to care. Data were analyzed using SPSS WIN 21.0 program. Results: Hospital based home care nurses showed a high level of service performance activity in the domain of clinical laboratory tests, medications and injections, therapeutic nursing, and education. Community based visiting nurses had a high level of service performance in the reference domain. Long term care insurance based visiting nurses showed a high level of performance in the service domains of fundamental nursing and counseling. Conclusion: The results show that although health care service provided by the three types of community health nurse overlapped, the focus of the service is differentiated. Therefore, these results suggest that existing home care services will need to be utilized efficiently in the development of a new nursing care service for patients living in the community after hospital discharge.

The Development and Effect of Doctor-Visiting Nurses ICT Communication System for the Elderly: Focusing on Visiting Nursing Services for Long-term Care Patients (고령자 대상 의사-의료인간 ICT활용 협진 모델개발 및 실증에 관한 연구: 장기요양서비스 중 재가노인대상 방문간호서비스를 중심으로)

  • Purpose: The purpose of this study was to examine effectiveness and usefulness of Information Communication Technology (ICT) in communication between physician and visiting nurses who provide visiting nursing services under long-term care insurance. Methods: Structured questionnaires were used to measure usefulness and satisfaction of the system, both accessibility and convenience to visiting nurses (31 people) and users (182 people). Results: From the user perspective, accessibility and convenience in terms of service users were both satisfactory as shown by users' percentage. No statistically significant difference was found for satisfaction between before and after using the system. The usefulness of the system for visiting nurses was satisfactory for most the nurses. Also most nurses answered that the system is needed and is very useful. Most of the participants (both visiting nurse and service users) were satisfied with use of the ICT system. However, there was no statistically significant difference in satisfaction between the pre and post service because the service provision period was too short (three months). Conclusion: The consensus from both users and service providers is that an ICT based visiting nursing system needs to be introduced but a more user - friendly environment for system development will be needed.
