일차보건의료조직에서의 방문간호사업 실태조사연구 (A Study on Visiting Nursing Service in Primary Health Care Units)

  • The visiting nursing service is an essential part of public health. The purpose of this study was to analyze the visiting nursing service in primary health service centers. The data were collected from visiting nurse records in Wonju City Health Center, Myun Health Center and Community Health Subcenters. The period of data collection was from April 6 to July 15, 1998. The major findings were as follows: 1. Characteristics of 36 service providers. 1) Age : Over 40 years old - 66.7% 2) Educational level: The proportion of registered nurses was 47.2% and nurse aids 52.8% 3) Career: The proportion of providers who worked over 11 years in Public Health Service was 7.8% 2. Characteristics of Subjects 1) The major health problems were cerebro vascular attack, hypertension, D. M., arthritis, gastrointestinal problems and psychiatric problems. The prevalence of chronic health problems increased with age, except for mental illness. 2) The prevalence of cancer was 4.3/1000. 3. Contents of Home Visiting Nursing Services. 1) The major service was education and counseling. 2) The other services were Direct Care(ROM exercise, wound care, physical therapy, basic nursing care etc,) as 56.5%, of the work involved Indirect Care(teaching, counseling, emotional support, etc,) 30.3%, medication - 11.7%, and referral to hospitals - 1.5%.

응급실 방문 환아 간호에 대한 만족도 (The degree of Satisfaction with Nursing Care of Pediatric patients visiting Emergency Center)

  • The purpose of this study was to provide qualified nursing care to examine the degree of satisfaction with nursing care of pediatric patients visiting emergency center. Data were collected from March, 8 to June, 5, 1999 from 304 subjects including 2 University hospitals located in Seoul. Using the 23 item questionnaire, which made out by Davis CHECSS tool through modification and supplement by researchers. The collected data were scored by use of means and standard deviations according to the degree of satisfaction with nursing care of pediatric patients visiting emergency center and each item as an independent variable was analysed by t-test and ANOVA test. The results are as follows. 1. The degree of satisfaction with the nursing care of pediatric patients visiting emergency center showed as an average of 3.69. 2. With the resept of general characteristics related to pediatric patients visiting emergency center, there were statiscally significant difference in duration of stay at emergency center(F=2.908, p=.014), type of disease(F=1.777, p=.046). The degree of satisfaction with the nursing care of pediatric patients of examiners showed relatively high, but emotional care and information supply are required.

의료기관 홍보에 대한 의료기관 종사자의 태도 및 인식 (A Study on the Attitudes and Awareness on Medical Public Relations by Employee of Hospitals)

  • The aim of this study is to provide basic data in establishing marketing strategies for hospital by analyzing effective public relations of hospital through attitude and awareness on medical public relations by employee of hospital. The awareness on medical public relations by employee of hospitals were highest among aged 50s or above and above university graduates from the level of education, and from hospital management from types of duties and from those who have worked for more than 10 years. The methods medical public relations positively regarded by employee of hospitals were 'Appearing at health programs' followed by 'Periodicals like magazines and books', 'Issuing of hospital bulletins to homes of patients visiting hospitals'.

의약분업이후 병원문전약국과 동네약국 이용자의 만족도 비교 (Comparison of patient's satisfaction on the pharmacy services between those using the pharmacy nearby hospital and those using the pharmacy in resident areas)

  • The purpose of the study was to compare patient's satisfaction on the pharmacy services after introducing the new system of separation of dispensing from prescribing medicines, between those filling their prescriptions from the pharmacy nearby hospitals and those from the pharmacy in their resident areas. To measure patient's satisfaction, a questionnaire survey was conducted with 354 outpatients who received prescriptions from any of the three university hospitals located in In-Cheon city. Study results showed that geographic accessibility to pharmacy was a main attribute to select pharmacy. Size of the pharmacy and availability of prescription drugs are the second major reasons for pharmacy selection for the patients from the pharmacy nearby hospital, whereas patronage is the second major reason for those from the pharmacy in resident areas. Overall satisfaction was higher among the patients from the pharmacy in resident areas than those from the pharmacy nearby hospitals, mainly due to better facilities(waiting area, public telephone, etc), kindness, cleanliness, shorter waiting time, and pharmacist's concern about patient's health. On the other hand, the patients of the pharmacy nearby hospitals showed low satisfaction because of long waiting time and the lack of pharmacist's knowledge and information about patients' health status and medication history. Patients visiting the hospital that has pharmacy-hospital cooperation system showed higher satisfaction as compared to those visiting the hospital without such system. This study provided an empirical evidence that it would be more advantageous for patients to receive pharmacy services from pharmacies located in their living areas than from pharmacies nearby hospitals. This implies that there is a strong need for adequate strategies to enhance the role of pharmacies in resident areas under the new system of separation of dispensing from prescribing medications.

강원지역 소재 한방병원 피부과 외래환자의 임상적 분석 (The Statistical Study of Dermatoses Outpatients in Oriental Hospitals Located in Gwangwon Province)

  • Objective : The purpose of this study is to expand our treatment field in oriental dermatology by understanding the patterns of outpatients who had visited the oriental hospitals located in Gangwon province. Methods : We studied 510 outpatients out of 2240 people from Sangji University Oriental Hospital(SUOH) and 333 outpatients out of 2030 people from Chuncheon Oriental Hospital(COH) from January 1, 2007 to December 31, 2009. We categorized the results by gender, age, disease types, residences, months and number of visiting times. Results : The results were as follows 1. Comparing the number of outpatients that had visited the hospital by gender in SUOH shows that female's visiting rate is 57% and male is 43%. In COH shows that female's visiting rate is 54% and male is 46%. 2. Most frequent visiting age group were 21-30 year-old group(30%) in SUOH and 11-20 year-old group(23%) in COH. 3. In regard to the monthly visiting period, January(13%) is the highest in SUOH and July(10%) is the highest in COH. 4. Regional distributions of the outpatients that had visited SUOH and COH show Wonju(73%) and Chuncheon(74%). 5. In terms of treatment methods, SUOH showed the external application(65%), herb medication(54%), and acupuncture(52%) while COH showed acupuncture(74%), the external application(69%), and herb medication(66%). 6. Visiting frequency of outpatients in SUOH showed that 1 time visiting cases were 51%. In COH case, 1 time visiting cases were 19%. 7. The most prevalent diseases were urticaria(15%), eczema(14%) acne(12%), and seborrheic dermatitis(11%) in SUOH and atopic dermatitis(27%), urticaria(20%), acne(16%), and psoriasis(10%) in COH. 8. Most outpatients had visited SUOH for 1 time(51%) and COH for over 11 times(32%) with the major 16 diseases. Conclusions : This study suggests that outpatients who had visited the department of oriental dermatology preferred to get a treatment for urticaria, eczema, atopic dermatitis and acne. Considering this aspect, it needs to improve current research to expand the oriental medical treatments further in dermatological field.

일반인 및 내원환자의 한방병원 진료에 대한 인식도 연구 (Study on Perception About Traditional Korean Medicine (TKM) Hospital Treatment among the General Population and Patients Visiting a TKM Hospital)

  • Objectives : This study was carried out to measure perceptions about Traditional Korean Medicine hospitals and provide data for improvement of TKM hospital management. Methods : We surveyed the general population from 1st to 17th December, 2008, and patients visiting a TKM hospital from 1st June to 9th December, 2008. Results : Result as follows. 1. The response ratio for the generally preferred medical institution was in the order of western medical clinic, pharmacy, general hospital, TKM clinic, TKM hospital. 2. The response ratio for the preferred medical institution for herbal medicine treatment was in the order of TKM clinic, herbal medicine store, TKM hospital, pharmacy. The preferred medical institution for acupuncture treatment was in the order of TKM clinic, TKM hospital, acupuncture clinic, public health center. 3. The most common response to reason for receiving traditional Korean medical care was "traditional Korean medical care is more effective than western medical care." 4. The proportion of first treatment and second treatment of patients visiting at TKM hospital was about 1:1. 5. 72.3% of patients had been to another medical institution. 6. The response ratio for the first visited medical institution for the same disease was in the order of general hospital, western medical hospital, western medical clinic, TKM clinic. 7. The response ratio for the concern about TKM treatment was in the order of expensive charge for TKM treatment, uncertainty of TKM treatment effect. Conclusions : Through this research, we can understand the perception about TKM hospital treatment of the general population and patients visiting TKM hospitals.

The Institutionalization Process of the Visiting Rehabilitation System in Japan and the Challenges Faced in the Process

  • Purpose: In Japan, the long-term care insurance and health insurance acts have stipulated the visiting rehabilitation system to provide support at the national level. The prior case of Japan would provide guidelines for seeking a suitable policy direction in South Korea. This study aims to examine the historical transition process of the visiting rehabilitation system in Japan, and the issues that emerged in the process of the institutionalization of this system. Methods: To examine the historical transition process of the system, the regulations announced by the government and their reports were reviewed. The relevant issues were qualitatively analyzed based on the opinions of scholars, therapists, and interested organizations that were reported in published papers on the topic. Result: The visiting rehabilitation system has been implemented in the following chronological order: The Health and Medical Service Act for the aged (1982-2006), the Health Insurance Act (1988-), and the Long-term Care Insurance Act (2000-). Currently, visiting rehabilitation is provided through hospitals, clinics, visiting nursing stations, etc. The following issues came to the fore in the process of the institutionalization of the system: (1) the complexity of the system, (2) the necessity of changing perceptions into a life model approach, (3) insufficient service provision by therapists, (4) the lack of human resources and an education system, (5) the lack of awareness of care managers and doctors about visiting rehabilitation, and (6) the necessity of quality enhancement through a team approach. Conclusion: It is deemed worthwhile to refer to the visiting rehabilitation system in Japan and the issues that emerged in the process of institutionalizing the system while seeking a policy direction for a similar system in South Korea.

Recognizable Investigation on Home Visiting Physical Therapy of Physical Therapist in Gwangju and Jeonnam

  • PURPOSE: This study seeks to examine the awareness and need of home visiting physical therapy among physical therapist in Gwangju and Jeonnam province. METHODS: Survey was conducted during the continuing education on physical therapist held in Gwangju in 2015 with 297 participants. The survey questionnaire consisted of 12 questions on the characteristics of physical therapists, 8 questions regarding the awareness of home visiting physical therapy, and 7 questions on need. RESULTS: Significant differences were found regarding service period, according to categories of the inconvenience associated with commuting to/from hospitals, long-term treatment for patients, high quality treatment with intensive care, treatment at a time of patients' choosing. The category of hospital fees reduced due to early discharge from the hospital also saw significant differences, depending on respondents' marital status, dependent family, and service period. Providing treatment without causing discomfort to patients in front of others also saw significant differences according to physical therapist career period. Home visiting physical therapy is easier for family members to take care of patients also saw significant differences according to education and physical therapist career period. CONCLUSION: Survey indicates that physical therapist province feel the need for home visiting physical therapy in many respects. However, since as many as half of the surveyed physical therapists have only heard of home visiting physical therapy or are have no awareness of the this form of treatment, it appears that there is a need to raise awareness of home visiting physical therapy among physical therapist and introduce appropriate policies.

소비자가 지각하는 간호 및 의료서비스에 대한 만족과 병원 재이용 의도 분석 (Perceived consumers satisfaction with nursing and medical service and hospital revisiting intent)

  • Purpose: This study is performed to measure the level of nursing and medical service satisfaction according to consumer's demographic and hospital using behavior characteristics, and investigate the relationship among the nursing service satisfaction and the medical service satisfaction and the hospital revisiting intent. Method: The questionnaire is distributed to 300 patients at the six general hospitals in provincial city, Korea. For data analysis, SPSS/PC program is used. Result: The characteristics of consumer's demographics and hospitals using behavior that make difference in nursing and medical service satisfaction are education, the average number of the visiting(by month) and hospitals. Nursing service satisfaction correlated with medical service satisfaction(r=.677) and hospital revisiting intent(r=.570), and medical service satisfaction correlated with hospital revisiting intent(r=.680). Nursing service satisfaction explained medical service satisfaction(45.9%) and hospital revisiting intent(32.5%), and medical service satisfaction explained hospital revisiting intent(54%). Conclusion: It is concluded that education, the average number of the visiting(by month) and hospitals make difference in nursing and medical service satisfaction, and there are very close relationship among nursing service satisfaction and medical service satisfaction and hospital revisiting intent. Nursing service is a very important factor to improve medical service satisfaction and hospital revisiting intent.

전국민을 대상으로 한 한의원과 한방병원 외래이용환자의 이용실태 및 특성비교연구 - 2011년 한방의료이용 및 한약소비실태조사 보고서(보건복지부)중 이용환자의 질병치료방법 및 치료효과를 중심으로 - (Nationwide Study on the Usage and Characteristics of Patients Visiting Korean Medical Facilities - Based on the Treatment of Major Disorders, Effectiveness, Satisfaction and Occurrence Rate of Side Effects From the Ministry of Welfares Report on Usage and Consumption of Korean Medicine in 2011 -)

  • A survey conducted on 1,103 patients or caretakers visited Korean medical clinics and hospitals between August 25, 2011 to September 30, 2011 by the Ministry of Health and Welfares and Korea Institute for Health and Social Affairs was analyzed and obtained following results: 1. For sociodemographic distribution, female(813) outnumbered male(290) patients with majority of patients ranging from 30s to 60s. Married patients(793) outnumbered unwed(150) patients and 65.0% with higher than high school education. Statistical significance was seen in gender, age, marital status, and education level but no significant difference for status of employment, income level, and types of insurance between the clinics and hospitals. (P<0.05). 45.1% had less than \2,000,000 in monthly salary and most of coverage was provided by either regional or work insurance. 2. 67.9% of the patients rated health conditions to be better than average and 32.1% listed as poor. People in good health showed tendency to visit Korean medical facilities. Musculo-skeletal conditions such as arthritis, ankle sprain, lumbago, muscular injury, and frozen shoulder were common conditions, followed by gastric disorders, common cold and herbal tonics. No significant difference was observed between the clinics and hospitals for above conditions, but significance was seen in atopic dermatitis, stroke, and sequela from traffic accidents (P<0.05). 3. Ten most common conditions addressed at Korean medical facilities were lumbago, arthritis, muscular injury, back sprain, gastric disorders, ankle sprain, common cold, herbal tonics, frozen shoulder and stroke. Major treatment modalities rendered were herbal medicine, herbal supplements, acupuncture and moxibustion, cupping, tuina, and Korean midical physical therapy. No significant difference existed between the clinics and hospitals. 4. All modalities showed at least 85% effectiveness. No statistical significant difference between the clinics and hospitals except for herbal decoction. (P=0.0452) 5. 88.3% of responses showed treatment satisfaction with significant difference between the clinics and hospitals (P=0.002). The occurrence of side effects was at 2%, mostly corning from treating digestive, skin, kidney disorders and neurological issues. No significant difference was observed between the clinics and hospitals. From the above results, the typical population visiting Korean medical facilities can be summarized as being middle aged female with relatively higher education and moderately low income. The health condition is generally good and the purpose of visit is to receive traditional treatments of acupuncture, herbal medicine, and physical therapy. Treatment efficacy and satisfaction were high with no significant differences between the clinics and hospitals.
