Correlation of the Berg Balance Scale and Smart Balance Master System for Chronic Hemiparetic Stroke (만성 편마비 뇌졸중 환자의 버그균형척도(Berg Balance Scale)와 Smart Balance Master System의 상관성)

  • The purpose of this study was to find correlation between the Berg Balance Scale which is most commonly used and the equilibrium scores from Smart Balance Master System which is used for the test of the balance ability for the patients with chronic hemiparetic stroke to predict their falling risk though examining the postural control. Twenty-two subjects who had first stroke participated in this study. Participants were measured the Berg Balance Scale and the Smart Balance Master System. The study found that there was a significant correlation between the Berg Balance Scale and the equilibrium scores from Smart Balance Master System. And there was no significant correlation at all "eye-closed and sway of surfaces" in the 6 conditions of Smart Balance Master System and there was a significant correlation with over median in the other 5 conditions. The study was uncovered that there was no meaningful correlation between the Berg Balance Scale and "eye-closed and sway of surfaces" because chronic hemiparetic stroke patients heavily relied on their vision to maintain their balance ability and the Berg Balance Scale did not properly exam it. Thus to predict their falling risk in clinical practice for the patients with chronic hemiparetic stroke, it would be more efficient ways for us to consider all the effects of sensory organs with more than one clinical evaluation method.

Transareolar-Perinipple Dual Pockets Breast Augmentation (횡유륜 유두주위절개를 통한 이중포켓 유방확대술)

  • Purpose: Many options are available for the incision and pocket selection in breast augmentation. Each method has its advantages and disadvantages. To leave an invisible operation scar and to achieve easier pocket dissection by the central location of the incision on the breast, we made a transareolar-perinipple incision. To overcome the disadvantages of the transareolar incision, originally advocated by Pitanguy in 1973, we modified the direction of incision line and dissection plane. Methods: To avoid the injury of 4th intercostal nerve responsible for nipple sensation, we made perinipple incision on the medial side of the nipple instead of trans-nipple incision and made the transareolar incision as 11-5 o'clock on the left side and 1-7 o'clock on the right side instead of 3-9 o'clock on both sides. To avoid the possible infection and breast feeding problem caused by the injury to the lactiferous duct, and the possible implant hernia caused by the incisions lying on a same plane of pocket dissection, we made a subcutaneous dissection just above the breast tissue medially down to the bottom of breast tissue and made a subglandular or subfascial pocket, which may avoid the injury of lactiferous duct and create different planes for skin incision and pocket dissection. Other advantages of the transareolar-perinipple incision include easier pocket dissection, less chance of hematoma, and as a result less postoperative pain because of the central location of the approach which allow finger dissection and meticulous bleeding control with direct vision, without any specialized instrument such as an endoscope or long mammary dissectors. As for pocket selection, we made dual pockets. We prefer subglandular or subfascial pocket. Also, we made a subpectoral pocket in the upper 1/4 of the pocket to add more volume on the upper part of the augmented breast, which can make aesthetically more desirable breasts in thin Asian women with small breasts. Possible disadvantages of our method are subclinical infection and scar widening, which could be overcome by meticulous operation techniques, antibiotic therapy, and intradermal tattooing. Results: From September, 2003 to August, 2005, 12 patients underwent breast augmentation using round smooth surface saline implants by our method. During the mean follow-up period of 13 months, there were no complications such as infection, hematoma, capsular contracture, and sensory change of nipple, and results were satisfactory. Conclusion: We suggest breast augmentation via transareolar-perinipple incision and dual pockets(subpectoral-subglandular or subfascial) as a valuable method in thin oriental women with small breasts.

A Case Report of Non-Motor Symptoms Evaluated Using the Non-Motor Symptom Scale in a Patient with Secondary Parkinsonism Presumed to be Probable Lewy Body Dementia and Improved with Combined Treatment with Herbal Medicine and Acupuncture (루이소체 치매로 추정되는 이차성 파킨슨증 환자의 Non-Motor Symptom Scale(NMSS)로 평가한 비운동성 증상을 한약과 침의 복합치료로 호전시킨 증례보고 1례)

  • Parkinson's syndrome is a degenerative brain disease that presents characteristic motor symptoms of tremor, rigidity, and gait disturbance. In addition to these motor symptoms, Parkinson's syndrome also presents non-motor symptoms (NMSs) such as sleep disturbance and cognitive decline. NMSs reduce patient's quality of life and psychosocial functioning and cause economic burden on the patient, so appropriate evaluation and treatment are required. Lewy body dementia is one of the several diseases belonging to Parkinson's syndrome. Its symptoms such as cognitive function, memory impairment, and hallucinations occur with Parkinsonism. Although drug therapy is being used with drug treatment to treat non-motor symptoms, it has limitations such as side effects, which stimulated interest in other complementary treatment methods such as oriental medicine treatment, dance, and yoga. The patient in this case complained of tremor in the right upper extremity, muscle hypertension and pain, and persistent vision, memory, and cognitive decline. The patient was diagnosed with probable Lewy body dementia. The patient was hospitalized for 4 months and received acupuncture and herbal medicines. After treatment, the patient's NMS scale scores decreased from 90 to 63, and the Unified Parkinson's Disease Rating Scale scores (summed I, II, and III) decreased from 17 points to 8 points. The Beck Depression Inventory score decreased from 22 points to 13 points. In addition, the patient's subjective evaluation revealed improvement. In this case, a patient diagnosed with probable Lewy body dementia who did not respond to the standard treatment and did not want to take medications showed improvement in not only motor symptoms but also NMSs after integrative Korean medicine treatment.

Why did she lose her sight? A case of visual damage due to methanol inhalation (메탄올 흡입 후 발생한 시각장애 : 증례보고)

  • Methanol is a clear, colorless, volatile, and poisonous liquid that is commonly used as an industrial solvent. Visual impairment is a common symptom of methanol poisoning; however, visual impairment rarely occurs after exposure through inhalation. Therefore, visual loss after methanol intoxication via respiration has rarely been reported. We report a case of visual damage associated with methanol poisoning via respiratory exposure in an industrial setting. In this case in South Korea, a 28-year-old woman who worked at a cell phone factory was admitted to the emergency department with mental changes. She had blurred vision that began two days prior, but she did not come to the hospital until she experienced mental changes. She ranked 9 on the Glasgow Coma Scale and presented with severe metabolic acidosis. So, she was admitted to intensive care, and continuous renal replacement therapy was performed. Finally, she was discharged after recovery of her mental state, but had to undergo rehabilitation for six months. Also, her visual impairment was permanent. Methanol intoxication can occur through inhalation, which is difficult to detect initially. However, treatment of methanol poisoning is time-critical. Therefore, doctors should always keep in mind that methanol intoxication may occur via respiration. If in doubt, treatment should be given as soon as possible.

Effect of hip positions in frontal plane on abdominal muscle activities during bridging exercise (교각운동시 이마면에서 엉덩관절 위치가 배근육 근활성도에 미치는 영향)

  • The purpose of this study is to investigate the effect of the hip positions in frontal plane on abdominal muscle activities during bridging exercise. Twenty six subjects who have healthy conditions were asked to perform bridging exercise in three starting positions which are hip abduction, neutral and hip adduction. We used surface electromyography to compare the activities of both external oblique, internal oblique and rectus abdominis muscle. We analyzed the data by using repeated one way ANOVA, The alpha level was set at 0.05. The results showed that the muscle activities of both sides of external oblique, and internal oblique were significantly different among three starting positions. The activity of both sides of rectus abdominis were insignificantly different among three positions. The muscle activities of both sides of external oblique muscle and left internal oblique in bridging exercise with hip adduction position were significantly greater than hip neutral positions. The muscle activity of right internal oblique in bridging exercise with hip abduction positions. Our results suggest that bridging exercise with hip adduction position is recommended to lumbar stabilization exercise and the factor of hip position is important for controlling exercise intensity when designing bridging exercise program.

Effect of Visual Difference on Balance and Walking Capacity in Life Care of College Students (대학생의 라이프케어에서 시력 차이가 균형과 보행능력에 미치는 영향)

  • This study studied the effect of visual difference on balance and walking ability in college students' life care. The study was conducted on 45 students attending H University in G City, divided into control groups (n=22, not wearing glasses and contact lenses) and experimental groups (n=23, wearing glasses and contact lenses). In not wearing glasses and contact lenses, the subjects measured visual acuity with logMAR charts, evaluated their balance ability with BIOrescue, and evaluated walking ability with G-Walk. The results of this study showed that the experimental group had statistically significantly lower vision than the control group in the visual acuity measurement(p<.01). Static balance ability was statistically significant increase in center of mass movement of the right foot in the experimental group compared to the control group(p<.05). Dynamic balance ability was statistically significantly reduce in limit of stability for groups of experiments compared to control group(p<.05). The walking ability was statistically significantly shorter on step length and stride length, swing of the experimental group compared to the control group in the right foot(p<.05). The findings showed that the visual difference in university students reduces balance and walking ability. Therefore, university students with poor visual acuity are recommended to correct of visual acuity to prevent collision and falls in their daily lives.

A Suggestions of Future Direction of the Integrated Community Care Business for Improvement of the Elderly's Life Care (노인의 라이프케어 향상을 위한 지역사회 통합돌봄사업 미래 방향에 대한 제시)

  • In this study, we investigated and the following conclusions are presented by identifying the current status and problems in order to expand the future's value of the community care project introduced and implemented to improve the quality and care for the elderly's life. First, the needs analysis of the elderly receiving services is composed of patient-centered rather than investigator-centered, and in particular, medical management through medical staff visits should be strengthened, and specialized service items according to gender, age, disability type, and personal preference should be strengthened. This will have to be gradually strengthened. Second, by analyzing the satisfaction, redundancy, and effectiveness of service items, we save money, and consider the consumer-oriented service item composition and application of items necessary for new services. Third, through the introduction of an integrated schedule management system, it is necessary to specialize in pre-booking and visit schedule management between the elderly and the direct service organizations that provide services. Fourth, as an effort to solve the financial problem, it is necessary to prepare a rational resource sharing system with health and medical finance, long-term care insurance system, and social welfare financial project. and it may consider that putting the medical personnel who are from local public medical college input. Through these proposals, the community care business will be able to complete and have future value as a universal aged care system.

A Study on the Use of Balance Scale for Life Care of the Elderly People (노인의 라이프케어를 위한 균형 평가척도 활용에 관한 연구)

  • The purpose of this study is to identify the effect not of therapeutic behavior but of the balance evaluation process itself on the balance ability of the elderly. For this, 36 elderly people over 65 age belonging to two elderly welfare centers in Jeonju city were divided into 3 groups of 12 each. All participants were divided into three groups, one was the daily evaluation, another was weekly, and the other group was twice at the beginning and the end of the study. And this study conducted for 4 weeks totally. The balance scale that we used one was Berg Balance Scale and for comparison within groups and between groups, Paired t-test and one-way ANOVA methods were used. The post-hoc test was used 'Scheffe test', and the significance level was α = .05. The results showed that the balance ability was significantly improved after 4 weeks in all three groups. Among all groups, the highest improvement was observed in the daily evaluation group and the lowest improvement was observed in the general evaluation group. In the comparison between the groups, it was confirmed between the daily evaluation group and the weekly evaluation group, between the daily evaluation group and the general evaluation group. These results show that the evaluation process using the balance function scale, rather than the therapeutic behavior, can affect the balance ability of the elderly. I would like to be able to utilize this basic research data that just evaluation of balance function using balance scale in the social facilities and institutions for the elderly can positively influence the improvement of the balance function and strengthen the postural controling ability necessary for the independent daily life practice.

A Case Report of an Idiopathic Bilateral Strabismus with Internal and External Palsy (내외전마비를 동반하는 특발성 양안사시 한의 증례보고 1례)

  • Objectives : The purpose of this study is to report the effect of Korean medical treatments on idiopathic bilateral strabismus. Methods : We treated a 43-year-old male idiopathic bilateral strabismus patient who was told to wait without a prescription as there was no problem in intraocular pressure test, fundus test, brain computed tomography, brain magnetic resonance imaging and angiography test, and cerebrospinal fluid test. The patient was admitted to Cheonan Korean medical hospital for 3 months to get Korean medical treatments including acupuncture, electroacupuncture, vapor treatment, pharmacopuncture therapy. The degree of improvement in idiopathic bilateral strabismus was determined by the objective measures of internal and external rotation disorders, and the patient's subjective opinions on eye pain and double vision discomfort. Results : The results of this patient showed distinct improvements on objective measures of internal and external rotation disorders in both eyes. Distance from the outer corner of the right eye when abducted decreased from 7mm to 0mm, and left eye decreased from 7mm to 0mm. Distance from the inner corner of the right eye when adducted decreased from 5mm to 0mm, and left eye decreased from 4mm to 0mm. In addition, eye pain decreased from 10 to 0 and diplopia decreased from 10 to 2 according to the patient's subjective opinions. Conclusions : This study shows the possibility that such medical treatment in Koreans can improve the rare idiopathic bilateral perspective among strabismus patients.
