A Study on the Effect of the Craniocervical Posture on Hyoid Bone Movement with Sitting Posture in Wheelchair (휠체어에 앉은 자세에서 두경부 자세가 설골 움직임에 미치는 효과에 관한 연구)

  • The purpose of this study was to identify the effect of the craniocervical posture on swallowing and hyoid bone movement with sitting posture in wheelchair, and to provide basic data for developing a treatment program for dysphagia. A total of sixty healthy adults without a history of dysphagia who were guardians or care assistants of inpatients at Hospital K underwent video fluoroscopy performed by physiatrists from April to July 2009. The results of the fluoroscopy were analyzed based on the measurement of hyoid bone movements. The hyoid bone movement(horizontal, vertical, and diagonal) was significantly high in the neutral craniocervical, chin-tuck, and sixty degrees extension postures, in descending order(p<.05). It was found that the neutral craniocervical posture is the most effective posture for hyoid bone movement with sitting posture in wheelchair.

CT-Guided Microcoil Localization of Small Peripheral Pulmonary Nodules to Direct Video-Assisted Thoracoscopic Resection without the Aid of Intraoperative Fluoroscopy

  • Objective: To evaluate the feasibility, safety, and effectiveness of CT-guided microcoil localization of solitary pulmonary nodules (SPNs) for guiding video-assisted thoracoscopic surgery (VATS). Materials and Methods: Between June 2016 and October 2019, 454 consecutive patients with 501 SPNs who received CT-guided microcoil localization before VATS in our institution were enrolled. The diameter of the nodules was 0.93 ± 0.49 cm, and the shortest distance from the nodules to the pleura was 1.41 ± 0.95 cm. The distal end of the microcoil was placed less than 1 cm away from the nodule, and the proximal end was placed outside the visceral pleura. VATS was performed under the guidance of implanted microcoils without the aid of intraoperative fluoroscopy. Results: All 501 nodules were marked with microcoils. The time required for microcoil localization was 12.8 ± 5.2 minutes. Microcoil localization-related complications occurred in 179 cases (39.4%). None of the complications required treatment. A total of 463 nodules were successfully resected under the guidance of implanted microcoils. VATS revealed 38 patients with dislocated microcoils, of which 28 underwent wedge resection (21 cases under the guidance of the bleeding points of pleural puncture, 7 cases through palpation), 5 underwent direct lobectomy, and the remaining 5 underwent a conversion to thoracotomy. In 4 cases, a portion of the microcoil remained in the lung parenchyma. Conclusion: CT-guided microcoil localization of SPNs is safe and reliable. Marking the nodule and pleura simultaneously with microcoils can effectively guide the resection of SPNs using VATS without the aid of intraoperative fluoroscopy.

  • A 3-month-old, 2.8 kg, female Cocker spaniel was presented with chronic history of dysphagia since weaning. Video fluoroscopic examination revealed swallowing problems in the upper esophageal sphincters. It was diagnosed as cricopharyngeal achalasia. Cricopharyngeal and thyropharyngeal myectomy was performed. One day after surgical management, normal swallowing movement was observed in the video fluoroscopic examination of the dog. There was no evidence of recurrence for 15 months.

  • With the recent advances in radiological science, there was radiographic techniques development and several researches to diagnosing dysphagia. We proposed the new Imaging technology based on Near Infrared radiation (NIR) for video fluoroscopic swallowing study (VFSS). To reduce the risk of the VFSS examination for swallowing rehabilitation, multi-NIR camera system comprised. Based on the multi-NIR camera imaging system, Computational simulation was conducted to identify the potential of the multi-NIR camera imaging system as a clinical tool (screening system). As a result of the simulation applied in this study, the proposed system has a potential to be a clinical solution although there is a few of limitations. we believe that it will be a good tool to support the VFSS as a screening technology in clinical fields.

  • Speech characteristics of patients with cleft palates are resonance disorders, articulatory disorders and voice disorders. The purpose of this study is to find the acoustic, physiological and articulatory characteristics of cleft palate speakers. Thirteen control groups and 3 cleft palate patients participated in this experiment. Test words were composed of simple vowels and consonants imbedded in low vowel /a/, /p 'ap'i/ and /sasi/ according to the evaluation experiments. CSL, Video fluoroscopy, Fiberscope and Nasometer were used to analyze VOT, vowel formants, profiles of articulator, VP port images and nasalance. The results are as follows : (1) The nasalance of cleft palate patients in the high vowel /i/, stop sounds and fricative sounds were 60%, 34.8% and 44.1%, respectively. These values were higher than those of the control group. (2) Posterior articulatory movements /k'a/ in patients with cleft palates showed backward movement in comparison with the control group on Video Fluoroscopic images and palatograms. These results suggested that patients with cleft palate have the compensatory oral sounds to close the VP port. (3) The VOT in patients with cleft palates was longer than that of the control group.

Comparison of Cervical Musculoskeletal Kinematics in Two Different Postures of Primate During Voluntary Head Tracking

  • We have examined the effect on neck-muscle activation of altering whole body posture. A Rhesus monkey (Macaca mulatta) was trained to produce sinusoidal (0.25 Hz) head tracking movements in the sagittal plane when seated with trunk and head vertical or while standing in the quadrupedal position. Video-fluoroscopic images of cervical vertebral motion, and electromyographic (EMG) responses were recorded simultaneously. Results demonstrated that vertebral motion varied with body posture, occurring synchronously between all joints in the upright position and primarily at skull-$C_1$ when in the quadrupedal position. Muscle EMG activation was significantly greater (P<0.001) in the quadrupedal position than when upright for all muscles except semispinalis cervicis. Peak activation of all the muscles occurred prior to peak head extension in the quadrupedal position, suggesting synchronous activity between muscles. Data suggest that, when upright, muscles were activated in functional groupings defined by their anatomical arrangement. In the quadrupedal position, gravity acting on the horizontally oriented head produced greater activation and a collective response of the muscles.

  • In this paper, we present the diagnosis system for swallowing disorder. There are some types of diagnosis device for swallowing disorder, for example, the video fluoroscopy, the nuclear medicine inspection, the endoscopy, EMG and motion analysis. But these systems need heavy devices or have dangerous nuclear exposure, so are uncomfortable for handicapped person. Our system has advantages of simplicity, accuracy and quantitative analysis. In addition to the diagnosis aspect, this system can be used to biofeedback treatment.

  • The use of cardiac angiography (CA) and the interventional procedures is rapidly increasing due to the increase in modern adult diseases. Cardiovascular intervention (CI) is an examination method where radiation is applied to the same area for a long period, and thus may cause skin injury. In this study, we investigate the diagnostic reference level (DRL) of the cardiovascular intervention (CI) carried out by medical institutions and use it as a tool to reduce patient exposure dose. In this study, the DRL was set by acquiring information about the cumulative fluoroscopy time, cumulative fluoroscopy dose-area product (DAP), radiography DAP, cumulative DAP, air kerma, number of video clips, and the total number of images from the cardiac angiography and interventional procedures performed on 147 patients. The DAPs corresponding to the DRL of cardiac angiography(CA) and that of the interventional procedures were shown to be $44.4Gy{\cdot}cm2$ and $298.6Gy{\cdot}cm2$, respectively; the corresponding DRLs of fluoroscopy time were shown to be 191.5s and 1935.3s, respectively. A DRL is not a strict upper bound for radiation exposure. However, the process of setting, enacting, and reviewing the DRLs for the dose by medical institutions will contribute to a reduction in the unnecessary exposure dose of patients.

  • Dysphagia is common and serious problems in post-stroke patients. The post-stroke dysphagia with aspiration is associated with dehydration, malnutrition, pneumonia, sepsis and death. Up to date, gag reflex and choking history used to decide the aspiration in clinical. The purpose of this study was to evaluate the aspiration and to choice the proper meal formation using by video fluoroscpic swallowing study(VFSS) with post-stroke dysphagia patients. 58 post-stroke patients and ten normal person participated to perform the VFSS with liquid type, puding, yoplait, rice mixed with barium(Ba). Two rehabilitation medicine doctors and a radiological technologist analysed the phase(oral, pharyngeal, esophageal phase)with video film, and checked the pharyngeal transition time(PTT). 38 patients showed abnormality in pharyngeal phase, 13 patients in oral and pharyngeal phase, 3 patients in oral phase, and 2 patients in pharyngeal and esophageal phase. 43 patients(65.2%) occured the aspiration, but 23 of 43 patients improved by the chin tuck position. Aspiration occured 34 patients in liquid type, 2patients in rice gruel and nothing in boiled rice. After VFSS, 13 of 23 patients change the intaking pathway from nasogastric tube(NGT) to oral, On the contrary with 3 of 42 patients from oral to NGT. Consequently VFSS is clearly effective to evaluate the aspiration with post-stroke dysphagia patients.

Analysis of Singer's Formant & Close Quotient During Change of the Larynx Position (후두위치의 변화에 따른 Singer's Formant와 성대접촉률의 변화 연구)

  • Background and Objectives : The purpose of this study is to analyze the difference of Fundamental Frequency(Hz), Closed Quotient(Qx ; %), Intensity(dB), Vocal tract length and width(cm), formant frequency(Hz), level of formant frequency(dB) depending on the larynx position. Materials and Methods : One professional male singer(career : 28 years) produced sustained vowel /a/,/e/,/i/,/o/,/u/ in two larynx position (higher, lower) with Dr. Speech and video fluoroscopy was used to quantify the vocal tract morphology. Results : In lower larynx position, CQ is increased 9.8% and Intensity is increased about 10% and level of Formant Frequency is increased. And also Vocal tract length is longer 2.4cm, Vocal tract width(Anterior width : 0.4cm, lateral width : 0.2cm) is wider than in higher larynx position. Conclusions : Singer's formant has a prominent spectrum envelope peak near 2400-2600Hz by clustering of F3, F4 and F5 near 3400Hz in lower larynx position.
