발목관절 가동술과 결합한 전신진동운동이 뇌졸중 환자의 보행 기능과 균형 능력에 미치는 영향: 무작위 대조 예비연구 (Effects of the Whole-body Vibration Exercise Combined with Ankle Joint Mobilization on the Gait Function and Balancing Ability in Stroke Patients: A Preliminary Randomized, Controlled Study)

  • PURPOSE: This study was performed to investigate the effects of the whole-body vibration exercise combined with ankle joint mobilization on the gait and balancing ability in patients with hemiplegic stroke. METHODS: A total of 19 patients at a rehabilitation hospital who had suffered a hemiplegic stroke were randomly assigned to the experimental group (whole-body vibration exercise combined with ankle joint mobilization, n=10) or control group (whole-body vibration exercise, n=9). All participants underwent 30 min of comprehensive rehabilitation therapy (5 × /week for 6 weeks). Additionally, the experimental group performed the whole body vibration exercise and ankle joint mobilization (15 minutes each, 30 minutes total, 3 × / week for 6 weeks). In the control group, only the whole- body vibration exercise was performed in the same manner and not the ankle joint mobilization. The gait and balancing abilities were measured before and after the 6-week training. RESULTS: Significant improvements were observed in the 10-m walk test, timed up-and-go (TUG) test, center of pressure (COP) path length, and COP path velocity in the experimental group (p < .05). The experimental group showed a larger decrease in the COP path length and velocity than the control group (COP path length, -10.27 mm vs. -3.67 mm, p < .05; COP path velocity, -.33 cm/sec vs. -.13 cm/sec, p < .05, respectively). CONCLUSION: The whole-body vibration exercise combined with ankle joint mobilization could be effective in improving the gait and balancing ability of stroke patients and could also be more effective for improving the static balance ability than the general whole-body vibration exercise alone.

진동이 무릎 푸쉬업플러스 운동 시 어깨뼈 익상이 있는 성인의 어깨안정근 근활성도와 익상 높이에 미치는 영향 (The Effects of Vibration on Shoulder Stabilizer Muscle Activation and Scapular Winging Length of Knee Push-up Plus Exercise in Adults with Scapular Winging)

  • Purpose : This study was investigated effects of vibration on scapular winging of knee push-up plus exercise. Methods : Twenty-eight female subjects with scapular winging were evaluated after performing knee push-up plus exercise, with or without vibration, three times a week for four weeks. Muscle activation of the serratus anterior and upper trapezius, and the ratio of activation of the upper trapezius to serratus anterior were measured using surface EMG; and scapular winging length was measured by using the caliper. Results : The findings showed that knee push-up plus exercise -with and without- vibration induced a significant increase in muscle activation of serratus anterior (p<.05) and a significant decrease in muscle activation of the upper trapezius (p<.05), the ratio of muscle activation of the upper trapezius to the serratus anterior (p<.05) and scapular winging length (p<.05). There was also a significant difference in muscle activation of the serratus anterior (p<.05) and upper trapezius (p<.05) between control and experimental group at post-test. Conclusion : This study suggests positive clinical effects of knee push-up plus exercise with vibration which enhanced muscle activation of serratus anterior and reduced muscle activation of upper trapezius in person with scapular winging. However, vibration had no effectiveness to improve muscle imbalance between serratus anterior and upper trapezius, and reduce the scapular winging length.

Effects of Active Vibration Exercise on Neck Pain, Disability Index, and Muscle Activity of Patients with Forward Head Posture

  • Purpose: The purpose of this study was to evaluate the impact of active vibration exercise on the neck pain, disability index, and muscle activity of patients with forward head posture. Methods: A total of 24 patients were randomly assigned to an experimental group or a control group (n=12 each). The experimental group performed active vibration exercise using a flexi-bar for 20 minutes a day, five times a week for four weeks. The study measured patient neck pain using a visual analog scale, neck pain related disability using the neck disability index, and muscle activity using electromyography. Results: The intragroup comparison showed significant differences in the visual analog scale score, neck disability index score and upper trapezius, lower trapezius and serratus anterior muscle activity values among patients in the experimental group. The intergroup comparison showed that differences in the visual analog scale score, neck disability index score and upper trapezius, lower trapezius and serratus anterior muscle activity values in the control group. Conclusion: This study showed that active vibration exercise was effective in improving the neck pain, disability index, and muscle activity of patients with forward head posture.

표면근전도를 이용한 진동운동기의 근수축 효과에 관한 연구 (The Study of Muscle Contraction Effect of Vibration Exercise Device Using Surface Electromyography)

  • The purpose of this study was to investigate the effects of vibration exercise using surface electromyography. Seven male collegiate wrestlers were participated in this study. Each subject stood on the platform and the vibration was induced for 1min. WEMG8 EMG system was used to record muscle activity from Vastus lateralis, Biceps Femoris, Tibialis Anterior, and Gastrocnemius. The EMG data were sampled for 30 sec. during non-vibration and vibration half squat position, respectively. The raw data were band pass filtered to remove noise and full wave rectified Paired sample t-test were performed to see the differences of maximum and average EMG between non-vibration and vibration trials. The results indicated that vibration produced much more muscle contraction than that of non-vibration trial for all selected muscles even though the significant difference was found only from Biceps Femoris. This phenomenon was due to the individual differences so care must be taken to evaluate vibration intensity and position before personal training.

Effect of Whole Body Horizontal Vibration Exercise in Chronic Low Back Pain Patients: Vertical Versus Horizontal Vibration Exercise

  • Objective To elucidate the effect of a 12-week horizontal vibration exercise (HVE) in chronic low back pain (CLBP) patients as compared to vertical vibration exercise (VVE). Methods Twenty-eight CLBP patients were randomly assigned to either the HVE or VVE group. All participants performed the exercise for 30 minutes each day, three times a week, for a total of 12 weeks. Altered pain and functional ability were evaluated using the visual analog scale (VAS) and Oswestry Disability Index (ODI), respectively. Changes in lumbar muscle strength, transverse abdominis (TrA) and multifidus muscle thicknesses, and standing balance were measured using an isokinetic dynamometer, ultrasonography, and balance parameters, respectively. These assessments were evaluated prior to treatment, 6 weeks and 12 weeks after the first treatment, and 4 weeks after the end of treatment (that is, 16 weeks after the first treatment). Results According to the repeated-measures analysis of variance, there were significant improvements with time on VAS, ODI, standing balance score, lumbar flexor, and extensor muscle strength (all p<0.001 in both groups) without any significant changes in TrA (p=0.153 in HVE, p=0.561 in VVE group) or multifidus (p=0.737 in HVE, p=0.380 in VVE group) muscle thickness. Further, there were no significant differences between groups according to time in any of the assessments. No adverse events were noticed during treatment in either group. Conclusion HVE is as effective as VVE in reducing pain, strengthening the lumbar muscle, and improving the balance and functional abilities of CLBP patients. Vibrational exercise increases muscle strength without inducing muscle hypertrophy.

진동자극을 이용한 저항운동이 앞십자인대 재건술 후 무릎관절 근력과 균형에 미치는 영향 (Effects of Resistance Exercise Using Vibration Stimulation on Knee Muscle Strength and Balance after Anterior Cruciate Ligament Reconstruction)

  • Background: The purpose of this study was to investigate how the resistive exercises with vibration stimuli could affect the strength of knee muscles and balance in patients with a history of reconstructive surgery. Methods: Thirty four subjects with anterior cruciate ligament reconstruction were randomly divided into 3 groups; Resistive exercises with vibration stimuli group (n=11), Resistive exercises (n=11) and control group (n=12). The paired t-test was used to show the variation before and after exercise in all three groups. One way ANOVA was used to separate the total variation between groups. Results: The results showed that there was significant improve in the strength of knee muscles in all three groups and resistive exercise with vibration stimuli he group showed a better result in all area than the other two groups. Comparisons of sway distances with open and closed eyes showed a statistically significant decrease before and after treatment in all three groups, there was no statistically significant difference between groups. There was a significant difference only in the sway distance with the eyes closed. Conclusions: Applying the resistive exercise with vibration stimuli as a therapeutic exercise program resulted in a positive effect to the functional activity not only in rapidly recovering the strength the weaken muscles and lost of balance ability, but also expecting an earlier return to daily life by advancing the date of the process for the functional activity.

전신진동 자극 훈련이 경직형 뇌성마비 아동의 고유수용감각 및 촉각에 미치는 영향 (Effect of Whole Body Vibration Training on Proprioception and Tactile in Spastic Cerebral Palsy)

  • Purpose: The purpose of this study was to investigate the effects of whole-body vibration stimulation on proprioception and tactile in patients with spastic cerebral palsy. Methods: This study was conducted on 9 children diagnosed with spastic cerebral palsy. Of the single case study methods, the ABAB design was employed in this study. Out of a 12-week study period, three weeks were allocated to each of two baseline periods and two intervention periods. The exercise was performed twice a week for 30 minutes. A general trunk stabilization exercise was performed during the baseline period and a trunk stabilization exercise accompanied with whole-body vibration was performed during the intervention period. Evaluation was performed five times in total: before the experiment, after baseline 1, after intervention 1, after baseline 2, and after intervention 2. To determine the effect of the exercise method, a skin sensory evaluation tool (monofilament kit) and a trunk proprioception sensor (digital dual inclinometer) were used. To compare the effects of the exercises at baseline and after intervention, an analysis of variance on repeated measures (repeated ANOVA) was performed to analyze the data. Results: The results showed that there were statistically significant increases from baseline in the means of proprioception and tactile during the intervention period with whole-body vibration (p <.05). Conclusion: Whole-body vibration can be proposed as an effective intervention method for improving the proprioception and tactile in children with spastic cerebral palsy, and this exercise method is expected to be actively used in clinical practice.

만성 경부통을 가진 시내버스 운전기사에게 복합 운동프로그램의 효과 : 사례연구 (The Effects of Combined Exercise Program to Intra-City Bus Driver With Chronic Neck Pain : Case Report)

  • Background: Although many of those who drive for a living are exposed to repetitive and awkward posture and whole body vibration which cause them cumulative trauma disorder, therapeutic studies related to the problems from the transport unit are still somewhat inadequate. The purpose of this case report is to identify the effect of combined exercise program on patients suffering chronic neck pain caused by long hour of intra-city bus driving. Method: A total of 4 subjects are selected as patients, all of whom have more than 10 years of experience in intra-city bus transportation services. These people have suffered from neck pain for years. We ran the combined exercise program 3 times a week for 4 weeks and respectively evaluated the results after the 2nd and 4th week. We implemented conventional physiotherapy for 40 minutes, another 40 minutes of combined exercise program, and then educated the patients to enable themselves to do active stretching program as a home program. The combined exercise program contained 3 different stages. 1st stage: active stretching program, 2nd stage combined stabilization exercise and strengthening exercise, 3rd stage: proprioceptive exercise Result: After 4 weeks of intervention, there was enhance in the range of motion. $12^{\circ}$ increase in flexion, $10^{\circ}$ in extension, $6^{\circ}$ in lateral flexion respectively, and $10^{\circ}$ in rotation on average. VAS(visual analgue scale) decreased by 33% on average, NDI(neck diability index) by 28% and fatigability by 23%. Conclusion: There were improved results in the range of motion, NDI, VAS, and in fatigability after applying combined exercise program to intra-city bus drivers exposed to whole body vibration and cumulative trauma disorder.

수직 진동을 동반한 Leg Press 운동 시 진동 크기와 주파수에 따른 하지 근육의 근전도 차이 분석 (Difference in sEMG on lower extremity during leg press exercise with whole body vibration with various amplitude and frequency)

  • As a prerequisite of developing muscle biofeedback system which can simulate analogous isokinetic exercise, the purpose of this study was to study the effects of frequency and amplitude of whole-body vibration on the difference in sEMG on lower extremities during leg press exercise with/without vibration. The amplitude of vibration was set to 20, 50, 80 and the frequency of vibration was set to 10, 20, 30, and 50 Hz. EMG were measured at Vastus lateralis muscle and Vastus medialis muscle. MP100 EMG module(BIOPAC system Inc., USA) was used for EMG measurement. The result showed that the combination of frequency of 30Hz and amplitude of 50 had more activated EMG than other combination with relatively small work load (30kg). It is necessary to experiment the frequency between 20 and 40Hz in detail, and to normalize sEMG using maximal voluntary contraction (MVC).

심상 기법을 이용한 단축발 운동이 만성 발목 불안정성에 미치는 효과 (Effect of Short Foot Exercise Using an Imagery Technique on Chronic Ankle Instability)

  • Purpose : This study was aimed at investgating the effect of short foot exercise(SFE) using an imagery technique on chronic ankle instability(CAI). Methods : Twenty subjects with chronic ankle instability were selected by using the Cumberland ankle instability tool. They were randomly assigned to one of two groups(10 in each group) : namely the short foot exercise group and imagery technique group. The short foot exercise group performed short foot exercise for 6 weeks(3 times a week, 15 min per session). The imagery technique group performed short foot exercise(3 times a week, 15 min per session) and the imagery technique(3 times a week, 5 min per session) for 6 weeks. We measured chronic ankle instability, balance and threshold of vibration sensation before and after exercise by using the Cumberland ankle instability tool, Biodex Balance System and VSA-II, respectively. Results : We found statistically significant differences in cumberland ankle instability tool, balance and threshold of vibration sensation between the groups(p<.05). Conclusion : We confirmed the effect of short foot exercise using an imagery technique. Thus, we thought these results could be used as basic data and reference for musculoskeletal therapy or intervention using an imagery technique.
