The Study on the Application of He-Ne Laser with Low Energy ILIB to the Superficial Venules (저용량(低容量) He-Ne 레이저침의 혈락적용(血絡適用) 연구(硏究))

  • Objective : The purpose of this study was to investigate the significance of the Oriental medical treatment using He-Ne laser with low energy intravascular Laser Irradiation of Blood(ILIB) through the superficial venules. Methods : The investigation of details connected with the superficial venules in the literature is performed. The investigation of details connected with the pricking blood techniques through the superficial venules in the literature is performed. The classification of the pricking blood techniques through the superficial venules by the blood-letting puncture methods in the literature is performed. The arrangement of domestic clinical treatises on the effectiveness of medical treatment using He-Ne laser with low energy ILIB through the superficial venules is performed. The consideration on the methodology for the improvement of the clinical effectiveness of He-Ne laser with low energy ILIB through superficial venules is performed. Results and Conclusions : The superficial venules are small arteries, veins and capillaries in the superficial region of the human body. In the pricking blood techniques, there are the blood-letting puncture using the implement of acupuncture to the Jing points, Extra points and superficial blood vessels and the acupuncture using the Hirudo. The methods of the blood-letting puncture are classified into the venous blood-letting puncture, the pricking , the picking out white fiber-like substances from the subcutaneous tissue, the cluster needling, the scattered needling, the blood-letting puncture of the tready collateral branch of the large channel and the blood-letting puncture of skin. The He-Ne laser with low energy ILIB through the superficial venules belongs to the Oriental medical treatment as the method of the blood-letting puncture in the vein of cubital fossa. The He-Ne laser with low energy ILIB has an effect on hyperfibrinogenemia, hyperlipidemia, speech and motor dysfunction in the case of cerebral infarction, headache, dizziness, pain and numbness. It is considered that fundamental research on the biological change of the human body, the experimental animal and the unicellular animal, and research on the effectiveness and the safety, and the development of He-Ne laser with low energy ILIB of an effective wavelength range are necessary.

Efficacy and patient satisfaction regarding lymphovenous bypass with sleeve-in anastomosis for extremity lymphedema

  • Background When performing lymphovenous anastomosis, it is sometimes difficult to find venules in the proximity of an ideal lymphatic vessel that have a similar diameter to that of the lymphatic vessel. In this situation, larger venules can be used. Methods The authors evaluated the efficacy of and patient satisfaction with lymphovenous bypass with sleeve-in anastomosis. Between January 2014 and December 2016, we performed this procedure in 18 patients (eight upper extremities and 10 lower extremities) with secondary lymphedema. Lymphovenous bypass with sleeve-in anastomosis was performed under microscopy after injecting indocyanine green dye. The circumferential diameter was measured before lymphovenous bypass and at 1, 2, and 6 months after the procedure. An outcomes survey that included patients' qualitative satisfaction with lymphovenous bypass was conducted at 6 months postoperatively. Results Almost all patients showed quantitative improvements after surgery. The circumferential reduction rate in patients with stage II lymphedema of both the upper and lower extremities was significantly greater than in their counterparts with stage III/IV lymphedema. The circumferential reduction rate was lower in lower-extremity patients than in upper-extremity patients. Conclusions Lymphovenous bypass surgery with sleeve-in anastomosis in lymphedema patients is beneficial, and appears to be effective, when adequately-sized venules cannot be found in the proximity of an ideal lymphatic vessel.

  • This study was performed to investigate the normal ocular fundus in 30 Korean native goats. The shape of two peaks of a mountain was shown in the tapetal fundus which was mostly greenish-blue color. The nontapetal fundus which was differentiated from the tapetal fundus, was generally a uniform shade with dark blue. The optic disc was nearly round and varied in color : orange, gray, tan and various combinations. It was mostly located at the junctional area of the nontapetal and the tapetal fundus. In the retinal vasculature, three or four major arterioles and two or three major venules emerged from the optic disc and the arterioles could be distinguished from the venules. The superior arteriole and venule in the tapetal fundus did not twist each other.

Role of Nitric Oxide in Leukocyte-Endothelial Interaction in Cerebral Venules during Reperfusion after Global Ischemia

  • Objective : Reactive oxygen metabolites and polymorphonuclear leukocytes have been implicated in the pathophysiology of reperfusion injury. The mechanisms involved in superoxide-mediated leukocyte adherence remain unclear, however, nitric oxide[NO] may contribute to this response. The present study is undertaken to elucidate mechamisms controlling NO based mechanisms that regulated leukocyte-endothelial interactions in the cerebral vasculature after global cerebral ischemia and reperfusion. Methods : Pial venular leukocyte adherence of anesthetized newborn piglets was quantified by in situ fluorescence videomicroscopy through closed cranial windows during basal conditions and during 2hours of reperfusion after global ischemia induced by 9minutes of asphyxia. Nitric oxide synthase[NOS] was inhibited by local window superfusion of L-nitroarginine[NA]; superfusion of sodium nitroprusside[SNP] was used to donate NO. Results : The mean number of adherent leukocytes to cerebral venules in the 9minutes asphyxia and 2hours reperfusion group were $161{\pm}19$ compared with $13{\pm}4$ in the nonasphyxial group. Superfusion of L-NA through the cranial window for 2hours resulted in leukocyte adherence similar to that observed during the initial 2hours of reperfusion after asphyxia. Leukocyte adherence was not additionally increased in asphyxic animal treated with L-NA. SNP inhibited asphyxia induced leukocyte adherence back to control levels. Conclusions : Nitric oxide inhibits leukocyte adherence to cerebral venules during the initial hours of reperfusion after asphyxia, and that NO supplementation inhibit asphyxia induced leukocyte adherence back to control levels. These results indicate that NO is an important factor in ischemia-reperfusion induced leukocyte adherence.

Leukocyte-Endothelial Cell Adhesion Induced by Ischemia and Reperfusion Observed with in vivo Videomicroscopy (허혈-재관류에 의해 유도된 백혈구-혈관내피세포 유착에 대한 Videomicroscopy 영상소견)

  • Purpose : Recent evidence suggests a possible role for leukocytes in brain injury following ischemia and reperfusion. This study examined the temporal profile of ischemic tissue damage and leukocyte response after transient middle cerebral artery occlusion(MCAO) with reperfusion in the mouse. Methods : Focal cerebral ischemia was made by temporary occluding of the stem of the proximal MCA. Two groups of the mouse were investigated : (1) sham operation(n=10), and (2)those having the arterial occlusion released after 90 minute(n=20). By 4 hours(n=10) and 24 hours(n=10) after the onset of ischemia-reperfusion, fluorescein videoimages were under-taken in the pial venules of the mouse using a closed cranial window technique. Rhodamine 6G was administered as a $80-100{\mu}l/min$ i.v. loading dose and a $30-40{\mu}l/min$ i.v. maintenance dose in saline to selectively label circulating leukocytes. Neuropathologic evaluation for brain injury was accomplished using the histochemical stain 2,3,5-triphen-yltetrazolium chloride(TTC) and hematoxylin and eosin(H & E) stain. Results : The mean number of adherent leukocytes to cerebral venules in the 90 minutes MCAO and 24 hours reperfusion group were $306{\pm}24$ compared with $72{\pm}8$ in the sham operation group. In the TTC staining method, the cortical infarct affecting 34.8% of hemispheric volume were created in all of animals (n=10) undergoing 90 minute MCAO with 24 hours reperfusion, but the infarcted area were not found in the other(sham operation and 90 minute MCAO with 4 hours reperfusion)groups. In the H & E stain, the brain tissue following 90 minute MCAO with 4 hours reperfusion revealed only a pyknosis of the nuclei with shrunken cytoplasm, but infiltrated leukocytes were not observed. After 24 hours of reperfusion, a many leukocytes were infiltrated within parenchyma and blood vessles. Conclusions : These findings demonstrate the feasiblity of continous in vivo monitoring of leukocyte adherence in cerebral venules and suggest that reperfusion induced leukocyte adherence to venular endothelium may contribute to tissue injury following focal cerebral ischemia.

  • I have come to next conclusions in consequence of documentary study about medical books of many generations regarding venesection therapy. 1. Venesection therapy is much used for five sensory organ disease. Besides that internal disease, pain paralysis disease of muscle and joints, sugical disease, disease of woman and children, fever sunstroke CVA emergency case follow that in the order of frequency of use. 2. It is used for swollen tongue, eye pain, pharyngitis, swelling and pain in the throat, bleeding from the eye ear nose mouth or subcutaneous tissue, tonsillitis, aphthae and so on in the five sensory organ disease. Focus, sosang, jinjin yuye, taiyang, baihui are used for five sensory organ disease in the order of frequency of use. 3. It is used for malaria, headache, precordial pain, head-wind, abdominal colic, diseases characterized by acute diarrhea and vomiting, and so on in the Internal disease. Superficial venules and lymph vessesls, taiyang, quze are used for Internal disease in the order of frequency of use. 4. It is used for low back pain, hypochondriac pain, numbness, knee pain, tinea pedis, red swelling pain of hand and arm, flaccidity-syndrome, and so on in the pain paralysis disease of muscle and joints. Weizhong, superficial venules and lymph vessesls, Ashi point, zhigou are used for pain paralysis disease of muscle and joints in the order of frequency of use. 5. It is used for furuncle, tinea capitis, and so on in the sugical disease. Focus, weizhong are used for sugical disease in the order of frequency of use. 6. It is used for inflammatory disease with redness of skin, and so on in the disease of woman and children. Focus, weizhong, yanglingquan, yaoshu, sanyinjiao are used for disease of woman and children in the order of frequency of use. 7. It is used for fever, CVA, sunstroke, cadaverous coma, common cold, and so on in the fever sunstroke CVA emergency case. Sosang, weizhong, chize are used for fever sunstroke CVA emergency case in the order of frequency of use. 8. The urinary bladder channel of foot-taiyang is most used. Next there are the du channel, the stomach channel of foot-yangming, the lung channel of hand-taiyin, the gall baldder channel of foot-shaoyang, the triple-warmer channel of hand-shaoyang, the large intestine channel of hand-yangming, the spleen channel of foot-taiyin, the kidney channel of foot-shaoyin the pericardium channel of hand-jueyin the liver channel of foot-jueyin, the ren channel, the heart channel of hand-shaoyin, the small intestine channel of hand-taiyang in the order of frequency in use. 9. Superficial venules and lymph vessesls, focus, five shu points, extra-point, back point are used in the venesection therapy, those are characteristic of locating an acupuncture point.

  • The HyeolRacRon(血絡論) of the YoungChu(靈樞) is one of the section associate with the study of bloodletting(depletion of blood) by acupunture. The bloodletting therapy, one of the treatment of Oriental Medicine, has been advanced in civilization. By studying of this section, I could find some conculution as follows: 1. It is the superficial venules(血絡) that has been included blood stasis which has been interrupted by pathogenic factor(邪氣), and still could not enter into the meridian's line. 2. When bloodletting therapy, we should carefully study of patient's health condition, because doctor's missed acupuncure leads to very serious side effects. 3. External shape of HyeolRac(血絡) is like acu-niddle or muscle, so its thickness and location were not fixed position, its color is redish, and its quality is hard. 4. It is reason of heating that fixation of acu-niddle and muscle fiber.

Potential of Cells and Cytokines/Chemokines to Regulate Tertiary Lymphoid Structures in Human Diseases

  • Tertiary lymphoid structures (TLS) are ectopic lymphoid tissues involved in chronic inflammation, autoimmune diseases, transplant rejection and cancer. They exhibit almost all the characteristics of secondary lymphoid organs (SLO), which are associated with adaptive immune responses; as such, they contain organized B-cell follicles with germinal centers, distinct areas containing T cells and dendritic cells, high endothelial venules, and lymphatics. In this review, we briefly describe the formation of SLO, and describe the cellular subsets and molecular cues involved in the formation and maintenance of TLS. Finally, we discuss the associations of TLS with human diseases, especially autoimmune diseases, and the potential for therapeutic targeting.

Anatomic Study of the Vessels of the Digital Nerves in the Fingers (손가락 신경의 혈관 구조에 대한 해부학적 연구)

  • Purpose: The vessels of peripheral nerves have been extensively studied since Breidenbach used vascularizd nerve grafts. Tayor and Pinel studied the course and distribution of the vessels of peripheral nerves. However, the vessels of digital nerves are still not well known. The objective of this study was to prove vessels of digital nerves and to investigate the pathway of that. Materials and Methods: 36 patients and 2 fresh human cadavers were studied under the microscope and histologic sections under the light microscope.. Results: We found that digital nerves had own arterioles and venules as well as peripheral nerves. This small vessels of digital nerves paralleled the digital nerves or run in a spiral. Digital nerves were abundantly vascularized throught their length by a succession of vessels and by their repeated divisions and anastomoses. Conclusion: The clinical implications of this results can be discussed in relation to the dissection of nerves, the possibility of vasculized nerve grafts.

  • Kimura's disease is uncommon and chronic inflammatory disease of unknown etiology. It manifests as solitary or multiple subcutaneous nodule, primarily located on the cervical area with local adenopathies and salivary gland hypertrophy. Its clinical course is benign nature. Histologic findings are hyperplastic lymphoid tissue, inflammatory infiltrates rich in eosinophils and proliferations of capillary venules. Treatment options are steroid therapy, radiation therapy and surgical excision. We report two cases of Kimura's disease on medial aspect of upper arm.
