Pulmonary and Cutaneous Evaporative Water Losses in Sahiwal and Sahiwal × Holstein Cattle During Solar Exposure

  • In order to assess the effect of solar exposure on pulmonary functions and evaporative losses from skin and pulmonary surfaces, in six healthy Sahiwal (S) and six Sahiwal ${\times}$ Holstein ($S{\times}H$) cattle were exposed to direct sun during summer. Breed differences were observed during exposure. Increase in sweating rate was higher in crossbreds (222%) than in Sahiwal (125%). Pre-exposure (ambient temperature, $32.6{\pm}0.85^\circ}C$, solar radiation, $0.9cal\;cm^{-2}min^{-1}$) evaporative loss from skin accounted for about 90% of the losses and remaining losses were contributed by pulmonary surfaces in Sahiwal. The contribution of evaporation through skin increased to 92% (ambient temperature $39.4{\pm}0.68^\circ}C$, solar radiation, $1.35cal\;cm^{-2}min^{-1}$). In crossbreds 80% of the evaporative losses were through skin before exposure which increased to 87% after exposure to solar radiations. Rectal temperature increase was higher in crossbreds ($1.5^{\circ}C$) than in Sahiwal ($0.8^{\circ}C$). With the increase in pulmonary evaporative losses, respiratory frequency increased to 2 fold in Sahiwal and pulmonary ventilation increased 1.6 times the resting value in Sahiwal due to solar exposure. In $S{\times}H$ crossbreds the respiratory frequency increased 3.5 times and pulmonary ventilation increased only to 1.8 times due to decrease in tidal volume. There was about 2 fold increase in alveolar ventilation in both the breeds, the increase in dead space ventilation was more in crossbreds than in Sahiwal. Behavioral symptoms exhibited by animals after exposure were profuse salivation, open mouth panting, tongue protrusion and general restlessness.

  • Purpose: The purpose of this study is to predict a reasonable direction to design a pertinent educational program in the future by evaluating an adult CPR(Cardiopulmonary Resuscitation) skill performed by EMTs engaged in fire services organization and comparing the CPR success rate of factors as like a duty place and licensed year. Methods: We studied CPR skill by the use of a CPR manikin(Skillmeter Resusci Annie, Laerdal company). The study population consisted of 320 EMTs. Every EMT performed four cycle after investigating the manikin for 2 minutes. We regarded chest compression with 100 times in a minute as the 100% success rate. We analysed the skill of chest compression, ventilation and chest compression times success rate by the records printed in the CPR paper. Results: The average success rate of chest compression was 59.42$\pm$29.26% and ventilation 49.22$\pm$29.65%. The success rate of manual CPR was different between chest compression and ventilation. Also the success rate of chest compression times was high relatively as a 87.32$\pm$9.14%(p=0.000). For the CPR skill, ventilation was lowest as 49.22%. The factors such as duty place and licensed year did not influence the CPR success rate (p>0.001). Conclusion: We could have conclusion that CPR training should be shared more time in ventilation than in chest compression. Also we could reach to a conclusion that it is important to increase the times of CPR training for improving the accuracy of CPR and that the continuing education of CPR training frequency might be more than four times in a year.

Increase In Mean Alveolar Pressure Due To Asymmetric Airway Geometry During High Frequency Ventilation

  • During high frequency ventilation (HFV), mean alveolar pressure has been measured to increase with mean airway opening pressure controlled at a constant level in both humans and experimental animals. Since this phenomenon could potentiate barotrauma limiting advantages of HFV, the present study theoretically predicted the difference between menu alveolar and airway opening pressures ($MP_{alv}$). In a Weibel's trumpet airway model, approximated formula for $MP_{alv}$ was derived based on momentum conservation assuming a uniform velocity profile. The prediction, equation was a func pion of gas density($\rho$), mean flow rate(Q), and diameter of the airway opening where the pressure measurement was made($D_0$) : $MP_{alv}=4{\rho}(Q/D_0^{2})^2$. This was a result of the difference in crosssectional area between the alveoli and the airway opening. A simple aireway model experiment was performed and the results well fitted to the prediction, which demonstrated the validity of the present analysis. Previously reported $MP_{alv}$ data from anesthetized dogs in supine position were comparable to the predicted values, indicating that the observed dissociation between mean alveolar and airway opening pressures during HFV can be explained by this innate geometric (or cross-sectional area) asymmetry of the airways. In lateral position, however, the prediction substantially underestimated the measurements suggesting involvement of other important physiological mechanisms.

Incentive Spirometer를 사용한 심호흡 방법이 폐환기 기능에 미치는 효과에 관한 연구 -상복부 수술 환자를 대상으로- (A Study on the Effect which the Method of Deep Breathing with the Use of Incentive Spirometer has on the function of pulmonary Ventilation - In Upper Abdominal Operation Patients -)

  • The nursing intervention for the prevention of the pulmonary complication and of the function lowering of pulmonary ventilation which emerge with high generation frequency during the nursing of operation patient is necessary for performing the qualitative nursing for operation patient. So, this researcher tried this study so as to obtain the data which can be utilized for the trial of nursing intervention, by grasping the effect that the deep breathing with Incentive Spirometer has on the function of pulmonary ventilation, analysing the factor to have influence on the function of pulmonary ventilation, and applying the effective method of deep breathing to the clinic. By making 42 patients who underwent the operation of upper abdomen after admitting G Hospital in Seoul from Mar. 7, 1991 to Apr.30, 1991 as the object, they were classified into the experiment group that the deep breathing was made with the use of Incentive Spiromenter and the comparison group that the deep breathing exercise was made without the use of Incentive Spirometer. And then, by measuring Tidal Volume and Forced Vital Capacity with Respirometer and $O_2$ Saturation with Pulse Oximeter at preoperation postoperation 24 hours, 72 hours, and 120 hours data were collected. The collected data were analyzed with of, average, standard deviation, x$^2$-test, t-test and ANOVA by SPSS. The result of this study is as follows : 1. As for the hypothesis that the function of pulmonary ventilation at postoperation 24 hours, 72 hours and 120 hours will be better in the experiment group that the deep breathing was made with the use of Incentive Spirometer, in comparison with the comparison group that deep breathing was made without the use of Incentive Spirometer, experiment group and comparison group didn't show the significant difference in Tidal Volume, Foreced Vital Capacity and $O_2$ Saturation at postoperation 24 hours and 72 hours. But experiment group and comparison group showed the significant difference in Tidal Volume at postoperation 120 hours (p<0.01). So, this hypothesis was supported partially. 2. The variables that there were the significant differences about the function of pulmonary ventilation in experiment group at postoperation 24 hours stastically were smoking existence (p<0.05), and the variables that there were not significant differences about the function of pulmonary ventilation were distinction of sex, age, anesthetic duration, smoking extent, body weight, surface area of body, existence of narcotic use, regular exercise existence, and past experience existence of respiratory disease. As above result, it appeared that the method of deep breathing with the use of the Incentive Spirometer is more effective for the function recovery of pulmonary ventilation, in comparison with the deep breathing without use of Incentive Spirometer and that smoking existence was the factor to have influence on the function of pulmonary ventilation. In the aspect of clinic, the trial of nursing intervention of deep breathing with use of Incentive Spirometer is expected. And, in the aspect of study, the study through various operative site patients about the effect of Incentive Spirometer use at the clinic will have to be confirmed.

짐나르기의 에너지 소요량 (Energy cost of loads carried on the hands, head, or feet)

  • Oxygen consumption, pulmonary ventilation, heart rate, and breathing frequency were measured on 8 men walking on a treadmill carrying load of 9 kg on hand, back, or head. Besides measurements were made on subjects carrying loads of 2.6 kg each on both feet. The speed of level walking was 4, 5, and 5.5km/hr and a fixed speed off km/hr with grades of 0, 3, 6, and 9%. Comparisons were made between free walking without load and walking with various types of loads. The following results were obtained. 1. In level or uphill walking the changes in oxygen consumption, pulmonary ventilation, breathing frequency and heart rate were smallest in back load walking, and largest in hand load walking. The method of back load was most efficient and hand load was the least efficient. The energy cost in head load walking was smaller than that of in hand load walking. It was assumed that foot load costed more energy than hand load. 2. In level walking the measured parameters increased abruptly at the speed of 5.5 km/hr. Oxygen consumption in a free walking at 4 km/hr was 11.4ml/kg b.wt., and 13.1 ml/kg b.wt. 5.5 km/hr, and in a hand load walking at 4 km/hr was 13.9, and 18.8 ml/kg b. wt. at 5.5 km/hr. 3. In uphill walking oxygen consumption and other parameters increased abruptly at the grade of 6%. Oxygen consumption at 4 km/hr and 0% grade was 11.4 ml/kg b. wt., 13.6 at 6% grade, and 16.21/kg b. wt. at 9% grade in a free walking. In back load walking oxygen consumption at 4km/hr and 0% grade was 12.3 ml/kg b.wt.,14.9 at 6% grade, and 18.7 ml/kg b.wt. In hand load walking the oxygen consumption was the greatest, namely, 13.9 at 0% grade, 17.9 at 6%, and 20.0 ml/kg b. wt. at 9% grade. 4. Both in level and uphill walking the changes in pulmonary ventilation and heart rate paralleled with oxygen consumption. 5. The changes in heart rate and breathing frequency in hand load were characteristic. Both in level and uphill walk breathing frequency increased to 30 per minute when a load was held on hand and showed a small increase as the exercise became severe. In the other method of load carrying the Peak value of breathing frequency was less than 30 Per minute. Heart rate showed 106 beats/minute even at a speed of 4 km/hr when a load was held on hand, whereas, heart rate was between, 53 and 100 beats/minute in the other types of load carriage. 6. Number of strides per minute in level walking increased as the speed increased. At the speed floater than 5 km/hr number of strides per minute of load carrying walk was greater than that of free walking. In uphill walk number of strides per minute decreased as the grade increased. Number of strides in hand load walk was greatest and back load walk showed the same number of strides as the free walk.

내과계 중환자의 비계획적 기관내관 발관과 계획적 기관내관 발관의 관련 특성 비교 (Comparison of Related Characteristics between Unplanned and Planned Extubation of Patients in Medical Intensive Unit)

  • Purpose: This retrospective study was designed to examine the frequency of unplanned extubation, and to identify the related factors of unplanned extubation in medical intensive care unit patients. Methods: Data were collected from medical records of patients who received intubation in a medical intensive care unit. One hundred eighteen patient charts were selected for the study. The Patient Severity Classification Scale and unplanned extubation risk factors were examined. Data were analyzed with descriptive statistics, $x^2$-test, Fisher's exact test and Mann-Whitney U test. Results: The incidence of unplanned extubation was 11 (9.32%) out of 118 patients who had undergone intubation. There were statistically significant differences between the unplanned extubation and work shift ($x^2$=61.52, p=.001), ventilation mode (p=.001), number of days of ventilator application (U=366.00, p=.038) and administration of sedatives (p=.025). Conclusion: Unplanned extubation is affected by the following variables: a) whether a nurse is in night shift, b) whether ventilation is mandatory, c) duration of ventilation use and d) administration of sedatives.

치과병·의원 환기방법에 따른 실내공기질 자각증상 수준 및 근무환경 만족요인 (A Subjective Symptom Level and Satisfaction Factor by Indoor Air Quality According to Ventilation System in Dental Clinic)

  • 본 연구는 치과위생사가 근무하는 병원의 미세먼지(PM-10), 포름알데히드(HCHO), 이산화탄소($CO_2$), 일산화탄소(CO), 휘발성유기화합물(VOCs) 등에 의해 오염된 실내공기가 치과위생사의 건강 및 작업능률에도 많은 영향을 끼친다는 사실을 인식하여 치과위생사를 대상으로 근무하는 병원의 냉 반방 방식 및 창문개폐를 통한 환기 횟수에 따른 실내공기질에 대한 신체자각증상 정도를 비교하고, 근무하는 병원의 실내공기질과 관련된 환경 만족에 미치는 요인을 분석하므로써 치과위생사의 건강관리를 위한 효율적인 근무환경개선방안을 제시하기 위하여 시도되었다. 조사대상자는 전국 치과병 의원에 근무하는 치과위생사로 하였으며 2010년 10월 1일 부터 2011년 1월 21일까지 자기기입식 설문조사 방식으로 582부의 설문지를 수집하여 SPSS 12.0 프로그램을 이용하여 분석하였다. 1. 냉 난방 방식별 창문 개폐를 통한 하루 환기 횟수를 비교한 결과 창문개폐를 통한 환기가 편리한 개별 냉 난방 방식에서 높은 것으로 나타났다. 2. 치과위생사는 실내공기질과 관련된 자각증상으로 인하여 불편함을 가장 많이 느끼는 시간은 오후 2시 전 후이며 불편함을 느끼기 시작한 지 1시간 경과 후인 3시 전 후에 불편함의 정도가 가장 심한 것으로 조사되었다. 3. 치과위생사의 자각증상에 따른 불편함 수준을 냉 난방 방식 및 하루 창문개폐를 통한 환기 횟수와 비교하여 분석한 결과 창문 개폐를 이용하여 자연환기가 유리한 개별 냉 난방 방식의 사무실에 근무하는 치과위생사의 수준이 낮은 것으로 나타났다. 4. 근무하는 병원의 만족에 미치는 요인을 실내공기 조건과 관련된 항목을 독립변수로 하여 중회귀 분석을 실시한 결과 "병원 공기의 냄새가 기분 좋게한다", "병원 실내 온도조건은 근무하기에 편하다", "병원의 조명은 일하기에 적당한 편이다", "병원의 환기가 잘 된다", "병원 시끄러움", "병원공기가 신선하다고 느낀다", "병원 더움" 순으로 유의하게 나타났으며 설명력은 42.9%인 것으로 나타났다.

Evaluation of a Fungal Spore Transportation in a Building under Uncertainty

  • A fungal spore transportation model that accounts for the concentration of airborne indoor spores and the amount of spores deposited on interior surfaces has been developed by extending the current aerosol model. This model is intended to be used for a building with a mechanical ventilation system, and considers HVAC filter efficiency and ventilation rate. The model also includes a surface-cleaning efficiency and frequency that removes a portion of spores deposited on surfaces. The developed model predicts indoor fungal spore concentration and provides an indoor/outdoor ratio that may increase or decrease mold growth risks in real, in-use building cases. To get a more useful outcome from the model simulation, an uncertainty analysis has been conducted in a real building case. By including uncertainties associated with the parameters in the spore transportation model, the simulation results provide probable ranges of indoor concentration and indoor/outdoor ratio. This paper describes the uncertainty quantification of each parameter that is specific to fungal spores, and uncertainty propagation using an appropriate statistical technique. The outcome of the uncertainty analysis showed an agreement with the results from the field measurement with air sampling in a real building.

배기용 Fan Scroll에 대한 연구 (A Study on the Fan and Scroll for Ventilation)

  • In the Over-The-Range, the outlet size is limited by the industrial standards. Therefor to enlarge the volume of cavity, the installation height of ventilation fan is become small, the system resistance is higher than before. For that reason, the important design variables such as the diameter of a fan, the scroll expansion angle, etc. which play the significant role on flow rate and noise, are confined. In this study, we made an experiment of the diameter of fans relation to scroll expansion angle and investigated flow rate of the length of fans in enlarged cavity volume of OTR, and then we designed the new scroll to improve the flow rate and noise level. As a result, flow rate increased to 110% compared to current scroll and the blade passing frequency of a fan is disappeared by inclined cut-off shapes.
