The response of thrombosis in the portal vein or hepatic vein in hepatocellular carcinoma to radiation therapy

  • Purpose: The purpose of current study is to evaluate the response of the patients with portal vein thrombosis (PVT) or hepatic vein thrombosis (HVT) in hepatocellular carcinoma (HCC) treated with three-dimensional conformal radiation therapy (3D-CRT). In addition, survival of patients and potential prognostic factors of the survival was evaluated. Materials and Methods: Forty-seven patients with PVT or HVT in HCC, referred to our department for radiotherapy, were retrospectively reviewed. For 3D-CRT plans, a gross tumor volume (GTV) was defined as a hypodense filling defect area in the portal vein (PV) or hepatic vein (HV). Survival of patients, and response to radiation therapy (RT) were analyzed. Potential prognostic factors for survival and response to RT were evaluated. Results: The median survival time of 47 patients was 8 months, with 1-year survival rate of 15% and response rate of 40%. Changes in Child-Pugh score, response to RT, Eastern cooperative oncology group performance status (ECOG PS), hepatitis C antibody (HCVAb) positivity, and additional post RT treatment were statistically significant prognostic factors for survival in univariate analysis (p = 0.000, p = 0.018, p = 0.000, p = 0.013, and p = 0.047, respectively). Of these factors, changes in Child-Pugh score, and response to RT were significant for patients' prognosis in multivariate analysis (p = 0.001 and p = 0.035, respectively). Conclusion: RT could constitute a reasonable treatment option for patients with PVT or HVT in HCC with acceptable toxicity. Changes in Child-Pugh score, and response to RT were statistically significant factors of survival of patients.

  • Spontaneous intracranial hypotension (SIH) can be a rare risk factor of cerebral venous thrombosis. We describe a case of isolated cortical vein thrombosis (CVT) secondary to SIH and discuss the value of susceptibility-weighted imaging for the detection of isolated CVT.

  • Trousseau's syndrome is an unexplained thrombotic event that precedes the diagnosis of an occult visceral malignancy or appears concomitantly with the tumor. Upper extremity deep vein thrombosis is prevalent in patients with a central venous catheter. Furthermore, a peripheral intravenous injection may cause upper extremity deep vein thrombosis as well. However, a deep vein thrombosis has not been reported in the form of Trousseau's syndrome with a catastrophic clinical course triggered by a single peripheral intravenous injection. A 48-year-old man presented with a swollen left arm on which he was given intravenous fluid at a local clinic due to flu symptoms. Contrast computed tomgraphy scans showed thromboses from the left distal brachial to the innominate vein. The patient developed multiple cerebral infarctions despite anticoagulation treatment. He was diagnosed with stomach cancer by endoscopic biopsy to evaluate melena and had a persistently positive lupus anticoagulant. After recurrent and multiple thromboembolic events occurred with treatment, he died on day 20.

한의학적 치료로 호전된 심부 정맥혈전증 치험 1례 (Case of Deep Vein Thrombosis Patient Treated by Korean Medicine)

  • This is a case report on a deep vein thrombosis(DVT) patient whose pain, numbness, and swelling in the left leg have been decreased with Korean Medicine. The 59 year-old male patient who was diagnosed with DVT and showed the classic symptoms of DVT(pain, numbness and swelling in the left leg). Although he was treated with medication(100mg of aspirin, 20mg of rivaroxaban) for five weeks, his symptoms did not improved. Then, he was gaven Sunkihwalhyul-tang three times a day and Jungsongouhyul pharmacopuncture once a day for two months. Conservative therapy including leg elevation and an elastic stocking was co-administered. During our treatment period, the size of the thrombus in the left external iliac vein was decreased according to sonography compared with the pre-hospital's sonography, and the patient felt his symptoms had improved. This case report suggests Korean Medicine may play a role in improving the deep vein thrombosis symptoms.

Deep Vein Thrombosis Due to Hematoma as a Rare Complication after Femoral Arterial Catheterization

  • Hematoma is quite a common complication of femoral arterial catheterization. However, to the best of our knowledge, there have been no previous studies regarding deep vein thrombosis (DVT) caused by compression of a vein due to a hematoma. We report a case of a hematoma developing after femoral arterial catheterization and causing extensive symptomatic DVT. A 59-year-old male was seen in our Emergency Department with right lower leg swelling 15 days after coronary stent implantation performed using right femoral artery access. Computed tomographic (CT) scanning revealed a large hematoma (45 mm in its longest diameter) compressing the common femoral vein and with DVT from the right external iliac vein to the popliteal vein. Due to the extensive DVT involvement, we decided to release the compressed common femoral vein by surgical evacuation of the large hematoma. However, even following evacuation of the hematoma, as the DVT did not resolve soon, further mechanical thrombectomy and catheter-directed thrombolysis were performed. Angiography then showed nearly resolved DVT, and the leg swelling was improved. The patient was discharged with the anticoagulation medication, warfarin.

  • The problem of phlebothrombosis is an important one, not only because of its local effects, but because of the propensity for portions of the venous clot to berak off and travel back to the heart and into the pulmonary Artery. The result of pulmonary embolism, unfortunately is still as prominent a cause of death today as it was before the advant of modern advances in treatment and prevention. This paper reports a case of thrombosis of left common iliac vein left hypogastric vein and left common femoral vein, who had been bedmidden for two weeks because of left knee joint arthritis. Successfully treated with transabdominal direst venous thrombectomy and anticoagulant heparin. Authors also reviewed the references.

Early Spontaneous Recanalization of Sigmoid Sinus Thrombosis Following a Closed Head Injury in a Pediatric Patient : A Case Report and Review of Literature

  • Cerebral venous sinus thrombosis (CVST) following a closed head injury in pediatric patients is a rare condition, and an early spontaneous recanalization of this condition is extremely rare. A 10-year-old boy was admitted with a mild, intermittent headache and nausea five days after a bicycle accident. The brain computed tomography showed an epidural hematoma at the right occipital area with pneumocephalus due to a fracture of the occipital skull bone. The brain magnetic resonance imaging and the magnetic resonance venography demonstrated a flow signal loss from the right sigmoid sinus to the right jugular vein. The diagnosis was sigmoid sinus thrombosis, so close observations were selected as a treatment for the patient because of his gradually improving symptoms; however, he complained of vomiting 14 days the after conservative treatment. The patient was readmitted for a further examination of his symptoms. The laboratory and the gastroenterological examinations were normal. Due to concern regarding the worsening of the sigmoid sinus thrombosis, the brain magnetic resonance venography was rechecked and it revealed the recanalization of the venous flow in the sigmoid sinus and in the jugular vein.

  • 급성 구획 증후군은 정형외과적 응급 질환이며, 구획 내의 압력이 증가하여 비가역적인 조직의 괴사를 유발한다. 이것은 기능적 장애, 하지의 손실 및 심할 경우 신 기능 저하를 유발하여 사망까지 이를 수 있다. 환자가 초기에 통증과 종창을 호소하는 경우 심부 정맥 혈전증과 유사하여 두 질환 간의 감별이 매우 중요하다고 알려져 있다. 저자들은 마사지 후 좌측 하퇴부의 통증과 종창을 보이는 환자를 심부 정맥 혈전증으로 오인하였고 항응고제를 투여하여 발생한 급성 구획 증후군에 근막 절개술을 시행하였고 만족스러운 치료 결과를 얻었으며, 문헌 고찰과 함께 본 증례를 보고하고자 한다.

  • 심부 정맥 혈전증(deep vein thrombosis)과 폐색전증(pulmonary embolism)은 정형외과적 수술 후 발생할 수 있는 심각하고 치명적인 합병증이다. 대부분의 시술은 인공관절 치환술등 절개가 큰 시술 후에 발생한다. 반면 슬관절의 관절경적 시술 같은 최소 침습적 수술의 경우, 그 위험도 및 예방적 조치에 대해 명확하게 밝혀지지 않고 있다. 슬관절의 관절경적 시술 후 발생한 폐색전증의 국외보고는 다소 있으나 국내보고는 아직 없어 2례에 대하여 보고하고자 한다.
