  • Pressure-induced decubitus is a serious disease among the elderly people. Interface pressure occluding vascular perfusion is known to be a cause of decubitus. Therefore, it is essential to quantify the relationship between vascular perfusion and interface pressure among the elderly people. Nine elderly normal people ($57.8{\pm}5.6\;years,\;63.3{\pm}7.0kg,\;1.68{\pm}0.05m$) were participated. Pressure was applied on the ischial tuberosity in the sitting posture from 0mmHg to 135mmHg as capillary vascular perfusion was recorded. The average interface pressure to occlude vascular perfusion under the ischial tuberosity is 120mmHg. Vascular perfusion values at the capillary occlusion is often lower than 60% of the vascular perfusion at 15mmHg. Higher sampling number is required to have more accurate results.

  • Pressure-induced decubitus is a serious disease among the elderly people. Interface pressure occluding vascular perfusion is known to be a cause of decubitus. Therefore, it is essential to quantify the relationship between vascular perfusion and interface pressure among the elderly people to understand more about decubitus. Nine healthy elderly people (57.8$\pm$ 5.6 years, 63.3$\pm$ 7.0kg, 1.68$\pm$ 0.05m) were participated. Three healthy young people (31.7$\pm$ 3.2 years, 74.7$\pm$ 8.4kg, 1.75$\pm$ 0.04m) were also examined to be compared with the elderly group. Capillary vascular perfusion on the ischial tuberosity was recorded in the sitting posture as pressures were applied from 15mmHg to 135mmHg. The average interface pressure to occlude vascular perfusion (the average occlusion pressure) under the ischial tuberosity was 115.7mmHg in the elderly group. This value was not significantly different from the average occlusion pressure of the young group. Obesity effect on the occlusion pressure was investigated among the elderly group. The result was not significantly different between the obesity and the normal group in this study. This is a preliminary study to unveil the complicated cause of pressure-induced decubitus associated with occlusion of vascular perfusion. More subjects are required for the future study.

  • Pulse waves continuously change with respect to the characteristics and status of the cardiovascular system and in relation to the blood pressure (BP) and the pulse wave velocity (PWV). Monitoring the vascular condition by analyzing the variations in pulse waveforms has been used to diagnose vascular disorders and in drug treatment of arteriosclerosis and peripheral circulatory obstruction. In this paper, we investigated the vascular characteristic index with regard to the BP and classified by pulse wave signals. The pressure pulse wave and photoplethysmography (PPG) were measured simultaneously while subjects exercised, producing changes in the BP, to analyze the variation in the vascular characteristic index. We investigated the correlation between the BP and vascular characteristic index with regard to the classification methods of the pulse wave. The reflection index (RI) and vascular stiffness index were correlated with the diastolic BP, but no correlation was found between these parameters and the systolic BP. These results suggest the possibility of estimating BP through simple measurements of pulse waves.

Low Systolic Blood Pressure and Mortality From All Causes and Vascular Diseases Among Older Middle-aged Men: Korean Veterans Health Study

  • Objectives: Recently, low systolic blood pressure (SBP) was found to be associated with an increased risk of death from vascular diseases in a rural elderly population in Korea. However, evidence on the association between low SBP and vascular diseases is scarce. The aim of this study was to prospectively examine the association between low SBP and mortality from all causes and vascular diseases in older middle-aged Korean men. Methods: From 2004 to 2010, 94 085 Korean Vietnam War veterans were followed-up for deaths. The adjusted hazard ratios (aHR) were calculated using the Cox proportional hazard model. A stratified analysis was conducted by age at enrollment. SBP was self-reported by a postal survey in 2004. Results: Among the participants aged 60 and older, the lowest SBP (<90 mmHg) category had an elevated aHR for mortality from all causes (aHR, 1.9; 95% confidence interval [CI], 1.2 to 3.1) and vascular diseases (International Classification of Disease, 10th revision, I00-I99; aHR, 3.2; 95% CI, 1.2 to 8.4) compared to those with an SBP of 100 to 119 mmHg. Those with an SBP below 80 mmHg (aHR, 4.5; 95% CI, 1.1 to 18.8) and those with an SBP of 80 to 89 mmHg (aHR, 3.1; 95% CI, 0.9 to 10.2) also had an increased risk of vascular mortality, compared to those with an SBP of 90 to 119 mmHg. This association was sustained when excluding the first two years of follow-up or preexisting vascular diseases. In men younger than 60 years, the association of low SBP was weaker than that in those aged 60 years or older. Conclusions: Our findings suggest that low SBP (<90 mmHg) may increase vascular mortality in Korean men aged 60 years or older.

  • Hypertension is a medical problem with no permanent cure. Extended hypertension can cause various cardio vascular diseases, cerebral vascular diseases, and circulatory system trouble. Medical treatment at present does not consider circadian variation of blood pressure in patients ; therefore, the problem of over-reduction of blood pressure through drugs sometimes occurs. This paper presents a prediction model of circadian variation or moon blood pressure employing the endocrine grand and the autonomic nervous system.

Diminished Vascular Guanylyl Cyclase Activity in Deoxycorticosterone Acetate-Salt Hypertension

  • Pathophysiological implications of the vascular nitric oxide (NO)/cGMP pathway in hypertension were investigated. Sprague-Dawley rats were made deoxycorticosterone acetate (DOCA)-salt hypertensive for six weeks. The protein expression of endothelial constitutive NO synthase (ecNOS) and the tissue content of NO were determined in the thoracic aorta. The protein expression and catalytic activity of soluble guanylyl cyclase (GC) were also determined. Systolic blood pressure measured on the day of experiment was significantly higher in the experimental group than in the control. The hypertension was associated with decreases in the vascular tissue content of NO metabolites, concomitantly with the expression of ecNOS proteins. The protein expression of GC was not affected, while its catalytic activity was significantly decreased in hypertension. These results indicate that the high blood pressure is associated with a decreased activity of vascular NO/cGMP pathway in DOCA-salt hypertension.

Smartphone-Attachable Vascular Compliance Monitoring Module (스마트폰 탈착형 혈관 탄성 모니터링 모듈)

  • This paper presents a smartphone-attachable vascular compliance monitoring module. The proposed sensor module measures photoplethysmogram (PPG) and reconstructs an accelerated PPG waveform. The feature points are extracted from the accelerated PPG waves, and vascular compliance is estimated using these extracted features. The module is powered via the smartphone's USB terminal and transmits the acquired waveforms along with vascular compliance values through Bluetooth. The transmitted waveforms and vascular compliance value are displayed through the smartphone application. This work proposes an assessment method for consistency of PPG instrumentation, and it was implemented in a processor of sensor module. The proposed sensor module can be easily attached to smartphone that does not support PPG instrumentation, providing simple measurment and numerical analysis of vascular compliance. To verify the performance of the implemented sensor module, we acquired vascular compliance and pulse pressure data from 29 subjects. Pulse pressure, which serves as a representative indicator of vascular compliance, was obtained using a commercial blood pressure monitor. The analysis results showed that the Pearson coefficient between vascular compliance and pulse pressure was 0.778, confirming a relatively high correlation between two metrics.

  • The purpose of this study was Changes of body composition and vascular flexibility check according to Pilates Mat exercise in elderly Women and in future, Exercise programs for seniors health promotion as evidence when tried to utilize. Female 30 elderly patients classified into the control group and the exercise group, who was conducted Pilates Mat exercise was gradually intensity applied for 60 minutes, three times per 1week, a total of 12 weeks. The following were result the study. Body composition showed statistically significant increased(p<.01) in skeletal muscle and body fat rates was decreased(p<.01), and there was no statistically significant difference in body fat mass. Vascular flexibility showed systolic pressure was statistically significant decreased(p<.01) and peripheral pulse pressure were statistically significant decreased(p<.01), and there was no statistically significantly difference in diastolic pressure and aorta pulse pressure. As a result of all these, it was judged that a positive effect for body composition, and vascular flexibility after pilates mat exercise in 12 weeks.

Effect of Aqueous Extract of Polygoni Multiflori Radix on Hypertension and Arterial Contraction in Animal Models (하수오(何首烏)가 고혈압과 수축혈관에 미치는 영향)

  • This research was aimed to examine the effect of Polygoni Multiflori Radix extract on the blood pressure in spontaneous hypertensive rat (SHR) and norepinephrine - induced arterial contraction in rabbit. In order to investigate the effect of Polygoni Multiflori Radix on rabbit's contracted vascular ring detached from common carotid artery, vascular ring with intact or damaged endothelium was used for the experiment using organ bath. To analyze the mechanism of Polygoni Multiflori Radix-induced relaxation, Polygoni Multiflori Radix extract was infused into contracted vascular ring which had been pretreated by $N{\omega}$-nitro-L-arginine(L-NNA), Methylene blue(MB), and $Ca^{2+}$ was infused into contracted vascular ring induced by NE after treatment of Polygoni Multiflori Radix extract in $Ca^{2+}$-free solution. The results were as follows: Systolic blood pressure was significantly attenuated by administration of Polygoni Multiflori Radix. Blood flow and aldosterone were significantly decreased, but velocity and renin were not affected by Polygoni Multiflori Radix. Polygoni Multiflori Radix had an effective relaxation to the contracted vascular ring by NE in 0.03 mg/ml, 0.1 mg/ml and 0.3 mg/ml level. Polygoni Multiflori Radix had an effective relaxation to the intact endothelium vascular ring, but when endothelium was removed, vascular ring did not relax. Polygoni Multiflori Radix-induced relaxation was inhibited by the pretreatment of L-NNA and MB. Pretreatment of Polygoni Multiflori Radix extract inhibit the contraction by influx of extra-$Ca^{2+}$ in contracted vascular ring induced by NE in $Ca^{2+}$-free solution. As mentioned above, we suggest that Polygoni Multiflori Radix relaxes vascular ring through suppress influx of extra-cellular $Ca^{2+}$ by the action of nitric oxide from endothelium.

Effect of Arsenic on Heat Shock Protein and Vascular Contractility of Rat Aorta (횐쥐 대동맥의 수축반응과 열충격단백질에 대한 비소의 영향)

  • In order to examine if arsenic, one of environmental stresses, contributes to hypertension as one of cardiovas cular pathological factors, this study was perfarmed in vivo and in vitro, using intacted or pithed rats and aorta ring preparation, respectively. And also the relationship between expression of heat shock protein (HSP) 90 and vasoactives-induced contractile response was elucidated. To measure blood pressure, the carotid arterial pressure was recorded on physiograph(Grass Co. 79E) connected to strain gauge. On the other hand, contractile response of vascular ring preparation isolated from rat was determined in organ bath and was recorded on physiograph connected to isometric transducer. And HSP was detacted by Western blotting whole cell Iysis. Preganglionic nerve stimulation was increased by 26.0% in arterial pressure of rat treated with arsenic. Vascular contractile response was monitored and HSP were measured by Western blotting of whole Iysis prepared from samples exposed with 0, 0.5, 1, 2 and 4 mM of arsenic for 8 hours. The dose-vascular responses of potassium chloride were augmented by increasing dose of arsenic in the strips exposed to arsenic for 8 hours, and were not augmented for 1, 3, 5 hours. And the response of relaxation of rat aorta induced by histamine was not influenced by arsenic stress. The increase of HSP 90 expression in rat aorta was pronounced at 8 hours after 4 mM of arsenic treatment, but HSP 60 expression was not. Arsenic stress not only increased the expression of HSP 90 in the rat aorta, but also augmented contractions to potassium chloride. These results indicated that arsenic stress was sufficient to induce heat shock protein 90, resulting in increased vascular contractility in rat aorta.
