Detection of Enterococci and their Vancomycin Resistance in Drinking Spring-Water (먹는물 약수터 장구균의 검출 특성과 반코마이신 내성 현황)

  • This study was performed to detect enterococci strain as an indicator of faecal contamination, to identify of 16S rDNA sequence and vancomycin resistance by MIC (Minimum Inhibitory Concentration) test from drinking spring-water samples in Seoul. The detection frequency of enterococci was 42 (19.8%) among 212 samples, and its concentration was ranged from 0 to 110 CFU/100 mL. These results were confirmed the possibility as an indicator microorganisms that similar to the frequency of E. coli detection (t test p-value 0.268, significant level 0.05). Isolated 56 enterococci samples were identified by 16S rDNA sequence data and their NCBI BLAST searching. They were identified to Enterococcus faecalis of 24 samples, E. faecium (10), E. casseliflavus (10), E. gallinarum (3), E. hirae (2), E. durans (2), E. sanguinicola (1). E. faecalis was dominant species that clinical case report of a domestic was similar. Vancomycin resistant enterococci (VRE) of 53 samples showed that vanB and vanC1/C2 type with 2 and 12 case, respectively. These results indicated that the drinking spring-water quarantined to fecal pollution for block out outbreak of gastrointestinal symptom with using such as disinfection process.

Monitoring of Antimicrobial Resistance and Genetic Analysis of Enterococcus spp. Isolated from Beef, Pork, Chicken and Fish in Korea (축산물과 수산물에서 분리된 장구균의 항생제 감수성 및 유전형 분석)

  • This study was performed to examine antimicrobial resistance of Enterococcus spp. strains from retail raw meat and fish products purchased in 2012. 43 Enterococcus spp. strains were isolated from a total of 207 samples (beef, pork, chicken, fish) with contamination rate of 20.8%. The isolated strains were identified as E. faecalis (22 strains), E. gallinarum, E. hirae (5 strains), E. avium (4 strains), E. faecium (3 strains), E. duram, E. casseliflavus (2 strains). Susceptibility to 10 antibiotics was tested, and the highest resistance was observed to tetracycline. And antimicrobial resistance rates were presented below 20% with most of the other antimicrobial agents. The isolated Enterococci from chicken showed higher resistance also to ciprofloxacin and erythromycin, not only to tetracycline, compared to the isolated Enterococci from beef, pork and fish. Sixteen isolates (37.2%) were sensitive to all antibiotics. Four isolates (9.3%) were resistant to 3 or more antibiotics. Vancomycin-resistant enterococci (VRE) was not identified. According to the results of genetic similarity pattern analysis via PFGE and rep-PCR, Enterococci strains showed different patterns from these collected in 2011. This indicates that there is no genetic similarity among all the strains.

Distribution Characteristics and Antibiotics Resistance of Enterococcus spp. in Nakdong River (낙동강에서 엔테로코커스의 분포특성 및 항생제별 내성률)

  • This study was it conducted on effluent or river water close to discharge locations from a treatment plant, which were analyzed for the presence, phenotype and antibiotic resistance rates of Vancomycin Resistant Enterococci (VRE). The test results for isolation and identification of Enterococcus spp. showed that they all were VRE positive, with a total of 32 strains detected. Multiplex PCR was conducted for 32 strains, each of which were identified as E. faecium, and the results indicated that they were all confirmed as VRE, corresponding to the Van A phenotype. The results of E. faecium concentrations measured at various locations indicated that they were, on average, higher at the location of sewage treatment plants. The frequency of positive tests as well as the number of colonies was higher downstream of treatment plants. The minimum inhibitory concentration was inspected for 26 strains of discharged water samples from sewage water treatment plants, and 6 strains of river samples. Out of 19 antibiotics, 14 and 5 species showed resistance and sensitivity, respectively.

Development and Evaluation of a Web-based Education Program for Nursing Students on Control of Vancomycin-resistant Enterococcus Infection (간호학생을 위한 웹기반 VRE 감염관리 교육프로그램의 개발 및 효과)

  • Purpose: The purpose of this study was to develop a web-based education program on control vancomycin-resistant enterococci (VRE) infections and to identify the effects of the program on knowledge and performance of nursing students. Methods: The web-based VRE infection control education program was developed by using the network-based instructional systems design model. The nursing students in the experimental group could access this web-based education program at any time, and as many times as they wanted, during the clinical training period. Effects were evaluated by assessing knowledge and performance of VRE infection control measures during the clinical training period. Results: The contents of the education program included diagnosis, transmission, and treatment of VRE, contact precautions, hand washing, personal protective equipment, environment management, and quizzes. The lecture portion was filmed in a virtual screen studio using flash animation, video, and sound effects, and it was uploaded on an internet site. The knowledge and performance scores of the experimental group after using the education program were significantly higher than those of the control group. Conclusion: The results suggest that the web-based VRE infection control education program is an effective educational method to enhance knowledge and performance of VRE infection control measures.

  • Linezolid, the first oxazolidinone introduced into human clinical use, has become a last resort antibiotic in treatment of serious infections caused by Gram-positive pathogens, including methicillin-resistant staphylococci and vancomycin-resistant enterococci. Although oxazolidinones are strictly prohibited for use in food-producing animals, occurrence of linezolid-resistant staphylococci has recently been reported in livestock farms in Korea. Here, we report the complete genome sequence of an optrA-positive linezolid-resistant Staphylococcus rostri strain PJFA-333 isolated from a pig farm in Korea.

A Study of Antibiotic Resistance of Enterococci Isolated from Recycling Water of Floor Fountains in Gwangju Area (광주지역내 바닥분수 재이용수에서 분리한 장구균의 항생제 내성 연구)

  • Objectives: This study aims to evaluate water quality in terms of microorganisms and identify the antibiotic resistance of Enterococci isolated from the recycling water in floor fountains at three parks and one reservoir in the Gwangju area. Methods: Water samples were analyzed for Enterococci using membrane Enterococcus indoxyl ${\beta}$ d glucoside agar (mEI) as described in USEPA Method 1600. The vancomycin-resistant Enterococci with VanA and VanB were identified by PCR. An examination of the antibiotic resistance of isolates against 14 antibiotics was performed by the disk diffusion method. Results: The drinking water quality criterion was exceeded for total colony counts in 68% of all recycling water samples. The average concentration of total califorms and fecal coliforms was 139,325 and 413 CFU/100 mL, respectively. VanA and VanB were not detected from the isolates. We found the antibiotic resistant Enterococci strains to be E. faecalis, E. faecium, E. durans, E. mundtii, E. hirae, and E. thailandicus. The isolates were resistant to Rifampin (50%), Erythromycin (25.8%), Tetracycline (10.2%), Nitrofurantoin (8.1%), Minocycline (3.1%), Erythromycin (1.2%), Penicillin (0.7%), Norfloxacin (0.5%), and Teicoplanin (0.5%) among the 14 antibiotics tested. Antibiotic resistance tests for Enterococci from the recycling water of floor fountains resulted in 30.2% showing resistance to two or more antibiotics. Conclusions: These results showed that the multi-antibiotic resistance of Enterococci, E. coli, and others should be investigated continuously in each environment field.

Clinical Observations in Vancomycin-Resistant Enterococci Isolated from Pediatric Patients (소아 환아에서 분리된 Vancomycin 내성 장구균에 대한 임상적 고찰)

  • Purpose : Since the first report of vancomycin-resistant enterococci(VRE) in 1986, the resistance to vancomycin in enterococci has been increasingly rapidly. In this study, we investigated the clinical manifestations of pediatric patients with VRE and the pattern of the antibiotic use with increasing the rate of VRE in pediatrics Methods : We studied retrospectively 36 pediatric patients who were isolated VRE from January 1998 to December 2000. We classified patients into ICU and non ICU groups and reviewed species of VRE, specimens in which VRE were first detected and procedures performed before VRE detected. Results : We have found that the number of pediatric patients isolated VRE is increasingly annually in this study. In addition, the number of VRE-isolation in the ICU group and in patients who were operated or who underwent active procedures is much higher than that of in the non ICU group and in patients who were taken medication only. Enterococcus faecium is the main species of VRE. VRE showed high resistance to almost all antibiotics except tetracycline, and resistance was closely related to the duration of hospitalization and history of the antibiotic use. The proportion of the cephalosporin use was higher than any other antibiotic before VRE detection. In contrast, that of teicoplanin was higher than any other antibiotic after VRE detection(P<0.05). The cases of superinfection is higher in the ICU group than in non ICU group. Conclusion : In the hospital level, prevention of nosocomial infection through proper administrative policies, through surveillance of high risk VRE regions and prudent antibiotic use can prevent VRE outbreaks and corresponding side effects.

Structure Determination of Macrolactin Compounds with Antibacterial Activities Isolated from Bacillus polyfermenticus KJS-2 (Bacillus polyfermenticus KJS-2에서 분리된 항생물질 마크로락틴류의 구조결정)

  • In this study, we isolated five macrolactin compounds from a fermentation broth of Bacillus polyfermenticus KJS-2. The macrolactin compounds were structurally identified as macrolactin A (MA), 7-O-malonyl macrolactin A (MMA), 7-O-succinyl macrolactin A (SMA), macrolactin E (ME) and macrolactin F (MF) via a variety of NMR techniques, COZY, DEPT, HMQC and HMBC, and mass and specific rotation assays. The three macrolactin compounds, MA, MMA and SMA, profoundly inhibited the growth of both vancomycin-resistant Enterococci (VREs) and methicillin-resistant Staphylococcus aureus (MRSA), the inhibition of which were estimated via a disc agar diffusion bioassay. MA, MMA, and SMA exhibited antibacterial activities superior to those of vancomycin and teicoplanin.

Detection of Vancomycin-Resistant Enterococci and Related Genes Using VITEK 2 System and Multiplex Real-time PCR Assay (VITEK 2 시스템과 Multiplex Real-time PCR을 이용한 반코마이신 내성 장알균(VRE)과 내성관련 유전자 검출)

  • In this study, using the VITEK 2 system, 74 samples (22.6%) out of 327 specimens were identified by the growth of Enterococcosel media (EV6 agar) supplemented with $6{\mu}g/mL$ of vancomycin. Enterococcus faecium was identified as 55 strains (74.3%), Enterococcus casseliflavus as 2 strains (2.7%), Enterococcus avium as 1 strain (1.4%), and Enterococcus gallinarum as 16 strains (21.6%). Among the 55 phenotypes of Enterococcus faecium, 42 (76.4%), 9 (16.4%), and 4 strains (7.3%) showed the vanA, vanB, and vanC phenotype, respectively. The 16 strains of Enterococcus gallinarum and 2 strains of Enterococcus casseliflavus showed the vanC phenotype and the 1 strain of Enterococcus avium had the vanB phenotype. The one strain of Enterococcus faecium propagated only in EV4 and was susceptible to both vancomycin and teicoplanin according to the antimicrobial susceptibility test using the VITEK 2 system. The vancomycin resistance phenotype gene was not detected by PCR. A total of 327 specimens were cultured in Enterococcosel broth supplemented with $6{\mu}g/mL$ of vancomycin (EV6 broth), and 120 strains (36.7%) were isolated. These 120 strains were subjected to vancomycin resistant genotyping by a multiplex real-time polymerase chain reaction and 51 strains (42.5%) showed vanA; 5 strains (4.2%) showed vanA and vanC; and 18 strains (15%) showed vanC. Vancomycin resistance genotypes were not detected in the remaining 46 strains (38.3%).
