Application of Immunohistochemical Technique in the Cytologic Diagnosis of Herpes Simplex Virus Infection (단순포진 Virus 감염의 세포학적 진단시 면역조직 화학법의 적용)

  • Herpes simplex virus type 1 and 2(HSV-1, HSV-2) are the ubiquitous human pathogens responsible for a variety of afflictions. HSV-2 is one of the viruses that were suspected of promoting carcinogenesis in the uterine cervix. Certainly, there is a need for the more sensitive and accurate laboratory techniques for HSV detection. We examined total 80 cases of smears including 17 Tzanck smears of skin and 63 cases of Papanicolaou smears from total 77 patients with clinical impression of herpetic infections, from September, 1985 through August, 1989. Immunohistochemical typings for HSV-1 and HSV-2 were performed together with routine cytologic findings and compared. The results are as follows : 1) Patients were 9 males and 33 females, and age distribution was between 5 and 71 years. 2) Subjective symptoms such as ulceration, vesicle, vaginal discharge, pruritus, and pain were complained in 36 patients and 38 cases were genital herpes. Recurrence was noted in 11 cases. 3) Positive results were obtained in 42 among 80 cases. 4) Both routine cytology and immunohistochemical staining were positive in 13 cases and in 24 cases only immunohistochemical staining were positive. 5 cases were positive only in routine cytologic smears. 5) The cases that immunocytochemical stain had been performed were 37 cases, which were all positive in type 2. Among the above 37 cases, type 1 also were positive in 5 cases. The results show that the immunoperoxidase technique is one of the rapid and reliable method to confirm the herpetic infection when suspected and that it is particularly useful when the Papanicolaou smear findings are equivocal.

A Case of Ovarian Serous Cystadenocarcinoma Diagnosed by Cervicovaginal Smear (자궁경부.질도말로 진단된 난소의 장액성 낭성암종 1예)

  • Although there have been a few reports of cases in which cancer cells of extrauterine origin were observed in vaginal smears, such findings are relatively uncommon. We recently experienced a case of ovarian serous cystadenocarcinoma diagnosed by cervicovaginal smear in a 56-year-old woman in routine work-up of carcinoma peritonei. The cellular features were several scattered cellular clusters of adenocarcinoma cells in clear background without tumor diathesis. Psammoma body was not present. Exploratory laparotomy confirmed the diagnosis of bilateral ovarian serous cystadenocarcinoma with multiple metastases.

Screening for the Hormonal Properties of the Chloroform Extract of Carica papaya Linn. Seeds for Antifertility Investigation

  • The chloroform extract of the seeds of Carica papaya has been screened for the hormonal properties using ovariectomized female rats for estrogenicity, estrogen primed immature rats for progestogenicity and castrated adult male rats for androgenicity. The results revealed that the extract lacks progestogenicity and androgenicity as evident from the failure of the extract treated animals to mimic progestogen and androgen related changes in the target tissues. The increased weight of vagina and uterus, open status of vagina, cornified and epithelial cells in the vaginal smears and hypertrophy in the uterine epithelium, endometrium and stroma with increased glycogen and sialic acid content in the uterus of the chloroform extract treated animals, which are comparable to those of the ovariectomized estrogen treated animals, suggest that the chloroform extract possesses mild estrogenic activity.

Economic Consideration of Mass Screening Program for Early Detection of Carcinoma of the Uterine Cervix (자궁부암의 조기발견을 위한 집단 적격 검사 계획의 경제학적 고려)

  • Authors reviewed 2,362 cases of consecutive vaginal and cervical smears submitted to the Department of Pathology, Pusan Gospel Hospital during one year period from Jan. 1, 1974 to Dec. 31, 1974. Prevalence of dysplasia, carcinoma in situ and invasive carcinoma of the uterine cervix was analyzed, and cost per a lesion was calculated. The followings are conclusions: 1. Prevalence of dysplasia, carcinoma in situ and invasive carcinoma was 2.88%, 0.34% and 2.58% restectively. 2. Cost per a lesion for dysplasia was calculated as 34,735 Won, for carcinoma in situ, as 295,250 Won and for invasive carcinoma as 38,721 Won. Cost per a lesion for dysplasia and carcinoma in situ was calculated as 31,079 Won and for dysplasia and for all the lesions as 17,248 Won. 3. The results obtained suggested that mass cytologic screeiding for detection of dysplasia, carcinoma in situ and invasive carcinoma was reasonable in the present status of economy.

Activation of MAPK Is Required for ROS Generation and Exocytosis in HMC-1 Cells Induced by Trichomonas vaginalis-Derived Secretory Products

  • Trichomonas vaginalis is a flagellated protozoan parasite that causes vaginitis and cervicitis in women and asymptomatic urethritis and prostatitis in men. Mast cells have been reported to be predominant in vaginal smears and vaginal walls of patients infected with T. vaginalis. Mitogen-activated protein kinase (MAPK), activated by various stimuli, have been shown to regulate the transcriptional activity of various cytokine genes in mast cells. In this study, we investigated whether MAPK is involved in ROS generation and exocytotic degranulation in HMC-1 cells induced by T. vaginalis-derived secretory products (TvSP). We found that TvSP induces the activation of MAPK and NADPH oxidase in HMC-1 cells. Stimulation with TvSP induced phosphorylation of MAPK and $p47^{phox}$ in HMC-1 cells. Stimulation with TvSP also induced up-regulation of CD63, a marker for exocytosis, along the surfaces of human mast cells. Pretreatment with MAPK inhibitors strongly inhibited TvSP-induced ROS generation and exocytotic degranulation. Finally, our results suggest that TvSP induces intracellular ROS generation and exocytotic degranulation in HMC-1 via MAPK signaling.

Cervical Cancer Screening in an Early Diagnosis and Screening Center in Mersin, Turkey

  • Cancer is a major public health problem due to the jeavy disease burden, fatality and tendency for increased incidence. Of all cancer types, cervical cancer is reported to be the fourth most common cancer in women, and the seventh overall, with an estimated 528,000 new cases worldwide in 2012. It features a long preclinical phase with slowly progressing precancerous lesions such as CIN 2 and 3 and adenocarcinoma in situ. Therefore, screening programs such as with Pap smear tests may play an important role in cervical cancer prevention. The purpose of this study was to present results of a Pap smear screening survey for cervical cancer targeting women living in an urban area in the province of Mersin, located in the Mediterranean region of Turkey. This community-based descriptive study included women living at Akdeniz county of Mersin province. A total of 1,032 screened women between 30 and 65 ages within the routine screening programme constituted the study population. The mean age of the participants was $43.8{\pm}8.6$ (min. 30, max. 65) years. The percentage of the participants who had previously undergone smears was 40.6%. Epithelial cell changes were found in 26 (2.5%) participants, with ASC-US in 18 (1.7%), ASC-H in 2 (0.2%), LSIL in 5 (0.5%) and HSIL in 1 (0.1%). The most common clinical presentation together with epithelial changes was abnormal vaginal discharge. Taking into account the presence of women who had never undergone Pap test; it should be offered at primary level of health care in the form of a community-based service to achieve reduced morbidity and mortality rates.

Distribution of Corynebacterium renale in Cattle (축우(畜牛)에서의 Corynebacterium renale의 분포(分布))

  • An attempt was made by the authors to survey the distribution of C. renale in apparently healthy Korean male cattle and dairy cattle, and to determine the types of C. renale isolated in Korea. A total of 153 urine samples and 240 vaginal smears were collected from 253 cows for examination, and 124 urine samples of Korean cattle were investigated. Of them, one case showed cystitis symptoms. The results obtained are summarized as follows: 1. The organism was detected from 8(6.5%) of 124 specimens of Korean cattle. The isolates studied in this survey belonged to type I (4.1%), type III (1.6%) and untypable(0.8%) of C. renale. 2. The rate of isolation of C. renale was 7.5% (19 of 253 individuals) in apparently healthy cow. The 26 strains isolated from the 19 dairy cattle belonged to type I (38.5%), type II (26.9%), type III (24.6%) and untypable(11.5%) in the serological classification. From the cow with clinical cystitis, type III strain was isolated. 3. It appears that the isolation rate depends on the history of pasture rather than the number of cattle; higher percentages were detected from the pasture which had fed for longer period. 4. From the findings mentioned above, it is clear that three types of C. renale were distributed in the apparently healthy Korean and dairy cattle in Korea. And this is the first report on the isolation and distribution of C. renale in Korea.

Knowledge, Attitude and Practice of the Pap Smear as a Screening Procedure Among Nurses in a Tertiary Hospital in North Eastern India

  • Background: Cancer of the uterine cervix is one of the most common cancers among women worldwide. Industrialized countries have dramatically reduced the incidence of mortality from cervical carcinoma in the last 50 years through aggressive screening programs utilizing pelvic examinations and Papanicolaou (Pap) smears but it still remains a major problem in the developing world. Objectives: This study was performed to determine knowledge, attitude and practice of Pap smear as a screening procedure among nurses in a tertiary hospital in north eastern India. Material and Methods: This cross sectional study was carried out with a questionnaire survey covering the socio demographic factors, knowledge, attitude and practices about Pap smear screening among 224 nurses in Regional Institute of Medical Sciences, Imphal, Manipur, India during December 2011. Results: Two hundred and twenty one participants (98.6%) had heard about cervical carcinoma but 18.3% lacked adequate knowledge regarding risk factors. Knowledge about the Pap smear was adequate in 88.8% of the respondents. Out of these, only 11.6% had Pap smear at least once previously. The most common reasons for non-participation in screening were lack of any symptoms (58.4%), lack of counselling (42.8%), physician does not request (29.9%) and fear of vaginal examination (20.5%). Conclusion: Although knowledge of Pap smear as a screening procedure for cervical cancer is high, practice is still low. The nurses who should be responsible for opportunistic screening of women they care for are not keen on getting screened themselves. If we can improve the practice of Pap smear screening in such experts, they should be able to readily provide appropriate and accurate information and motivate the general population to join screening programs.

Rehmannioside D mitigates disease progression in rats with experimental-induced diminished ovarian reserve via Forkhead Box O1/KLOTHO axis

  • This study aims to explore the impact of Rehmannioside D (RD) on ovarian functions of rats with diminished ovarian reserve (DOR) and its underlying mechanisms of action. A single injection of cyclophosphamide was performed to establish a DOR rat model, and fourteen days after the injection, the rats were intragastrically administrated with RD for two weeks. Rat estrus cycles were tested using vaginal smears. Ovarian tissues were histologically evaluated, the number of primordial, mature, and atretic follicles was calculated, and the apoptotic rate of granulosa cells. Follicle-stimulating hormone (FSH), luteinizing hormone (LH), and estradiol (E2) levels were determined by ELISA assays. Protein levels of Forkhead Box O1 (FOXO1), KLOTHO, Bcl-2, and Bax were investigated in ovarian tissues of DOR rats. The binding between FOXO1 and KLOTHO was verified by ChIP assay. High-dose administration of RD into DOR rats improved their estrus cycles, increased ovarian index, enhanced the number of primordial and mature follicles, reduced the number of atretic follicle number, and ovarian granulosa cell apoptosis in addition to inhibiting FSH and LH levels and upregulating E2 expression. FOXO1 and KLOTHO were significantly suppressed in DOR rats. FOXO1 knockdown partially suppressed the protective effects of RD on DOR rats, and KLOTHO overexpression could restore RD-induced blockade of DOR development despite knocking down FOXO1. FOXO1 antibody enriched KLOTHO promoter, and the binding between them was reduced in DOR group compared to that in sham group. RD improved ovarian functions in DOR rats and diminished granulosa cell apoptosis via the FOXO1/KLOTHO axis.
