반코마이신 내성 Staphylococcus aureus 억제 물질 생산 Streptomyces sp.의 분리 및 동정 (Isolation and Identification of Streptomyces sp. Producing Anti-vancomycin Resistant Staphylococcus aureus Substance)

  • An Actinomycetes producing an anti-VRSA (vancomycin-resistant Staphylococcus aureus) substance was isolated from soil. The cultural, morphological, physiological and phylogenetic analyses of an isolated strain were investigated for identification. Cultural characteristics based on ISP (International Streptomyces Project) were as follows: white aerial mycelium, yellow reverse side, and good growth on various medium. Also, the isolate did not produce the soluble pigment. Morphological characteristics were showed cylindrical spore chain and smooth spore surface by SEM (Scanning Electron Microscope). Physiological characteristics were showed LL-type by DAP isomer analysis and detected glycine, glutamic acid and alanine. A phylogenetic analysis of the 16S rDNA provided a clue that the isolated strain was actually a member of the genus Streptomyces, because the determined sequence exhibited a higher homology with Streptomyces echinatus. The isolate was identified to be a genus of Streptomyces sp.. The optimal culture conditions for the maximum production of anti-VRSA substance by Streptomyces sp. were attained in a culture medium composed of $2.0\%$ (w/v) glucose, and $0.4\%$ (w/v) yeast extract. The anti-VRSA substance was highly produced after 5 days of culture. Optimal pH and temperature conditions for the production of anti-VRSA substance were pH 7.0 and $28^{\circ}C$, respectively.

국내 200병상이상 의료기관의 다약제 내성균 감염관리 실태조사 (Current Status of Infection Control of Multidrug Resistant Organisms in Hospitals with more than 200beds in the Republic of Korea)

  • This study was performed to investigate the current status of isolation precautions of multidrug resistant organisms(MDROs) in general hospitals with more than 200bed. The questionnaires were mailed from 3rd, April 2013 to 30th, April 2013. Methicillin resistant Staphylococcus aureus:(MRSA), vancomycin resistant Staphylococcus aureus:(VRSA), vancomycin resistant Enterococcus(VRE), multidrug-resistant Acinetobacter baumannii, carbapenem-resistant Enterobacteriaceae multidrug-resistant Pseudomonas aeruginosa were enrolled. MRSA(100.0%) and VRE(98.7%) were isolated respectively. VRE(97.3%) and MRSA(64.0%) were regulated strictly respectively. VRE(91.5%) and VRSA(50.7%) were isolated in a single room respectively. Hospital being located in Seoul(p<0.001), and beds(${\geq}600$)(p=0.008) were different significantly. The isolation space limitation(71.1%) was the highest difficulty. The development of refunding the costs of the extra supplies and other hygienic materials for infection control was discovered as the most urgent strategy.

치과병원 진료실 내에서 메티실린 또는 반코마이신 저항성 Staphylococcus aureus의 검출 (DETECTION OF METHICILLIN OR VANCOMYCIN-RESISTANT STAPHYLOCOCCUS AUREUS FROM DENTAL HOSPITAL)

  • 본 연구는 조선대학교 치과병원의 진료환경 및 진료요원으로부터 기회감염성 병원체로 알려진 methicillin 또는 vancomycin 저항성 황색포도상 구균 (methicillin-or vancomycin-resistant Staphylococcus aureus. MRSA or VRSA)의 존재 여부를 조사하여, 이를 광주지역 개원치과와 비교분석을 통해 현재 조선대학교 치과병원의 MRSA와 VRSA의 오염정도를 파악하고자 하였다. 이를 위해 진료실 환경 및 진료요원으로부터 분리한 S. aureus 균주들의 8종 항생제에 대한 감수성 조사를 시행하고, 기존에 알려진 항생제 내성 유전자 존재 여부를 PCR법을 이용하여 확인하였다. 그 결과, 조선대학교 치과병원의 진료요원에서 채취한 샘플 중 1개 (2.3%), 개원 치과에서는 2명 (10%)의 진료요원의 샘플에서 S. aureus가 분리되었으며, 진료환경에서는 두 곳 모두에서 S. aureus가 검출되지 않았다. 조선대학교 치과병원과 개원치과에서 분리된 S. aureus는 amoxicillin, penicillin G, ciprofloxacin clindamycin, vancomycin에 내성을 보이며 oxacillin, cefuroxime에는 균주에 따라 감수성 또는 내성을 보였다. 조선대학교 치과병원에서 분리된 S. aureus는 erythromycin과 clindamycin에 내성 유전자인 ermA가 존재하였으며, 개원치과에서 분리된 3개의 S. aureus 중 2개에서 penicillin과 oxacillin에 내성 유전자 mecA가 존재하는 것으로 나타났다. Vancomycin 내성 유전자인 vanA, vanB는 어떠한 샘플에서도 검출되지 않았다. 이상의 결과를 종합할 때, 본 연구는 조선대학교 치과병원과 개원치과의 S. aurues분포 및 MRSA 또는 VRSA의 존재여부를 조사하여 MRSA와 VRSA의 확산예방을 위한 치과진료 환경의 개선과 적절한 항생제 사용에 대한 기초 자료를 제공할 것으로 사료된다.

Amycolatopsis sp. KCTC 29142로부터 유래된 siderochelin A의 다제 내성 균주에 대한 항균활성 (Anti-multi drug resistant pathogen activity of siderochelin A, produced by a novel Amycolatopsis sp. KCTC 29142)

  • 본 연구에서는 신규 Amycolatopsis 균주 KCTC 29142를 분리하여 형태학적 관찰, 계통분석 및 화학분류학적 분석 등 다상 분류분석을 통해 분석하였다. KCTC 29142 균주의 에틸아세테이트추출물은 강한 항균활성을 나타났고, 활성물질은 철 이온 킬레이트 물질인 siderochelin A로 동정되었다. 본 연구에서 분리된 siderochelin A는 다제내성균인 Acinetobacter baumanii, methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant Staphylococcus aureus (VRSA), 및 Escherichia coli (E. coli)에 대해 강한 활성을 보였고, 임상에서 분리된 다제내성균에 대한 MIC를 결정하였다.

최근 10년 동안 일개 대학병원 상처 배양에서 분리된 포도알균의 항생제 내성 변화 연구 (A Study on Changes in Antimicrobial Resistant Staphylococcus aureus from Wound Isolates in a South Korean University Hospital for the Past 10 Years (2006, 2016))

  • 일반적으로 지역사회 관련 경로로 감염되기 쉬운 S. aureus는 대부분이 MSSA로 알려져 있었으나 최근 CA-MRSA가 증가하고 있다. 항생제와 glycopeptides의 지속적인 남용은 VISA, VRSA가 더욱 증가할 가능성이 있다. 본 연구는 최근 10년간 일개 대학병원의 상처에서 분리한 황색 포도상 구균의 항생제 감수성 패턴과 항생제 내성의 유병률의 변화를 조사하기 위하여 실시 하였다. 본 연구에서는 남성이 여성보다 항생제 내성이 높게 나타났다. 그리고 연령에 따라 항생제 내성률이 증가하였다. 2006년 항생제 내성률은 penicillin, oxacillin, erythromycin, gentamicin, clindamycin, tetracycline, ciprofloxacin, fusidic acid, trimethoprim/sulfamethoxazole, clindamycin, rifampicin에서 각각 97.7%, 60.5%, 57.4%, 48.8%, 41.1%, 44.2%, 44.2%, 14.7%, 13.2%, 3.9% 나타났으며, 2016년 항생제 내성률은 penicillin, oxacillin, erythromycin, gentamicin, clindamycin, tetracycline, ciprofloxacin, fusidic acid, trimethoprim/sulfamethoxazole, clindamycin, rifampicin에서 각각 95.9%, 62.6%, 55.7%, 28.6%, 50.3%, 34.7%, 38.8%, 34.0%, 2.7%, 8.2%로 나타났다. Vancomycin, linezolid, quinupristin/dalfopristin, teicoplanin 는 항생제 감수성에서 100%를 나타내었다. 특히, fusidic acid는 2006년에 비하여 2016년에 내성이 19.3% 증가하였고, 감수성이 12.4% 감소하였으며, 통계적으로 유의하였다. 따라서, 미생물 및 항생제 변화의 주기적인 검토와 이해를 통해 적절한 항생제 감수성을 지속적으로 연구해야 할 것으로 사료된다.

Antimicrobial and Antioxidant Peptide from Bacillus Strain CBS73 Isolated from Korean Food

  • An antimicrobial peptides-producing Bacillus strain CBS73 was isolated from fermented food (kimchi) that produces low-molecular-weight proteins with broad-spectrum antimicrobial activity. Our goal was to explore the therapeutic potential of antimicrobial substances produced by Bacillus species. Peptide CBS73 was purified from Bacillus subtilis subsp. subtilis with identity of 99.79%. It was found to be stable at pH 4.0-10.0 and temp $20-60^{\circ}C$. A protein band around 5.2 kDa was detected in tricine-SDS-PAGE and band was confirmed by MALDI-TOF test. Peptide CBS73 showed antimicrobial activity against MDR bacteria. The minimal inhibitory concentration (MIC) of peptide CBS73 for vancomycin-resistant S. aureus (VRSA), vancomycin resistant Enterococci (VRE) and Salmonella typhimurium ranged from $10-40{\mu}g/mL$. The antioxidant activity of peptide CBS73 was measured by DPPH scavenging, reducing power activity and total phenolic content. Cell viability and NO production result showed less cytotoxic effect upto $12{\mu}g/mL$. Peptide CBS73 could be a promising antimicrobial agent for clinical application.

부산지역 중·고등학생 휴대전화에서 분리한 식중독균의 특성 연구 - 대장균, 황색포도상구균, 바실러스 세레우스를 중심으로 - (Study on the Characteristics of Food-borne Pathogens Isolated from Students' Mobile Phones in Busan)

  • Objectives: Mobile phones have become one of the most essential accessories in daily life. However, they may act as reservoir of infectious pathogens if they are used without hygienic practices in their handling. Therefore, this study aimed to isolate food-borne pathogens from mobile phones and investigate the characteristics of toxin genes and antibiotic susceptibility patterns. Methods: A total of 146 mobile phones were collected from 83 middle- and 63 high-school students in Busan. The surfaces of the mobile phones were aseptically swabbed. Results: Among the food-borne pathogens, Staphylococcus aureus, Bacillus cereus and Escherichia coli were detected in 26 (17.8%), 20 (13.7%) and four (2.7%) samples, respectively. There were no statistically significant differences according to school level, gender or phone type. None of four E. coli strains had pathogenic toxic genes. All of the B. cereus strains carried at least three different toxin genes among the nine enterotoxin and emetic toxin genes. Three out of 20 B. cereus strains (15%) possessed emetic toxin genes, which are rarely detected in food-poisoning cases in Korea. Among the 26 strains of S. aureus, the detection rate of staphylococcal enterotoxin genes, toxic shock syndrome toxin (tsst) and factors essential for methicillin resistance (femA) were 84.6%, 7.7% and 100%, respectively. In the antibiotic susceptibility test, there was no methicillin-resistant S. aureus (MRSA) or vancomycin-resistant S. aureus (VRSA). Conclusion: The results show that students' mobile phones in Busan were contaminated by food-borne pathogens which carried various toxic genes. Therefore, regular phone disinfection and hand hygiene is important in order to reduce cross-contamination.

Nationwide Surveillance Study of Vancomycin-Intermediate Staphylococcus aureus Strains in Korean Hospitals from 2001 to 2006

  • We investigated the prevalence and the molecular characteristics of vancomycin-intermediate Staphylococcus aureus (VISA) among methicillin-resistant Staphylococcus aureus (MRSA) strains isolated from clinical samples at tertiary or general hospitals participating in a nationwide surveillance program for VISA and vancomycin-resistant Staphylococcus aureus (VRSA) in Korea during an 8-week period in each year from 2001 to 2006. Of 41,639 MRSAs isolated, 37,856 were screened and 169 grew on brain heart infusion agar supplemented with 4 ${\mu}g/ml$ vancomycin. A vancomycin MIC of 4 ${\mu}g/ml$ was confirmed for 33 VISA isolates of the 169 isolates. Eighteen of the 33 isolates were classified as hetero-VISA (hVISA) by the population analysis profile (PAP) method. All VISA isolates were susceptible to linezolid, tigecycline, and quinupristin-dalfopristin. Most VISA isolates (MIC 4 ${\mu}g/ml$) showed a PFGE C pattern with sec, seg, and sei enterotoxin genes, including ST5-SCCmec type II, or a PFGE A pattern with sea, including ST239-SCCmec type III.

외과계 입원환자에 대한 Vancomycin의 약물사용 평가 (Drug Use Evaluation of Vancomycin in Hospitalized Patients of Surgery Departments)

  • Over the last 50 years, a number of antibiotic agents have been developed and clinically used in the area of infectious diseases. Due to antimicrobial resistance problems and increasing health care costs, the rational use of antibiotics has been required. As a drug of choice to treat infections caused by MRSA, vancomycin has been extensively prescribed since the late 1970's. Recently, reports of vancomycin-resistant organisms such as VRE and VRSA have been increased to draw medical concerns. The objectives of this study were to evaluate the rational use of vancomycin and the appropriateness of the Restrictional Program of Antibiotic Utilization (RPAU) which has been operated at Samsung Medical Center. A retrospective chart review was performed in 132 hospitalized patients treated with vancomycin in the surgery departments from. January to June 1998. The guidelines of ASHP and HICPAC for vancomycin were modified and used as our criteria to determine the vancomycin DUE. In one hundred out of the patients, uses of vancomycin were approved by the Department of Infectious Diseases (DID) based on the RPAU. Vancomycin was appropriately used in $62.5\%$ of the 100 patients according to the criteria of justification of use, while $60.0\%,\;60.0\%,\;79.0\%,\;and\;51.0\%$ of the patients showed appropriate according to those of lab reports such as applicable culture obtained, pretreatment SCr, WBC and serum drug concentration monitoring, respectively. Although the rest 32 patients were not approved to receive vancomycin by the DID, twenty two percent continued receiving vancomycin treatment. This might result from the fact that the RPAU was started not before the use of antibiotics but in the middle of antimicrobial therapy. Continual education should be provide to the related health professionals and the RPAU should be simultaneously modified in order to increase the rate of appropriate uses of antibiotics.

Impact of antimicrobial resistance in the $21^{st}$ century

  • Antimicrobial resistance has been a well-recognized problem ever since the introduction of penicillin into clinical use. History of antimicrobial development can be categorized based on the major antibiotics that had been developed against emerging resistant $pathogens^1$. In the first period from 1940 to 1960, penicillin was a dominating antibiotic called as a "magic bullet", although S.aureus armed with penicillinase led antimicrobial era to the second period in 1960s and 1970s. The second stage was characterized by broad-spectrum penicillins and early generation cephalosporins. During this period, nosocomial infections due to gram-negative bacilli became more prevalent, while those caused by S.aureus declined. A variety of new antimicrobial agents with distinct mechanism of action including new generation cephalosporins, monobactams, carbapenems, ${\beta}$-lactamase inhibitors, and quinolones characterized the third period from 1980s to 1990s. However, extensive use of wide variety of antibiotics in the community and hospitals has fueled the crisis in emerging antimicrobial resistance. Newly appeared drug-resistant Streptococcus pneumoniae (DRSP), vancomycin-resistant enterococci (VRE), extended-spectrum ${\beta}$-lactamase-producing Klebsiella, and VRSA have posed a serious threat in many parts of the world. Given the recent epidemiology of antimicrobial resistance and its clinical impact, there is no greater challenge related to emerging infections than the emergence of antibiotic resistance. Problems of antimicrobial resistance can be amplified by the fact that resistant clones or genes can spread within or between the species as well as to geographically distant areas which leads to a global concern$^2$. Antimicrobial resistance is primarily generated and promoted by increased use of antimicrobial agents. Unfortunately, as many as 50 % of prescriptions for antibiotics are reported to be inappropriate$^3$. Injudicious use of antibiotics even for viral upper respiratory infections is a universal phenomenon in every part of the world. The use of large quantities of antibiotics in the animal health industry and farming is another major factor contributing to selection of antibiotic resistance. In addition to these background factors, the tremendous increase in the immunocompromised hosts, popular use of invasive medical interventions, and increase in travel and mixing of human populations are contributing to the resurgence and spread of antimicrobial resistance$^4$. Antimicrobial resistance has critical impact on modem medicine both in clinical and economic aspect. Patients with previously treatable infections may have fatal outcome due to therapeutic failure that is unusual event no more. The potential economic impact of antimicrobial resistance is actually uncountable. With the increase in the problems of resistant organisms in the 21st century, however, additional health care costs for this problem must be enormously increasing.
