Effects of Saseuptang Water Extracts on the Renal Function in Rats (사습탕(瀉濕湯) 전탕액(煎湯液)이 백서(白鼠)의 신장기능(腎臟機能)에 미치는 영향(影響))

  • The aim of this experiment was to elucidate the effects of Saseuptang water extract on the renal function plasma renin activity and plasma levels of atrial natriuretic peptide and aldosterone in rat The results were as follows; 1. Water balance decreased significantly after the administration of Saseuptang water extract, 0.4 and 0.8ml/kg. 2. Urine volume increased significantly after the administration of Saseuptang water extract, $0.4\;m{\ell}/kg$. 3. Urinary excretion of chloride increased significantly after the administration of Saseupthang water extract, $0.8\;m{\ell}/kg$. 4. Free water clearance increased significantly after the administration of Saseuptang water extract, $0.8\;m{\ell}/kg$. 5. Urinary excretion of creatinine increased significantly after the administration of Saseuptang water extract, $0.8\;m{\ell}/kg$. 6. Plasma levels of atrial natriuretic peptide (ANP) decreased significantly after administration of Saseupthang water extract, $0.8\;m{\ell}/kg$. These results suggest that the changes of urine volume after the administration of Saseuptang water extracts are related to the increments of glomerular filtration rate and free water clearance, and it is suggested that the changes of renal function by which Saseuotang may related to the renin-angiotensin and atrial natriuretic peptide system.

The Study on the Effect of Ikryung-tang(yiling-tang) and Ikryung-tangkamibang(yiling-tangjiilweifang) on Rats with Nephrosis Induced by Puromycin Aminonucleoside (익령탕과 익령탕가미방이 Puromycin Aminonucleoside로 유발된 백서의 현증에 미치는 영향)

  • Objectives : This experimental study was performed to prove the effect of Ikryung-Tang (yiling-tang; IRT) and Ikryung-tangkamibang (yiling-tangjiaweifang; IRT I, IRT II) on rats with nephrosis induced by a single intravenous injection of puromycin aminonucleoside (PAN). Methods : The effects of IRT, IRT I and IRT II on PAN nephrosis were evaluated by measuring the gene expression of IL-4, $IFN-{\gamma}$ and the concentrations of IgE, albumin, total protein, cholesterol, triglyceride, creatinine, and BUN in the serum and the volume & amount of protein of the 24hrs' urine. Results : In the gene expression linked T cell specialization, IRT II inhibited IL-4 and IgE but IRT and IRT I showed no significant difference compared with control group. On the other hand, IRT, IRT I and IRT II increased $IFN-{\gamma}$ compared with the control group. In the urine protein, serum albumin, total protein, BUN, and creatinine, IRT I especially showed more. significant effect than other groups. In the serum cholesterol and triglyceride, IRT II especially showed more significant effect than other groups. In the urine volume during 24 hrs, IRT especially showed more significant effect than other groups. Conclusions : According to the above results, it is suggested that IRT is effective for the treatment of edema, IRT I is effective for the treatment of hypoproteinemia and kidney dysfunction, IRT II is effective for immune modulation and the treatment of cholesteremia. Therefore IRT, IRT I and IRT II seem to be available for treating nephrosis in clinical practice.

Preliminary Investigation into Urinary 1-Hydroxypyrene as a Biomarker for Polycyclic Aromatic Hydrocarbons exposure among Charcoal Workers in Ogun and Oyo States, Nigeria

  • Background: Urinary 1-hydroxypyrene (1-OHP) has been widely used as a biomarker of polycyclic aromatic hydrocarbons (PAHs) in occupationally exposed workers. The objective of this study is to investigate the concentration of urinary 1-OHP among charcoal workers as subjects and non-charcoal workers as controls. Methods: Early morning urine samples were collected from 68 persons (25 charcoal workers in Igbo-Ora, 20 charcoal workers in Alabata, and 23 non-charcoal workers) who volunteered to participate in this study. 1-OHP determination in urine samples was carried out using high performance liquid chromatography after hydrolysis. Descriptive and inferential statistics were used for data analysis at p < 0.05. Results: The mean urinary 1-OHP concentration (${\mu}mol/mol$ creatinine) among charcoal workers at Igbo-Ora and Alabata and non-charcoal workers were $2.22{\pm}1.27$, $1.32{\pm}0.65$, and $0.32{\pm}0.26$ (p < 0.01). There existed a relationship between respondent type and 1-OHP concentration. Charcoal workers were 3.14 times more at risk of having 1-OHP concentrations that exceed the American Conference of Governmental Industrial Hygienists guideline of $0.49{\mu}mol/mol$ creatinine than non-charcoal workers (relative risk = 3.14, 95% confidence interval: 1.7-5.8, p < 0.01). Conclusion: Charcoal workers are exposed to PAHs during charcoal production and are at risk of experiencing deleterious effects of PAH exposure. Routine air quality assessment should be carried out in communities where charcoal production takes place. Assessment of urinary 1-OHP concentration and use of personal protective equipment should also be encouraged among charcoal workers.

On Exposure of Workers in Sections Handling Organic Solvents within Hospitals to Benzene, Xylene, and Toluene (병원 내 근로자 중 유기용제를 취급하는 부서에서 벤젠, 크실렌, 톨루엔의 노출 여부에 관한 연구)

  • Due to the development and expansion of industries and medical standards, the number of workers who handle organic solvents within hospitals is increasing. The authors in this study intended to investigate the actual conditions of the handling of the solvents and to encourage the recognition that the exposure to such solvents may be possible because of insensitivity to safety. In order to investigate the actual conditions of exposure of workers within hospitals, the experimental group included the three sections (Pathology department, Central Supply Room, and Operating Room), which handle organic solvents most frequently. Meanwhile, university interns were selected to be the control group. This study was conducted between May 1 to May 30, 2007, and urine was taken as samples. Hippuric acid, mandelic acid, methylhippuric acid, and phenylglyoxylic acid were analyzed through gas chromatography, and a total of 52 subjects of the experimental and the control groups were performed of the analysis. As for the results of the analysis of the experimental group, the exposure to benzene was $42.27{\pm}3.70mg/g$ creatinine, that of xylene was $1.30{\pm}0.18g/g$ creatinine, and that of toluene was $2.36{\pm}0.24g/g$ creatinine. Meanwhile, the results of the analysis of the control group showed that the exposure to benzene was $15.54{\pm}2.85mg/g$ creatinine, that of xylene was $0.52{\pm}0.02g/g$ creatinine, and that of toluene was $0.85{\pm}0.20g/g$ creatinine. The amount of exposure to benzene, xylene, and toluene was shown to be under the maximum permitted level, but as for Pathology department, it was required to educate on working conditions, to educate workers on prevention management about their health, and to install fume hood and exhaust system to improve the environments.

The Change of Renal Function in Diabetic Nephropathy and Chronic Renal Failure Patients with Long Term Herb Medication by Frequently Prescribed Formular (상용 한약복합 처방의 장기간 연용 투여가 당뇨병성 신증 및 만성 신부전 환자의 신기능에 미치는 영향)

  • Twenty-one diabetic nephropathy patients with normal serum BUN(Blood Urea Nitrogen), creatinine levels and ten chronic renal failure patients with abnormal high BUN, creatinine levels were investigated to evaluate the renal function change after long term herb medicine administration. The hospitalized patients were administrated three times a day with herb medicine, which were prescribe frequently in practical oriental medicine such as many hospital and local clinics. Blood Urea Nitrogen, creatinine and glomerular filtration rate (GFR) were measured immediately after 7days medication. Serum BUN, creatinine levels in diabetic nephropathy patients changed from 17.63±4.38㎎/㎗, 1.09±0.26㎎/㎗(mean±SD) of pre-medication levels to 14.13±3.24 1,20±0.37, 14.75±2.21 1.23±0.55, 12.34±2.89 1.18±0.42 at 7th, 14th, 21th days after herb medicine administration respectively. Also 24hr urine total protein changed from 632.25±254.43㎎/㎗ of pre-medication levels to 623.18±231.56㎎/㎗ after herb medicine administration(P>0.05). Serum BUN, creatinine levels and GFR in chronic renal failure patients changed from 67.45±13.86㎎/㎗, 6.74±2.91㎎/㎗, 13.73±4.21㎖/min pre-medication levels to 61.23±17.75 6.43±2.29 15.49±3.56, 58.84±19.36 5.83±2.51 16.38±2.85, 56.39±20.33 5.64±2.52 16.73±3.40 at 7th, 14th, 21th days after herb medicine administration respectively. Therefore, there was not clinically remarkable difference in the serum BUN, creatinine, GFR levels between pre-medication and post-medication in both Group.

Development of objective indicators for quantitative analysis of sodium intake: the sodium to potassium ratio of second-void urine is correlated with 24-hour urinary sodium excretion

  • BACKGROUND/OBJECTIVES: To date, sodium intake has been evaluated based on spot urine instead of 24-hour (hr) urine collection. Nevertheless, the optimal method for assessing daily sodium intake remains unclear. SUBJECTS/METHODS: Fifteen male (age 32.7 ± 6.5 years) participants were offered 3 meals with a total of 9-10 g salt over 24 hours, and 24-hr urine was collected from the second-void urine of the first day to the first-void urine of the second day. Twenty-four-hr urinary sodium (24UNa) was estimated using Tanaka's equation and the Korean formula, and spot urine Na, potassium (K), chloride (Cl), urea nitrogen (UN), creatinine (Cr), specific gravity (SG) and osmolality (Osm) were measured. The ratios of urinary Na to other parameters were calculated, and correlations with total measured 24UNa were identified. RESULTS: Average 24-hr urine volume was 1,403 ± 475 mL, and measured 24UNa was 143.9 ± 42.1 mEq (range, 87.1-239.4 mEq). Measured 24UNa was significantly correlated with urinary Na/UN (r = 0.560, P < 0.01), urinary Na/Osm (r = 0.510, P < 0.01), urinary Na/Cr (r = 0.392, P < 0.01), urinary Na/K (r = 0.290, P < 0.01), 24UNa estimated using Tanaka's equation (r = 0.452, P < 0.01) and the Korean formula (r = 0.414, P < 0.01), age (r = 0.548, P < 0.01), weight (r = 0.497, P < 0.01), and height (r = 0.393, P < 0.01) in all spot urine samples. Estimated 24UNa based on the second-void spot urine of the first day tended to be more closely correlated with measured 24UNa than were estimates from the other spot urine samples. The significant parameters correlated with the second-void urine of the first day were urinary Na/K (r = 0.647, P < 0.01), urinary Na/Cr (r = 0.558, P < 0.05), and estimated 24UNa using Tanaka's equation (r = 0.616, P < 0.05) and the Korean formula (r = 0.588, P < 0.05). CONCLUSIONS: Second-void urine is more reliable than first-void urine for estimating 24UNa. Urinary Na/K in the second-void urine on the first day is significantly correlated with 24UNa. Further studies are needed to establish the most reliable index and the optimal time of urine sampling for predicting 24UNa.

Evaluation of Serum and Urine Protein Electrophoresis in Patients with Renal Disease (신장질환 환자에서 혈청과 요단백 전기영동 분획 평가)

  • One of the cardinal findings of the renal diseases is proteinuria, which appears in the early phase of kidney diseases and is very important in diagnosis, prognosis and decision making in the treatment process and results of the treatment. The study subjects were 126 patients who visited the nephrology department of Kangbuk Samsung Hospital. Serum was requested for urine protein electrophoresis. Total protein was measured with Bayer Advia 1650 (Biuret). Quantitation of each fraction was done by multiplying the percentage of each fraction by the total protein. Serum creatinine and BUN were also measured with Bayer Advia 1650 (Jaffe and Urease). Serum protein EP was done with REP(rapid electrophoresis) using Helena Kit reagents (REP Ultra SPE Kit, Ponceau S stain, Acetic acid, Methanol, EP Control). Concentrated urine was used for urine protein EP. The SPSS package was used for statistics analysis. Percentage and quantitation of the level of albumin in renal diseases were significantly lower than those in healthy controls. Total protein was correlated with albumin. In terms of proportion, ${\alpha}1$-globulin, ${\alpha}2$-globulin, ${\beta}$-globulin, and ${\gamma}$-globulin fractions were increased in the disease group. But, in the quantified level, ${\alpha}2$-globulin was increased and ${\beta}$-globulin and ${\gamma}$-globulin were decreased. ESRD patients showed an increased secretion of high molecular proteins in urine protein EP. A decreased level in serum total protein correlated with the decreased level of serum albumin and the total amount of urine total protein. This study revealed the variety in the level of serum and urine proteins and their subgroups by EP.

Impact of the Sealant on Salivary and Urinary Bisphenol-A Concentration in Children (어린이에서 실란트 시술에 의한 타액 및 요 중 비스페놀-A 농도변화)

  • The purpose of this study was to quantify urinary and salivary bisphenol-A (BPA) concentrations according to sealant procedure among children. Nine students who had never treated with composite resin or sealant before, were recruited from one elementary school, Daegu, Korea from August 2013 to April 2014. Before sealant procedure, saliva and urine sample were collected. Immediately after sealant procedure saliva sample was collected and 24 hours after the procedure urine sample was collected. Creatinine was measured and adjusted to calculate urinary BPA concentration. Salivary and urinary BPA concentration after sealant procedure were $2.43{\pm}1.54{\mu}g/L$, $4.08{\pm}3.05{\mu}g/g{\cdot}$creatinine respectively, which were relatively higher than those before sealant procedure ($1.41{\pm}1.06{\mu}g/L$, $2.89{\pm}2.91{\mu}g/g{\cdot}$creatinine) but these were not statistically significant. We suggest that more large scale studies considering environmental confounders which have an effect on BPA are needed to establish the relationship between sealant exposure and BPA among children.

A study on the Establishment of Korean PTWI for Cadmium Based on The Epidemiological Data (국내역학조사에 기초한 한국인의 카드뮴 PTWI 설정 연구)

  • There are differences of Cadmium (Cd) urinary concentration which is considered as indicator of renal tubular dysfunction in other countries, so we have reviewed domestic epidemiological data and suggested Korean health based guidance value (HBGV) for Cd to improve an efficiency of risk management. We decided to apply the WHO calculation model which considered the relationship between dietary intake and Cd concentration in urine sample. It is determined that Cd concentration 2.5 ug/g creatinine in urine as the prevalence of renal tubular dysfunction based on epidemiological data, because there is no renal tubular dysfunction and injury/lesion such as proteinuria at the concentration of 11.63 ug/g creatinine which is the highest Cd concentration in urine from the domestic epidemiological data. It is identified that the ratio between the Cd dietary consumption (8.3~10.4 ug/day) and Cd urinary concentration (0.38 ug/g creatinine) in Korean adult who predicting never been exposed to Cd are 21.8~27.3 and then it is applied to the corresponding model suggested by WHO. Also it is applied that 10% of bioavailability and 50% of excretion rate of absorbed to body (the ratio is 24) were assumed. The estimate of daily Cd consumption level which begins tubular dysfunction is 1 ug/kg bw/day, so we suggest the Korean provisional tolerable weekly intake (PTWI) as 7 ug/kg bw/week.

Protective effect of Ganopoly and Ganopoly/C+ on nephrotoxicity induced by cisplatin in rats (Cisplatin의 신장독성에 대한 영지추출물 복합제제의 보호효과)

  • In this paper, the effect of Ganopoly(extracts of Ganoderma lucidum) and Ganopoly/C+(70% Ganopoly + 30% chitosan) on cisplastin-induced nephrotoxicity was investigated in Sprague-Dawley rats. A single dose of cisplastin(5 ㎎/㎏) kg) was administered intraperitoneally after pretreatment of saline, Ganopoly and Ganopoly/C+ for 7 days. The nephrotoxicity and renal function were manifestated by the changes of body weight, blood pressure, biochemical changes and solute in urine and plasma. After the treatment of CDDP(cis-dichlorodiamineplatinum), a significant elevation of kidney weight, serum urea, cretinine, urine volume for 24 hours, urine magnesium, and a severe or significant decrease in body weight, blood pressure, creatinine clearance, urine osmolarity, serum albumin, etc. The nephrotoxicity was further confirmed by a significant decrease in glutathione S-transferase(GSH) in urine and kidney homogenate, GSH, glutathione peroxidase(GSH-Px) and catalase in kidney tissue. And also the lipid peroxidation was significantly increased in kidney homogenate. These signs of nephrotoxicity was ameliorated by the pretreatment and consecutive administration of Ganopoly and Ganopoly/C+ for 14 days after the Lp. injection of CDDP on 7th day after pretreatment of Ganopoly and Ganopoly/C+. The amelioration of nephrotoxicity was evidenced by significant reduction in serum urea and creatinine concentration, and improvement of other index of renal function. And The activity of antioxidant enzymes were partially recovered in kidney tissue of rats treated by CDDP and the administration of Ganopoly and Ganopoly/C+. These results indicate the cispastin induced nephrotoxicity is due to an impairment of tubular reabsorption systems enhanced by necrosis of proximal tubule, and the Ganopoly and Ganopoly/C+ has a partial protective effect on nephrotoxicity induced by CDDP. The polysacchride of Ganoderma lucidum may improve the therapeutic index of nephrotoxicity induced by CDDP. However, it is needed to elucidate the mechanism for confirming the therapeutic effect.
