방향족 유기용제 폭로근로자들의 요중 N-Acetyl-β-Glucosaminidase Activity와 마뇨산농도 (NAG Activity and Urinary Excretion of Hippuric Acid Among Workers Exposed to Aromatic Organic Solvents)

  • In order to investingate the renal dysfunction in workers exposed to aromatic organic solvents, 105 male exposed workers and 109 controls were participated in this study. This study was conducted to examine the correlation among the concentration of aromatic organic solvents in air, worker's urinary NAG activity and urinary hippuric acid. The results were as follows : 1. The exposure level of aromatic organic solvent mixture caculated by the equation proposed by ACGIH was ranged from 0.27 to 0.54 and toluene concentration was ranged from 18.3ppm to 48.3ppm. 2. NAG activity in the urine of control and exposed group were $36.1{\pm}14.2nmolMU/mg$ creatinine and $52.4{\pm}26.1nmolMU/mg$ creatinine, respectively. Hippuric acid concentration in the urine of control and exposed group were $191.1{\pm}194.2mg/g$ creatinine and $789.3{\pm}784.1mg/g$ creatinine, respectively. 3. Correlation coefficent between the exposure level of aromatic organic solvent mixture was significantly related to urinary NAG activity(r=0.542) and urinary hippuric acid(r=0.752). 4. Correlation coefficient between NAG activity and hippuric acid concentration was 0.357. This study suggested that urinary NAG activity was a good renal function index for aromatic organic solvents exposure and urinary hippuric acid was an index for the biological monitoring of toluene exposure.

항암제 유발 말초신경병증환자와 유기산검사 마커와의 상관성 연구 (Correlation Analysis of Organic Acid Comprehensive Profile Markers with Chemotherapy Induced Peripheral Neuropathy in Cancer Patients)

  • Objectives: The purpose of this study is to evaluate the urinary organic acid comprehensive profile for chemotherapy induced peripheral neuropathy (CIPN). Methods: Participants are 66 patients with CIPN who had symptom (Visual analog scale ${\geq}30mm$, Eastern Cooperative Oncology Group ${\leq}2$). Participants were tested with organic acid comprehensive profile markers. Results: Positive Correlation was observed in the neurotransmitter metabolism markers, N-methyl-D-aspartate (NMDA) modulators markers, detoxification markers, energy production markers, amino acid metabolism markers, and intestinal dysbiosis markers. Especially, all the neurotransmitter metabolism markers were showed positive rate of 44%. In addition, neuro-endo-immune was associated with energy metabolism (mitochondrial dysfunction) in CIPN of cancer patient. especially detoxification, intestinal bacterial hyperplasia, vitamin deficiency (folate, complex B group, vitamin C). Conclusions: Significant urinary organic acid comprehensive profile results were obtained in cancer patients who induced peripheral neuropathy by chemotherapy.

유기용제의 생물학적 폭로기준 설정에 관한 연구 (Biological Exposure Indices of Organic Solvents for Korean Workers)

  • Biological exposure indices (BEI) of toluene, perchloroethylene (PCE) and methyl ethyl ketone (MEK) for Korean workers were studied respectively. Environmental exposures in workplace to organic solvent were measured by personal sampling. Blood toluene, blood perchloroethylene, urinary trichloroacetic acid and urinary MEK were determined by headspace gas chromatography. Urinary hippuric acid were determined by HPLC and corrected by creatinine. BEIs for Korean workers were calculated as the levels of determinants which are correspond to permissible exposure limits in Korea. Blood toluene level of 2.2mg/l and urinary hippuric acid level of 1.7g/g creatinine are correspond to an exposure of 100 ppm toluene. Blood PCE concentration of 1.6mg/l and urinary trichloroacetic acid concentration of 2.9mg/l are correspond to an exposure of 50ppm PCE. Urinary MEK concentration of 1.0mg/l is correspond to an exposure of 200ppm of MEK. BEIs for Korean workers determined in this study are very different to ACGIH's BEI as urinary determinants are much lower and blood determinants are much higher than ACGIH's BEI.

자동차 정비공의 혈액 및 뇨검사 소견과 자각증상에 대한 조사 (Laboratory Findings and Subjective Symptoms of Car Repair Workers)

  • To evaluate the effects on health by exposure to low dose organic solvents, the author analyzed the air concentration of mixed organic solvents (toluene, xylene, butylacetate) at worker's breathing zone during painting, some laboratory findings of blood (WBC, RBC, Hb, Hct, SGOT, SGPT, TC, TG, HDL-C, LDL-C, IgG, IgM, IgA and IgE) and urine (hippuric acid, urinary protein, urobilinogen), surveyed the subjective symptoms those were obtained from 35 male workers exposed to mixed organic solvents, and 25 male workers not exposed to organic solvents who worked in car repair workshops in Taejon area from December 1, 1995, to February 29, 1996. The results were as follows: 1. The mean concentration of urinary hippuric acid of car painting worker group (organic solvent exposure group) was 0.76$\pm$ 0.21 g/l, which is significantly higher than that of non-exposed group. 2. In hematologic findings, the values of RBC, TC, LDL-C, IgG and IgE in the exposure group were significantly lower than those of the nonexposed group, but SGOT and SGPT in the exposure group were significantly higher than those of the non-exposed group. 3. Urinary hippuric acid levels showed positively correlated with toluene, urobilinogen and HDL-C levels, but those were negatively correlated with RBC, LDL-C, IgM levels. 4. Rates of the subjective symptoms such as "dizziness", "appetite loss", "weight loss", "palpitation", "chest tightness", "sore throat and eye discomfort", "tingling sense and acrodynia", "illusion or hallucination" and "decreaased motor power" were significantly higher in the exposure group than those of the non-exposed group.

분석 방법에 따른 요중 마뇨산의 분석값 비교 (Comparison of analytical methods of hippuric acid in urine)

  • 이 연구의 목적은, 톨루엔에 노출된 근로자의 생물학적모니터링에 있어 혼재하는 유기산의 종류와 분석방법에 따른 요중 마뇨산 분석값을 비교하여, 다른 유기산이 혼재할 경우 UV법으로 분석했을 때 일어날수 있는 방해효과를 측정하는데 있다. 마뇨산만 포함된 소변 표준시료와 마뇨산, 메틸마뇨산, 페닐글리옥실산, 만델산 등 각각 다른 유기용제의 대사물이 포함된 소변 표준시료를 UV법, HPLC법으로 분석하고 유기산의 방해효과(IE; Inteference Efect)를 계산하였다. 또한 이들 소변 표준시료에 대해 전국 100개 검진기관을 대상으로 한 외부정도관리 결과, 참가기관들이 사용한 분석방법에 따른 요중 마뇨산 분석값을 비교하였다. 톨루엔과 크실렌에 노출된 근로자의 소변을 UV법으로 분석할 경우 크실렌 대사물인 메틸마뇨산은 마뇨산 분석값에 영향을 주어, 마뇨산 농도 0.33 g/L에서 0.5-2.0 g/L의 농도로 공존시 60-82%의 방해 영향을 가진 것으로 나타났다. 그러나 스티렌 대사물인 만델산, 페닐글리옥실산은 마뇨산 분석값에 거의 영향이 없었다. 따라서 톨루엔과 크실렌이 함께 노출되는 경우 소변 모니터링시 UV법을 사용하는 것보다 GC법이나 HPLC법을 사용하는 것이 바람직하다. 특히 특수검진기관의 의무 보유장비인 GC법은 경제적이고 신속하며 정확도가 높은 방법으로 UV법에 대체하여 사용할 수 있는 것으로 나타났다.

신발제조업체의 접착제 사용에 따른 직접·간접폭로 근로자들의 복합유기용제 폭로량과 자각증상 비교 (A Study on the Mixed Organic Solvent Dose and Subjective Symptoms of Direct and Indirect Bonding workers in Shoes Manufacturing Industrial)

  • This study was performed to find out the exposure level of mixed organic solvents, excretion of urinary hippuricacid and subjective symptoms according to the exposure of organic solvents of female workers who works on 5 shoes manufacturing industries in Taejon City from 24, september to 20, october 1993. The studied groups were divided into 3 groups that were consist of direct exposure group(48 workers), indirect exposure group(49 workers) and non-exposure group(68 workers) to the organic solvents. The exposure levels of toluene of direct exposure group which $89.86{\pm}56.20ppm$ had higher than that of indirect exposure group which had $40.23{\pm}47.21ppm$. In the exposure level of mixed organic solvent(R-value), direct exposure group was $2.84{\pm}1.53$ and exceeded approximatly 3 times the R-value. Whereas, indirect exposure group was not exceeded the R-value as $0.80{\pm}0.61$. In the excretion level of urinary hippuric acid, direct exposure group was $1.78{\pm}1.25g/l$, indirect exposure group was $1.22{\pm}0.93g/l$ and non-exposure group was $0.51{\pm}0.18g/l$ respectively. Therefore both direct exposure group and indirect exposure group were significantly higher than non-exposure group(P<0.01). In the correlation between toluene levels and urinary hippuric acid level, the direct exposure group had positive correlation(R=0.8309, P<0.01), also indirect exposure group had positive correlation(R=0.5859, P<0.05) and also in the correlation between the R value of mixed organic solvents and the urinary hippuric acid levels, the direct exposure group had positive correlation(R=0.4492, P<0.05), and indirect exposure group had ositive correlation(R=0.7911, P<0.01). In the complain rates of the worker's subjective symptoms at work, both direct exposure group and indirect exposure group were higher than non-exposure group(P<0.05, P<0.01). But the sujective symptoms of "floating sensation" of direct exposure group had significantly higher than indirect exposure group. In the percent of subjective symptoms complaints during the worker's daily life, both direct exposure group and indirect exposure group had generally more statistical significance than nonexposure group(P<0.01), direct exposure group had not statistical significant difference from indirect exposure group. As the results mentioned above, it has been analysed that the indirect at the adjacent manufaturing process are exposed to the considerable amount of solvent. Therefore, I think that there should be the betterment of surrounding through the complete working environment management to the occurrence source of the organic solvent, the changes of health management system to the indirect-exposed workers, and the systematic management of the special medical examination and the like.

유기용제와 소음에 폭로된 근로자들의 청력 손실 (Hearing Loss in the Workers Exposed to Organic Solvents and Noise)

  • 유기용제와 소음의 폭로가 청력손실에 미치는 영향을 알아보기 위해 유기용제 폭로군 32명, 소음 폭로군 31명, 소음과 유기용제에 동시에 폭로된 군 31명 그리고 대조군 53명을 대상으로 소음과 유기용제 폭로 정도와 청력손실치를 계산하여 얻어진 결과는 다음과 같다. 1. 조사 대상자들은 소음과 유기용제에 허용기준치 이하로 폭로되었으며, 요중 마뇨산 배설량도 참고치 이하로 배설되었다. 소음의 경우 소음과 유기용제 동시 폭로군과 소음 폭로군이 유기용제 폭로군보다 높았으며(p<0.05) 요중 마뇨산은 소음 유기용제 동시폭로군과 유기용제 폭로군이 소음폭로군보다 배설량이 더 많았다(p<0.05). 2. 주파수별 기도청력은 소음과 유기용제 동시 폭로군이 소음군보다 우측귀의 500Hz, 좌측귀의 500, 2000Hz에서 유의하게 더 높은 청력손실치를 나타내었고(p<0.05), 유기용제군과의 비교시에는 왼쪽귀 4000Hz에서 청력손실이 유의하게 더 높았다(p<0.05). 3. 조사 대상자 147명중 43명이 청력손실군에 해당되었으며 청력손실군의 연령이 42.6세로 정상군 38.0세 보다 더 높았으며 요중마뇨산 배설량도 유의하게 더 많았다(p<0.05). 4. 청력손실자는 소음군에서는 대상자의 38.7 $\%$(12명), 유기용제군 40.6 $\%$(13명), 소음과 유기용제 동시폭로군은 51.6 $\%$ (16명), 그리고 비폭로군은 3.8 $\%$(2명)의 빈도를 보였으며 비폭로군에 비해 폭로군에서 청력손실자가 많이 발생하였으나 폭로군 사이에서는 유의한 차이를 보이지 않았다. 5. 청력손실과 유의한 상관관계를 보이는 변수는 연령과 요중마뇨산 배설량이었으며, 유기용제로 인한 청력손실의 영향을 보기위해 연령을 보정하였을 때는 요중마뇨산 배설량만이 상관관계를 보였다(p<0.05). 6. 대조군과 폭로군사이의 청력손실에 대한 교차비에서는 비폭로군에 비해 소음폭로군은 6.1배(95 $\%$ CI,: 3.3-8.7),유기용제 폭로군은 7.4배(95 $\%$ CI,: 3.5-14.6) 그리고 동시폭로군은 1$\ulcorner$.2배(95 $\%$ Cl,: 5.31.8)로 더 켰다(p<0.01). 이상의 결과로 볼 때 유기용제 폭로 근로자에 대해서도 청력장애에 대한 건강진단이 실시되어야 할 것으로 생각된다.

유기산성약물이 푸로세미드의 흡수, 배설및 이뇨효과에 미치는 영향 (The Effects of Organic Acidic Drugs on the Absorption, Excretion and Diuretic Action of Furosemide)

  • The effects of organic acidic drugs on the absorption, excretion and diuretic action of furosemide were studied. Cefalexin, p-aminohippuric acid (PAH), ibuprofen and p-amino salicylic acid (PAS) were selected as organic acidic drugs. The in situ absorption rate and absorption rate constant of furosemide (30$\mu{M}$) were significantly (p<0.05) decreased by 30$\mu{M}$ of cefalexin, PAH, ibuprofen and PAS in rat small intestine. The plasma concentration of furosemide was significantly (p<0.01) increased by cefalexin, PAH and ibuprofen in rabbits. But the urinary excretion rate, renal clearance and diuretic action of furosemide were significantly (p<0.05) decreased by cefalexin, PAH, ibuprofen and PAS in rabbits.

Measurement of Microbial Protein Supply in Murrah Buffaloes (Bubalus bubalis) Using Urinary Purine Derivatives Excretion and PDC Index

  • A study was conducted to predict the rumen microbial protein production based on urinary excretion of purine derivatives in buffaloes fed a diet of wheat straw and concentrate (40:60) at four fixed levels of feed intake. (95, 80, 60 and 40% of preliminary voluntary feed intake) following experimental protocol of IAEA (Phase I). The buffaloes were allocated according to a $4{\times}4$ latin square design. The urinary allantoin, uric acid, total PD excretion (mmol/d) in treatments L-95, L-80, L-60 and L-40 was 20.13, 16.00, 12.96 and 9.17; 1.88, 2.12, 2.11 and 2.15; 22.01, 18.12, 15.07 and 11.32, respectively and were significantly (p<0.05) different among treatments except for uric acid. The rate of PD excretion (mmol/d) was positively correlated with the digestible organic matter intake. Variations were observed in PD and creatinine concentration in spot samples collected at 6-hour interval. However, daily PD:Creatinine ratio (PDC index) appears to be a reasonably good predictor of microbial-N supply. The contribution of basal purine excretion to total excretion of purine derivatives (PD) was determined in pre-fasting period followed by a fasting period of 6 d (Phase II). Daily PD and creatinine excretion (mmol/kg $W^{0.75}$) during fasting averaged 0.117 and 0.456 respectively for buffaloes. The excretion rates of PD decreased significantly (p<0.01) during fasting compare to pre-fasting period, the urinary creatinine excretion remained almost similar. Except for creatinine, plasma concentration of target parameters significantly (p<0.01) declined during fasting. Likewise, glomerular filtration rate (GFR) and renal clearance of allantoin and uric acid also decreased. Based on the PD excretion rates during fasting and at different levels of feed intake obtained in this study, a relationship between daily urinary PD excretion (Y-mmol) and microbial purine absorption (X-mmol) was developed for buffaloes as Y = 0.74X+0.117 kg $W^{0.75}$. The microbial N supply (g/kg DOMI) remained statistically similar irrespective of dietary treatment. The results showed that excretion of urinary purine derivatives is positively correlated with the levels of feed intake in Murrah buffaloes and thus, estimation of urinary purine derivatives and PDC index could be used to determine microbial nitrogen supply when there is large variation in level of feed intake.
