A Case Study of the Febrile Soeumin Patient Caused by UTI(Urinary Tract Infection) (요로감염(尿路感染)으로 인한 소음인(少陰人) 발열(發熱) 치험례(治驗例))

  • 1. Objectives In the western Medicine, UTI generally bring on fever, and treated by antibotics. In the traditional way of korean Medicine, our predecessors used the herb for the febrile disease that has the property of coldness. However, Lee Je-ma tried to make the Yang ascend with the herb, considering thar the febrile illness in Soeumin is made by the Exterior Heat not ascending. In the aspect of the Sasang Consititutional Medicine(SCM), Hwanggigyeji-tang(황지계지탕(黃芷桂技場)) & Palmoolgunja-tang (팔물군자탕(八物君子湯)) belongs to the kind of the warm medication. This study is reported to evaluate rhe effected of Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), Palmulgoonja tang(팔물군자탕(八物君子湯)) to the febrile patient. 2. methods To heal the febrile patient who was diagnosed as UTI & Soeumin exterior disease, we used soeumin Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), Palmulgoonja tang(팔물군자탕(八物君子湯)). 3. Results This parient's main symptoms are fever, sweating impotently feeling, anorexia. So we treated the patient with Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), (팔물군자탕(八物君子湯)) in ${\ulcorner}$Dongyi Suse Bowon(동의수세보원(東醫壽世保元))${\lrcorner}$ and improved the patient?s symptoms. 4. Conclusions We suggest that Soeumin Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), (팔물군자탕(八物君子湯)) are effective significantly to the patient with febrile illness, bur then we consider that the studies on the case like this are more needed to convince that Hwanggigyeji-tang(황지계지탕(黃芷桂技場)), (팔물군자탕(八物君子湯))are very much effective.

  • This work was performed to investigate the possibility of using J96 hemolysin(Hly, Hly A) vaccine against urinary tract infecting Escherichia coli. Based on the known sequence of J96 hemolysin which was originally isolated from a pyelonephritis patient, ten 20-mer oligonucleotide probes were synthesized. Radioactive labelled 8 probes showed positive colony blots against most of the hemolysin producing wild type E. coli, while HA484 and HA661 showed 28.3, 71.7% positive blots, respectively. This result means that hemolysin genes are highly conserved. Also, 12 anti-Hly MABs(monoclonal antibodies) showed more than 90% positive immunoblots against secreted hemolysin from wild type E. coli. Especially, the result that MAB132 neutralized hemolysin from all of the wild type E. coli augments the idea that hemolysin will be effective as a vaccine.

  • Polyps of the male urethra are relatively uncommon lesions. They usually arise from the prostatic urethra mostly verumontanum or Just lateral to the verumontanum. Posterior urethral polyp often presents with hematuria, hemospermia and sometimes lower urinary tract obstruction. We recently experienced a pedunculated prostatic urethral polyp in a 63-year-old man who complained of gross total hematuria and difficult micturition for several years. On rectal examination the prostate was slightly enlarged with normal consistency. Excretory urogram showed a round filling defect in the right side of the bladder neck measuring about $2{\times}2cm.$ in size. Cystourethroscopy revealed trabeculation of the vesical wall, mild bilateral prostatic hypertrophy and a round cystic mass with a long stalk arising from the prostatic floor 1 cm. Proximal to the verumontanum which caused ball-valve obstruction in the bladder neck. This pedunculated polypoid mass was then resected at its base with resectoscope and was removed transurethrally using Lowsley's grasping forceps. The specimen was proved as fibrous polyp histologically.

The Comparison of Kupperman's Index and MENQOL between Women with Surgical Menopause and Women with Natural Menopause (수술적 폐경을 경험한 여성과 자연 폐경 된 여성의 Kupperman's Index, MENQOL 비교)

  • Purpose: To investigate the change of the climacteric syndrome with women with induced menopause by hysterectomy or ovariectomy compared with women with natural menopause. Methods: Menopausal women(aged 45-55) were selected at two oriental medicine hospitals, Sam-se Oriental Medicine Hospital and Daegu Hannny Oriental Medicine Hospital. They were asked about Kupperman's Index, MENQOL and a few additional questions. They were divided into two groups: women with induced menopause by hysterectomy or ovariectomy and women with natural menopause. To analyse the Kupperman's index and MENQOL, SPSS for Windows 12.0 and Wilcoxon Ranked Sum Test were used. Results: The total sums of Kupperman's index and MENQOL were significantly correlated with women with induced menopause and women with natural menopause and women with induced menopause had higher total sums. Both the renal and urinary tract disorder and the sexual disorder were significantly correlated with women with induced menopause and women with natural menopause. The gastrointestinal and psychosocial disorder of Kupperman's index and MENQOL were not significantly correlated with women with induced menopause and women with natural menopause. Conclusions: Women with induced menopause by hysterectomy or ovariectomy experienced climacteric syndrome more severely compared with women with natural menopause, specifically the renal and urinary tract disorder and sexual disorder.

Febrile urinary tract infection in children: changes in epidemiology, etiology, and antibiotic resistance patterns over a decade

  • Background: Understanding the epidemiology and prevalence of febrile urinary tract infection (fUTI) in children is important for risk stratification and selecting appropriate urine sample collection candidates to aid in its diagnosis and treatment. Purpose: This study aimed to analyze the epidemiology, etiology, and changes in antibiotic susceptibility patterns of the first fUTI in children. Methods: This retrospective observational cohort study included children younger than 19 years of age who were diagnosed and treated for their first fUTI in 2006-2016. Electronic medical records were analyzed and radiologic images were evaluated. Results: A total of 359 patients (median age, 5.1 months; interquartile range, 3.0-10.5 months) fit the inclusion criteria; of them, 78.0% (n=280) were younger than 12 months old. The male to female ratio was 5.3:1 for patients aged 0-2 months, 2.1:1 for those 3-5 months, and 1.6:1 for those 6-11 months. Beyond 12 months of age, there was a female predominance. Escherichia coli was the leading cause (83.8%), followed by Enterococcus species (6.7%), and Klebsiella pneumoniae (3.6%). Significant yearly increases in the proportions of multidrug-resistant strains (P<0.001) and extended-spectrum beta-lactamase (ESBL) producers (P<0.001) were observed. In patients with vesicoureteral reflux (VUR), the overall recurrence rate was 53.6% (n=15). A significantly higher recurrence rate was observed when the fUTI was caused by an ESBL versus non-ESBL producer (75.0% vs. 30.0%, P=0.03). Conclusion: fUTI was most prevalent in children younger than 12 months of age and showed a female predominance in patients older than 12 months of age. The proportion of ESBL producers causing fUTI is increasing. Carbapenems, rather than noncarbapenems, should be considered for treating fUTI caused by ESBL-producing enteric gram-negative rods to reduce short-term recurrence rates in children with VUR.

Prevalence of Multi-Antibiotic Resistant Bacteria Isolated from Children with Urinary Tract Infection from Baghdad, Iraq

  • Urinary tract infections (UTIs) are one of the most common infections in different age groups, including children. Bacteria are the main etiological agents of UTIs. The aim of the present study was to isolate, identify, and determine the antibiotic susceptibility of bacteria isolated from children with UTIs from Baghdad, Iraq. Three hundred and two urine samples were collected from children aged 6 months to 12 years. The samples were cultured on blood agar and MacConkey agar. The selected colonies were subjected to biochemical tests and antibiotic susceptibility analysis using the Vitek® 2 Compact automated microbial identification system. In this sample, 299 bacteria were identified, of which, 267 were gram-negative bacteria, and 32 were gram-positive bacteria. Escherichia coli (56%) was the most commonly isolated gram-negative bacteria, followed by Pseudomonas aeruginosa (14%), Enterobacter spp. (10.48%), Klebsiella pneumoniae (9.36%), Proteus spp. (7.8%), Acinetobacter baumannii (1.5%), and Morganella morganii (0.37%). Enterococcus faecalis (62.5%) was the most commonly detected gram-positive bacteria, followed by Staphylococcus aureus (37.5%). E. coli and P. aeruginosa were the most antibiotic-resistant bacteria. Among the tested antibiotics, meropenem showed 100% sensitivity, followed by imipenem (97.4%), amikacin (91.8%), and tobramycin (83.5%). In contrast, the high frequencies of resistance were observed with cefixime (93.2%), cefotaxime (78.7%), and ceftriaxone/cefotaxime (71.2%). In conclusion, carbapenems and aminoglycosides are highly recommended for the empirical treatment of UTIs, while, Quinolones, penicillins, and cephalosporins are not suggested. Frequent antibiotics susceptibility testing are warranted to determine the resistance pattern of UTI bacteria.

Prevalence and Cytotoxic Effects of Some Colibactin and cnf Genes among Escherichia coli Isolated from Urinary Tract Infections

  • Colibactins (clb) and Cytotoxic Necrotizing Factors (cnf) are virulence factors that impact cell cycle through cellular differentiation, proliferation, and apoptosis. Urinary tract infections (UTIs) are the most common among type of infection among outpatients, with a lifetime incidence of about 60-65% in adult females. Here, we sought to isolate uropathogenic Escherichia coli (UPCE) from urine specimens and investigates the prevalence of clb A, B and cnf 1, 2 genes among these isolates. A total of 110 E. coli isolates were collected from patients with UTIs. All the isolates were examined for their hemolytic activity and only 46 isolates showed a halo zone of hemolysis on blood agar. The collected UPEC isolates were screened for the existence of clb A, B and cnf genes. The results revealed that out of 110 isolates, 28 harbored the clbA gene, 40 harbored clb B, and 24 isolates harboured cnf1. 13 isolates harbored clbA, clbB, and cnf1 genes, while no cnf2 gene was detected among isolates. The molecular detection revealed that 8 out of 28 hemolytic isolates carrying the clbA, 11 out of 40 were carrying clbB, 1 out of 24 were carrying cnf 1, and 5 out of 9 carrying clbA+clbB. Furthermore, 7 out of 13 isolates were hemolytic and carrying clbA, clbB, and cnf1 genes. Finally, we investigated the cytotoxicity of E. coli harboring clb and cnf genes, eukaryotic REF cells were exposed to E. coli producing colibactin, which induces DNA damage and leads to cell cycle arrest, senescence and death.

Plasma Neutrophil Gelatinase-associated Lipocalin and Leukocyte Differential Count in Children with Febrile Urinary Tract Infection

  • Purpose: We aimed to study the association of plasma neutrophil gelatinase-associated lipocalin (pNGAL) and leukocyte differential count in children with febrile urinary tract infection (UTI). Methods: Medical records of 154 children aged 1 month to 13 years with febrile UTI who were hospitalized were retrospectively reviewed. Associations between pNGAL levels and blood leukocyte differential count at admission and after 48 hours of treatment were investigated in children with or without acute pyelonephritis (APN). Results: The APN group (n=82) showed higher pNGAL levels, neutrophil count, monocyte count, and neutrophil-to-lymphocyte ratio (NLR), compared to the non-APN group (n=72) (all P<0.05). After adjustment for age and sex, pNGAL showed positive correlations with neutrophil count and NLR in both groups (all P<0.05). Additionally, it was correlated with the monocyte-to-lymphocyte ratio (MLR) only in the APN group (P<0.05). Before and after treatment, pNGAL was positively correlated with neutrophil count, NLR, and MLR in patients with APN while it was related with neutrophil count and NLR in those without APN (all P<0.05). Areas under the receiver operating curve of pNGAL, neutrophil count, NLR, and MLR for predicting APN were 0.804, 0.760, 0.730, and 0.636, respectively (all P<0.05). Only pNGAL was independently associated with the presence of APN in a multivariable logistic regression analysis (P<0.05). Conclusion: In children with febrile UTIs, pNGAL might be associated with leukocyte differential count and the presence of APN.

Comparison of O-serogroups, Virulence Factors and Phylogenetic Groups of Uropathogenic Escherichia coli Isolated from Patients with Urinary Tract Infections between 2 Time Periods of 1989 and 2010-2014 at Gangwon Province in Korea

  • Uropathogenic Escherichia coli (UPEC) is main causative agent of urinary tract infections. They are classified based on various types of O antigen. UPEC strains commonly possess many genes encoding virulece-associated factors. E. coli strains are generally divided into four main phylogenetic groups. The virulence factor (VF) profiles of UPEC are related with their O-serogroups in each strains. A total of 681 strains of UPEC clinical isolates were collected from Korean healthcare facility (1989: 123 strains and 2010-2014: 558 strains). The UPEC clinical isolates were analyzed by polymerase chain reaction (PCR) methods. A total of 14 O-serotypes (O1, O2, O4, O6, O7, O8, O15, O16, O18, O21, O22, O25, O75 and O83), 6 virulence factors (papC, fimG/H, sfaD/E, hly1, cnf1 and usp) and phylogenetic groups were identified. The most prevalent O-serogroups were O6 (11.1%) in 1989 UPEC strains and O25 (21.0%) in 2010-2014 UPEC strains. The identified VFs, phylogenetic groups in 1989 UPEC strains and 2010-2014 UPEC strains were fimG/H and B2 group. In this study, O6 serotype was revealed the close relationships with VFs. Also, the distribution of prevalence O-serogroups of UPEC has been changed from O6 to O25 and virulence of UPEC strains was increased during past twenty-one years.

Animal Models for the Study of Post-Micturition Dribble in Aged Male (고령 남성의 배뇨 후 요실금 연구를 위한 동물 모델)

  • Since attaining middle age, many patients suffer from the post-micturition dribble (PMD) syndrome but do not receive proper treatment. This is because the etiology and treatment mechanisms of PMD differ from conventional lower urinary tract symptoms. Several pathophysiologies have been proposed, but the mechanisms we observed are the weakening of the bulbocavernosus muscle and dilation of the urethra due to atrophy of the cavernosal tissue. This study investigates the mechanism of PMD by observing anatomical changes in the corpus cavernosum and urethra. SD male rats were categorized by age into the young rats group (8 weeks old) and the old rats group (24 months or more). The ratio area of the bulbous urethra and corpus cavernosum to the total penis was statistically significant between both groups (P<0.05). Significant differences were obtained between the two groups for restored smooth muscle contents and several other parameters related to PMD mechanisms (nNOS, α-SMA) (P<0.05). The changes in size and parameters of the urethra and corpus cavernosum in young and old rats were confirmed. The results from this study are expected to contribute to the study of PMD in the future.
