Intrapelvic Urethral Anastomosis in a Dog with Complete Obstruction of Proximal Membranous Urethra (막성요도 근위부 완전폐쇄를 지닌 개에서 골반내 요도문합술)

  • A 4.65 kg 13-month-old male Pekingese dog was referred to veterinary teaching hospital of Konkuk University for evaluation of dysuria. On physical examination, severe distention of urinary bladder was found in abdominal palpation. Urinary catheter could not be guided into urinary bladder. On serum biochemistry, blood urea nitrogen (35.6 mg/dl) and creatinine (1.9 mg/dl) were increased. Obstruction part of proximal membranous urethra was founded on urethrogram. The length (13 mm) of obstruction part was callipered by cystourethrogram and urethrogram on operation. Surgical resection of obstruction part of urethra was performed without pubic osteotomy, and anastomosis was performed with 5-0 polyglycolic acid. Omentum was placed around the urethral anastomosis. On first day after surgery, appetite was good. On day 5, complete blood count and serum biochemistry showed normal range but mild urinary incontinence was showed after removing catheter. On day 7, urinalysis showed normal condition. On day 14, no leakage of surgical site was observed in excretory urogram. On day 21, no more urinary incontinence and good micturition were found. On 1 year later, the patient showed healthy condition without recurrence.

  • A cross breed dog (6-year-old, 6 kg, intact male) was referred with hematuria. The dog had been treated for years owing to the urinary bladder calculi. On abdominal radiography, prostatic calculus was demonstrated in the prostatic area. In addition, ultrasonography and computed tomography (CT) scan would confirm that the part of calculus protruded within the prostatic urethra. The patient underwent a prostatolithotomy and traumatic prostatic urethra was carefully sutured and the omentum was filled with the prostate lumen. A crystallographic analysis of the stone showed 80% magnesium ammonium phosphate (struvite) and 20% carbonate apatite. The leakage of the urine was not observed post-operation and the hematuria improved and there was no specific problem at the 6 months follow-up.

  • An eight-year-old, castrated male, a Schnauzer dog was presented with dysuria, hematuria and intermittent vomiting. Retrograde positive contrast urethrography was performed under fluoroscope. On the contrast examination, the leakage was observed in the prostatic urethra region and irregular filling defect was also shown from the prostatic urethra and the level of the trigon of the bladder. Surgical treatment was selected because of partial urethral obstruction and dysuria. The metastasis at the time of surgery was suspected, and the owner decided euthanasia. We detected lymph metastasis with immunohistochemical staining for pan-cytokeratins (CK) AE1/AE3, confirming their epithelial origin. This report described typical clinical findings, diagnostic imaging and histopathological examination of a prostatic adenocarcinoma.

Surgical Repair of the Traumatic Urethral Transection with Urethrocutaneous Fistula in a Pung-san Dog (풍산 개에서 발생한 외상성 요도 절단과 요도 피부 누공의 수술적 교정)

  • A five-year-old, castrated male, Pung-san dog was referred to Konkuk University Veterinary Teaching Hospital with trauma and persistent hemorrhage in the inguinal region. The dog had a history of being wounded by a wild boar at 5 days prior to presentation. Rupture of the membranous urethra and urethrocutaneous fistula were demonstrated by the retrograde positive contrast urethrography. A urinary catheter was placed to identify the urethra and urethrocutaneous fistula. The necrotic tissues and damaged tissues by urine leakage around urethrocutaneous fistula were debrided. An urethral anastomosis over an indwelling catheter was performed. The dog maintained normal urination without other complications including dysuria and hematouria at the follow-up evaluation after 1 month postoperatively. A retrograde urethrogram repeated at 2 months after surgery showed no urethral stricture.

  • Polyps of the male urethra are relatively uncommon lesions. They usually arise from the prostatic urethra mostly verumontanum or Just lateral to the verumontanum. Posterior urethral polyp often presents with hematuria, hemospermia and sometimes lower urinary tract obstruction. We recently experienced a pedunculated prostatic urethral polyp in a 63-year-old man who complained of gross total hematuria and difficult micturition for several years. On rectal examination the prostate was slightly enlarged with normal consistency. Excretory urogram showed a round filling defect in the right side of the bladder neck measuring about $2{\times}2cm.$ in size. Cystourethroscopy revealed trabeculation of the vesical wall, mild bilateral prostatic hypertrophy and a round cystic mass with a long stalk arising from the prostatic floor 1 cm. Proximal to the verumontanum which caused ball-valve obstruction in the bladder neck. This pedunculated polypoid mass was then resected at its base with resectoscope and was removed transurethrally using Lowsley's grasping forceps. The specimen was proved as fibrous polyp histologically.

The Effects of Ginseng Saponin on Relaxation of Smooth Muscle in the Lower Urinary Tract and the Corpus Cavernosum (인삼 사포닌이 하부요로와 음경해면체 평활근의 이완작용에 미치는 효과)

  • Background: Korean ginseng (KG) has been used as a general tonic, and for voiding dysfunction for a long time in oriental society. However, scientific basic studies on the use of KG, have been rare, especially for voiding and erectile dysfunction. This study was performed to investigate the effects of KG on voiding and erectile function by examining the effects of total saponin (TS) on the bladder, urethral and penile cavernosal smooth muscle. Materials and methods: To examine the effects of TS, NewZeland white rabbits were used to obtain tissue strips from the smooth muscle of the bladder, proximal urethra and corpus cavernosum. Adult Sprague Dawley rats were used to examine the changes in urodynamic findings and penile erection after administration of TS. Results: In proximal urethral strips, the rate of relaxation of the proximal urethra was increased from $9.0{\pm}2.9$ to $33.7{\pm}4.8%$ in a dose-dependent manner when the concentration of TS was added accumulatively from 0.25 mg/ml to 4.0 mg/ml (p<0.05). However, no significant response was observed in the bladder strips within these concentration ranges. For the corpus cavernosal strips, the rate of relaxation ranged from $5.8{\pm}2.1$ to $36.7{\pm}5.8%$, increasing in a dose-dependent manner when TS was increased from 1.0 mg/ml to 4.0 mg/ml (p<0.05). After administration of 0.1 ml of TS (32 mg/ml) in the rat, the bladder pressure was $37.5{\pm}8.5$ mmHg at $52.1{\pm}7.0$ sec. during isovolumetric bladder contraction, showing no significant differences from $35.7{\pm}7.8mmHg$ and $50.7{\pm}7.2$ sec, respectively, before treatment. However, when 0.1 ml of TS (32 mg/ml) was administered, the relative reduction of urethral pressure was $6.9{\pm}0.5mmHg$ at $62{\pm}7.5$ sec, which was significantly higher compared to $4.6{\pm}1.1mmHg$ at $45{\pm}10$ sec before treatment (p<0.05). For the cavernosal injection study, the change in intracavernosal pressure (${\Delta}ICP$) was examined after administering 0.1 ml of TS. The cumulative additions of TS at concentrations from 0.5 mg/ml to 32 mg/ml increased ${\Delta}ICP$ from $1.3{\pm}0.5$ to $21.3{\pm}7.8mmHg$ in a dose-dependent manner (p<0.05). The duration of tumescence was from $0.3{\pm}0.1$ to $5.2{\pm}0.2$ min, showing dose-dependent increase (p<0.05). Furthermore, the cumulative addition of TS at concentrations from 0.5 mg/ml upto 32 mg/ml did not cause any significant change in systemic blood pressure. Conclusion: These results suggest that ginseng improves voiding functions, which is mainly achieved by TS relaxing the proximal urethra, the most important part of the bladder outlet function. In addition, ginseng safely induced a penile erection hemodynamically by relaxing the corpus cavernosum.

  • Hinman syndrome is a condition representing urinary voiding dysfunction in the neurologically intact child. The syndrome is probably caused by acquired behavioral and psychosocial disorders manifested by bladder and/or bowel dysfunction mimicking neurologic disease. Clinically, the symptom complex may include day and night time enuresis, encopresis, constipation, and recurrent urinary tract infections. Cystoscopy frequently demonstrates normal vesicourethral anatomy. Voiding films usually demonstarate a carrot-shaped proximal urethra with a persistent narrowing at the external sphincter. The bladder is large and often appears trabeculated with a thickened wall and significant postvoid residual. A 13-year-old male child was admitted due to fever, urinary tract infection, enuresis and flank pain. His neurologic examination was normal. Renal sonograms showed moderate hydronephrosis. Voiding cystourethrograms showed a huge, trabeculated bladder without vesicourethral reflux and urethral valves. No abnormal findings was found in spinal MRI.

Diagnosis and Treatment of Benign Prostate Hyperplasia in a Shih-tzu dog (Shih-tzu견에서 발생한 전립선 비대증 진단 및 치료 1례)

  • A 3-year-old, male Shih-tzu dog with clinical signs of dysuria and hematuria was brought to the Veterinary Teaching Hospital. Chonnam National University. The patient was suspected urolithiasis by history taking and clinical signs but urinary and urethra calculus was not found in the radiographic examination. Enlarged prostatic gland (29.0$\times$28.0 mm) and severe finding of cystitis was observed from the ultrasonographic examination. The dog was finally diagnosed as the secondary urethral obstruction by benign prostatic hyperplasia. Castration was performed to treatment of acute hyperplasia of prostatic gland and prostatic cysts. After 26 days of operation, the prostate was defined small, hypoechoic, symmetric and decreased size (18.5$\times$18.0mm) by ultrasonography and dysuria cured completely.

  • A 6-day-old male Korean native calf was referred to Veterinary Teaching Hospital, College of Veterinary Medicine Kyungpook National University because of dyschezia with atresia me. Clinical signs included anorexia, dyschezia, abdominal distention and depression. In radiographic examination, the intestinal loops are filled with gas and there is a fistula connecting the large intestine to the urethra. Colostomy was performed immediately and atresia am was healed on 28 day after colostomy without Buy other complication.

  • Two dogs were presented urinary disorder in the associated with urolithiasis. Clinical signs were increased frequency of urination with mild straining pollakiuria, general depression and anorexia. On physical examinations, the pain was revealed at the palpation of the urethral area. Urinalysis showed high specific gravity, high pH, and triple phosphates. Radiography showed an increased radiopacity, and ultrasonography showed hyperecho in the just behind of os penis. Urolithiasis was diagnosed on the basis of clinical signs, radiography, and ultrasonography. In the prescrotal urethrotomy, urethra on the midline was incised and uroliths were eliminated. After elimination of uroliths, incision area was closed with 4-0 synthetic absorbable suture. In postoperative, there was good prognosis without hemorrhage, inflammation, and urethral stricture.
