Dorsal Neck Muscle Fatigue Affects Cervical Range of Motion and Proprioception in Adults with the Forward Head Posture

  • Purpose: This study was to investigate the effect of dorsal neck muscle fatigue on the cervical range of motion (CROM) and proprioception in adults with the forward head posture (FHP). Methods: Thirty pain-free subjects were enrolled in this study. All subjects were measured the forward head angle by taking the capture of the sagittal plane of their upper body to determine the FHP. Subjects were distributed into two groups: the FHP group (n=14) and Control group (n=16). All subjects were measured the CROM and the Head repositioning accuracy (HRA) for joint proprioception before and after inducing muscle fatigue of the dorsal neck. The CROM and HRA were measured in neck flexion, extension, right-left lateral flexion, and right-left rotation. Sorenson's test was used to induce muscle fatigue of the dorsal neck. Results: Total CROMs were significantly decreased after dorsal neck muscle fatigue in both groups (p<0.05). Total HRAs were significantly increased after dorsal neck muscle fatigue in the FHP group (p<0.05), but there were no significant differences in the control group (p>0.05). Total CROM changes were not significant differences between groups (p>0.05), but total HRA changes were significant differences between groups (p<0.05) except for right and left lateral flexion (p>0.05). Conclusion: Immediate CROM and proprioception reduction after the dorsal neck muscle fatigue were observed in adults with the FHP. Therefore, FHP can significantly affect the CROM and positioning consistency of cervical proprioception.

두경부에 발생한 점막형 악성흑색종의 임상적 특성 (Clinical Characteristics of Head and Neck Mucosal Melanoma)

  • 점막형 악성흑색종 10례에 대한 임상분석 결과 비강에서 가장 호발하였고 증상은 비폐색이 가장 많았다. 치료 성적은 수술을 우선적으로 선택한 후 방사선요법을 시행한 경우군에서 평균생존기간이 20.8개월이었고 방사선치료를 우선적으로 선택한 경우군에서는 14.7개월로, 접막형 악성흑색종의 경우 광범위 절제술 후 방사선요법을 시행하는 것이 도움이 될 것으로 생각되나 윈격전이 6례를 포함한 8례의 환자에서 재발이 있어 예후가 극히 불량한 암종으로 생각된다.

두경부암 환자에 병발된 위암 2예 (Two Cases of Stomach Cancer in Patients with Head and Neck Cancer)

  • The causes of treatment failure in head and neck cancer are locoregional recurrence, distant metastasis and second primary cancer. The favorite sites of second primary cancer are head and neck, lung and esophagus. But, the incidence of stomach cancer in Korea is the highest of all, high incidence of stomach cancer is expected in head and neck cancer patients. We experienced 2 cases of stomach cancer after successful treatment of primary head and neck cancer at Kang Nam St. Mary's hospital. The first case was a 60-year-old male with nasopharyngeal non-keratinizing carcinoma(stage III, T1N1M0). He received three cycles of induction chemotherapy including cisplatin and 5-fluorouracil followed by radiotherapy which resulted in complete response. Five months after completion of radiotherapy. stomach adenocarcinoma(stage IV, T4N2M1) was diagnosed. He received one cycle of FAM chemotherapy and died 4 months after diagnosis of stomach cancer. The second case was a 50-year-old male with pyriform sinus squamous cell carcinoma(stage II, T2N0M0). He received curative partial pharyngolaryngectomy followed by radiotherapy which result in free of disease. Four months after completion of radiotherapy. stomach adenocarcinoma(stage IV, T4N2M1) was diagnosed. Bypass gastrojejunostomy was performed. The screening test for stomach cancer including upper gastrointestinal X-xay series and/or endoscope of the stomach should be performed periodically for the patients with head and neck cancer in Korea.

자가 신장운동이 전방머리자세를 동반한 만성 목통증 환자의 자세개선에 미치는 영향 (Effect of Self-Stretching Exercises on Postural Improvement in Patients with Chronic Neck Pain Caused by Forward Head Posture)

  • PURPOSE: The purpose of this study was to provide clinical basic data to reduce pain and improve function by comparing neck muscle activity and neck alignment using self-stretching and passive stretching exercises for chronic neck pain caused by forward head posture. METHODS: The subjects were divided into 15 subjects assigned to perform self-stretching exercise and 15 subjects assigned to perform passive stretching exercise. The intervention was conducted for a total of 4 weeks. The muscle activity in the neck was measured by surface electromyography (EMG) before intervention, and craniovertebral and cranial rotation angles were measured by X-ray. The 4-week intervention was conducted and the above items re-measured in the same manner and analyzed. RESULTS: Muscle activity within both groups after intervention using self-stretching or passive stretching exercise was significantly different (p < .05)(p < .01). Neck alignment of both groups was significantly different (p < .001)(p < .01). Further, muscle activities of the upper trapezius and splenius capitis muscles showed significant differences (p<.05). Lastly, neck alignment showed statistically significant difference (p < .05). CONCLUSION: Self-stretching exercise activated motor nerves as a posture correction exercise, thereby improving inhibition of muscle activity, muscle contraction delay, and pathological conditions of the muscle. For future research, interventions of self-stretching exercise will be needed for patients with chronic back pain accompanied by forward head posture, and various clinical studies on postural improvement of forward head posture by maintaining a normal muscle tone state are needed.

Age-Related Prevalence of Periodontoid Calcification and Its Associations with Acute Cervical Pain

  • Study Design: Prospective study. Purpose: To assess the prevalence of periodontoid calcification and its associations with acute cervical pain. Overview of Literature: Calcium pyrophosphate dihydrate (CPPD) deposition disease is a common rheumatological disorder that occurs especially in elderly patients. Although CPPD crystals induce acute arthritis, these crystals are not usually symptomatic. Calcification surrounding the odontoid process (periodontoid calcification) has been reported to induce inflammation, resulting in acute neck pain. This disease is called crowned dens syndrome. Whether calcification induces inflammation or whether the crystals are symptomatic remains unclear. Methods: The prevalence of periodontoid calcification at the atlas transverse ligament was examined by computed tomography of the upper cervical spine in patients suspected of brain disease but no cervical pain (control group, n=296), patients with pseudogout of the peripheral joints but no cervical pain (arthritis group, n=41), and patients with acute neck pain (neck pain group, n=22). Next, the correlation between the prevalence of periodontoid calcification and symptoms was analyzed. Results: In the control group, 40 patients (13.5%) showed periodontoid calcification with no significant difference in the prevalence with gender. The prevalence of calcification increased significantly with age (p=0.002). In the arthritis group, 26 patients (63.4%) reported periodontoid calcification. In the neck pain group, 14 patients (63.6%) reported periodontoid calcification. Multiple logistic regression analysis by age and group revealed that higher age, inclusion in the arthritis group, and inclusion in the neck pain group significantly affected the prevalence of calcification. Conclusions: Our results cumulatively suggest that periodontoid calcification is an aging-related reaction and that calcification per se does not always cause neck pain. Periodontoid calcification was observed more frequently in patients with pseudogout of the peripheral joints and in those with acute neck pain than in asymptomatic control patients.

측두하부와에 발생한 영아섬유종증 1례 (A Case of Infantile Fibromatosis of the Infratemporal Fossa)

  • The fibromatoses are defined as a group of non encapsulated, non metastasizing, fibrous tumors that have a tendency for infiltrative growth and local recurrence following surgical excision and can be divided into fascial and musculoaponeurotic fibromatoses. Infantile fibromatosis is childhood counterpart of musculoaponeurotic fibromatosis and is more common in the head and neck, the shoulder and upper arm and the thigh. Treatment includes excision, radiation, steroid and chemotheraphy, etc. Treatment of choice is complete excision but postoperative deformity and sequelae should be considered. Recently, we have experienced a case of infantile fibromatosis originating from infratemporal fossa and report this case with a review of literatures.

후두기관식도열 1례 (A case of Laryngortacheoesophageal cleft)

  • Laryngotracheoesophageal clefts are rare congenital anmalies of the upper aerodigestive tract. They range in severity from small soft tissue defects in the interarytenoid region to complete clefts of the larynx, trachea, and esophagus. Minor clefts with mild symptoms can be managed conservatively. But, major clefts requires prompt surgical managemet fur the best result. We report a case of a 1 month old male with laryngotracheoesophageal cleft. This cleft was corrected by thoracotomy and anterior laryngofissure approach.

토들러복 브랜드의 내의(內衣) 패턴 조사(調査) 연구(硏究) (A Study on the Underwear Pattern Sizes among Toddlers' Brands)

  • In order to compare the pattern sizes of toddlers' brands, the apparel size #95 and #100, #110 were surveyed, while upper body lengths, bust circumferences and waists, hip circumferences, shoulder length, sleeve length, neck circumferences of toddlers' underwears were examined. As a result, it was found that upper body lengths, chest circumferences and lower body lengths of toddlers' underwears differed more or by $2.0cm{\sim}3.0cm$ among brands than other sizes. In case of underwear size #95, the average upper body length was 32.8cm; the upper body size of "c" brand was longest or 33.5cm, while that of "e" brand was shortest or 32.0cm. In case of size #80, the average upper body length was 36.0cm; the upper body size of "a" brand was longest or 37.0cm, while that of "e" brand was shortest of 35.0cm. Such findings may be attributable to the fact that the main customers of "a" and "b" brands whose pattern sizes are larger are middle or lower class people who tend to buy larger toddlers' apparels than their children's actual body sizes. In contrast, "e" brand seems to target the upper class who prefers the apparels almost fitting their children's actual body sizes.

Comparison of Upper and Lower Cervical Muscle Strengths Between Subjects With and Without Forward Head Posture

  • Background: Forward head posture (FHP) is common postural malalignment. FHP is described relatively extension to upper cervical and lower cervical is relatively flexion. Although several researchers mentioned the lower cervical flexion posture in FHP, most of the studies related to FHP is focused on the deep cervical flexor function. Objects: The purposes of present study is to compare the cervical strength (upper cervical extension [UCE], lower cervical extension [LCE], upper cervical flexion [UCF], lower cervical flexion [LCF]) between individuals with and without FHP. Methods: Fifty-one participants are recruited. Participants who have the craniovertebral angle (CVA) less than 48 degree were classified to the FHP group (n = 24) and the others were included in without FHP group (n = 27). The cervical strength (UCE, LCE, UCF, LCF) were measured using Smart KEMA strength sensor and the strength data was normalized by body weight. All strength measurement conducted at head and neck neutral position in sitting. Independent t-test was used to compare the cervical strength between individuals with and without FHP. Results: The mean value of CVA was greater in without FHP group than with FHP group (p < 0.000). The strength value of UCF (p < 0.002) and LCE (p < 0.001) was significant less in FHP group than without FHP group. But no significant differences were seen in the LCF and UCE strength between two groups. Conclusion: UCF and LCE weakness in FHP group should be considered to evaluate and manage the individuals with FHP.
