Lymphatic vessel mapping in the upper extremities of a healthy Korean population

  • Background Intraoperative indocyanine green (ICG) lymphography can effectively detect functioning lymph vessels in edematous limbs. However, it is sometimes difficult to clearly identify their course in later-stage edematous limbs. For this reason, many surgeons rely on experience when they decide where to make the skin incision to locate the lymphatic vessels. The purpose of this study was to elucidate lymphatic vessel flow patterns in healthy upper extremities in a Korean population and to use these findings as a reference for lymphedema treatment. Methods ICG fluorescence lymphography was performed by injecting 1 mL of ICG into the second web space of the hand. After 4 hours, fluorescence images of lymphatic vessels were obtained with a near-infrared camera, and the lymphatic vessels were marked. Three landmarks were designated: the radial styloid process, the mid-portion of the cubital fossa, and the lower border of the deltopectoral groove. A straight line connecting the points was drawn, and the distance between the connected lines and the marked lymphatic vessels was measured at 8 points. Results There were 30 healthy upper extremities (15 right and 15 left). The average course of the main lymph vessels passed $26.0{\pm}11.6mm$ dorsal to the styloid process, $5.7{\pm}40.7mm$ medial to the mid-cubital fossa, and $31.3{\pm}26.1mm$ medial to the three-quarters point of the upper landmark line. Conclusions The main functioning lymphatic vessel follows the course of the cephalic vein at the forearm level, crosses the mid-cubital point, and travels medially toward the mid-axilla.

완전 몰입형 가상현실 기기를 이용한 작업치료 중재가 만성 뇌졸중 환자의 상지기능에 미치는 영향: 사례연구 (The Effects of Occupational Therapy Intervention Using Fully Immersive Virtual Reality Device on Upper Extremity Function of Patients With Chronic Stoke: Case Study)

  • 목적 : 본 연구는 만성 뇌졸중 환자를 대상으로 완전 몰입형 가상현실 기기를 이용한 작업치료 중재가 환자의 상지기능 향상에 미치는 효과를 알아보고자 하였다. 연구방법 : 본 연구의 연구 대상은 좌측 편마비를 가지고 있는 만성 뇌졸중 환자 1명이며, 개별사례 실험연구 중 ABA 설계를 사용하였다. 기초선 기간 4회, 중재 기간 12회, 기초선 회귀기간 4회로 총 20회의 실험을 휴지기간을 포함하여 총 10주간 진행하였다. 독립변수인 완전 몰입형 가상현실 기기를 이용한 작업치료 중재는 매 회기 30분씩 시행되었으며 종속변수 중 Fugl-Meyer Assessment(FMA) 평가는 각 기간 마다 1회씩 측정하였다. Box and Block Test(BBT) 평가와 Wolf Motor Function Test(WMFT) 평가는 매 회기 마다 시행하여 그 측정 결과를 꺾은선 그래프를 통해 제시하였다. 결과 : 완전 몰입형 가상현실 기기를 이용한 작업치료 중재 후 대상자의 환측 상지기능이 향상되었다. 기초선 회귀 기간에는 중재를 제거한 뒤 치료유지 효과를 확인하였지만 큰 변화는 나타나지 않았다. 결론 : 본 연구의 결과, 완전 몰입형 가상현실 기기를 이용한 작업치료 중재가 만성 뇌졸중 환자의 상지기능에 효과적인 중재라는 것을 알 수 있었다. 다만 치료유지 효과는 크지 않았기 때문에 보다 사용이 간편한 가정용 중재 프로그램 등의 개발이 필요하다.

자기효능증진전략을 이용한 건측억제유도운동이 편마비 환자의 상지기능에 미치는 효과 (Effects of Constraint-Induced Movement Using Self-Efficacy Enhancing Strategies on the Upper Extremity Function of Chronic Hemiplegic Patients)

  • Purpose: The purpose of this study was to investigate the effects of constraint-induced (CI) movement using self-efficacy on U/E function of chronic hemiplegic patients. CI movement discourages the use of the unaffected U/E, combined with intensive training of the affected U/E. Method: A non-equivalent pretest-posttest design was used. Study subjects were 40 hemiplegic patients conveniently selected from 2 different community health centers. The experimental subjects participated in the CI movement program for 6 hours daily over a period of two weeks. The exercises for affected U/E consisted of warming up, main exercise and ADL practice. To encourage the participants' behaviors self-efficacy enhancing strategies were used, which included performance accomplishment, vicarious experience, verbal persuasion and emotional arousal. Result: After 2 weeks of treatment, the grip power, pinch power, wrist flexion/extension, elbow flexion, and shoulder flexion/extension were significantly higher in the experimental subjects than in the control subjects. However, there was no significant difference in hand functions of the two groups. Conclusion: The above results show that the constraint-induced movement using self-efficacy could be an effective nursing intervention for improving U/E function of chronic hemiplegic patients. Long term studies are needed to determine the lasting effects of constraint-induced movement.

Performing Ultrasound-Guided Pharmacopuncture and Acupotomy for Nerve Entrapment in the Upper Extremity: A Guide for Teaching Procedural Skills

  • The use of ultrasound (US)-guided interventions has rapidly increased in Korean medicine (KM) to ensure the safety and accuracy of invasive procedures, such as pharmacopuncture and acupotomy. Although hands-on training is important for the acquisition of skills, it requires considerable time and cost. A detailed guide on the procedure and treatment regions is needed to ensure hygiene and safety during US-guided procedures in KM practice. In this study, we present the overall procedure, target structures, and treatment approaches of US-guided pharmacopuncture and acupotomy for nerve entrapment in the upper extremities of the cubital and radial tunnel, posterior interosseous nerve, carpal tunnel, and Guyon's canal syndrome. We believe that the findings of our study will serve as a foundation for future clinical research, practice, and education on US-guided KM procedures. Further research involving US-guided interventions should specify target structures in three-dimension to delineate the treatment areas.

Benzodiazepine withdrawal syndrome에 의한 Status Epilepticus 1례 (Status Epilepticus as a Benzodiazepine withdrawal syndrome)

  • A 57-year-old man was transferred to our emergency department with decreased mental status after organophosphate intoxication. He had a four year history of benzodiazepine and hypnotic medication use for chronic insomnia and a depressive mood disorder. He had no previous history of seizures, diabetes mellitus, and hypertension. By hospital day 5, the patient was noted to be awake and to have repetitive jerking movements involving the left upper extremity, and appeared apathetic, depressed and less responsive to external stimuli. A benzodiazepine withdrawal syndrome was subsequently apparent when he developed several generalized tonic clonic seizures and status epilepticus. Using a continuous midazolam intravenous infusion, we successfully controlled the refractory seizure without complications. We present a rare case of status epilepticus from a benzodiazepine withdrawal that developed during the treatment for organophosphate intoxication.

자동차 조립 작업자들에서 상지 근골격계의 인간공학적 작업평가(Rapid Upper Limb Assessment) 결과와 자각증상과의 연관성 (The Relation Between Work-Related Musculoskeletal Symptoms and Rapid Upper Limb Assessment(RULA) among Vehicle Assembly Workers)

  • 본 연구는 Rapid Upper Limb Assessment(RULA)를 이용하여 자동차 조립작 업에 대한 인간공학적 작업평가를 하고, 이 결과와 작업관련 상지 근골격계 질환의 자각증상율 및 작업특성 변수들의 연관성을 분석함으로써, RULA결과가 관련 신체부위의 통증이나 불편함으로 보고되는 근골격계 부하의 좋은 지표가 될 수 있는지, RULA평가체계를 이용하여 작업 위험도가 적합한지 여부를 평가할 수 있는가를 보고자 하였다. 자동차 제조업 작업자 314명을 대상으로 근골격계질환 자각증상설문과 RULA를 이용한 작업평가를 하였으며, 다음과 같은 결과를 얻었다. 1. NIOSH의 작업관련 근골격계 질환의 감시기준(surveillence criteria)에 따라 자각증상을 조사한 결과, 연구대상자 중 목 32.8%, 어깨 26.4%, 팔 10.5%, 손 29.3%, 허리 41.4%였으며, 한가지 이상 의 증상을 보이는 경우는 62.4%의 분포를 보였다. 2. 품질관리부에 비해 조립부서 작업자 틀이 전체 증상과 어깨, 허리, 팔 및 손부 위 증상에서 더 높은 유증상률을 보였다(p<0.05). 3. 부서별 RULA를 이용한 작업평가 결과, 서브/조립반과 의장반의 공정이 높은 위험점수를 보였다. 전체 조립부서에 대한 평가결과 적합한 작업은 3.02%였고, 62.8%가 부적합한 작업으로 작업전환이나 중재조치가 필요한 경우였다. 자각증상과 RULA를 이용한 작업평가 결과를 비교한 결과 RULA점수가 클수록 자각증상율이 높음을 보였다.(p<0.05) 4. RULA평가지표와 자각증상사이의 관련성을 보기 위하여, RULA지표와 기타 작업관련요인들을 독립변수로, 자각증상유무를 종속변수로 하여 다중 로지스틱 회귀분석을 실시하였다. 최종적으로 증상유무를 가장 잘 설명하는 회귀모형에 대한 분석결과는 다음과 같다. 모든 증상군에 대해서는 반복작업(OR 2.183), 진동공구 사용(OR 2.775)과 총 위험도 점수(OR 2.250); 목 부위 증상군에서는 상완의 자세점수(OR 1.786), 상완 및 손부 위의 총점수(OR 1.634) ; 어깨 증상군은 상지의 근사용(OR 3.076), 상완 및 손부 위의 총점수(OR 1.798); 어깨와 목 부위를 합한 증상군은 상완 및 손부위의 총점 수(OR 1.715)와 상지의 근사용 점수(OR 2.057); 팔 증상군에서는 상지의 근사용 점수(OR 10.662) ; 손 부위 증상군에 대해서는 손목의 자세/손목 비틀림 지표 (OR 2.068)와 상지의 근사용 점수(OR 2.215); 허리부위 증상군에서는 하지의 근사용 점수(OR 2.601)가 통계적으로 유의한 지표였다.(p<0.05) 이상의 결과에서 작업관련 상지근골격계 자각증상과 RULA 점수사이에는 연관성이 있음이 관찰되었다. 이는 RULA가 직업성 질환을 유발할 수 있는 근골격계 부하에 폭로된 작업자들을 평가하는, 일차적인 작업 위험도 평가도구(Screening tool)로서 쓰일 수 있음을 말해준다. 다만 향후 RULA 사용에 있어서 상지(upper limb)와 허리부위(back)의 평가를 구분하고, 기타 다른 작업관련 요인들에 대한 평가체계를 보완하는 것이 필요하다.

Clinical outcomes of a low-cost single-channel myoelectric-interface three-dimensional hand prosthesis

  • Background Prosthetic hands with a myoelectric interface have recently received interest within the broader category of hand prostheses, but their high cost is a major barrier to use. Modern three-dimensional (3D) printing technology has enabled more widespread development and cost-effectiveness in the field of prostheses. The objective of the present study was to evaluate the clinical impact of a low-cost 3D-printed myoelectric-interface prosthetic hand on patients' daily life. Methods A prospective review of all upper-arm transradial amputation amputees who used 3D-printed myoelectric interface prostheses (Mark V) between January 2016 and August 2017 was conducted. The functional outcomes of prosthesis usage over a 3-month follow-up period were measured using a validated method (Orthotics Prosthetics User Survey-Upper Extremity Functional Status [OPUS-UEFS]). In addition, the correlation between the length of the amputated radius and changes in OPUS-UEFS scores was analyzed. Results Ten patients were included in the study. After use of the 3D-printed myoelectric single electromyography channel prosthesis for 3 months, the average OPUS-UEFS score significantly increased from 45.50 to 60.10. The Spearman correlation coefficient (r) of the correlation between radius length and OPUS-UEFS at the 3rd month of prosthetic use was 0.815. Conclusions This low-cost 3D-printed myoelectric-interface prosthetic hand with a single reliable myoelectrical signal shows the potential to positively impact amputees' quality of life through daily usage. The emergence of a low-cost 3D-printed myoelectric prosthesis could lead to new market trends, with such a device gaining popularity via reduced production costs and increased market demand.

Changes in muscle activity of the abdominal muscles according to exercise method and speed during dead bug exercise

  • Objective: There are many types of exercises with upper and lower-limb action for activation of abdominal muscles for trunk stabilization. A comparison of the different exercise methods been very useful to enhance the result from the exercise for treatment. The purpose of this study was to investigate through surface electromyography (EMG) the changes in abdominal muscle activity during the performance of three different dead-bug exercise methods performed at three different speeds. Design: Cross-sectional study. Methods: The subjects were 30 healthy adults (13 males and 17 females). We instructed the subjects to perform three different dead-bug exercises. We also applied three different speeds to the dead-bug exercises; 60 bpm, 90 bpm, and 120 bpm with use of a metronome. The assessment of EMG was percentage of maximal voluntary isometric contraction on the rectus abdominis (RA), external oblique (EO), and internal oblique (IO). Results: EMG activation of the RA, EO and IO muscles was significantly greater at the higher speed (p<0.05). There was a significant increase in abdominal muscle activity during the dead-bug exercise performed with both the upper and lower extremities compared to that with only the upper extremity or the lower extremities (p<0.05). Conclusions: These findings demonstrate that the performance of the dead-bug exercise with both the upper and lower extremities combined at a high speed is more effective compared to other exercises. Therefore, it is suggested that more favourable and effective outcomes may occur when the type and speed of the exercise is chosen appropriately.

만성 뇌졸중 환자의 대칭적, 비대칭적 양측성 상지훈련의 상지기능회복 효과 (The Effect of Symmetrical and Asymmetric Bilateral Training for Chronic Stroke Patients in Upper Extremity Recovery)

  • 목적 : 양측성 상지 활동의 대칭적, 비대칭적 양측성 상지훈련이 상지기능회복에 미치는 영향을 알아보고자 하였다. 연구방법 : 원주에 소재한 ${\bigcirc}{\bigcirc}$병원의 연구 뇌졸중 환자 15명을 무작위로 대칭적 움직임훈련 그룹과 비대칭적 움직임 훈련 그룹으로 나누었다. 중재는 하루 30분, 주5회, 4주 동안, 총 20회기를 받았다. 결과 : 양측 상지의 실제 사용량은 대칭적 움직임 훈련은 건측, 환측 모두 유의한 변화를 보였으며, 비대칭 움직임 훈련도 환측에서 유의한 변화를 보였다. 또한 환측에서 그룹 간 유의한 변화차이가 있었다. 두 훈련 그룹 모두 수행도와 만족도, 상지기능 변화는 그룹 내 유의미한 변화가 있었지만 그룹 간 차이는 없었다. 결론 : 대칭적 움직임 훈련은 비대칭 움직임 훈련보다 대칭적 움직임은 비대칭적 움직임 보다 운동수행에서의 향상을 보였지만, 보다 명확한 차이를 얻기 위해서는 fMRI와 같은 뇌신경학적 평가도구를 사용하는 것이 필요할 것이라 판단되며, 보다 명확한 훈련 프로토콜이 필요하고, 보다 많은 뇌졸중 환자를 대상으로 한 후속연구의 필요성이 제시된다.

Effects of Differential Stability on Control of Multi-Joint Coordination in the Upper Extremity: A Torque Component Analysis

  • Purpose: The purpose of the present current study was to examine control of upper limb multi-joint movements with differential coordination stability. To achieve the goals of the study, torque analyses were utilized to answer questions about how torque components were differed among various elbow-wrist coordination patterns. Methods: Eight self-reported right-handed college students (3 males and 5 females, mean age=20.6 yr) were volunteered. The task required participants to rhythmically coordinate the flexion-extension motions of their elbow and wrist with coordination relationship of $0^{\circ}$, $90^{\circ}$, and $180^{\circ}$relative phases between the two joints. Mean relative phase and phase stability (standard deviation of relative phase) were computed to for analysisze of overall coordination performance. To determine the figure out characteristics of torque components in elbow and wrist joints, impulse values of muscle torque (MT) and interactive torque (IT) and MT as a percentage of cycle duration (MT-PCD) were analyzed. Results: Torque results showed that the proximal elbow joint generated motions with mainly muscle efforts regardless of coordination patterns, while the distal wrist joint adjusted the coordination patterns by changing amount of MT. Impulse analyses showed that the least stable $90^{\circ}$ pattern was performed by utilizing a similar coordination strategy of the most stable $0^{\circ}$ pattern. Conclusion: The present current study suggests that the roles of distal and proximal joints differ in order to achieve various multi-joint coordination movements. This study provides information for use in gives an idea to development of rehabilitation or training programs for to persons with an impaired upper limb motor ability.
