Effect of upper thoracic mobilization on cervical alignment in stroke patients with forward head posture: A case study

  • The purpose of this study was to identify the effect of upper thoracic mobilization on cervical alignment in stroke patients with forward head posture. The subject's of this study were two stroke patients with forward head posture and a cervical curve angle (four-line Cobb's method; FLCM) less than $40^{\circ}$. The intervention, central posterior-anterior (PA) mobilization, was applied to the T1-T4 vertebrae (upper thoracic spine) following the Maitland concept. This mobilization was applied three times per week for four weeks. In the results, the cervical curve angle (FLCM) increased for both subject 1 and 2. However, Jochumsen method score was decreased in subject 1, while it was increased in subject 2. These results demonstrate that upper thoracic mobilization had the positive effect on the cervical curve angle but not on Jochumsen method score. These findings suggest that PA mobilization on the upper thoracic spine could correct cervical curve angle measured by FLMC in stroke patients with FHP.

Effects of Manual Lymphatic Drainage and High Frequency Diathermy on Pain, Volume, Function of Upper Extremity and Quality of Life in Breast Cancer Patients with Axillary Web Syndrome : a Study of Five Case Reports (겨드랑이막증후군을 가진 유방암 환자들의 도수림프배출법과 고주파 투열치료가 통증, 부피, 기능 및 삶의 질에 미치는 효과 : 사례군 연구)

  • Purpose : The purpose of this study was to apply manual lymphatic drainage (MLD) and high-frequency diathermy (HFD) to patients with axillary web syndrome (AWS), one of the side effects of breast cancer surgery, and to treat upper extremity pain, volume, function of the upper extremity, (joint range of motion; ROM, disabilities of the arm, shoulder and hand; DASH) and quality of life before and after treatment. It is to determine the effect of treatment by checking the level change. Methods : This study is a case series. A total of 5 patients diagnosed with AWS after breast cancer surgery voluntarily participated in this study. The intervention program consisted of stretching, MLD and HFD. It was conducted 3 times a week for 30 minutes for 4 weeks. In order to compare the effects of pain (numeric pain rating scale; NPRS), volume, upper limb function (ROM, DASH) and quality of life (the European organization for research and treatment of cancer quality of life questionnaire-breast, EORTC QLQ-BR23) evaluations were compared before and after 4 weeks of intervention. All measured variables were analyzed and expressed as mean, standard deviation and percentage. Results : The shoulder NPRS level of the subjects in all case groups decreased, the volume decreased and the shoulder flexion, abduction ROM increased. It showed improvement in DASH and quality of life, QLQ-BR23. Conclusion : After breast cancer surgery, we confirmed the possibility that MLD and HFD treatments could be effective in improving pain, decreasing volume, increasing upper extremity function, and quality of life for patients who have difficulties with AWS. The possibility has been confirmed, and additional research is needed by increasing the number of participants in the experiment in the future.

Effects of Covering Parts of Body with Garments on Human Thermoregulation and Sensation (신체의 부위별 피복이 체온조절 및 주곤적인 감각에 미치는 영향(I))

  • The physiological significances of the upper and lower body on thermoregulation and sensation were studied in this paper. Experiments were carried out on 4 females in a climatic chamber conditioned at 1) $25^{\circ}C\rightarrow35^{\circ}C\rightarrow25^{\circ}C$, 2) $25^{\circ}C\rightarrow15^{\circ}C\rightarrow25^{\circ}C$, both with 50% R.H., covering the upper body (U) or lower body (L) with garments. 1. When the upper or lower body is covered or exposured respectively, the mean skin tempterature of upper body is higher than that of lower body. And upper body is more easily influenced by the environmental temperature than lower body. It means the skin temperatures of the upper body change faster than those of the lower body following the environmental changes. 2. In U and L, the skin temperatures of the upper limbs (thighs, upper arms) are lower than those of the peripherals (hands, feet). 3. Warm sensations and skin temperatures of the upper body showed high correlation and it was the case with cold sensations and skin temperatures of the lower body. 4. In high temperature condition $(25^{\circ}C\rightarrow35^{\circ}C\rightarrow25^{\circ}C)$, mean skill temperature and rectal temperature in L were lower than in U. This lower rectal temperature in L is probably due to the insulation of the lower body with garments that promotes the heat radiation only in the high temperature environment.

Improvement of Upper Extremity Function and Leisure Satisfaction of Children with Brain Lesions through Sports Stacking Activities: A Case Study

  • Purpose: This study aimed to investigate changes in upper extremity joints and leisure satisfaction in children with brain lesions through sports stacking activities. Methods: A sports stacking program was conducted on three children with brain lesions who had upper extremity joint limitations and joint range of motion lower than the normal range. It was conducted 10 times, 1 to 2 times a week, 40 minutes each time. Upper extremity joint angles were measured using a goniometer in the order of shoulder, elbow, wrist, and fingers, and leisure satisfaction was measured using a smile evaluation. Results: As a result of measuring the upper extremity joint angles, all three children showed slight angle changes in the shoulder, elbow, and wrist areas. Differences in joint angles appeared differently for each child. Smile evaluation results were evaluated in various psychological, educational, and physical aspects. Only child A was evaluated for Smile Evaluation No. 1. Conclusion: Sports stacking activities changed the upper extremity function of children with brain lesion disorders and showed differences in psychological, physical, and educational aspects of leisure satisfaction. As this is a short-term study result, the change in upper extremity function is minimal, but if sports stacking activities are continued, it will be a rehabilitation program that can prevent upper extremity dysfunction and improve physical strength. Accordingly, continuous attention should be paid to increasing accessibility and enjoyment of daily life according to individual characteristics and level.

The Effects of Hold-Relax and Active Stretching on Recoveries of Muscle Fatigue after Computer Work (컴퓨터 작업 후 유지-이완운동과 능동신장이 근피로 회복에 미치는 영향)

  • Purpose : The purpose of this study was to identify that the effect of hold-relax and active stretching on recoveries of upper trapezius and sternocleidomastoid muscle fatigue after computer work. Methods : This study was conducted by 30 volunteer at the age of 23~33(resting group=10, hold-relax group=10, active stretching group=10). The muscle fatigue measured by tracing the median frequency(MDF) decrement from power spectrum analyses of EMG signals and the data were analyzed repeated ANOVA. Result : In case of upper trapezius, there was significant difference in group I,II and group II, III. In case of sternocleidomastoid, there was significant difference in group I,II and group I,III. Conclusion : The active stretching was effective for sternocleidomastoid and the hold-relax was effective for upper trapezius and sternocleidomastoid. The hold-relax was effective better than active stretching on recoveries of muscle fatigue.

  • The incidence of contrast media extravasation has been increasing gradually. Most contrast media extravasations only can cause pain, swelling and erythema. However, in more severe cases, skin necrosis, ulceration, or compartment syndrome may occur, often necessitating a surgery. Early diagnosis and treatment should be established on the spot in order to avoid such a severe damage. We present a case of 42-year-old woman with a compartment syndrome in the right upper extremity caused by extravasation of computed tomography contrast media.

  • This study presents a two-level inventory distribution system with one-upper level facility(warehouse) in first echelon and n-lower level facilities(stores) in the second echelon. The demand process at the upper level is induced by the aggregated backorder processes of n independently operated lower level facilities in parallel. For the upper level and the lower level we find the decision points and the distribution function of the aggregate backorders from n-stores. Optimal allocation units and expected system profits are obtained. If the product is of great enough importance to the customer, echelon structure will include a shadow installation. Also, this case is analyzed. Numerical examples are presented to illustrate the results for each case.

  • It is very important that the ratio of Soft Lean Mass and Body Fat Mass in body composition. Generally we make an effort not only weight reduction but body fat mass reduction, and finally percent body fat is reduced. And we make an effort to reduce of partial obesity. I have one case that improvement of imbalance on the upper and lower part of the body by Oriental Medicine. We have the result that the upper part of the body composition is increased and the lower part of the body composition is decreased.

  • In this paper, we consider the robust pole assignment and the upper bound of quadratic cost function for the linear systems with time-varying uncertainy. The considered uncertainties are both the norm bounded unstructured case and the structured case that has the matrix polytope type uncertain structure. We derve conditions that guarantee the robust pole assignment inside a disk in the L.H.P. and the robust stability. Also, we derive the upper bound of quadratic cost for thil pole assigned systems. Finally, we show the usefulness of our results by an example.

  • Bleomycin is known as a antibiotic agent for malignant tumors especially sguamous cell carcinoma. We have treated 2 cases of malignant tumors, each one esophageal and lung carcinoma with bleomycin, 630 mg and 510 mg respectively. In case of esophageal carcinoma. the subjective symptoms such as dysphagea and swallowing disturbance are temporally relieved, but the irregular filling defect is not significantly changed in esophagogram. In case of lung carcinoma, the atelectasis of right upper lobe on chest X-ray was slightly regressed in its size without effective improvement of subjective symptoms. However, following additional radioactive $^60{Co}$ irradiation therapy (5200r), marked regression of tumor density and aeration of right upper lobe was observed. But 2 weeks later of ceasatioil of irradiatio:J, atelectasis of right upper lobe was again developed. Fever, anorexia, headache and eruption were developed during the treatmeat with bleomycin in both cases but the sign or symptoms of hone marrow depression, renal or liver damage were not noted.
