clinical evaluation of spontaneous pneumothorax -A review of 186 cases- (자연기흉의 임상적 고찰)

  • In this study, 186 cases of spontaneous pneumothorax experienced at the Dept. of thoracic and Cardiovascular surgery, chosun University Hospital during from Jan. 1978 to Mar. 1987 were analysed retrospectically. The results were as follows: 1. The sex ratio of the stadied patient was 2.5:! in male prominence and the incidence and the incidence was highest in the adolescence between 20-29 year of age. 2. The clinical symptoms were frequently Dyspnea (52.2%) and Chest pain (38.7%). The etiologic factors were as follows, Tuberculous origin in 55.9%, Unknown origin in 17.2%, Bullae in 11.8%. 3. In the method of treatment, closed thoracotomy was the most effective procedure in achieving the expansion of collapsed lung in 87.1%. Other methods were bed rest with high oxygen inhalation, thoracentesis and open thoracotomy. 4. The incidences of complication were developed in 12.4% and recurrent rates were seen in 9.7%.

  • Objectives : We report an unusual case of a 57-year old woman with spasticity following fever of unknown origin (the origin of her fever was not certain by western medical diagnosis). Malignant hyperthemia with spasticity couldn't be cured by general western medical therapy, and furthermore the function of liver and renal system was worsened as a consequence of drugs. Methods : We diagnosed the watery state of the patient as seupon (습온) with gyeonggwol (경궐) through pattern identification (변증) of symptoms and signs. Results : This patient who had spasticity with malignant hyperthermia was treated by optical management of herb medicine and acupuncture, and should need long-term observation. Conclusions : We want to show that trial by febrile diseases can solve fever and spasticity.

Infectious Myositis of the Jaw Presenting as Trismus of Unknown Origin

  • Infectious myositis, an infection of the skeletal muscles, is a rare condition and potentially life-threatening if not detected and treated in the early stages. This clinical entity may arise from various pathogens, such as bacteria, fungi, parasites, and viruses. A propagation of contiguous infection, penetrating trauma, vascular insufficiency, or hematogenous spreading of microorganisms can cause infectious myositis. Though several cases have been reported in large muscle groups in the lower extremities, there are only a few reports on infectious myositis of the masticatory muscles. We report three cases of infectious myositis presenting jaw pain and trismus. Unlike a common head and neck infection caused by the spreading of odontogenic origin, the early diagnosis of infectious myositis was difficult because no specific lesion suspected to be the infection source was observed in the physical examination and the plain radiographs. Advanced imaging modalities such as computed tomography and magnetic resonance imaging, and laboratory evaluation is useful for the early detection of infectious myositis.

Approximation-Based Decentralized Adaptive Output-Feedback Control for Nonlinear Interconnected Time-Delay Systems (비선형 상호 연결된 시간 지연 시스템을 위한 함수 예측 기법에 기반한 분산 적응 출력 궤환 제어)

  • This paper proposes a decentralized adaptive output-feedback controller design for nonlinear interconnected systems with unknown time delays. The interaction terms with unknown delays are related to all states of subsystems. The time-delayed functions are compensated by using appropriate Lyapunov-Krasovskii functionals and function approximation technique. The observer dynamic surface design technique is employed to design the proposed memoryless local controller for each subsystem. In addition, we prove that all signals in the closed-loop system are semiglobally uniformly bounded and control errors converge to an adjustable neighborhood of the origin.

  • Shapiro and Wilk(1972) developed a test for exponentiality with origin and scale unknown. The procedure consists of comparing the generalized least squares estimate of scale with the estimate of scale given by the sample variance. However the test based on the statistic is inconsistent Kim(2001a) proposed a modified Shapiro-Wilk's test statistic using the ratio of two asymptotically efficient estimators of scale. In this paper, we study the consistency of the proposed test.

  • Progressive systemic sclerosis is a disease of unknown origin and uncommon in the east area. We experienced one case of severe peptic esophageal stricture due to esophageal scleroderma. The patient`s life quality was improved with the esophageal reconstructive procedure using reversed tubed gastric pedicle and his body weight was normalized with regular diet.

Poland`s Syndrome (Poland 증후군 1례 보고)

  • The Poland`s syndrome is very rare anomaly, which consists of congenital unilateral absence of the sternocostal part of the pectoralis major muscle, with ipsilateral hand deformities. The clinical features are variable but all patients have absence of at least the sternal head of the pectoralis major muscle. The syndrome is not hereditary and is of unknown origin. Early recognition of Poland`s syndrome may give the provision of psychologic and genetic counseling for anxious parents. We have encountered two patients with this entity, and one of them underwent successful surgical correction.

In this paper, we study the asymptotic behavior of the statistic using the approximation of the quantile process by a sequence of Brownian bridges and represent the limit null distribution as an integral of a Brownian bridge.
