편측성 II급 1류 부정교합의 교합특성에 관한 연구 (A STUDY OF THE ETIOLOGY OF UNILATERAL CLASS II, DIVISION 1 MALOCCLUSION)

  • The purpose of this study was to verify the class I molar relationship in skeletal class II and class II molar relationship in skeletal clan I malocclusion with unilateral class II, division 1 malocclusion. The sample consisted of lateral cephalometric radiographs and upper and lower dental casts of 30 unilateral class II, division 1 malocclusion. The results of this study were as follows: 1. Skeletal class I malocclusion was $43\%$, and skeletal class II malocclusion was $57\%$ in 30 cases of unilateral class II, division 1 malocclusion. 2. In the skeletal class II with unilateral class II, division 1 malocclusion, mandibular first molar on the class I side showed more mesial migration than the opposite side. 3. In the skeletal class I with unilateral class II, division 1 malocclusion, maxillary first molar on the class II side showed more mesial migration than the opposite side. 4. Midline deviation of upper or lower dental arch was $90\%$ in 30 cases of unilateral class II, division 1 malocclusion.

교정치료를 위한 발치빈도에 관한 연구 (A STUDY ON THE FREQUENCY OF TOOTH EXTRACT10N FOR ORTHODONTIC TREATMENT)

  • To study diagnosis and treatment plan of malocclusion, comparison of race and nation and consideration of treatment technique, 885 orthodontic patients of the department of orthodontics, Infirmary of dental college, Chosun University were evaluated. The frequency and pattern of tooth extraction were studied according to malocclusion types and developmental stages of occlusion and sex. Thefollowingresultswereobtained. 1. Average frequency of tooth extraction for orthodontic treatment was $38.3\%$. 2. The frequency of tooth extraction was $43.7\%$ in class I malocclusion, $34.2\%$ in class II malocclusion and $32.1\%$ in class III malocclusion. 3. The drequency of tooth extraction was the highest in permanent dentition $(49.2\%)$. 4. The frequency of tooth extraction was not different between male and female. 5. The frequency of 3-4 premolars extraction was the highest $(57.5\%)$ and that of lower incisor extraction was the lowest $(2.9\%)$ in patterns of tooth extraction. 6. Both frequency of unilateral and single arch extraction were the highest in class III malocclusion.

부정교합 치료기간의 예측에 관한 연구 (A STUDY HO THE PREDICTION OF DURATION OF ORTHODONTIC TREATMENT IN MALOCCLUSION)

  • To predict duration of orthodontic treatment of malocclusion, 304 male and female patients whose orthodontic treatment was finished in department of orthodontics, Infirmary of dental college, Chosun University were studied. The duration of treatment was studied according to types of malocclusion, beginning ages of treatment, Hellman's dental ages, the determination of tooth extraction, types of tooth extraction and types of orthodontic appliance. The following results were obtained. 1. The duration of treatment was 18 months in Class I malocclusion, 20 months in Class II malocclusion and 24 months in Class III malocclusion. 2. The more early treatment was done, the more duration of treatment was needed according to beginning age of treatment and Hellman's dental ages. 3. Treatment of tooth extraction case was needed for 78 days more than that of non-extraction case. 4. The duration of treatment with unilateral extraction of premolars was the shortest (598 days) and that with single arch extraction of premolars was the longest (685 days) according to types of tooth extraction. 5. The duration of treatment by removable appliance was the shortest (237 days) and the combination of removable appliance, headgear and full banded appliance was the longest (1425 days) according to types of orthodontic appliance. 6. The duration of treatment was 18 months in typical orthodontic treatment with 4 extraction of premolar and full banded appliance.

Three-dimensional finite element analysis of unilateral mastication in malocclusion cases using cone-beam computed tomography and a motion capture system

  • Purpose: Stress distribution and mandible distortion during lateral movements are known to be closely linked to bruxism, dental implant placement, and temporomandibular joint disorder. The present study was performed to determine stress distribution and distortion patterns of the mandible during lateral movements in Class I, II, and III relationships. Methods: Five Korean volunteers (one normal, two Class II, and two Class III occlusion cases) were selected. Finite element (FE) modeling was performed using information from cone-beam computed tomographic (CBCT) scans of the subjects' skulls, scanned images of dental casts, and incisor movement captured by an optical motion-capture system. Results: In the Class I and II cases, maximum stress load occurred at the condyle of the balancing side, but, in the Class III cases, the maximum stress was loaded on the condyle of the working side. Maximum distortion was observed on the menton at the midline in every case, regardless of loading force. The distortion was greatest in Class III cases and smallest in Class II cases. Conclusions: The stress distribution along and accompanying distortion of a mandible seems to be affected by the anteroposterior position of the mandible. Additionally, 3-D modeling of the craniofacial skeleton using CBCT and an optical laser scanner and reproduction of mandibular movement by way of the optical motion-capture technique used in this study are reliable techniques for investigating the masticatory system.

Unilateral maxillary central incisor root resorption after orthodontic treatment for Angle Class II, division 1 malocclusion with significant maxillary midline deviation: A possible correlation with root proximity to the incisive canal

  • Root resorption can be caused by several factors, including contact with the cortical bone. Here we report a case involving a 21-year-old female with Angle Class II, division 1 malocclusion who exhibited significant root resorption in the maxillary right central incisor after orthodontic treatment. The patient presented with significant left-sided deviation of the maxillary incisors due to lingual dislocation of the left lateral incisor and a Class II molar relationship. Cephalometric analysis demonstrated a Class I skeletal relationship (A point-nasion-B point, 2.5°) and proclined maxillary anterior teeth (upper incisor to sella-nasion plane angle, 113.4°). The primary treatment objectives were the achievement of stable occlusion with midline agreement between the maxillary and mandibular dentitions and appropriate maxillary anterior tooth axes and molar relationship. A panoramic radiograph obtained after active treatment showed significant root resorption in the maxillary right central incisor; therefore, we performed cone-beam computed tomography, which confirmed root resorption along the cortical bone around the incisive canal. The findings from this case, where different degrees of root resorption were observed despite comparable degrees of orthodontic movement in the bilateral maxillary central incisors, suggest that the incisive canal could be an inducing factor for root resorption. However, further investigation is necessary to confirm this assumption.

Asymmetric Head-Gear의 견인력의 차이에 따른 상악 제 1 대구치에 나타나는 힘과 변위 및 초기 응력분포에 관한 유한요소법적 비교 연구 (A FEM comparison study about the force, displacement and initial stress distribution on the maxillary first molars by the application of Asymmetric Head-Gears with the different traction forces)

  • 편측성 II급 부정교합의 치료 시 다양한 mechanics가 사용되어지는데 그중 한가지가 asymmetric face-bow를 가진 head gear이다. 이 asymmetric head gear의 편측 효과에 대해 다양한 연구결과를 보고하고 있는데 이에 저자는 편측성 II급 부정교합의 상악 치열 모델링과 임상에서 자주 사용되는 Power arm asymmetric face-bow를 모델링하고, 견인력을 달리하여 유한 요소법을 통해 치근막에서 발생되는 응력의 분포와 그에 따른 각 방향의 반력, 그리고 그로 인한 변위를 관찰하여 역계를 이해하고 치료효과를 예견하고자 실험을 고안하였다. 발치나 교정치료의 경험이 없는 25세 남자 정상 교합자를 대상으로 컴퓨터 단층 촬영을 시행하여 얻은 방사선필름을 기초로 하여 유한 요소 상악 모델과 치주인대 모델을 제작하고 그후 다시 좌측 제 1 대구치가 근심 편위되어있는 편측성 상악 II급 부정교합 모델로 제작하고, RMO 사의 Face-bow (Penta-$^{TM}$/Medium size)를 기본 모델로 0.045 inch 직경의 inner-bow와 0.072 inch 직경의 outer-bow를 가지는 asymmetric face-bow를 우측을 25mm 짧게 하여 모델링한 후, 좌우측 제 1 대구치에 각각, 250g, 300g, 350g 씩 견인력을 부여하여 다음과 같은 결론을 얻었다. 1. 양쪽 제1 대구치가 받는 힘의 총합은 견인력이 증가함에 따라 증가하는 경향을 보였는데 대체로 근심위치된 치아가 정상 위치한 치아보다 더 많은 힘을 받는 것으로 나타났으며, 두치아 모두 윈심으로의 힘을 받으며, 원심으로 이동하는 양상을 보여주었다. 2. 측방력은 두 치아 모두 협측으로의 힘을 받는 것으로 나타났는데, 힘의 성분을 분석하여 보면 견인력이 증가함에 따라 X축으로의 힘이 근심 위치된 치아에서 점점 작아지며, 정상 위치한 치아에서 점점 증가하여 측방력의 성분이 힘을 많이 받는 쪽에서 적게 받는 쪽으로 이동하는 양상을 보여주었다. 3. 원심으로의 이동과 동시에 회전과 경사이동 양상을 보여주었는데 견인력의 증가시 함께 증가하는 양상을 보였고, 힘을 많이 받는 쪽, 즉 근심 위치된 치아가 더 많은 회전과 경사이동을 보였으며 적은 양이나마 정상 위치된 치아에 서도 같은 양상의 변위를 보였다.

Orthodontic treatment in a patient with Moebius syndrome: A case report

  • Moebius syndrome (MBS) is a congenital neurologic disorder that causes cranio-facial abnormalities. It involves paralysis of the VI and VII cranial nerves and causes bilateral or unilateral facial paralysis, eye movement disorder, and deformation of the upper and lower limbs. The orofacial dysfunctions include microstomia, micrognathia, hypotonic mimetic and lip muscles, dental enamel hypoplasia, tongue deformity, open bite or deep overbite, maxillary hypoplasia, high arched palate, mandibular hyperplasia or features indicating mandibular hypoplasia. This case report presents a 7-year-old male patient who was diagnosed with MBS at the age 2 years. The patient displayed typical clinical symptoms and was diagnosed with Class II malocclusion with a large overjet/overbite, tongue deformity and motion limitation, and lip closure incompetency. Treatment was initiated using a removable appliance for left scissor bite correction. After permanent tooth eruption, fixed appliance treatment was performed for correction of the arch width discrepancy and deep overbite. A self-ligation system and wide-width arch form wire were used during the treatment to expand the arch width. After 30 months of phase II treatment, the alignment of the dental arch and stable molar occlusion was achieved. Function and occlusion remained stable with a Class I canine and molar relationship, and a normal overjet/overbite was maintained after 9.4 years of retainer use. In MBS patients, it is important to achieve an accurate early diagnosis, and implement a multidisciplinary treatment approach and long-term retention and follow-up.

Positional changes of the mandibular condyle in unilateral sagittal split ramus osteotomy combined with intraoral vertical ramus osteotomy for asymmetric class III malocclusion

  • Objectives: In the present study, the effects of sagittal split ramus osteotomy (SSRO) combined with intraoral vertical ramus osteotomy (IVRO) for the treatment of asymmetric mandible in class III malocclusion patients were assessed and the postoperative stability of the mandibular condyle and the symptoms of temporomandibular joint disorder (TMD) evaluated. Materials and Methods: A total of 82 patients who underwent orthognathic surgery for the treatment of facial asymmetry or mandibular asymmetry at the Department of Oral and Maxillofacial Surgery, Dong-A University Hospital, from 2016 to 2021 were selected. The patients that underwent SSRO with IVRO were assigned to Group I (n=8) and patients that received bilateral SSRO (BSSRO) to Group II (n=10, simple random sampling). Preoperative and postoperative three-dimensional computed tomography (CT) axial images obtained for each group were superimposed. The condylar position changes and degree of rotation on the superimposed images were measured, and the changes in condyle based on the amount of chin movement for each surgical method were statistically analyzed. Results: Group I showed a greater amount of postoperative chin movement. For the amount of mediolateral condylar displacement on the deviated side, Groups I and II showed an average lateral displacement of 0.07 mm and 1.62 mm, respectively, and statistically significantly correlated with the amount of chin movement (P=0.004). Most of the TMD symptoms in Group I patients who underwent SSRO with IVRO showed improvement. Conclusion: When a large amount of mandibular rotation is required to match the menton to the midline of the face, IVRO on the deviated side is considered a technique to prevent condylar torque. In the present study, worsening of TMD symptoms did not occur after orthognathic surgery in any of the 18 patients.

Occlusal deviations in adolescents with idiopathic and congenital scoliosis

  • Objective: This cross-sectional study aimed to investigate the characteristics of malocclusions in scoliotic patients through clinical examinations. Methods: Fifty-eight patients with idiopathic scoliosis (IS) and 48 patients with congenital scoliosis (CS) participated in the study. A randomly selected group of 152 orthopedically healthy children served as the control group. Standardized orthodontic and orthopedic examination protocols were used to record the occlusal patterns and type of scoliosis. Assessments were made by three experienced orthodontists and a spinal surgery team. The differences in the frequency distribution of occlusal patterns were evaluated by the chi-squared test. Results: In comparison with patients showing IS, patients with CS showed a higher incidence of Cobb angle ≥ 45° (p = 0.020) and included a higher proportion of patients receiving surgical treatments (p < 0.001). The distribution of the Angle Class II subgroup was significantly higher in the IS (p < 0.001) and CS (p = 0.031) groups than in the control group. In comparison with the healthy controls, the CS and IS groups showed significantly higher (p < 0.05) frequencies of asymmetric molar and asymmetric canine relationships, upper and lower middle line deviations, anterior deep overbite, unilateral posterior crossbite, and canted occlusal plane, with the frequencies being especially higher in CS patients and to a lesser extent in IS patients. Conclusions: Patients with scoliosis showed a high frequency of malocclusions, which were most obvious in patients with CS.

최근 11년간 서울대학교병원 교정과에 내원한 순구개열 환자의 내원 현황에 관한 연구(1988.3 - 1999.2) (The study on the cleft lip and/or palate patients who visited Dept. of Orthodontics, Seoul National University Dental Hospital during last 11 years (1988.3-1999.2))

  • 순구개열은 악안면 선천성 기형 중에서 발생율이 가장 높으며, 여러 선학들의 조사연구에 의하면 순구개열의 발생빈도가 증가하는 것으로 알려져 있다. 따라서 순구개열 환자의 치료에서 중요한 부분을 담당하는 교정과의 순구개열환자의 내원동향에 관한 역학조사가 필요하다고 생각된다. 이에 저자들은 서울대학교병원 교정과에 내원한 순구개열 교정환자들의 연도별, 종류별, 성별, 연령별, Angle씨 부정교합군별 분포에 따른 역학적 특성과 국내병원에서의 수술시기 등의 치료현황에 대한 임상자료 등을 파악하고, 이를 순구개열 환자의 교정 진단 및 치료계획 수립에 중요한 기초자료로 사용하기 위하여 본 연구를 시행하였다. 1988년 3월 1일부터 1999년 2월 28일까지 서울대학교병원 교정과에 내원한 순구개열 환자(총 250명)와 그 부모들을 대상으로 한 초진시의 문진, 시진을 통하여 기록한 교정 chart및 cleft chart내용, X-tay film과 모형을 대상으로 조사를 시행하여 다음과 같은 결론을 얻었다. 1. 본 병원 교정과에 내원한 순구개열 환자의 수는 1988년부터 1990년까지 증가한 후 1992년까지 감소추세를 보였고, 1993년부터 1996년까지 비교적 일정한 추세를 보이다가 1997년 이후 현저한 증가 추세를 보였다. 2. 내원한 환자를 순구개열의 종류에 따라 조사한 결과 구순열:구순치조열:구개열:구순구개열이 7.6:19.2:9.6:63.6의 비율을 보였다. 편측 대 양측의 발생빈도는 구순열은 79:21, 구순치조열은 77:23,구순구개열은 75.5:24.5으로서 편측의 발생빈도가 양측에 비해서 높았다. 그리고 편측성에서 좌, 우측간의 발생빈도는 구순열이 53.3:46.7, 구순치조열이 59.5:40.5, 구순구개열이 59.2:40.8 으로서 좌측의 발생빈도가 우측에 비해서 높았다. 3. 순구개열의 남:여 발생빈도는 구순열은 57.9:42.1, 구순치조열은 68.8:31.2, 구순구개열은 76.1:23.9 로서 남자의 발생빈도가 여자에 비해서 높았다. 그러나 구개열에서는 41.7:58.3으로서 여자의 발생빈도가 남자에 비해서 높게 나타났다. 4. 내원 환자를 연령군 별로 조사한 결과 7-12세 군이 $52\%$로서 압도적으로 많았고, 0-6세 군 ($20.4\%$), 13-18세 군($17.2\%$), 18세 이상 군 ($10.4\%$)의 순이었다. 5. 구순열의 봉합수술시기로는 0-3개월 군이 $60.3\%$로서 가장 많았고, 4-6개월 군이 $17.9\%$로 두 번째였다. 6. 구개열의 봉합수술시기로는 1-2세군이 $31.7\%$로 가장 많았고, 0-1세군은 $25.6\%$, 2-3세군이 $12.1\%$였다. 구개 및 상악 성장이 어느 정도 이루어진 5세 이상 군은 $11.6\%$를 차지하였다.7. 구순 반흔 제거수술시기로는 4-6세군 ($27.5\%$), 6-8세군 ($19.6\%$), 2-4세군 ($13.7\%$)이 $60\%$이상을 차지하여 초등학교 취학 전에 구순의 반흔을 제거하려 함을 알 수 있었다. 8. 비변형 교정수술시기로는 0-2세군 ($7.1\%$), 2-4세군 ($14.3\%$), 4-6세군 ($21.4\%$), 6-8세군 ($14.3\%$)으로 초등학교 취학이전이 $57.1\%$로서 최근의 조기 치료경향을 반영하는 것으로 보인다. 9. 인두피판술은 평균 6세에 시행되었으며, 수술 시행 시기별의 차이를 보이지 않고 고른 분포를 보였다. 10. 내원한 환자를 순구개열 종류와 Angle씨 분류법에 의해 조사한 결과, 구순열군은 I급이 가장 많았고 III, II 급의 순이었으며, 구순치조열, 구개열, 구순구개열군은 III급이 가장 많았고, I, II 급의 순이었다. 그리고 III급의 발생빈도의 비율차이는 구순치조열은 $61.7\%$, 구개열은 $73.9\%$, 구순구개열군에서 $79.3\%$로서 구순구개열에서 압도적으로 III급의 발생빈도가 높게 나타났다. 11. 모든 연령군에서 III급 부정교합의 빈도가 가장 많아서($72.7\%$) 전치부의 반대교합이 주된 내원 동기가 됨을 간접적으로 알 수 있었다.
