Neuroblastoma: Review of 20-year Experiences (신경모세포종: 20년 치료 경험의 분석)

  • Neuroblastoma is the most common extracranial solid tumor in children. We retrospectively analyzed the results of neuroblastoma treatment of 191 patients (116 males and 75 females) treated between January 1986 and December 2005 at the Department of Pediatric Surgery and the Department of Pediatrics, Seoul National University Children's Hospital. The mean age at diagnosis was 3.1 years (0.1 yrs - 13.5 yrs). Forty-seven patients were under 1 year of age. The mean follow-up period was 57.3 months (24 days - 19.1 yrs). Patients were classified into two groups according to the completeness of resection of the primary tumor; (1) gross total resection (GTR) and (2) incomplete resection (IR). The number of patients in stages I, II, III, IV, IV-S were 17 (8.9 %), 12 (6.3 %), 43 (22.5 %), 114 (59.7 %), 4 (2.1 %), respectively. GTR was achieved in 120 patients and IR in 71 (22 stage III, 47 stage IV, 1 stage IV-S, 1 brain). Overall survival (OS) was 65.2 % and event-free survival (EFS) was 48.6 %. EFS were 100 %, 75 %, 66.8 %, 31.3 %, 75 % at stage I, II, III, IV, IV-S, respectively. There was no significant difference in EFS according to the completeness of resection. EFS was improved in GTR group (p=ns) of stage III, but by contrast, stage IV patients showed worse EFS in GTR group. EFS was improved significantly after the introduction of autologous stem cell transplantation (ASCT) (58.1% vs. 40.6%, p=.029). The EFS improved significantly after the introduction of ASCT in IR group (p=.009) rather than GTR group (p=ns). The EFS of the patients under 1 year of age (N=47) was better than the patients over 1 year of age (N=144) significantly (75.5 % vs. 39.4 %, p=.0034). The prognosis of neuroblastoma was related to the INSS stage and age at diagnosis. The survival of IR group significantly improved after ASCT.

The Analysis of the Type and Characteristics of Playable Furniture for Preschool Children (미취학 아동의 놀이가구 유형 및 특성 분석)

  • The interest and expenditure for children is increasing than in the past because the number of nuclear family is growing, the children's space is configured independently in most homes from infancy, therefore, the market of children's furniture is expanding in spite of the reduction in the population of children. There are not many products considering age, gender, and play behavior in children's furniture market in Korea. The bed, desk, chair, and closet for children under 2 years hold a large majority of children's furniture, there are not a lot of preschool children's furniture for the children aged 4 to 7. As the children aged 4 to 7 experience physical, emotional, and cognitive growth at the same time, the space configuration and furniture choice for these children is educationally very important. Therefore, in this study, we present the importance and necessity of playable furniture to promote the physical, social, emotional, linguistic, and cognitive development of the children aged 4 to 7 among preschool children. We found the concept and basic condition of playable furniture to collect the base data for developing playable furniture and provided the basic data for the development of an ideal playable furniture by comparing and analyzing the cases of 157 items of 14 domestic playable furniture companies and 365 items of 30 overseas in aspect of play functions, forms, materials and colors.

  • The author has conducted survey on the status of physical growth and morbidity of the children for christian children's fund programme, as a means of collecting basic data for the anticipated establishment of a health planning. A total 345 children aged 9 to 16 underwent C.C.F. programme while as a control, a total of 480 children of same ages from the middle-class school children in Jeonju area was also studied. As results of survey conducted for a period of one month (form July 1 to 31, 1974) on a total 429 children in 347 households living in Jeonju area. I. Socio-economic background 1. By educational status of the children, 39.9 per cent of the total children was attending at primary school, 33.8 per cent in middle school and 15.6 per cent in high school. 2. The greatest proportion or 28.8 per cent of the household head were engaged in labor, 17.9 per cent in peddler and 13.2 per cent in retail. 3. As for the living standard of the households, low class constitued 90.1 per cent, middle and high classes only 9.9 per cent. 4. 39.5 per cent of the households had their own house, 39.1 per cent lived in rent deposit house or rooms and 14.6 per cent in monthly rented house and rooms. II. Physical growth and nutritional status 1. The growth of children for C.C.F. programme in terms of height was found to be slightly smaller than the school children. The ages frm 9 to 16 corespond to the 'secondary growth and replenishment period and this period was regarded to be the one most affected by environmental and nutritional factors of all the other periods of growth and developmet. 2. The body weight of the children for C.C.F. presented a quite different pattern from that of the school children. The above findings appeared thin-and-long stature from the famillies with higher living standard while those from the household with low standard of living had a short-and-plump one. 3. According to the values of Rohrer's index, the children of C.C.F carried a higher degree of 'replenishment' than the children in Jeonju area and adolesecence comes later for the girls under C.C.F. programme. III. Morbidity 1. The monthly prevalence rate was 110.0 per thousand persons for the children under C.C.F. programme. 2. The total number of case was classified by timing of the incidence as follws. 40.0 per cent was constituted by diseases carried over from tile previous month and 60.0 per cent by new incidences. 3. The diseases were broken down by W.H.O. disease classification into the greatest proportion or 39.1 per thousand person constituted by disease of the digestive system.

The Study for Apical Pulse Measurement Technique Through Hospitalized Children (입원한 영유아의 심첨 맥박 측정 방법에 관한 연구)

  • The purpose of this study was to determine the most accurate technique measuring the apical pulse rate, using three counting duration 15, 30 and 60 seconds, and two methods start ‘0’ and start ‘1’. The instrument used in the study was the EKG monitor, stethoscope and stopwatch. Data was analyzed by utilizing SPSSWIN program. General characteristics of the subjects were analyzed by frequency, percentile, mean, SD. The subject of this research is made up of 46 children and 20 nurses. The children were infants, & under the age of 5. They were hospitalised in PICU & NICU in 2 tertiary hospitals in seoul from Jan. 1. 1998 to Sep. 10. 1998. The measurement of starting 1 & measurement of starting ‘0’ used the T-test to find out the measurement error. Apical pulse duration of 15, 30, 60 seconds were used to find out measurement error, the measurement error depend on experience of Nurse were analyzed by using ANOVA. The result of this study are as follows. 1. When comparing the starting poin of apical pulse 0&1, starting with 1 the measurement error is less, but not statiscally significant. 2. When counting the apical pulse by 15, 30,60 sec. ; 60 seconds counting duration was more accurate, but not statistically significant. 3. The mean of measure error ; Group under 100/min, is 10.33 ; from 100 re 119/min, is 8.30 ; from 120 to 139/min, is 4.76 ; from 140 to 159/min, is 6.09 ; above 160, is 17.83. The differences of these groups are statistically significant. When 60sec were counted, under 140/min the mean of measurement error is 3.4. Also when 30 seconds were counted from 140/min to 159/min the measurement error is 7.14, above 160/min the measurement error is 16.4. That measurement mean is the smallest than the other durations. During the 15 sec. count the measurement error was the largest of them all. 4. By the experience of the nurses, the apical pulse count measurement error was discovered. Under a year experience this measurement error was the largest(11.09), 1 year to under 3 years, the error is the smallest(4.86). 3 year to under 6 years the error is 8.33, 5 years above the error is 6.11 but this is not statistical significant. Under a year experience when counting 15, 30, 60 seconds the error is the largest. The group of the nurses from a year to under 3 years, the measurement error is the smallest of all the groups. The result of the study is to determine the technique measuring the apical pulse rate, Hargest (1974), starting point ‘0’ is not proved. When the pulse rate increases the 30 sec measurement rate is accurate. Under 140/min the 60 sec measurement rate is the most accurate. Depending on the nurses experiences, there is a variable difference to the apical pulse rate measurement. Especially new nurses training courses should enforce the children’s pulse rate count and the basic vital signs.

Survey on Nutritional Status for Preschool Children in a County in Jeju Island (제주도 1개 군 지역에서의 취학 전 아동의 영양상태 조사)

  • The purpose of this survey was to assess the nutritional status of preschool children a county of Jeju Island. This study used health examinations results for preschool aged children, performed by the public health center of Pukjeju-gun in Jeju-do for over three years from 1999 to 2001, of children in kindergartens and children homes. The target children totaled 5,990 from the ages three to six. For a control group, 316 children from the nearby Jeju-city areas were included as well. The items of this research included height, weight, and hemoglobin values. 1. The average height of boys from ages three to six were 96.35cm, 102.14cm, 109.94cm, 111.00cm respectively, and girls were 94.96cm, 100.93cm, 108.33cm, 110.54cm respectively. The average weights of boys from ages three to six were 15.42kg, 16.93kg, 19.65kg, 19.67kg respectively, and the weight of girls were 14.90kg, 16.45kg, 18.88kg, 19.50kg respectively. 2. The percentages of children who did not reach 90% of the Korean standard height were 4.3% in boys 4.1% in girls. The percentages of children with less than 80% of the Korean standard weight were 7.6% in boys and 6.8% girls. The percentages of children over 120% of Korean standard weight were 10.4% in boys and 11.4% in girls. 3. As for the obesity level, the percentage of boys under-weighed(under 10% for standard weight for height) were 11.6% and girls, 9.5%, and the percentages of boys and girls with obesity($\geq$20%) were 3.6% and 4.4% respectively. 4. The mean hemoglobin value of boys were 11.83g/$d\ell$ and girls, 11.83g/$d\ell$. These were lower than the value of average normal Korean children (12.5g/$d\ell$). The mena hemoglobin values of the children in Pukjeju-gun were considerably lower than that of the children living in Jeju-city(12.3g/$d\ell$) as well. Anemia of Children of Pukjeju-gun were estimated at 38.1%(male) and 37.2%(female), by using Hemoglobin level(<11.5g/$d\ell$ 5. The rates of children included within the normal range of obesity level in Jeju-city and Pukjeju- gun were boys 80.2%, 71.6% in boys, and 77.4%, 72.4% in girls. The percentage of children living in PukJeju-gun included within the normal range were considerably low. 6. There were no changes in the Body Mass Index (BMI) during the three years from 1999 to 2001, but the percentage of children with anemia significantly increased. Health care for preschool aged children, especially in the rural areas, is very important. Centering on public health centers, it is necessary to systematically promote health care in the rural areas.

Trends of conscious sedation in the Department of Pediatric Dentistry at the Dankook University Dental Hospital for 11 Years

  • Background: Anxiety and fear in children's dental care are major impediments to successful dental care. High-quality dental treatment can be achieved using various behavioral control methods; however, conscious sedation using drugs can be used if behavioral control is difficult, owing to excessive fear and anxiety. This study aimed to examine the trends in conscious sedation implemented in pediatric dentistry at the Dankook University Dental Hospital over the past 11 years. Methods: This study included 6,438 cases of dental treatment under conscious sedation conducted over 11 years between January 2011 and December 2021 in the Department of Pediatric Dentistry at Dankook University Dental Hospital. Results: Over the past 11 years, the number of dental treatments under sedation has increased. In the case of inhalation sedation using nitrous oxide, the rate of increase was approximately twice every year, and the use of midazolam gradually decreased. The average age of children who underwent sedation was 5.11 years, and the rate of sedation treatment in children aged <4 years tended to decrease, while that of children aged >5 years tended to increase. This is related to the trend of changes in drugs used. In a sex-based survey, sedation treatment rate was higher in males than that in females. Conclusion: Appropriate selection of sedatives can reduce the frequency of general anesthesia and minimize complications through efficient and safe dental treatments. Trend analysis of sedation by year will help provide guidelines for the appropriate selection of sedation for dental treatment of children and patients with disability.

Interior Conditions and User Satisfaction of Elementary School Children's Rooms (주택 아동실의 실내공간 구성현황 및 만족도 조사에 관한 연구 - 초등학교 저학년 학생을 대상으로 -)

  • This study examined the current interior conditions of children's rooms, and investigated the children's satisfaction of the rooms and their needs for improvement. Survey questionnaires were given to 205 elementary school children in the third or lower year grades and to their parents. Frequency analysis, t-tests, analysis of variance, and Duncan tests were mainly employed for data analysis. The findings indicated that the respondents' satisfaction of the children's rooms differed in their grade, room size, and the main color of the rooms. In general, those in the third grade, those in the rooms under 16.5$m^2$, or mainly painted in cold colors, tended to respond more negatively. Meanwhile, they expected to exchange their furniture for study, such as desks and bookshelves, and to individually possess electric appliances in their rooms. Most children's rooms were designed on the basis of the parents' preferences: The more the rooms reflected the children's opinions, the higher satisfaction with the furniture they showed.

Research On General Attitude Of Parents Toward Their Cerevral Palsy Children (뇌성마비환자 부모들의 일반적인 개념조사)

  • This Research has been made through the questionnaire(enquete) sent to parents whose children are under cerebral palsy treatment in the hospitals of Daejon, Daegu and Pusan area and in other social welfare organizations such as Korea C.P. Welfare institution and Social Welfare Corporation during the period of Februay to August 1997. Form this research, we have obtained following results : 1. Mostly, the blood type of parents of cerebral palsy children is "0" type in father side (24 persons : 30%) and "A" type in mother side(28 persons : 35%), 2. For their(parents) baby-delivery, it shows that the nature delivery was adoped in most cases i. e. 50 persons(62.5%). 3. The birth weight of cerebral palsy children appears to be less than 2.4Kg in most cases(24 persons : 30%) 4. Most of parents positively want to take a medical treatment for their children with hope, but parents of 5 patients(6.3%) have a negative attitude against medical treatment and its cure. 5. Parents of 25 patients (31.7%) appear to have a sense of shame about the fact that their children are C.P. patients. 6. The 65 parents(81.3%) are depending on medical treatment and physical therapy as a means of cure.

  • Oxidative stress-induced mitochondrial dysfunction is implicated in the pathogenesis of intervertebral disc degeneration (IVDD). Sirtuin 3 (SIRT3), a sirtuin family protein located in mitochondria, is essential for mitochondrial homeostasis; however, the role of SIRT3 in the process of IVDD has remained elusive. Here, we explored the expression of SIRT3 in IVDD in vivo and in vitro; we also explored the role of SIRT3 in senescence, apoptosis, and mitochondrial homeostasis under oxidative stress. We subsequently activated SIRT3 using honokiol to evaluate its therapeutic potential for IVDD. We assessed SIRT3 expression in degenerative nucleus pulposus (NP) tissues and oxidative stress-induced nucleus pulposus cells (NPCs). SIRT3 was knocked down by lentivirus and activated by honokiol to determine its role in oxidative stress-induced NPCs. The mechanism by which honokiol affected SIRT3 regulation was investigated in vitro, and the therapeutic potential of honokiol was assessed in vitro and in vivo. We found that the expression of SIRT3 decreased with IVDD, and SIRT3 knockdown reduced the tolerance of NPCs to oxidative stress. Honokiol ($10{\mu}M$) improved the viability of NPCs under oxidative stress and promoted their properties of anti-oxidation, mitochondrial dynamics and mitophagy in a SIRT3-dependent manner. Furthermore, honokiol activated SIRT3 through the AMPK-PGC-$1{\alpha}$ signaling pathway. Moreover, honokiol treatment ameliorated IVDD in rats. Our study indicated that SIRT3 is involved in IVDD and showed the potential of the SIRT3 agonist honokiol for the treatment of IVDD.

Appropriate Size and Dish Combination of Nutritional-Balanced Lunch Boxes Delivered to Children Under the Government-Funded Meal Service Program in Korea (영양적으로 균형 있는 아동급식사업용 도시락의 적정한 용기 크기 및 음식 구성)

  • The purpose of this study was to propose the appropriate sizes and dish combination for nutritional-balanced lunch boxes delivered to children under the government-funded meal service program in Korea. The study was based on the 3 : 1 : 2 Meal Box Magic, a nutrition education method developed in Japan. A total of 290 lunch menus, comprising of 10 day menus from 29 organizations having delivered lunch boxes to children during summer vacation of 2008, were analyzed and used as the base data for lunch box combination. Dishes of the menus were classified into 6 groups: Rice group, Protein side dish group (including meat, fish, egg, and bean dishes), Vegetable side dish group, Kimchi group (including kimchi and jangajji), Soup stew group, and the other group. Nutrient analysis was conducted for 100 ml of these dishes by CAN Pro 3.0 utilizing volume and weight conversion data used for analysis of the Korea Health and Nutrition Examination Survey and those from the Nutrient Composition of Food for Consumers. All the possible combinations of 5 dishes, comprising of 3 dishes from Rice group, Protein side dish group, and Kimchi group, respectively, and 2 dishes from Vegetable side dish group, were made using the frequently served dishes from the respective dish group. Nutrient analysis of each combination was conducted based on the assumption that a lunch box was 600 ml and filled up to 80% by dishes using the volume ratio of 3 : 1 : 1.5 : 0.5 for Rice group : Protein side dish group : Vegetable side dish group : Kimchi group. The mean and standard deviation of energy and nutrients of all combinations calculated by weighting the serving frequency of each dish selected for the combinations were $621\pm81$ kcal for Energy, $22.1\pm5.0$ g for Protein, $120\pm45$ mg for Calcium, $4.1\pm1.1$ mg for Iron, $201\pm130\;{\mu}g$ RE for Vitamin A, $0.34\pm0.10$ mg for Thiamin, $0.27\pm0.10$ mg for Riboflavin, and $24.3\pm9.6$ mg for Vitamin C. The energy percentages from Carbohydrate, Protein and Fat were 66%, 14% and 20%. The analysis results met the nutrition standard of lunch boxes for male elementary students in grades 4 through 6 under the government-funded meal service program regarding calories, nutrients except calcium and riboflavin, and macronutrient distribution ranges. Accordingly appropriate box sizes were suggested for different age and sex groups to meet the respective nutrition standards. In addition, milk or dairy products were suggested to accompany lunch boxes to supplement calcium and riboflavin intake. The method of selecting box sizes and making dish combination suggested in this study could be useful for the organizations preparing lunch boxes under the government-funded children's meal service program where nutrition professionals are not available.
