하대정맥 손상후 일차봉합술로 인해 발생한 버드-키아리 증후군 환자를 혈관성형술로 치료한 증례보고 (Balloon Angioplasty for Budd-Chiari Syndrome Resulting from Primary Repair of an Inferior Vena Cava Injury)

  • Budd-Chiari syndrome is an uncommon condition characterized by hepatic outflow obstruction. Direct suture of the injured Inferior vena cava in a patient with blunt hepatic trauma also may cause an equivalent condition. However, early diagnosis is possible with common symptoms and radiologic evaluation. Moreover, a transluminal approach with balloon angioplasty could prevent long-term complications of Budd-Chiari syndrome without repeated abdominal surgery.

  • Osteoma is a benign osteogenic tumor composed of cancellous or compact bone, classified as peripheral, central, or extraskeletal. Peripheral osteomas are uncommon. Excluding the maxillary sinuses, the maxilla is a rare site for osteomas. The purpose of this report was to describe clinicopathological and radiological features of two peripheral osteomas occurring in the jaws, one located in the mandible and another in the edentulous maxillary alveolar ridge. The tumors were asymptomatic and were fully excised without any complications or recurrence. The lesions were submitted to histopathological analysis and diagnosed as peripheral osteoma, compact type.

  • Intraabdominal cystic lymphangioma is an uncommon lesion. It is usually found incidentally in patients presenting with an acute abdomen. Laparoscopic excision of intraabdominal cystic lymphangioma is an easy and safe procedure in children. We report one case of cystic lymphangioma in a 6.year-old female. The lesion was located on the left side of the transverse mesocolon. Laparocopic excision of the cyst was performed without complications.

  • Esophageal foreign body are not uncommon problems in the otolaryngologic field. Esophageal foreign body may cause severe complications such as esophageal ulceration, esophageal perforation, periesophagitis, tracheo-esophageal fistula, mediastinitis, pneumothorax pyothorax according to the kinds, shape, size, duration of lodgement of foreign body. The majority of esophageal foreign U which lodge in the esophagus can be removed endoscopically, but the following type of foreign body may require removal by external route. 1. an impacted foreign body 2. a foreign body producing esophagitis after unsuccessful attempts at removal through the esopahgoscope 3. a periesophageal abscess with a foreign body lodging in the abscess itself. Recently, we experienced a case of esophageal foreign body (fish bone) which penetrate the cervical esophageal wall and formed retropharyngeal abscess in 54-year old female. The foreign body are successfully removed and abscess was drained by external route through the lat neck.

Ossifying fibroma in the maxilla and orbital floor: report of an uncommon case

  • Benign fibro-osseous lesions occur when normal bone is replaced by cellular fibrous connective tissue and mineralized structures. One rare type of these lesions is the ossifying fibroma (OF). The aim of this study is to report an unusual case of OF in a 57-year-old female. Physical examination showed facial asymmetry without any tenderness, fluctuation, ocular pain, or ophthalmoplegia. Imaging exams revealed a solid mass involving the left maxilla and orbital floor. Surgical resection was performed without any complications or sequelae, and the histopathological results confirmed OF. Although recurrence is rare in this condition, the patient remains under follow-up.

  • 기도이물은 이비인후과 영역에서 중요한 위치를 차지하고 있는 질환으로서 때로는 순식간에 생명을 위협하거나 중증의 합병등을 초래하기도 하며, 이들의 종류와 형태도 다양할 뿐 아니라 그 개재 부위도 달라서 이물 적출은 항상 낙관을 불허하는 것으로 수술적 방법을 요할때도 있다. 저자들은 최근에 경험한 흥미있다고 생각되는 기도이물 8례를 문헌적 고찰과 함께 보고하는 바이다.

  • Intra-osseous ganglion is an uncommon benign bone tumor caused by an intramedullary metaplastic event. Two cases of the intraosseous ganglion of the foot are presented; one in the metatarsus and the other in the talus. Follow-up averaged 14 months. Satisfactory results were obtained with curettage, cancellous bone graft from the ipsilateral calcaneus. No recurrence and complications occurred during the period of follow-up.

외상성 기관지 파열 -1례 보고- (Tracheobronchial Rupture following Blunt Chest Trauma -1 case report-)

  • The rupture of tracheobronchial tree caused by non penetrating blunt trauma is being increased in incidence though it is still rarely occurred on now Because this type of injury is uncommon, a high index of suspicion should be maintained in all crush injuries involving the chest. Early diagnosis and primary repair not only restore normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair. We experienced a case of tracheobronchial rupture caused by non penetrating blunt chest trauma without rib fracture. The patient was a 16 year old male who was a high school student. He was compressed on anterior chest by hand ball goal post being failed down on the morning of admission day. After this accident, he was suffered from progressively developing dyspnea and subcutaneous emphysema on face, neck and anterior chest. The diagnosis, tracheal rupture, was made by chest CT and bronchoscopy. After right thoracotomy, the ruptured site was directly closed by using interrupted suture. Post-operative course was uneventful. Thus we report this case of traumatic tracheal rupture with review of literature.

  • Paroxysmal nocturnal hemoglobinuria (PNH) is an uncommon hematologic disease characterized by an abnormal sensitivity of blood cells to the lytic action of serum complement. We experienced one case of PNH in Yeungnam University Hospital from May 1983 to May 1989. The patients was followed up without severe complications for 4 years since diagnosis with the only consevating treatments such as washed blood transfusion, adrenal corticosteroids, androgens, folate and iron preparation, intermittently.

Pseudotumor and Subsequent Implant Loosening as a Complication of Revision Total Hip Arthroplasty with Ceramic-on-Metal Bearing: A Case Report

  • Pseudotumors are not uncommon complications after total hip arthroplasty (THA) and may occur due to differences in bearing surfaces of the head and the liner ranging from soft to hard articulation. The most common causes of pseudotumors are foreign-body reaction, hypersensitivity and wear debris. The spectrum of pseudotumor presentation following THA varies greatly-from completely asymptomatic to clear implant failure. We report a case of pseudo-tumor formation with acetabular cup aseptic loosening after revision ceramic-on-metal hip arthroplasty. The patient described herein underwent pseudotumor excision and re-revision complex arthroplasty using a trabecular metal shell and buttress with ceramic-on-polyethylene THA. Surgeons should be aware of the possibility of a pseudotumor when dealing with revisions to help prevent rapid progression of cup loosening and implant failure, and should intervene early to avoid complex arthroplasty procedures.
