미세전류치료와 초음파치료가 슬관절전치환술 후 통증경감과 기능회복에 미치는 영향 (The Effects of Microcurrent Treatment and Ultrasound Treatment on the Pain Relief and Functional Recovery after Total Knee Replacement)

  • Purpose: The purpose of this study was to investigate pain relief and functional recovery after total knee replacement. Methods: The treatment was performed by dividing individuals into a control group ($n_1=5$), ultrasound treatment group ($n_2=5$), and micro-current treatment group ($n_3=5$). The control group applied the hot pack for 15 minutes, Transcutaneous Electrical Nerve Stimulation (TENS) for 15 minutes, and Continuous Passive Movement (CPM) for 40 minutes. The ultrasound therapy group applied the frequency of 1 MHz, intensity of 1.0 $W/cm^2$ for five minutes following the same treatment as the control group. The micro-current therapy group applied the intensity of 25 ${\mu}A$, and pulsation frequency 5 pps for 15 minutes following the same treatment as the control group. After treatment, Visual Analogue Scale (VAS), Korean Western Ontario and McMaster Universities Arthritis Index (K-WOMAC), Berg Balance Scale (BBS), Range of Movement (ROM) and wound length was measured. Results: VAS showed significant effect in the control group and micro-current therapy group during the treatment period. According to the treatment of K-WOMACK, BBS, ROM, and Healing wounds showed main effects between groups. Conclusion: According to the results of this study, data showed improvement of pain relief, wound healing effects, and range of motion recovery. Thus, these selected treatments were effective after total knee replacement. In other words, electrical treatment continues to influence pain relief and functional recovery after total knee replacement.

분리정제봉약침의 초음파 유도하 시술과 한의복합치료를 병행한 늑골 골절 증례 보고 (The Effectiveness of Ultrasound-Guided Essential Bee Venom Pharmacopuncture Combined with Integrative Korean Medical Treatment for Rib Fracture: A Case Study)

  • This study is to report the usefulness of ultrasound in diagnosing rib fracture and the effectiveness of ultrasound-guided essential bee venom pharmacopuncture as a treatment of it. A 46-year-old unable to find a fracture by chest radiography was diagnosed it on ultrasound. The patient was applied integrative Korean medical treatments and ultrasound-guided essential bee venom pharmacopuncture at the fracture site for 4 weeks, 18 times total. Numeral rating scale (NRS) was used to evaluate the amount of pain. The chest pain when turning over the body was reduced from NRS 8 to 2 for 4 weeks. The remarkable aspect is that the pain was immediately relieved after phamacopuncture and its effect lasted for 3 hours. This report suggests ultrasound can be useful for diagnosing rib fracture and also helpful for safety and accuracy of pharmacopuncture. Additionally, essential bee venom can be considered one of the methods of treating rib fracture.

완관절과 수부 질환의 초음파 소견과 치료 (Ultrasound Findings and Treatment of Wrist and Hand Diseases)

  • There are many wrist and hand diseases in which ultrasound can help in the diagnosis and treatment. Because many small structures are located, identifying anatomical locations and pathways is especially important. In De Quervain's syndrome, it is necessary to find tendon lesions located in the first compartment of the wrist. If injection therapy is required, administer the regimen accurately within the tendon sheath through ultrasound. In carpal tunnel syndrome, there are several methods to diagnose a disease by measuring cross-sectional area of the median nerve. Ultrasound has the advantage of administering injection therapy without damaging the nerve. Intersection syndrome can be diagnosed by observing swelling and hypoechoic appearances at the point where the tendons of the first compartment cross over the second compartment of the wrist. Ultrasound-guided injection is also safe and efficient. If there is a trigger finger lesion, the most representative findings is to observe a nodular hypoechoic thickening of the involved A1 pulley. When injection therapy is performed, it is effective to administer medication between pulley and flexor tendons as much as possible to reduce pressure on the attached structures.

  • This study examined the effectiveness of Korean-Western cooperative treatment for patients with plantar fasciitis. Fifty patients received Korean medicine treatments (acupuncture, pharmacopuncture, herbal medicine) and Western medicine treatments (polydeoxyribonucleotide, and extracorporeal shock wave therapy). Evaluation methods used were comparison before and after ultrasound (P9), and numeric rating scale scores. Results revealed a significant improvement in the level of pain and evaluation of improvement using ultrasound. Moreover, it was suggested that Korean-Western cooperative medicine treatment may be effective for the treatment of plantar fasciitis.

초음파 자극이 현미발아 촉진에 미치는 영향 (Effects of Ultrasound Stimuli on Acceleration of Brown Rice Germination)

  • The effects of ultrasound stimuli on the germination and sprout growth of brown rice were investigated. Ultrasound was applied to brown rice at the frequencies of 28, 40, and 60 kHz before germination test and it was germinated in three methods (Type I, II and III). Type I was to soak brown rice into water for 60 hours. Type II was to expose brown rice to air for 48 hours after soaking them into water for 12 hours. Type III was a repetitive method of water-soaking and air exposure for 12 hours respectively. The most effective method for the germination was Type III without ultrasound. However, Type I was a best method after ultrasound treatment. As power of ultrasound increased, sprouts grew faster after brown rice were treated in 40%, 70%, and 100% power (0.137, 0.241, and 0.344 $W/cm^2$) at 40 kHz. The good treatments for fast sprout growth of brown rice at each frequency were the 28 kHz-10min group, the 40 kHz-5min group, and the 60 kHz-20min group of Type I. The best effective treatment was the 40 kHz-5min group at 0.344 $W/cm^2$ and at that condition the time required for sprout growth of 2.5 m was 51.9 hours. The ultrasound stimuli was very effective in the beginning of the rice germination, and the germination ratio was more than 95% in all ultrasound treatments.

중둔건 석회화 건염의 초음파 유도하 정확한 조준에 의한 체외충격파치료 -증례 보고- (Ultrasound-guided Exact Focusing of Extracorporeal Shock Wave Therapy for the Calcific Tendinitis of Gluteus Medius - A Case Report -)

  • 석회화 건염은 건-골 부착부에 침착된 칼슘 수산화 인회석 결정 주위로 염증이 생기는 질환인데 체외충격파치료가 석회화 건염에 대한 비침습적 치료로써 효과적으로 많이 사용되고 있다. 실제 임상에서는 병변 부위에 정확히 체외충격파를 시행하기보다는 주로 촉진과 압통 부위 확인만으로 충격파 시행범위를 정하고 있다. 방사선 투시하에 석회 침착부위에 정확히 체외충격파를 시행하는 것이 임상 결과가 월등히 우수하다는 것은 알려져 있다. 초음파는 간편하고 비싸지 않으며 방사선 피폭이 없으면서 건 질환을 진단하는데 아주 효과적인 진단 기기이지만 체외충격파 치료의 조준에 있어서의 역할은 확립되어 있지 않다. 저자들은 중둔건에 발생한 거대 석회화 건염을 초음파를 이용하여 병변 부위의 위치를 정확히 조준하면서 체외충격파치료를 시행함으로써 석회 침착의 완전 소실을 얻은 증례를 경험하였기에 문헌 고찰과 함께 보고하고자 한다.

  • 초음파을 통하여 신생아 고관절의 형태와 안정성 여부를 관찰 할 수 있음이 알려진 이후 2개의 상반된 견해가 형성되었다. 신생아기에 관찰되는 관절 불안정이 치료를 필요로 하는 중요 병리라는 의견은 신생아기 비구이형성이 중요한 역할을 한다는 의견과 상충되어 왔다. Graf방법같이 신생아 비구이형성을 포함하여 치료의 필요성 여부를 판단하면 고관절 불안정성만을 기준으로 한 경우보다 치료와 추시를 하는 빈도가 훨씬 더 높았었다. 현재는 기술적 검사 방법의 발달과 질환의 적절한 이해로 그 차이는 크지 않은 것으로 여겨지고 있어 여러 방법은 유사한 결과를 보이고 있다. 중요한 것은 방법의 선택이 아니라 표준화한 검사를 할 수 있는 충분한 경험과 이해를 가지고 있어야 한다는 것이다. 이를 위하여 고관절 이형성증(DDH)의 진단에서의 여러 초음파 검사 방법과 의의, 치료시의 유용성에 대하여 논의 하였다.

Ultrasound-Assisted Mental Nerve Block and Pulsed Radiofrequency Treatment for Intractable Postherpetic Neuralgia: Three Case Studies

  • Postherpetic neuralgia (PHN) is one of the most difficult pain syndromes to treat. Invasive treatments may be considered when patients fail to obtain adequate pain relief from noninvasive treatment approaches. Here, we present three cases of PHN in the mandibular branch treated with ultrasound-assisted mental nerve block and pulsed radiofrequency treatment. None of the patients had adequate pain relief from the medical therapy, so we performed the mental nerve block on the affected side under ultrasound assistance. Two patients showed satisfactory pain relief continuously over 12 months without any further interventions, whereas one patient only had short-term pain relief. For the patient had short-term pain relief we performed pulsed radiofrequency treatment (PRFT) on the left mental nerve under ultrasound assistance. After PRFT, the patient had adequate pain relief for 6 months and there was no need for further management.

전처리 방법에 따른 영지버섯 추출액의 품질특성 변화 (Comparison of Pretreatment Methods for Extraction of Selected Components from Ganoderma lucidum)

  • 영지버섯(Ganoderma lucidum)으로부터 영양 및 기능성 성분의 열수추출 수율을 증대시킬 수 있는 방법을 개발하기 위하여 추출 전처리 및 방법의 효과를 비교, 검토하였다. 영지를 분쇄하여 사별하고 초음파와 마이크로파 처리를 10분 동안 실시한 후 물을 용매로 상압 및 가압추출을 각각 2시간 동안 실시하였다. 추출전 전처리를 행함으로서 총당, 단백질함량 및 페놀성물질의 추출수율을 증가시켰고, 유리기 소거능이 증가하는 경향을 보였으며 마이크로파 전처리가 초음파처리 보다 우수한 것으로 나타났다. 또한 상압추출보다 가압추출에 의해 가용성 고형물, 총당, 단백질 및 페놀성 물질의 추출수율이 증대되는 경향을 보였다. 따라서 초음파나 마이크로파 전처리와 가압추출은 영지버섯의 추출효율 향상에 유효한 방법인 것으로 판단되었다.

  • Recently, ultrasound is becoming a universal way of treatment when it comes to clinic. The thermal effect of therapeutic ultrasound can raise the temperature of tissue without damage and thus increases the flow rate, and this leads to the oxygen transportation velocity of peripheral tissue and ultimately facilitates the metabolism.
