구강악안면영역에서 초음파영상 진단 (Clinical Usefulness of Ultrasonography in Oral and Maxillofacial Regions)

  • Ultrasound sonography(US) is used to evaluate various diseases of maxillofacial region including salivary glands, soft tissue and jaw lesions because of easy accessibility and no hazard of ionizing radiation. Also, US can offer dynamic study showing real-time images during diagnostic or surgical procedure. US images provide accurate information about the internal features of lesions on the jaw prior to surgical treatment. Doppler images are used to visualize the vascular distribution of the lesions and to provide additional information to enhance diagnostic value. Nevertheless, the clinical application of US imaging is limited in the dental field. This is due to the lack of knowledge about the US image and the image characteristics of the anatomical structures. It is necessary to evaluate the diagnostic value of US and evaluate its usefulness by looking at clinical cases using US images. Therefore, US imaging may be recommended as an assistant image in evaluating jaw lesions. US images provided accurate information about the internal structure of lesions on the jaw prior to surgical treatment, and diagnostic value was enhanced by visualizing the vascular distribution of the lesion using doppler imaging.

  • 결절종은 수부나 손목 관절의 관절막 혹은 건초에서 흔히 발생하는 연부 조직 양성 종양으로 초음파상 경계가 명확한 저 에코성의 낭성 종괴로 보여 비교적 정확하게 진단할 수 있다. 저자들은 손목 관절의 수장부 요측에서 촉지되는 종괴에 대하여, 술전에 시행한 초음파 검사 상 고 에코성 소견을 보여서 고형성 종괴로 추정하였으나, 수술 소견 및 조직학적 검사에서 결절종으로 진단되었던 예를 경험하였기에 고 에코를 보이는 결절종에 대한 문헌 고찰과 함께 보고하고자 한다.

Dorsal midline cutaneous stigmata associated with occult spinal dysraphism in pediatric patients

  • Purpose: To investigate the prevalence of occult spinal dysraphism (OSD) and subsequent neurosurgery in pediatric patients with isolated or combined dorsal midline cutaneous stigmata with or without other congenital malformations. Methods: We carried out a retrospective review of patients who underwent sonography or magnetic resonance imaging (MRI) for OSD because of suspicion of dorsal midline cutaneous stigmata (presumed to be a marker for OSD) between January 2012 and June 2017. Information about patient characteristics, physical examination findings, spinal ultrasound and MRI results, neurosurgical notes, and accompanying congenital anomalies was collected. Results: Totally 250 patients (249 ultrasound and one MRI screening) were enrolled for analysis. Eleven patients underwent secondary MRI examinations. The prevalence of OSD confirmed by an MRI was 2.4% (6 patients including one MRI screening). Five patients (2%) had tethered cord and underwent prophylactic neurosurgery, 3 of whom had a sacrococcygeal dimple and a fibrofatty mass. Prevalence of tethered cord increased as markers associated with a sacrococcygeal dimple increased (0.5% of the isolated marker group, 8.1% of the 2-marker group, and 50% of the 3-marker group). Incidence of OSD with surgical detethering in 17 other congenital anomaly patients was 11.8%, which was higher than the 1.3% in 233 patients without other congenital anomalies. Conclusion: Our results suggest that the presence of dorsal midline cutaneous stigmata, particularly fibrofatty masses, along with a sacrococcygeal dimple is associated with OSD or cord tethering requiring surgery. OSD should be suspected in patients with concurrent occurrence of other congenital anomalies.

An Ultrasonographic Evaluation of Masseter Muscle Thickness in Patients Having Parafunctional Habit

  • Purpose: The purpose of this study was to investigate the masseter muscle thickness before and after treatment using ultrasound sonography in patients with parafunctional habits. Materials and Methods: From September 2019 to March 2020, a total of 27 patients who visited the Department of Oral and Maxillofacial Surgery at Ewha Womans University Seoul Hospital were collected. The thickness of both masseter muscles was measured using a tablet ultrasound scanner. Statistical analysis was performed by using the IBM SPSS version 26.0 statistical package (IBM Corp) with significance level at 0.05. Result: According to the statistical results, the thickness of the masseter muscle was thicker on the right side than on the left, with no correlation with sex or age. The severity and duration of pain did not have a significant correlation with the thickness of the masseter muscle. Botulinum A toxin injection in the masseter muscle was the most effective way to reduce pain and reduce the thickness of the masseter muscle. Splint treatment also showed some effects in reducing the thickness of the masseter muscle. Conclusion: Based on the findings, it can be claimed that ultrasonography is simple, inexpensive and easily repeatable method to get real-time diagnosis and treatment results for masseter muscles.

손목 관절과 발목 관절 손상 환자의 진단에 있어 응급 초음파의 유용성 (The Utility of Emergency Ultrasound for Diagnosing Wrist and Ankle Injuries)

  • Purpose: Ultrasound is of proven accuracy in abdominal and thoracic trauma and may be useful for diagnosing extremity injury in situations where radiography is not available, such as disasters and military and space applications. However, the diagnosis of fractures is suggested by history and physical examination and is typically confirmed with radiography. As a alternative to radiography, we prospectively evaluated the utility of extremity ultrasound performed by trained residents of emergency medicine (EM) one patient with wrist and ankle extremity injuries. Methods: Initially, residents of EM performed physical examinations for fractures. The emergency ultrasound (EM US) was performed by trained residents, who used a portable ultrasound device with a 10- to 5-MHz linear transducer, on suspected patients before radiography examination. The results of emergency ultrasound and radiography and the final diagnosis were recorded, and correlation among them were determined by using Kappa s test Results: Thirty-nine patients were enrolled in our study. The average age was $36.6\;{\pm}\;19.3$ years. There were radius Fx. (n=21), radius-ulna Fx. (n=1), ulna Fx. (n=1), and contusion (n=2) injuries among the wrist injury and lat.-med. malleolar Fx. (n=13), lat. malleolar Fx. (n=6), and med. malleolar Fx. (n=3) injuries among the ankle injury. Comparing EM US with radiography, we found the sensitivity, specificity, positive predictive value, and negative predictive value of EM US for Fx. diagnosis to be 100%, 66.7%, 97.3%, 100% and those of radiography to be 97.2%, 100%, 100%, and 75%, respectively. Kappa s test for a correlation between the Fx. diagnosis of EM US and the final diagnosis of Fx was performed, and Kappa's value was 0.787 (P = 0.004).Conclusion: EM US for Fx. can be performed quickly and accurately by EM residents with excellent accuracy in remote locations such as disaster areas and in military and aerospace applications. EM US was as useful as radiography in our study and had a high correlation to the final diagnosis of Fx. Therefore, ultrasound should performed on patients with extremity injury to determine whether extremity evaluation should be added to the FAST (focused abdominal sonography trauma) examination.

  • Controlled ovarian hyperstimulation is one of the major steps of in vitro fertilization. The inaccessibility or non-visualization of developing follicles on transvaginal sonography (the preferred imaging method) may be misjudged as a poor response, resulting in cycle cancellation. It is necessary to scrupulously appraise proxy indicators for ovarian response, such as estradiol levels, endometrial thickness, and other individual clinical characteristics. This can prompt meticulous transabdominal ultrasound follicular monitoring and oocyte retrieval with the goal of averting cycle cancellation and improving treatment outcomes.

초음파적 내측 반월상 연골 탈출의 진단 가치 (Diagnostic Value of Sonographic Medial Meniscal Extrusion)

  • 목적: 초음파상 진단되는 내측 반월상 연골 탈출의 반월상 연골 파열과 관련된 의의에 대해 자기 공명 영상과 관절경 소견을 연계하여 알아보고자 하였다. 대상 및 방법: 슬관절 내측 반월상 연골의 파열이 있어 관절경적 수술을 시행하였던 32예로서 내측 반월상 연골 파열 14예, 내측 및 외측 동시 파열 18예를 대상으로 하였다. 반월상 연골 파열이 없는 내측 반월상 연골의 탈출의 정도는 자기공명 영상에서 반월상 연골 파열이 없는 24예를 대상으로 하였다. 내측 반월상 연골 탈출의 정도는 골극을 제외한 경골의 내측 관절선부터 내측 반월상 연골의 변연부까지 계측하였다. 단순 방사선 사진에서 Kellgren-Lawrence (K-L) 등급을 평가하였고, 관절경 수술 시에 병변의 위치를 평가하였다. 각각의 소견을 비교하여 자기 공명 영상과 초음파에서 반월상 연골의 탈출의 정도를 비교 분석하였다. 결과: 관절경 소견 상 반월상 연골 파열이 있는 군 전체, 내측 반월상 연골 파열이 있는 군, 내측 및 외측 반월상 연골이 동반 파열된 군 들과 파열이 없는 정상 군 사이에 통계학적으로 유의한 차이를 보였다(P<0.05). 결론: 초음파 영상에서 5 mm 이상의 탈출이 있을 때 반월상 연골 파열을 의심해야 할 것으로 생각된다.

초음파를 이용한 슬관절 내측 불안정 평가 -측정기법- (Evaluation of Medial Instability of the Knee with Ultrasonography - Technical note -)

  • 목적: 슬관절의 내측 측부 인대 손상으로 인한 내측 불안정의 정도 측정 시에 초음파를 이용하여 내반 및 외반 상태에서 동적으로 측정하는 기법을 기술하고자 한다. 대상 및 방법: 2008년 1월부터 2008년 4월까지 내측 측부 인대의 손상이 있었던 7예를 대상으로 하였다. 추시 관찰 시에 손상 직후 운동제한 보조기(limited motion brace)를 착용시켜 보존적 치료를 시행하였고 수상 직후, 6주 및 12주에 초음파로 내측 불안정성의 치유 경과를 관찰하였다. 평가는 슬관절을 30도 굴곡시키고 내반 및 외반 상태에서 대퇴골의 내측f 상과의 직하부에서 경골의 골 기시부까지의 거리를 계측하였다. 결과: 초음파 이용 시 내측 상과(medial epicondyle) 직하부에 골피(cortex)가 직선으로 내려가기 시작하는 부위와 경골 근위부의 관절 연골 둥근 부분 직하부의 골피 시작 부위 사이의 거리를 계측하였고 내반 및 외반은 하지 중력 만을 이용한 생리적 상태로 초음파 시야를 조정하면서 동적으로 측정하여 항상성 있는 결과를 얻을 수 있었다. 결론: 초음파는 내측 불안정의 정도 평가 시에 내측 측부 인대의 추가 손상 없이 내반 및 외반의 동적 상태로 측정할 수 있는 방법이다.

초음파 검사를 이용한 비알코올성 지방간에 운동이 미치는 영향 평가 (Evaluation of the Effect of Exercise on Nonalcoholic Fatty Liver By Sonography)

  • 비알코올성 지방간 질환(NAFLD)은 알코올을 과도하게 섭취하지 않고도 간세포 내에 지방이 축적되어 있는 상태이며, 이는 비만과 밀접한 관련이 있다고 연구되어 지고 있다. 이 연구의 목표는 NAFLD의 위험인자를 파악하여 예방하거나 위험요인을 관리하는데 있다. 이 연구는 2011년 5월 1일부터 10월 31일까지 6개월 동안 진행되었고, 복부초음파 검사를 시행한 83명 중 연구 실험에 적합한 11명을 특별히 심사해서 선정하였다. 연구결과는 다음과 같이 나타났는데 첫째, 체중과 체질량지수에 있어 유의한 감소경향을 나타냈고, 둘째, 초음파 진단 상 지방간 심화정도의 변화에서 유의한 감소경향을 나타났으며, 셋째, 칼로리에 상관없이 꾸준한 운동을 하면 지방간 심화정도가 감소경향을 나타냈다. 따라서, 이 연구에서는 장기적인 운동 프로그램을 적용할 경우에는 지방간 치료에 긍정적인 효과를 나타낼 수 있을 것으로 추론해 본다.

The Effect of Ultrasound-guided TAPB on Pain Management after Total Abdominal Hysterectomy

  • Background: Incisional pain is particularly troublesome after hysterectomy. A method called transversus abdominis plane block (TAPB) has shown promise in managing postoperative pain. In this study, we evaluated the analgesic efficacy of ultrasound-guided TAPB after hysterectomy at different time points and at each time point separately for 48 hours. Methods: Forty-two patients (ASA I, II) who were electively chosen to undergo total abdominal hysterectomy were divided into 2 groups, control (group C) and intervention (group I). Twenty-one patients underwent TAPB (group I) and 21 patients received only the standard treatment with a fentanyl pump (group C). Both groups received standard general anesthesia. For patients in group I, following the surgery and before emergence from anesthesia, 0.5 mg/kg of ropivacaine 0.2% (about 20 cc) was injected bilaterally between the internal oblique and transverse abdominis muscles using sonography. Pain scores using the Visual Analogue Scale (VAS) and drug consumption were measured at 2, 6, 12, 24, and 48 hours after TAPB. Results: There were no significant differences in demographics between the two groups. VAS scores appeared to be lower in group I, although there was no interaction with time when we compared mean VAS measurements at different time points between group I and group C (P > 0.05). The amount of fentanyl flow was consistently higher in group C, but when we compared the two groups at each time point separately, the observed difference was not statistically significant (P < 0.053). The incidence of vomiting was 10% in group I and 28% in group C. There were no complaints of itching, and sedation score was 0 to 3. There were no complications. Conclusions: This study showed that TAPB did not result in a statistically significant decrease in VAS scores at different time points. TAPB did lead to decreased fentanyl flow, but when we compared the two groups at each time point separately, the observed difference was not statistically significant.
