Ultrasound Diagnosis of Double Crush Syndrome of the Ulnar Nerve by the Anconeus Epitrochlearis and a Ganglion

  • Double compression of the ulnar nerve, including Guyon's canal syndrome associated with cubital tunnel syndrome caused by the anconeus epitrochlearis muscle, is a very rare condition. We present a case of double crush syndrome of the ulnar nerve at the wrist and elbow in a 55-year-old man, as well as a brief review of the literature. Although electrodiagnostic findings were consistent with an ulnar nerve lesion only at the elbow, ultrasonography revealed a ganglion compressing the ulnar nerve at the hypothenar area and the anconeus epitrochlearis muscle lying in the cubital tunnel. Careful physical examination and ultrasound assessment of the elbow and wrist confirmed the clinical diagnosis prior to surgery.

  • This study was performed to investigate the effect of ultrasound irradiation on $TGF-\beta_1$ expression in the surgically injured achilles tendons of rats. The results of this study were as following: 1. The control group (3 days after injury) expressed little immunoreactivity for $TGF-\beta_1$. 2. In the experimental groups, $TGF-\beta_1$ immunoreactivity of group 11(applied US for 3 days) was increased markedly than that of group 1(applied US for 1 day). 3, The experimental group 11(applied US for 3 days) expressed higher immunoreactivity for $TGF-\beta_1$ than control group. These findings suggest that ultrasound irradiation on the injured Achilles tendon may be of benefit such as increasing $TGF-\beta$ release in the inflammatory phase of heal ins process.

Multi-vessel intractable coronary spasm development in a patient with aborted sudden cardiac death: a case study with intravascular ultrasound findings

  • Coronary spasm generally occurs in patients with minimal atherosclerotic plaque lesion, and it has a rather favorable prognosis. However, in some cases, coronary spasm may induce myocardial infarction and even sudden cardiac death (SCD). Here, we report a case in which multi-vessel intractable coronary vasospasm suddenly occurred in a diffuse atherosclerotic lesion after percutaneous coronary intervention (PCI) in a patient with aborted SCD. We identified the characteristics of the spasm portion in intravascular ultrasound (IVUS) images and conducted percutaneous cardiopulmonary bypass support-PCI with stenting as treatment. Intima and media thickening and a large attenuated plaque burden with rupture were identified in IVUS images at the obstructive spasm portion.

괴사성 림프절염에 대한 임상적 고찰 (A Clinical Consideration of Necrotizing Lymphadenitis)

  • Background and Objectives: Necrotizing lymphadenitis or Kikuchi's disease is characterized by cervical lymphadenopathy of unknown etiology with unique histologic findings in young female patients. The importance of this disease lies in the fact that it can be easily misdiagnosed as malignant lymphoma, hence, clinicians need to aware of this disease entity. The purpose of this study is to report the clinicopathologic findings, radiographic findings, and many laboratory tests in order to contribute to the diagnosis and treatment of necrotizing lymphadenitis. Materials and Methods: We evaluated 31 patients, who were diagnosed as necrotizing lymphadenitis by excisional biopsy or fine needle aspiration cytology or ultrasound guided 18G cutting needle biopsy, retrospectively. Result : The median age was 24.8 years (range 12 to 43 years) and the male to female ratio was 1 : 2.4(9:22), with 14 females (45.1%) under 30 years. The common chief complaints were neck mass, easy fatigue and fever. Lymph node enlargement was limited to the cervical area in most cases (28cases : 90.3%). The involved lymph nodes were usually multiple (20cases : 64.5%), unilateral (26cases 83.9%) and small sized. Leukopenia (19cases : 61.3%) and elevation of ESR (18cases : 58.1%) appeared most frequently in the abnormal laboratory data. These symptoms will be gone spontaneoulsy without any specific treatment in several weeks or months. Conclusion : We should consider open biopsy or fine needle aspiration cytology or ultrasound guided cutting needle biopsy with lymph node in patients who have cervical lymphadenopathy with easy fatigue and fever, especially young women to exclude other conditions such as malignant lymphoma and tuberculosis, etc.

슬관절 초음파 개론 (Overview of the Sonography of the Knee Joint)

  • 전통적으로 슬관절 질병은 주로 방사선 촬영과 자기 공명 영상에 근거하여 진단과 치료를 하고 있다. 그러나 방사선 진단은 골 내부의 변화에는 좋은 영상을 가져다 주지만 연부 조직의 영상은 잘 볼 수 없는 단점이 있고 외래 진료실에서 실시간으로 할 수 없는 단점이 있다. 또 자기 공명 영상은 고가이고 역시 실시간으로 볼 수 없으며 정적인 정보만 제공한다. 초음파는 동적인 영상을 실시간으로 외래 진료실에서 즉시 볼 수 있고 비용이 저렴하며 동적 영상을 제공하고 연조직 영상을 잘 보여준다. 슬관절에서는 연조직 및 골 표면의 변화까지 관찰이 가능하므로 진단에 중요한 역할을 한다. 이 글에서는 정상적인 구조물의 초음파 영상과 병적 소견을 방사선 또는 자기 공명 영상 소견과 연계하여 소개하고 그 용도의 개략적인 것들을 설명하였다.

지간 신경종에서 보존적 치료 및 수술적 치료의 임상적 결과 (Clinical Result of Conservative Treatment and Operative Treatment for Interdigital Neuroma)

  • Purpose: To evaluate the usefulness of ultrasound and MRI in diagnosing Morton's Neuroma, and the effect and significance of stepwise treatment. Materials and Methods: Out of all patients suspected of Morton's neuroma through history taking and physical examination, 77 patients (84 feet, 95 lesions) in whom Morton's neuroma was confirmed by ultrasound or MR imaging study or was clinically suspected with negative imaging studies, and followed up for over 3 months were included. In all cases, history taking and imaging study were done, and by comparison with operative findings of the patients, the sensitivity of ultrasound and MRI was checked. Postoperative evaluation was done using the AOFAS scale. The patient's satisfaction was also examined. Results: Morton's neuroma occurred most frequently at the $3^{rd}$ web space of the foot (56%), followed by the $2^{nd}$ web space (44%). Out of 15 cases suspected of morton's neuroma through ultrasound study, 13 were pathologically positive showing a sensitivity of 85.7%. Out of 16 cases suspected of morton's neuroma through MRI, 14 were pathologically positive showing a sensitivity of 83.3%. There was no significant difference in sensitivity between the two imaging modalities. Conclusion: In diagnosing Morton's neuroma, ultrasound examination had a similar sensitivity with MRI, therefore can be used as a screening study.

근골격계 초음파 진단기기를 활용한 견관절 통증 환자의 초음파 소견: 후향적 관찰 연구 (Musculoskeletal Ultrasound Findings of Shoulder Pain Patients: A Retrospective Observational Study)

  • Objectives The aim of this study is to retrospectively evaluate the shoulder lesions in patients experiencing shoulder pain through the use of musculoskeletal ultrasound during Korean medicine intervention treatments. Methods A total of 20 cases were collected, including biceps tendinitis (n=4), calcification (n=3), SASD bursitis (n=4), partial tear (n=2), full-thickness tear (n=1), tendinopathy (n=5), and impingement syndrome (n=1). Musculoskeletal ultrasound was used by Korean medicine doctors to perform real-time scanning and to explain the patient's condition during treatment sessions. Results The use of musculoskeletal ultrasound allowed Korean medicine doctors to perform treatments such as pharmacoacupuncture, acupotomy, and acupuncture more safely and effectively. Patients were able to better understand their conditions through real-time imaging and explanations provided by the doctors. Conclusions Musculoskeletal ultrasound has the potential to enhance the safety and efficacy of Korean medicine intervention treatments for patients with shoulder pain. Institutional or governmental support is needed to further promote the use of medical devices by Korean medicine doctors, ultimately leading to an increase in cases and clinical evidence.

Ultrasonography Findings of the Carpal Tunnel after Endoscopic Carpal Tunnel Release for Carpal Tunnel Syndrome

  • Objective: To investigate changes in the median nerve, retinaculum, and carpal tunnel on ultrasound after successful endoscopic carpal tunnel release (ECTR). Materials and Methods: This prospective study involved 37 wrists in 35 patients (5 male, 30 female; mean age ± standard deviation [SD], 56.9 ± 6.7 years) with primary carpal tunnel syndrome (CTS). An in-house developed scoring system (0-3) was used to gauge the clinical improvement after ECTR. Ultrasound was performed before ECTR, and at 1, 3, and 12 months post-ECTR. Changes in the median nerve, flexor retinaculum, and carpal tunnel morphology on ultrasound after ECTR were analyzed. Ultrasound parameters for different clinical improvement groups were compared. Results: All patients improved clinically after ECTR. The average clinical improvement score ± SD at 12 months post-ECTR was 2.2 ± 0.7. The median nerve cross-sectional area proximal and distal to the tunnel decreased at all time intervals post-ECTR but remained swollen compared to normal values. Serial changes in the median nerve caliber and retinacular bowing after ECTR were more pronounced at the tunnel outlet than at the tunnel inlet. The flexor retinaculum had reformed in 25 (68%) of 37 wrists after 12 months. Conclusion: Postoperative changes in median nerve and retinaculum parameters were most pronounced at the tunnel outlet. Even in patients with clinical improvement after ECTR, nearly all ultrasound parameters remain abnormal at one year post-ECTR. These ultrasound parameters should not necessarily be relied upon to diagnose persistent CTS after ECTR.

흉부수술전 흉막유착에 대한 초음파검사 (Ultrasound for Detecting Pleural Adhesion before Video-Assisted Thoracic Surgery)

  • 배경: 비디오흉강경수술은 폐를 허탈시켜 흉강내의 공간을 확보함으로써 시행되는데 흉막유착은 이러한 공간을 확보하는데 커다란 장애요인이 되어 폐손상 및 출혈을 유발할 수 있으며, 개흉술로 전환하는 원인이 되기도 한다. 흉강경수술을 시행하기 전에 흉막유착 유무를 확인하면 투관침의 위치선정 및 삽입방법을 포함한 수술과정을 계획하는데 매우 유용한 정보를 얻을 수 있다. 대상 및 방법: 2009년 6월부터 2009년 11월까지 비디오흉강경수술을 시행한 24명의 환자에서 수술 전에 흉막유착을 확인하기 위하여 초음파검사를 시행하고 흉강경수술시의 소견과 비교하였다. 검사부위는 투관침 삽입부위인 전액와선에서 후액와선까지의 흉부 측면을 주로 검사하였으며, 경우에 따라서는 흉부 전면이나 후면을 관찰하기도 하였다. 결과: 환자는 다한증 6예, 간질성폐병변 8예, 폐종양 5예, 종격동종양 2예, 말초폐동맥색전증 1예, 전이성폐종양 1예, Sarcoidosis 1예로 구성되었다. 초음파검사를 통해 흉막유착이 없다고 진단한 22예 중 4예는 검사부위와 동떨어진 곳에 일부 국한되어 경미한 유착이 있었으나 투관침 삽입부위와는 관련이 없었다. 흉막유착이 있다고 진단한 2예는 수술시 동일한 소견이었다. 수술중 투관침 삽입으로 인한 폐손상은 없었으며 개흉으로의 전환도 없었다. 결론: 초음파검사는 짧은 시간내에 검사가 이루어지며, 비디오흉강경수술을 계획하고 시행하는데 초음파를 이용한 흉막유착 여부의 확인은 매우 유익한 시술법이라고 사료된다.

유한차분법을 이용한 기능성 위장 장애 진단용 초음파 시스템의 개발 (Development of Ultrasound Diagnostic System for Functional Gastrointestinal Disorders using Finite Difference Method)

  • The disaster from functional gastrointestinal disorders (FGID) has detrimental impact on the quality of life of the affected population. There are, however, rare diagnostic methods for FGID. Our research group identified recently that the gastrointestinal tract well of the patients with FGID became more rigid than that of healthy people when palpating the abdominal regions overlaying the gastrointestinal tract. The objective of the current study is, therefore, to identify feasibility of a diagnostic system for FGID based on ultrasound technique, which can quantify the characteristics above. Two-dimensional finite difference (FD) models (one normal and two rigid models) were developed to analyze the reflective characteristic (displacement) on each soft-tissue layer responded after application of ultrasound signals. Based on the results from FD analysis, the ultrasound system for diagnosis of the FGID was developed and clinically tested via application of it to 40 human subjects with/without FGID who were assigned to Normal and Patient Groups. The results from FD analysis showed that the maximum displacement amplitude in the rigid models (0.12 and 0.16) at the interface between the fat and muscle layers was explicitly less than that in the normal model (0.29). The results from actual specimens showed that the maximum amplitude of the ultrasound reflective signal in the rigid models $(0.2{\pm}0.1Vp-p)$ at the interface between the fat and muscle layers was explicitly higher than that in the normal model $(0.1{\pm}0.0Vp-p)$. Clinical tests using our customized ultrasound system showed that the maximum amplitudes of the ultrasound reflective signals near to the gastrointestinal tract well for the patient group $(2.6{\pm}0.3Vp-p)$ were generally higher than those in normal group $(0.1{\pm}0.2Vp-p)$. These findings suggest that our customized ultrasound system using the ultrasound reflective signal may be helpful to the diagnosis of the FGID.
