Intraductal ultrasonography for biliary strictures

  • When diagnosing the nature of biliary strictures, it is sometimes difficult to perform non-invasive methods such as ultrasound, spiral computed imaging, magnetic resonance imaging, or endoscopic ultrasonography. Thus, treatment decisions are usually based on biopsy results. However, brush cytology or biopsy, which is widely used for biliary stenosis, has limitations owing to its low sensitivity and negative predictive value for malignancy. Currently, the most accurate method is bile duct tissue biopsy under direct cholangioscopy. On the other hand, intraductal ultrasonography administered under the guidance of a guidewire has the advantages of easy administration and being less invasive, allowing for adequate examination of the biliary tract and surrounding organs. This review discusses the usefulness and drawbacks of intraductal ultrasonography for biliary strictures.

Elastography for Breast Cancer Diagnosis: a Useful Tool for Small and BI-RADS 4 Lesions

  • The present study aimed at evaluating and comparing the diagnostic performance of B-mode ultrasound (US), elastography score (ES), and strain ratio (SR) for the differentiation of breast lesions. This retrospective study enrolled 431 lesions from 417 in-hospital patients. All patients were examined with both conventional ultrasound and elastography. Two experienced radiologists reviewed ultrasound and elasticity images. The histopathologic result obtained from ultrasound-guided core biopsy or operation excisions were used as the reference standard. Pathologic examination revealed 276 malignant lesions (64%) and 155 benign lesions (36%). A cut-off point of 4.15 (area under the curve, 0.891) allowed significant differentiation of malignant and benign lesions. ROC (receiver-operating characteristic) curves showed a higher value for combination of B-mode ultrasound and elastography for the diagnosis of breast lesions. Conventional ultrasound combined elastography showed high sensitivity, specificity, and accuracy for group II lesions (10mm${\leq}20mm$). Elastography combined with conventional ultrasound show high specificity and accuracy for differentiation of benign and malignant breast lesions. Elastography is particularly important for the diagnosis of BI-RADS 4 and small breast lesions.

연부조직 종물의 진단에서 초음파 유도하 중심부 침생검 (Ultrasound-guided Core Needle Biopsy in Diagnosis of Soft Tissue Masses)

  • 목적: 근골격계 종물의 진단에 있어 초음파 유도하 생검의 유용성을 알아보고자 하였다. 대상 및 방법: 2002년 3월부터 2003년 8월 사이에 본원 정형외과를 내원한 환자 중 연부조직 종물을 가지는 환자의 진단을 위하여 조직 생검이 필요하였던 5 5명 환자를 대상으로 하여 전향적인 연구를 하였다. 조직검사는 초음파 유도하에서 조직의 채취 부위에 국소마취하에 시행하였으며 14 gauge 또는 18 gauge의 굵은 바늘을 사용하여 5~6개의 조직을 얻을때까지 시행하였다. 통계적인 분석은 임상적으로 치료가 가능하였던 11명의 환자와 수술적 절제 후 조직의 최종 확인이 가능한 38명을 포함하여 49명을 대상으로 하였다. 결과: 불충분한 조직을 얻은 경우나 조직학적 진단이 불가능한 6명의 환자를 제외하고 정확한 진단은 49명의 환자중 47명의 환자에서 얻을 수 있어 정확도는 96%였고 민감도는 90%, 특이도는 100%였다. 전체 55명의 환자중 89%(49명)에서 조직학적 진단이 가능한 충분한 조직을 얻을 수 있었다. 모든 환자에서 특별한 부작용은 없었다. 결론: 초음파 유도하 생검은 연부 조직 종물의 정확한 위치에 바늘을 위치시킬 수 있어 연부조직 종양의 진단이나 골종양의 진단에서 골외 부분이 있는 경우 시행할 수 있는 안전하면서 유용한 방법이라 생각된다.

  • Cryptococcal pneumonia usually occurs in immunocompromised patients with malignancy, acquired immune deficiency syndrome, organ transplantations, immunosuppressive chemotherapies, catheter insertion, or dialysis. It can be diagnosed by gaining tissues in lung parenchyma or detecting antigen in blood or bronchoalveolar lavage fluid. Here we report an immunocompetent 32-year-old male patient with diabetes mellitus diagnosed with cryptococcal pneumonia after a ultrasound-guided percutaneous supraclavicular lymph node core needle biopsy. We treated him with fluconazole at 400 mg/day for 9 months according to the guideline. This is the first case that cryptococcal pneumonia was diagnosed from a percutaneous lymph node biopsy in South Korea.

Histologic evaluation of low-intensity pulsed ultrasound effects on bone regeneration in sinus lift

  • Purpose: Many techniques have been described for achieving vertical augmentation of the maxillary sinus. The aim of this study is to evaluate the effect of low-intensity pulsed ultrasound (LIPUS) to enhance bone regeneration after sinus floor elevation. Methods: The sinus lifting technique was performed through a lateral approach on 8 different sites of 5 patients (3 males and 2 females) and their mean age was 45.7 years old. The sites were randomly assigned to the control or test groups. The control group had 4 sites that received lateral sinus lifting procedure only, while the test group had 4 sites that received LIPUS application after the lateral sinus lifting procedure. 24-32 weeks (an average of 29 weeks) postoperatively, new bone formation in the augmented sinus sites was evaluated through histologic and histomorphometric analyses of the biopsy specimens obtained during implant placement. Results: In the test group, the mean percentage of newly formed bone was $19.0{\pm}2.8$%. In the control group, the mean percentage of newly formed bone was $15.2{\pm}3.1$%. The percentage of newly formed bone was approximately 4% higher in those cases where the sinus was treated by LIPUS than the percentage in those cases where it was not used. The difference was statistically significant. Conclusions: Within the scope of this study, low-intensity pulsed ultrasound application after sinus lifting appeared to have a significant effect on the development of new bone formation.

유방 초음파 영상의 컴퓨터 보조 진단을 위한 특성 분석 (Analysis of characteristics for computer-aided diagnosis of breast ultrasound imaging)

  • 지난 몇년간 유방 초음파영상을 이용한 신호 및 영상처리 기술과 자동 영상 최적화 기술, 유방 종괴 자동 검출 및 분류 기술 등, 컴퓨터 보조 진단(computer-aided diagnosis, CAD)을 활용하는 연구들이 활발히 진행되어지고 있다. 컴퓨터진단기술이 개발될수록 암의 조기 발견이 정확하고 빠르게 진행되어 건강 보험과 환자의 검사 빙용을 줄일 수 있고 조직 검사에 대한 불안감을 없앨 수 있을 것으로 기대된다. 본 논문에서는 GLCM(gray level co-occurrence matrix)을 사용하여 초음파 영상에서 종양의 정량적 분석을 진행하여 컴퓨터보조 진단에 활용 가능성을 실험하였다.

유방 초음파 팬텀의 제작과 특성에 관한 연구 (The study on Development and characteristic of ultrasound biopsy training phantom of breast)

  • 한국 여성의 유방암 발생은 전체 암 발생의 16.1%로 가장 많으며, 1995년 12.5%에서 2001년 16.1%로 증가하였고, 발생 연령군이 낮아지고 있다. 한국중앙 암 등록사업 연례 보고서, 2001(ICD_ $O_2$분류)에 따르면 유방암 등록분율은 7.1%, 사망분율은 2.1%(사망원인 통계연보, 2001)(ICD 10분류))로 나타나 조기발견의 중요성을 알 수 있다. 따라서 유방 초음파 팬텀을 이용해 주사와 생검 훈련을 하여 정밀한 유방초음파검사를 시행한다는 측면에서, 생검용 유방 초음파 팬텀을 제작하여 다음과 같은 결론을 얻었다. (1) PU를 주제로 하고 C와 $TiO_2$ 첨가제의 농도와 경도를 조정하여 인체 유방과 유사한 에코 양상을 나타내는 TMM을 합성하였으며, (2) 합성한 C와 $TiO_2$ TMM은 첨가량의 농도 변화에 따라 휘도와 대조도의 변화를 보였으며, (3) C형 TMM의 경도 증가에 따라 투과심도가 감소하는 것으로 나타났으며, (4) $TiO_2$ 형 TMM은 C형 TMM에 비해 낮은 휘도와 높은 투과도를 보였으며, (5) $TiO_2$형 TMM과 C형 TMM의 전파속도와 감쇠는 연부조직의 그것과 유사하게 나타났다. 따라서 생검용 유방초음파 팬텀은 $TiO_2$ 4.10 %w/v TMM으로, 표적물질은 C 2.09 %w/v로 제작하여 에코 양상을 평가하였다. 본 팬텀의 개발은 유방초음파검사의 주사 능력 향상은 물론 표재성장기의 TM 팬텀을 개발하는데 기여할 것이다.

Comparison of Pre-Operation Diagnosis of Thyroid Cancer with Fine Needle Aspiration and Core-needle Biopsy: a Meta-analysis

  • Background: The aim of this meta-analysis was to compare sensitivities and specificities of fine needle aspiration (FNA) and core needle biopsy (CNB) in the diagnosis of thyroid cancer. Materials and Methods: Articles were screened in Medline, the Cochrane Library, EMBASE and Google Scholar, and subsequently included and excluded based on the patient/problem-intervention-comparison-outcome (PICO) principle. Primary outcome was defined in terms of diagnostic values (sensitivity and specificity) of FNA and CNB for thyroid cancer. Secondary outcome was defined as the accuracy of diagnosis. Compiled FNA and CNB results from the final studies selected as appropriate for meta-analysis were compared with cases for which final pathology diagnoses were available. Statistical analyses were performed for FNA and CNB for all of the selected studies together, and for individual studies using the leave-one-out approach. Results: Article selection and screening yielded five studies for meta-analysis, two of which were prospective and the other three retrospective, for a total of 1,264 patients. Pooled diagnostic sensitivities of FNA and CNB methods were 0.68 and 0.83, respectively, with specificities of 0.93 and 0.94. The areas under the summary ROC curves were 0.905 (${\pm}0.030$) for FNA and 0.745 (${\pm}0.095$) for CNB, with no significant difference between the two. No one study had greater influence than any other on the pooled estimates for diagnostic sensitivity and specificity. Conclusions: FNA and CNB do not differ significantly in sensitivity and specificity for diagnosis of thyroid cancer.

Utility of Radial Probe Endobronchial Ultrasound-Guided Transbronchial Lung Biopsy in Diffuse Lung Lesions

  • Background: Radial probe endobronchial ultrasound (R-EBUS) is widely used for diagnosing peripheral pulmonary lesions. However, the utility of R-EBUS-guided transbronchial lung biopsy (TBLB) for diffuse lung lesions (DLLs) remains unknown. We designed this study to evaluate the utility of R-EBUS-guided TBLB in DLLs. Methods: This retrospective study enrolled patients admitted from January 2016 to November 2017 who underwent TBLB for DLLs. The R-EBUS-guided TBLB and blind TBLB groups were compared. DLL was defined as any lung disorder that involved more than one segment of the lung. In both the groups, fluoroscopy and guided sheath were not used during TBLB. Results: A total of 127 patients underwent TBLB for DLLs (67 patients in the R-EBUS-guided TBLB group and 60 in the blind TBLB group). There were no differences in age, sex, and comorbid illnesses between the two groups. Furthermore, there was no difference in the TBLB diagnostic yield of the two groups (p=0.660) although more samples were collected from the R-EBUS-guided TBLB group (p=0.003). Procedure time was significantly longer in the R-EBUS-guided TBLB group than in the blind TBLB group (p<0.001). Thus, incidence of pneumothorax was significantly lower in the R-EBUS-guided TBLB group than in the blind TBLB group (p=0.032). Conclusion: Diagnostic yield in DLLs did not differ between the R-EBUS-guided TBLB and blind TBLB groups. Findings show that R-EBUS-guided TBLB in DLLs may reduce risk of pneumothorax.
