Urachal Anomalies in Children (소아 요막관 기형)

  • Failure of the urachus to regress completely results in anomalies that may be classified as patent urachus, urachal sinus, urachal cyst and bladder diverticula. The presenting symptoms of children with urachal anomalies are variable and uniform guidelines for diagnosis and treatment are lacking. The purpose of this study was to analyze our experience and develop conclusions regarding the presentation, diagnosis and treatment of urachal anomalies. We retrospectively analyzed the records of 32 patients who were admitted for urachal anomalies from March 1995 to February 2005. The age distribution of these patients at presentation ranged from 1 day to 14 years old (median age 1 month). There were 20 boys and 12 girls. The 32 cases comprised 13 cases of urachal sinus (40.6 %), 10 urachal cyst (31.3 %), and 9 patent urchus (28.1 %). In 30 patients ultrasonography was used for diagnosis and 2 patients with patent urachus were explored without using a diagnostic method. Twenty-three patients were confirmed by ultrasonography alone and 7 patients were examined using additional modalities, namely, computed tomography for 2 patients with an urachal cyst, magnetic resonance imaging for 1 patient with an urachal cyst, and fistulography for 3 patients with an urachal sinus. The presenting symptoms were umbilical discharge (14 patients), umbilical granuloma (8), abdominal pain and fever (3), fever (3), abdominal pain (2), and a low abdominal mass (2). Excision was performed in 29 patients, and 3 patients were conservatively managed. Urachal anomalies in children most frequently presented in neonates, and the most common complaint was umbilical discharge with infection. Urachal anomalies can be diagnosed by a physical examination and an appropriate radiographic test. Ultrasound was the most useful diagnostic method. Complete surgical excision of an urachal anomaly is recommended to avoid recurrence, and the rare development of carcinoma.

Added Value of Structured Reporting for US of the Pediatric Appendix: Additional CT Examinations and Negative Appendectomy (소아 충수 초음파 검사에서 구조화 판독문의 부가가치: 추가 CT 검사 및 음성 충수절제술의 관점에서)

  • Purpose This study aimed to determine the incremental value of using a structured report (SR) for US examinations of the pediatric appendix. Materials and Methods Between January 2009 and June 2016, 1150 pediatric patients with suspected appendicitis who underwent US examinations of the appendix were included retrospectively. In November 2012, we developed a five-point scale SR for appendix US examinations. The patients were divided into two groups according to the form of the US report: free-text or SR. The primary clinical outcomes were compared between the two groups, including the rate of CT imaging following US examinations, the negative appendectomy rate (NAR), and the appendiceal perforation rate (PR). Results In total, 550 patients were included in the free-text group and 600 patients in the SR group. The rate of additional CT examinations decreased by 5.3% in the SR group (8.2%, p = 0.003), and the NAR decreased by 8.4% in the SR group (7.8%, p = 0.028). There was no statistical difference in the appendiceal PR (37.6% vs. 48.0%, p = 0.078). Conclusion The use of an SR to evaluate US examinations for suspected pediatric appendicitis results in lower CT use and fewer negative appendectomies without an increase in appendiceal PR.

Analysis of the Accuracy about Abdominal Ultrasonography and Clinical Chemical Test with Fatty Liver Patients on the Medical Examination (종합검진 후 지방간 환자의 복부 초음파 검사와 임상화학적 검사결과의 정확성 분석)

  • Ultrasonography is one of the fields on the imaging studies within intra-abdominal organs. A blood test is a typical method of clinical chemical examination on the in vivo test of clinical symptoms. Fatty liver is performed simultaneously two kinds of diagnostics methods, intra-abdominal ultrasound and blood tests. At an implementing this, the standard of blood tests value was the TBIL, TC, AST, ALT, ALP, GGT, TG, HDL-C, GLU. In this study analyzing the accuracy of the two kinds of test, ultrasonography and blood test, on patients with fatty liver. From January to March 2012, patients 459 determined fatty liver within 1350, who received health examination simultaneously on the intra-abdominal ultrasonography and blood tests. Same result of ultrasonography and blood test's patients were 459 ie, 60.8%. and different result of those examinations was 280 ie, 39.2%. Consequently diagnostic accuracy was 60.8%. It presumably caused by differences in the diagnostic performance of ultrasound professionals(Radiologist or Radiological Technologist). In order to overcome this, ongoing educational and study will be needed.

The Effect of Application of a Non-Elastic Fixation Belt on the Balance Ability and Fall Prevention in Elderly Women (비탄력 고정식 벨트가 노인 여성의 균형능력과 낙상예방에 미치는 영향)

  • The aim of this study was to determine the effect of a non-elastic fixation belt on the balance ability and fall prevention in elderly women. Elderly women experience reduced balance ability and an increased risk of falls due to a weakening of the surrounding sacroiliac joint and pelvic muscles during childbirth and menopause. On the other hand, specific studies are still needed. The subjects were allocated randomly to two groups: control (n=20) and experimental (n=20). The experimental group used a non-elastic fixation belt, whereas the control group had no fixation belt. The balance ability and the fall index were measured in all subjects using a balance measurement device, and the low abdominal muscle thickness was determined in the experimental group using ultrasound imaging for the exact application of the non-elastic fixation belt. The following statistical analysis was performed: an independent t-test for the general characteristics of the subjects, $2{\times}2$ analysis of variance with repeated measures for the balance and fall index score, and a paired t-test for the abdominal muscle thickness. The group ${\times}$ time interaction effect showed significant improvement in the General Stability Index (F1,38=47.24, p=0.001), Fourier Harmony Index (F1,38=88.83, p=0.001), Weight Distribution Index (F1,38=50.21, p=0.001), and Fall Index (F1,38=21.59, p=0.001). The thicknesses of the transverse abdominal (p=0.001) and internal oblique (p=0.001) muscles were increased significantly in the experimental group after using the non-elastic fixation belt. Overall, the application of a non-elastic fixation belt could be effective in improving the balance ability and fall prevention in elderly women.

Variability of Transrectal Shear Wave Elastography in a Phantom Model (팬텀연구에서 경직장 전단파탄성초음파의 가변성)

  • Purpose This study aimed to assess the variability of transrectal shear wave elastography (SWE) using a designed phantom. Materials and Methods In a phantom, the SWE values were examined by two radiologists using agarose and emulsion silicone of different sizes (1, 2, and 3 cm) and shapes (round, cubic) at three depths (1, 2, and 3 cm), two region of interest (ROI) and locations (central, peripheral) using two ultrasound machines (A, B from different vendors). Variability was evaluated using the coefficient of variation (CV). Results The CVs decreased with increasing phantom size. Significant changes in SWE values included; agarose phantom at 3 cm depth (p < 0.001; machine A), 1 cm depth (p = 0.01; machine B), emulsion silicone at 2 cm depth (p = 0.047, p = 0.020; both machines). The CVs increased with increasing depth. Significant changes in SWE values included; 1 cm agarose (p = 0.037, p = 0.021; both machines) and 2 cm agarose phantom (p = 0.047; machine A). Significant differences in SWE values were observed between the shapes for emulsion silicone phantom (p = 0.032; machines A) and between ROI locations on machine B (p ≤ 0.001). The SWE values differed significantly between the two machines (p < 0.05). The intra-/inter-operator agreements were excellent (intraclass correlation coefficient > 0.9). Conclusion The phantom size, depth, and different machines affected the variability of transrectal SWE.

Bradycardia after Dobutamine Administration in a Dog (Dobutamine 투여 후 발생한 개의 서맥 1례)

  • A 13-year-old, castrated male, Shih Tzu dog with a history of acute ataxia was referred to veterinary medical teaching hospital and anesthetized for diagnostic magnetic resonance imaging of cervical intervertebral disk disease. After preanesthetic evaluation including physical examination, blood chemistry, radiography and ultrasound, the patient was premedicated with intravenous butorphanol (0.2 mg/kg). Anesthesia was induced by intravenous propofol (6 mg/kg) and maintained with isoflurane at 1.2 minimal alveolar concentrations. Because the mean arterial pressure (MAP) decreased from 70 to 58 mmHg at 70 minutes after induction, dobutamine was administered by constant rate infusion ($5{\mu}g/kg/min$) to treat hypotension. However MAP did not increase, and heart rate rapidly decreased from 100 to 55 beats per minute (bpm). To treat bradycardia, intravenous glycopyrrolate ($5{\mu}g/kg$) was administered, and heart rate increased to 165 bpm. After extubation of endotracheal tube, the patient showed normal recovery without any problems related to cardiovascular system. Unexpected dobutamine-induced bradycardia was considered as Bezold-Jarisch reflex. It is recommended that clinicians know and prepare the possibility of bradycardia during dobutamine therapy under general anesthesia.

Extraction of Lumbar Multifidus Muscle using Ultrasound Imaging (초음파 영상에서 다열근 추출)

  • In this paper, we propose a new method for extracting muscles from lumbar images. The proposed method sets areas without distortions with field expert's assistance as areas of measuring interest and removing noises from initial ultrasonic videos. Then, the method emphasizes the brightness contrast with Ends-in search stretching algorithm and separate thoracic vertebra from subcutaneous fat area using morphological characteristics. 4-directions contour tracing algorithm is applied to extract the bottom of subcutaneous fat area. Extracting thoracic vertebra area requires noise removal and morphological characteristics as well among candidate areas obtained by controlling min-max brightness. The thickness of muscles is then defined as the length between subcutaneous fat area and extracted thoracic vertebra. The experiment which consists of 368 image analysis verifies that the proposed method is more effective in measuring the thickness of muscles than before.

Change of Muscle Thickness on Exercise Type of Core Stabilization in Aged Men (남성노인의 코어 안정화 운동 형태가 근 두께에 미치는 영향)

  • Purpose : The purpose of this study was to provide more effective interventions for elderly men with weak core muscles by measuring the thickness of the muscles according to the five core stabilization exercise and comparing the thickness differences in muscles in each posture. Methods : The study selected 29 elderly men aged 65 to 80 years old among outpatient patients at S Medical Center in B city, and measured the muscle thickness by exercise posture once. In order to find out the thickness of the external oblique (EO), internal oblique (IO), and transverse abdominis (TrA) muscles were measured by using rehabilitative ultrasound imaging (RUSI) in five exercise conditions. Results : A significant change in the thickness of the EO muscles in each group was measured by the method of motion, followed by the abdominal crunches (1.67±0.15), the lower body rotations (1.54±0.07). As a result of measuring the thickness of the IO muscles of each group according to the exercise method, the bridge group (1.14±0.22) was the highest, followed by the abdominal drawing group (1.05±0.03). As a result of measuring the thickness of the TrA muscles of each group according to the exercise method, the abdominal crunches (0.98±1.00) were the highest, and the bridge group (0.57±0.05) were higher in order of magnitude. Conclusion : Consequently, the five core stabilization exercises all affect changes in abdominal thickness and are expected to continue to require training studies on muscle posture.

Evaluation of normal masseter muscles on ultrasonography (초음파영상을 이용한 정상 교근의 평가)

  • Purpose: To assess the internal echo intensity and morphological variability of masseter muscles on ultrasonography and to establish diagnostic criterion of estimation. Materials and Methods: Participants consisted of 50 young adults (male 25, female 25) without pathologic conditions and with full natural dentitions. Sonographic examinations were done with real time ultrasound equipment as Logiq 500 (GE Medical Systems, Seoul, Korea) at 3 parts according to lines paralleling with ala-tragus line as reference line. The thickness and area of masseter muscles according to reference line in cross-sectional images were measured at rest and at maximum contraction. The visibility and width of the internal echogenic intensity of the masseter muscles were also assessed and the muscle appearance was classified into 4 types. Data were statistically analyzed by paired t-test and $x^2$-test. Results: 1. When comparing the thickness and area of masseter muscles concerning with gender, there was few significant difference between right and left sides, however, there were significant differences between males and females except for the greatest thickness of left side. 2. The changes of the greatest thickness and the area between rest and maximum contraction showed that the part of the least thickness manifested more increase at maximum contraction. 3. Each part the manifestations of the internal echogenic intensity of the masseter muscles were different depending on the locations. But there was no statistically significance. Conclusion: Changes of muscles thickness with contraction and internal echogenic intensity with locations showed great disparity within the masseter muscles, which will be diagnostic criteria for pathophysiologic and anatomic changes of masseter muscles.

Correlation between lateral abdominal, rectus femoris, and triceps brachii muscle thickness and endurance during prone bridge exercise in healthy young adults

  • Objective: Prone bridge exercise is one of the core strengthening exercise for improving abdominal, lower and upper extremity muscles. In addition, coactivation of the trunk muscles and extremities is important for treatment of low back pain. This study aimed to investigate the correlation between the thickness, cross-sectional area of the target muscle, and endurance during prone bridge exercise. Our hypothesis was that an increase in muscle thickness is positively related to the hold time for the static prone bridge exercise. Design: Cross-sectional study. Methods: Fourteen healthy university students (8 men and 6 women) voluntarily participated in the study at Sahmyook University. Hold time for the prone bridge with one and both legs was measured. The resting and contracted thickness of the lateral abdominal, rectus femoris, and triceps muscles was measured using rehabilitative ultrasound imaging. The correlation between muscle thickness and endurance for maintenance time was evaluated. Results: The prone bridge with both legs and the contraction thickness of the triceps muscle showed a positive correlation (r=0.692, p<0.05); the prone bridge with one leg and the contraction thickness of the internal oblique and transversus abdominis muscles showed a positive correlation (r=0.545, 0.574, p<0.05, 0.05, respectively). Conclusions: Endurance for the prone bridge exercise with a stable support surface is correlated with the contraction thickness of arm muscles; the prone bridge exercise with an unstable support surface is correlated with the contraction thickness of the deep abdominal muscles.
