기관사 직무 스트레스가 인적오류에 미치는 영향에 관한 연구 (A Study on the Effect of Railway Driver's Task Stress on Human Error)

  • Although the up-to-date(high) technique is developing and automating in many fields, the center of the all fields is closely connected with the human resource. Also, the railway driver is required to have both high technique and high level of mental work. Therefore, this research will grasp the types of the railway driver groups through railway driver's task stress analyze(JCQ and Job Content Questionnaire), and then analyze the human errors which the types can occur. Finally, this research will propose the way to reduce the human errors.

  • The human error or error involved with human is still the major portion of the causes of marine casualties and attracting a great concern in ship operation. However, there are not so many researches conducted to investigate or develop methodologies for analyzing such causes of human error in maritime industry, which may be caused by the variety of factors affecting the performance of ship operation and the characteristics of human being. This paper aims to study the methodologies used in investigating human factors or errors in maritime field through the investigation of researches performed so far. The methodology for human factors can be usually classified into one of three types; descriptive studies, experimental research and evaluation research. Also there are many different kinds of applied researches for some specific subjects. Prior to the investigation of the root cause of marine accident related with human error, clear concept of the human factors and systematic taxonomy shall be established. Moreover, it is very important considerations in case of casualty inspection on human factors that the inspector on the accident shall have adequate knowledges, understandings on the concept of human factors including the way of research more than those of methodologies and techniques.

A Study on a Trend of Human Error Types Observed in a Simulated Computerized Nuclear Power Plant Control Room

  • Objective: The aim of this study is to investigate a trend of human error types observed in a series of verification and validation experiments for an Advanced Control Room(ACR) equipped with Lager Display Panel(LDP), Work Station Flat Panel Display(WS FPD), list type Alarm System(AS), Soft Control(SC) and Computerized Procedure System(CPS). Background: Operator behaviors in a fully computerized control room are quite different from those in a traditional hard-wired control room. Operators in an ACR all together monitor plant status and variables through their own interface system such as LDP and WS FPD, are notified of abnormal plant status through their own list type AS, control the plant through their own SC, and follow the structured procedure through their own CPS whereas operators in a traditional control room only separately do their duty directed by their supervisor. Especially the secondary task such as manipulating the user interface of ACR can be an extra burden to all the operators including the supervisor. Method: The Reason's human error classification method was applied to operators' behavioral data collected from a series of verification and validation experiments where operators showed their plant operational behaviors under a couple of harsh scenarios using the ACR simulator. Results: As operators accustomed to the new ACR system, knowledge or rule based mistakes appearing frequently in the early series of experiments decreased drastically in the latest stage of the series. Slip and lapse types of errors were observed throughout the series of experiments. Conclusion: Education and training can be one of the most important factors for the operators accustomed to the traditional control room to be adapted to the new system and to run the ACR successfully. Application: The results of this study implied that knowledge or rule based mistakes can be reduced by training and education but that lapse type errors might be reduced only through innovative improvement in human-system interface design or teamwork culture design including a new leadership style suitable for ACR.

원자력 산업의 위반오류 발생 메커니즘 개발 및 유형 분류 (Conceptual Models of Violation Error in a Nuclear Power Plant)

  • Although many studies have been conducted to find solutions to deal with human errors effectively, violations have been rarely studied in depth. The violation is a type of human error when an employee takes an action with intention but does not intend harmful outcomes. Violations have characteristics similar to other types of human errors but it is difficult to understand the intention of an employee from accident reports. The objective of this study is to develop a conceptual model of violation errors for preventing accidents/failures in a nuclear power plant from violation errors. Based on the previous studies, the characteristics of violations were collected in 9 categories and 136 items. They were classified into three-kinds of characteristics (human-related, task-related, organization-related characteristics) to construct conceptual models of routine/situational violations. The representative cases of accidents/failures in a nuclear power plant were analyzed to derive the specific types of routine/situational violation occurrence. Three types of conceptual model for each violation were derived according to whether the basic, human-related, and task-related characteristics are included or not. The conceptual models can be utilized to develop guidelines to support employees preventing routine/situational violations and to develop supportive system in nuclear power plant.

NASA-TLX 방법에 의한 KTX 운전 직무부하 분석 (Task Load Analysis of KTX Operation by Using NASA-TLX Method)

  • Human factors still plays a significant role in railway accidents. The accidents often resulted from multiple causes of hardware failures and human errors. So to ensure the safety of railway operations, human error should be effectively prevented and managed. Among several factors influencing human performance, task load (or task complexity) is well known as a major contributor to human error. In order to reduce the potential of human error, a systematic analysis should be undertaken to evaluate task load and to reduce it by modifying task process and/or education&training. In this paper, we proposed a systematic framework for railway industry to perform task analysis and to evaluate task load, and applied it to KTX operational tasks. According to the application study, we identified 14 generic task types of KTX operation. And also this paper shows the quantitative task load of those generic tasks which were analyzed by NASA-TLX method.

디지털 기기 기반 직무 수행 시 인적오류위험성에 대한 실험적 평가 (An Experimental Evaluation on Human Error Hazards of Task using Digital Device)

  • The application of advanced Main Control Room(MCR) is accompanied with lots of changes and different forms and features through the virtue of new digital technologies. The characteristics of these digital technologies and devices give many opportunities to the interface management, and can be integrated into a compact single workstation in advanced MCR so that workers can operate the plant with minimum physical burden under any operation conditions. However, these devices may introduce new types of human errors and thus a means to evaluate and prevent such errors is needed, especially those related to characteristics of digital devices. This paper reviewed the new type of human error hazards of tasks based on digital devices and surveyed researches on physiological assessment related to human error. An experiment was performed to verify human error hazards by physiological responses such as EEG which was measured to evaluate the cognitive workload of operators. And also, the performances of four tasks which are representative in human error hazard tasks based on digital devices were compared. Response time, ${\beta}$ power spectrum rate of each task by EEG, and mental workload by NASA-TLX were evaluated. In the results of the experiment, the rate of the ${\beta}$ power was increased in the task 1 and task 4 which are searching and navigating task and memory task of hierarchical information, respectively. In case of the mental workload, in most of evaluation items, task 1 and 4 were highly rated comparatively. In this paper, human error hazards might be identified by highly cognitive workload. Conclusively, it was concluded that the predictive method which is utilized in this paper and an experimental verification can be used to ensure the safety when applying the digital devices in Nuclear Power Plants (NPPs).

Human Error Analysis in a Permit to Work System: A Case Study in a Chemical Plant

  • Background: A permit to work (PTW) is a formal written system to control certain types of work which are identified as potentially hazardous. However, human error in PTW processes can lead to an accident. Methods: This cross-sectional, descriptive study was conducted to estimate the probability of human errors in PTWprocesses in a chemical plant in Iran. In the first stage, through interviewing the personnel and studying the procedure in the plant, the PTW process was analyzed using the hierarchical task analysis technique. In doing so, PTWwas considered as a goal and detailed tasks to achieve the goal were analyzed. In the next step, the standardized plant analysis risk-human (SPAR-H) reliability analysis method was applied for estimation of human error probability. Results: The mean probability of human error in the PTW system was estimated to be 0.11. The highest probability of human error in the PTW process was related to flammable gas testing (50.7%). Conclusion: The SPAR-H method applied in this study could analyze and quantify the potential human errors and extract the required measures for reducing the error probabilities in PTW system. Some suggestions to reduce the likelihood of errors, especially in the field of modifying the performance shaping factors and dependencies among tasks are provided.

도시철도 기관사의 인적오류 위험 비교조사 및 개선방안 연구 (A Study on the Comparative Survey and Improvement of Human Error Risk Factors in Urban Railway Driver)

  • 본 논문은 서울교통공사 및 한국철도공사의 기관사를 대상으로 인적오류 위험요인과 관련 경험빈도에 대한 설문을 수행하여 기관별 운행시스템과 호선별 운행시스템 관점에서 오류유발 요인을 분석하였다. 도시철도 사고유발의 근본적인 원인 제거와 감소를 목적으로 하며 도시철도를 운영하는 시스템에서 발생하는 인적오류는 사고발생의 종류와 상황에 따라 대처하는 문제와 인지적 문제를 포함하고 있기 때문에 요인별 관점에서의 연구를 기반으로 접근하고자 한다. 인적오류를 감소시키는데 기여하여 도시철도 사고를 저감 하고 지속적으로 안전한 철도 운행이 될 수 있도록 하고자 한다.

철도 관제사 직무 유형 및 특성 분석 (Analysis of Types and Characteristics of the Railway Traffic Signaller's Tasks)

  • Railway accidents/incidents occur because of many reasons. Human error by the railway staff or personnel takes a major part of those reasons. In order to reduce and prevent railway accidents/incidents, appropriate measures should be developed to remove root causes induced in human error. A systematic study for analysing causes of human errors and for developing an effective management is necessary. Detailed analysis of the railway traffic signaller's tasks should be preferentially performed for this purpose. This paper introduces the results of analysis for the types and characteristics of the railway traffic signaller's tasks. As a result of the analysis, the railway traffic signaller's tasks can be decomposed into 24 tasks in total, and also be divided into the general task and the emergency task. The railway traffic signaller's tasks are characterized to require continual confirmation and observation activities, and sometimes immediate decision and action.

원자력 발전소 사고관리 직무의 인간신뢰도분석을 위한 수행영향인자의 선정 (Selection of Influencing Factors for Human Reliability Analysis of Accident Management Tasks in Nuclear Power Plants)

  • This paper deals with the selection of the important Influencing Factors (IFs) under accident management situations in nuclear power plants for use in the assessment of human errors. In order to achieve this goal, we collected two types of IF taxonomies, one is the full set IF list mainly developed for human error analysis. and the other is the IFs for human reliability analysis (HRA) in probabilistic safety assessment (PSA). Five sets of IF taxonomy among the full set IF list and ten sets of IF taxonomy among HRA methodologies were collected in the study. From the review and analysis of BRA IFs, we could obtain some insights for the selection of HRA IFs. By considering the situational characteristics of the accident management domain, candidate IFs are chosen. Finally, those IFs are structured hierarchically to be appropriate for the use in the assessment of human error under accident management situation. Three nuclear accidents such as TMI. Chernobyl and JCO were analysed to validate the proposed taxonomy.
