Clinical Study about Acupuncture and Moxibustion Treatment according to the different type of the Meridian Pathways of the Patients who have Shoulder Pain (견비통 환자의 경락 유형별 침구 치료에 대한 임상적 고찰)

  • Objectives : This clinical trial is to know how significant the acupuncture treatments are which were done on appropriate acupuncture-points with using method of classification by meridian pathways along the parts of the shoulder pain. Methods : 27 shoulder pain patients who participated in the clinical study in the Bukpyeoung haemalkeun oriental medical clinic which is located in Donghae city were classified 3 types(Yangmyeng, Soyang, Taeyang meridian type) according to their different parts of the shoulder pain. They have been acupunctured in their applied acupoints in the body 3 times a week for 4 weeks. Results : After 4 weeks of the treatments, there were statistically meaningful treatment effect in all index in Yangmyeng meridian type. In Soyang meridian type, there were statistically meaningful treatment effect in 'subjective symptom of shoulder pain','shoulder pain and disability index', 'visual analog scale'. In Taeyang meridian type, statistically effective treatment result was found in 'shoulder pain and disability index' Conclusions : Every type has statistically meaningful treatment effects according to the analysis of the results.

Clinical Study of Atopic Dermatitis ; the Classification of Oriental Medical Clinical type and Treatment (아토피 피부염 환자에 대한 임상적 연구; 한의학적인 임상유형분류 및 치료)

  • Objectives : Atopic dermatitis(AD) assume an remarkable clinical aspect and it s diagnosis almost depends on clinical symptoms. Therefore, we aimed to study the clinical diagnostic standard of AD for more accurate treatment. We repert as follows; Methods : For 6 months from March to August in 2000 we selected fifty outpatients who were prognosis of AD in the department of dermatology, Oriental medical hospitol, Dong-eui University. Results and Conclusions : 1. We classified of the grade, the condition of AD patient was slight and severe, by the sum of total by the clinical index of AD (diagnostic features). 2. By consulting previous oriental medical theories, we divided symptom-complex of AD into two type ; one was damp-heat type and the other was deficiency of blood- wind-dryness type. 3. Male to female ratio was 17 : 33 and the third stage, more than half of the patients were adolescents. 4. According to the results of symptom-complex of AD patients, on the first examination damp-heat type was more than deficiency of blood-wind-dryness type and in progressing treatment, the condition has been change to deficiency of blood-wind-dryness type. 5. In the lesions of AD, arm and knee were most serious skin lesions and in symptoms of AD, pruritus was most complained, and in progressing treatment, erosion and erythema were greatly improved. 6. When we measured the levels of serum Total IgE, that of thirty eight patientswere higher than that of normal, but elevation of serum IgE levels was not correlated with the severity of AD.

Case Study of Chronic Headache Patient by Oriental Medical Treatment (한방치료로 호전된 만성 두통 환자 치험 1례)

  • The purpose of this case study is to report the effect of oriental medical treatment on chronic tension-type headache. Despite the patient was treated by cervical nerve block for headache, headache was not improved. We diagnosed chronic tension-type headache according to ICHD-II(The International Classification of Headache Disorders) and Qi deficiency, dampness and phlegm by oriental differential diagnosis of symptom and signs. We applied herbal medicine, acupuncture, moxibustion and cupping therapy for hospitalization(7 days). Oriental medical treatment may have effective results in treating chronic tension-type headache that was not improved by cervical nerve block treatment. But this is a single case study, so further case-series research should be compiled.

  • Objectives: The aim of this study is to report the effects of Korean medical treatment with ByeolGab pharmacopuncture on a patient with a tension-type headache. Methods: We treated a tension-type headache using Korean medical treatment including ByeolGab pharmacopuncture for 6 days a week for 2 weeks. As a measurement, the numerical rating scale (NRS), the Henry Ford Headache Disability Inventory (HDI), and a six point Likert scale were used to confirm pharmacopuncture therapy efficacy. Results: After treatment, symptoms related to tension-type headache and other complaints were decreased and evaluation using the NRS, HDI, and six-point scale showed improvement in the patient. Conclusions: This study shows that Korean medical treatment with ByeolGab pharmacopuncture has beneficial effects on control of tension-type headache and improvement in quality of life for patients.

The Subjectivity of Attitudes Toward Life Support Care (성인의 생명연장술 태도에 관한 주관성 연구)

  • Purpose: The purpose of this study was to categorize adult's subjectivity of their attitudes towards life sustaining treatment, and thereby understand the differences among these life sustaining treatment types using Q methodology. Methods: Q-methodology, which provides a method of analyzing the subjectivity of each item, was used. Thirty selected Q-statements received from 52 adults were classified into a shape of normal distribution using a 7 point scale. The collected data was analyzed using a QUANL pc program. Result: Four types of attitudes toward life sustaining treatment were identified. Type I is called one's autonomy type. Type II is called potentiality of resuscitation type. Type III is called DNR (Do not resuscitation) type. Type IV is calledone's effort type. Conclusion: The results of the study indicate that different approaches of life support care programs are recommended based on the four types of life sustaining treatment attitudes among Korean adults.

A Study on the Optimization of Ballast Water Treatment System for Scale-up (선박평형수 처리장치의 Scale-up을 위한 조건 최적화 연구)

  • In the prior work, we studied a ballast water treatment apparatus, which is secondary pollution free by using physical treatment of shear stress. The principle of this apparatus is smashing various microorganisms by shear stress generated between stationary outer cylinder and revolving inner cylinder. Because of various magnitude of shear stress according to the inner cylinder surface type and revolution speed, an appropriate surface type and optimum revolution speed should be studied by consecutive experiment to determine the reference data for commercial apparatus. Through a comparative study of disk type and cylinder type of ballast water treatment apparatus, cylinder type is turned out to be superb to disk type. In this study, we studied to determine the superior collaboration of cylinder type, groove type and knurling type of inner cylinder to non patterned outer cylinder, and to optimize the revolution speed and flow rate according to the gap between inner cylinder and outer cylinder. As a result, we could get perfect sterilization effect at groove type under the conditions of 250 mL/min of flow rate at 8,000 rpm and 500 mL/min of flow rate at 10,000 rpm respectively.

  • Objectives : By analyzing symptoms of tinnitus, we tried to find out the relationship between deficiency and excess(虛實) and visceral pattern identification(臟腑辨證). By testing Tinnitus Handicap Inventory(THI), we evaluated the quality of life of people suffering from various aspects of tinnitus, as well as comparing treatment efficiency. Methods : 52 patients were recruited in this study who was ill with tinnitus. They wrote out the questionnaire about tinnitus and the THI. Results : The vast majority of high-grade tiny noise patients are deficiency type(虛症). The vast majority of low-grade roar patients are excess type(實症). Tinnitus persistence and feeling of ear occlusion were prominent in low-grade roar patients. low-grade roar patients suggest that overall quality of life is lower than high-grade tiny noise patients. In terms of treatment efficiency, spleen-stomach weakness(脾胃虛弱)-type, stomach heat(胃熱)-type and phlegm-fire(痰火)-type was higher than kidney essence depletion(腎精虧損)-type and liver qi depression(肝氣鬱結)-type regardless of the aspect of tinnitus. Conclusions : The present study suggests that tinnitus pattern may be helpful in differentiating patients with tinnitus, and the effectiveness of treatment can be predicted through differentiation.

Study of Crease Resistant Finish on Hemp Fabrics(Andongpo) (대마직물(안동포)의 방추가공에 관한 연구)

  • Andongpo, 100% Korean hemp fabric was treated with the glyoxale resin type finishing agent and/or the soluble urethane type finishing agent to determine the optimum process condition of the crease resistant finish and the crease recovery of treated sample fabrics was evaluated for the study. The treatment conditions for the study were 6 conditions, such as, A-1~A-6, in which A-1 was the condition of treatment glyoxale resin type finishing agent only and A-2~A-6 were the condition of treatment both glyoxale resin type finishing agent and soluble urethane type finishing agent. Among the 6 conditions, the crease recovery of the sample treated with A-4 condition was $148^{\circ}$(angle of recovery method) and grade 3.2(appearance method) and so, these samples showed the excellent crease recovery. From the result, 15g/l of the catalyst conc., 50g/l of the glyoxale resin type finishing agent cone., and 40g/l of the softner were the optimum treatment condition for the crease resistant finish of the andongpo.

A Case Report of Systemic Type Juvenile Idiopathic Arthritis with Night Fever (야간 발열을 동반하는 전신형(Systemic type) 소아기 류마티스 관절염(Juvenile Idiopathic Arthritis; JIA) 환아 증례보고)

  • Objectives: This study is to report a case that has an effective result to systemic type juvenile idiopathic arthritis patient with an oriental medicine treatment. Methods: We treated the patient with an oriental medicine and tapered down with the western medicine. We followed up the laboratory blood tests every two or three months and through telephone at least six days in a week. Results: The symptoms of systemic type juvenile idiopathic arthritis were vanished and the patient maintains his condition with oriental medicine treatment after discontinued all Disease-Modifying Antirheumatic Drugs(DMARDs). His ESR, CRP levels and other blood test results were back to normal range. Now he is in clinical remission status. Conclusions: According to the result, the oriental medicine treatment is considered to be effective on the systemic juvenile idiopathic arthritis and further studies will be needed with more cases.

Glutaric Aciduria Type I: Overview

  • Glutaric aciduria type 1 (GA1; OMIM #231670) is a rare autosomal recessive-inherited neurometabolic disorder caused by the deficiency of glutaryl-CoA dehydrogenase (GCDH), which is encoded by the GCDH gene. It results in the accumulation of glutaric acid (GA), 3-hydroxyglutaric acid (3-OH-GA), glutaconic acid, and glutarylcarnitine (C5DC). These metabolites are considered to damage the striatum through an excitotoxic mechanism. The treatments of GA1 known to date are metabolic maintenance treatment based on a low-lysine diet and emergency treatment during acute illness. However, treatment after the onset of neurological symptoms has limited effectiveness and is associated with poor outcomes, and the effect of treatment and disease course after treatment are not good. After the implementation of newborn screening, the incidence of acute encephalopathic crisis fell to 10%-20% with early diagnosis, preventative dietary management, and aggressive medical intervention during acute episodes. Recently, several cohort studies have been published on the natural course and treatment of GA1 patients. This mini review will cover the clinical symptoms, natural history, and treatment of GA1 through a literature review.
