갑상선과 후두에 발생한 다발성 원발암 1례 (A Case of Metachronous Multiple Primary Tumor Involving the Thyroid and the Larynx)

  • Multiple primary tumors in the head and neck are not uncommon, however those in the thyroid and the larynx are known to be very rare. In most cases of multiple primary tumors involving the thyroid and the larynx, lesions are observed usually simultaneously and thyroid tumors are found incidentally during the laryngeal tumor surgery. In rare cases, thyroid tumors are found metachronously after radiation therapy of laryngeal cancer. The authors recently experienced a case of multiple primary tumor involving the thyroid and the larynx, in which thyroid papillary carcinoma was the index tumor and the laryngeal squamous carcinoma was the meatachronous second tumor. Both tumors showed aggressive local extension and regional nodal meatastasis with tumor collision in the same node. The patient died of recurrent or of residual squamous carcinoma shortly after main surgical treatment index thyroid cancer.

Pheochromocytoma with Brain Metastasis: A Extremely Rare Case in Worldwide

  • Pheochromocytoma (PCC) is a neuroendocrine tumor that mainly arises from the medulla of the adrenal gland. Some PCCs become malignant and metastasize to other organs. For example, it typically involves skeletal system, liver, lung, and regional lymph nodes. However, only a few cases of PCC with brain metastasis have been reported worldwide. We report a case of metastatic brain tumor from PCC in South Korea in 2016. A 52-year-old man presented with headache, dizziness and motor aphasia. He had a medical history of PCC with multi-organ metastasis, previously underwent several operations, and was treated with chemotherapy and radiotherapy. Brain MRIs showed a brain tumor on the left parietal lobe. Postoperative pathology confirmed that the metastatic brain tumor derived from malignant PCC. This is the first report PCC with brain metastasis in South Korea.

Multi-Class Classification Framework for Brain Tumor MR Image Classification by Using Deep CNN with Grid-Search Hyper Parameter Optimization Algorithm

  • Histopathological analysis of biopsy specimens is still used for diagnosis and classifying the brain tumors today. The available procedures are intrusive, time consuming, and inclined to human error. To overcome these disadvantages, need of implementing a fully automated deep learning-based model to classify brain tumor into multiple classes. The proposed CNN model with an accuracy of 92.98 % for categorizing tumors into five classes such as normal tumor, glioma tumor, meningioma tumor, pituitary tumor, and metastatic tumor. Using the grid search optimization approach, all of the critical hyper parameters of suggested CNN framework were instantly assigned. Alex Net, Inception v3, Res Net -50, VGG -16, and Google - Net are all examples of cutting-edge CNN models that are compared to the suggested CNN model. Using huge, publicly available clinical datasets, satisfactory classification results were produced. Physicians and radiologists can use the suggested CNN model to confirm their first screening for brain tumor Multi-classification.

Retrospective review of 108 breast reconstructions using the round block technique after breast-conserving surgery: Indications, complications, and outcomes

  • Background Several oncoplastic approaches have been implemented in recent years to enhance cosmetic results and to reduce complications. The round block technique is a volume displacement technique for breast reconstruction after breast-conserving surgery (BCS). However, its indications are currently limited according to tumor location, and its cosmetic results and complications have not been clearly established. We hypothesized that the round block technique could produce favorable cosmetic results without major complications regardless of tumor location or nipple-tumor distance, below a certain resected tumor volume and tumor-breast volume ratio. Methods All breast reconstructions using the round block technique after BCS were included in this analysis. Patients' data were reviewed retrospectively to investigate complications during follow-up, and clinical photos were used to evaluate cosmetic results. The relationships of tumor location, nipple-tumor distance, tumor volume, and the tumor-breast volume ratio with cosmetic results were investigated. Results In total, 108 breasts were reconstructed. The mean resected tumor volume was 30.2±15.0 mL. The cosmetic score was 4.5±0.6 out of 5. Tumor location, nipple-tumor distance, tumor volume, tumor-breast volume ratio, radiotherapy, and chemotherapy had no significant effects on cosmetic results or complications. There were no major complications requiring reoperation. Conclusions Breast reconstruction using the round block technique after BCS can lead to good cosmetic results without major complications regardless of the tumor location, nippletumor distance, radiotherapy, or chemotherapy. Below the maximum tumor volume (79.2 mL) and the maximum tumor-breast volume ratio (14%), favorable results were consistently obtained.

대칭성 분석과 레벨셋을 이용한 자기공명 뇌영상의 자동 종양 영역 분할 방법 (Automatic Tumor Segmentation Method using Symmetry Analysis and Level Set Algorithm in MR Brain Image)

  • 본 논문은 자기공명 뇌영상을 대상으로 뇌종양 영역을 자동으로 분할하기 위한 방법을 제안한다. 정상적인 뇌영상은 좌우로 대칭인 특징을 지니는 반면에 종양이 존재하는 뇌영상은 종양세포와 부종 및 괴사로 인해 비대칭적인 특징을 가진다. 본 논문에서는 이러한 대칭성을 뇌영상내에 종양영역의 존재 유무를 판별할 수 있는 기준으로 이용한다. 대칭성 분석을 위해서 뇌영역의 윤곽선 정보를 이용해 중심축을 생성하였으며 이는 사전정보를 이용하지 않고 영상의 자체 정보만을 해석해서 중심축을 추출할 수 있다는 점에서 기존의 영상 정합을 통해 해부학적 위치 정보를 추출하고 이를 이용하여 중심축을 찾는 방법과 구별된다. 자기공명 영상에서 정상뇌의 조직은 크게 3가지 클러스터로 분할되며 각 클러스터가 포함하는 영역은 백질과 회백질영역을 포함하는 뇌 실질영역, 뇌척수액(csf)영역, 두개골, 지방 및 뇌막 영역 등으로 나뉜다. 종양이 포함된 영상은 종양과 부종 및 괴사 영역이 추가적으로 존재하며 이는 클러스터링을 이용한 분할을 통해서 구분될 수 있다. 분할된 종양 영역의 중심점은 다음 슬라이스의 종양 영역의 경계를 검출하기 위한 레벨셋 알고리즘에 적용되어 전체 볼륨의 종양 영역의 경계선을 추출하기 위한 초기 시드로 이용된다. 본 논문에서는 3차원 볼륨의 영상(슬라이스)중에서 종양 영역이 존재하는 슬라이스의 종양 영역을 분할하여 이후의 슬라이스에서는 분할작업을 수행하지 않고 영역의 경계선만 추출한다. 자카드 지수와 처리 시간의 비교 분석을 통해 기존의 방법과 비슷한 성능과 빠른 속도로 종양 영역을 분할할 수 있다는 것을 보인다.

인삼의 Crown Gall Tumor형성에 관한 연구 (Formation of Crown Gall Tumor in Panax ginseng C.A. Meyer)

  • 인삼에 이용할 수 있는 vector system의 개발연구의 일환으로 우선 Agrobacterium spp.를 인삼의 잎, 줄기 및 뿌리에 접종하여 crown gall tumor의 형성 및 탈분화 그리고 Agrobacterium spp.의 opine화합물의 이용정도등을 조사하였던 바 그 결과를 요약하면 다음과 같다. 1. Agrobacterium tumefaciens C58은 인삼의 모든 부위에서 crown gall tumor를 형성하였으나 secondary tumor나 teratoma는 형성하지 못했다. 2. Wild type Agrobacterium tumefaciens Y101, Y104, Y109는 crown gall tumor를 형성하였으며, tumor의 형태, 크기 그리고 생장 정도는 strain별로 차이가 있었다. 3. Agrobacterium tumefaciens Y194는 특히 amorphic tumor를 형성하였다. 4. 줄기에서 형성된 tumor조직에서 callus를 유기하고자 phytohormone free배지 및 2,4-D 첨가 배지에 접종한 결과 전혀 callus가 형성되지 않았다. 5. 뿌리에서 형성된 callus가 형성되긴 하였으나 출현빈도가 극히 낮았으며, 정상 조직과는 달리 2,4-D의 효과가 미미하였다. 6. Agrobacterium spp.에 의한 opone화합물의 이용능력을 조사한 결과, Agrobacterium tumefacciens Y104, Y110 과 C58은 nopaline type이었고 Y109는 octopine type이었으며, Y101은 nopaline과 octopine 어느것도 이용하지 못하였다.

폐암의 면역세포 치료: 동물 모델에서 수지상 세포를 이용한 Adjuvant Therapy 가능성 연구 (Immunocell Therapy for Lung Cancer: Dendritic Cell Based Adjuvant Therapy in Mouse Lung Cancer Model)

  • Background: The anti-tumor therapeutic effect of autologous tumor cell lysate pulseddendritic cells (DCs) was studied for non-immunogenic and immune suppressive lung cancer model. To test the possibility as an adjuvant therapy, minimal residual disease model was considered in mouse in vivo experiments. Methods: Syngeneic 3LL lung cancer cells were inoculated intravenously into the C57BL/6 mouse. Autologous tumor cell (3LL) or allogeneic leukemia cell (WEHI-3) lysate pulsed-DCs were injected twice in two weeks. Intraperitoneal DC injection was started one day (MRD model) after tumor cell inoculation. Two weeks after the final DC injection, tumor formation in the lung and the tumor-specific systemic immunity were observed. Tumor-specific lymphocyte proliferation and the IFN-${\gamma}$ secretion were analyzed for the immune monitoring. Therapeutic DCs were cultured from the bone marrow myeloid lineage cells with GM-CSF and IL-4 for 7 days and pulsed with tumor cell lysate for 18 hrs. Results: Compared to the saline treated group, tumor formation was suppressed in 3LL tumor cell lysate pulsed-DC treated group, while 3LL-specific immune stimulation was minimum. WEHI-3-specific immune stimulation occurred in WEHI-3 lysate-pulsed DC treated group, which had no correlation with tumor regression. Conclusion: The data suggest the possible anti-tumor effect of cultured DCs as an adjuvant therapy for minimal residual disease state of lung cancer. The significance of immune modulation in DC therapy including the possible involvement of NK cell as well as antigen-specific cytotoxic T cell activity induction was discussed.

Tumor Size as a Prognostic Factor in Gastric Cancer Patient

  • Purpose: The purpose of this study is to investigate the prognostic significance of tumor size for 5-year survival rate in patients with gastric cancer. Materials and Methods: A total of 1,697 patients with gastric cancer, who underwent potentially curative gastrectomy, were evaluated. Patients were divided into 4 groups as follows, according to the median size of early and advanced gastric cancer, respectively: small early gastric cancer (tumor size ${\leq}3$ cm), large early gastric cancer (tumor size >3 cm), small advanced gastric cancer (tumor size ${\leq}$ 6 cm), and large advanced gastric cancer (tumor size >6 cm). The prognostic value of tumor size for 5-year survival rate was investigated. Results: In a univariate analysis, tumor size is a significant prognostic factor in advanced gastric cancer, but not in early gastric cancer. Multivariate analysis showed that tumor size is an independent prognostic factor for 5-year survival rate in advanced gastric cancer (P=0.003, hazard ratio=1.372, 95% confidence interval=1.115~1.690). When advanced gastric cancer is subdivided into 2 groups, according to serosa invasion: Group 1; serosa negative (T2 and T3, 7th AJCC), and Group 2; serosa positive (T4a and T4b, 7th AJCC), tumor size is an independent prognostic factor in Group 1 (P=0.011, hazard ratio=1.810, 95% confidence interval=1.149~2.852) and in Group 2 (P=0.033, hazard ratio=1.288, 95% confidence interval=1.020~1.627), respectively. Conclusions: Tumor size is an independent prognostic factor in advanced gastric cancer irrespective of the serosa invasion, but not in early gastric cancer.

척수 종양 654예의 임상 분석(1973-1999) (Spinal Cord Tumors : An Analysis of 654 Cases(1973-1999))

  • Objective : The spinal cord tumors(including vertebral tumors) are increasingly diagnosed and operated due to development of refined diagnostic and therapeutic tools. It is necessary to re-evaluate clinical features and surgical results of spinal cord tumors with increasing cases and developing treatment modalities. The authors reviewed the spinal cord tumor cases to evaluate their clinical characteristics. Material and Methods : The retrospective review of 654 cases of spinal cord tumors between 1973 and 1999 was done. The clinical features, pathological analysis and surgical results were analyzed and compared to the literature. The results of the study are analyzed with a more detailed consideration of each of major pathologies : neurogenic tumors, meningeal tumors, neuroepithelial tumors, and metastatic tumors. Results and Conclusion : The spinal cord tumor was most common in the 5th decade of age(145 cases, 22.1%) and 78 cases(11.9%) were found in children under 15 years of age. The ratio of male to female was 1.2 : 1. The pathologic diagnosis was neurogenic tumor in 266 cases(40.7%), neuroepithelial tumor in 131(20.0%), metastatic tumor in 118(18.0%), and meningeal tumor in 94(14.4%) in the order of frequency. The tumor was located most frequently in the thoracic area(36.5%) and in the intradural extramedullary space(38.1%). The most common initial presentation was pain(40.1%) and the mean duration for presentation to operation was 14.8 months. The total or gross total removal was possible in 404 cases(61.7%) and the surgical result on the postoperative one month was recovery or improvement in 424 cases(64.8%), stationary in 188(28.7%), progression in 42(6.4%). As a surgical complication, there was a spinal deformity(12 cases), wound infection(5 cases), aspiration pneumonia(5 cases) etc. Neurogenic tumors and menigiomas showed good surgical results, whereas neuroepithelial tumors(except ependymoma) and metastatic tumors showed relatively poor prognosis.

Brachial Plexus Tumors in a Consecutive Series of Twenty One Patients

  • Objective : This is a retrospective review of 22 surgically treated benign and malignant tumors of brachial plexus region to describe clinical presentation, the characteristics of brachial plexus tumor and clinical outcomes with a literature review. Methods : Twenty-one patients with consecutive 22 surgeries for primary brachial plexus tumors were enrolled between February 2002 and November 2011 were included in this study. The medical records of all patients were reviewed. Results : Eleven male and 10 female patients were enrolled. Mean age was 39 years. Three patients had brachial plexus tumor associated with neurofibromatosis (13.6%). Presenting signs and symptoms included parenthesis and numbness (54.5%), radiating pain (22.7%), direct tenderness and pain (27.2%), palpable mass (77.3%). Twelve patients presented preoperative sensory deficit (54.5%) and 9 patients presented preoperative motor deficit (40.9%). Twenty tumors (90.9%) were benign and 2 tumors (9.1%) were malignant. Benign tumors included 15 schwannomas (68.2%), 4 neurofibromas (18.2%) and 1 granular cell tumor (4.5%). There were 1 malignant peripheral nerve sheath tumor (MPNST) and 1 malignant granular cell tumor. Gross total resection was achieved in 16 patients (72.7%), including all schwannomas, 1 neurofibroma. Subtotal resection was performed in 6 tumors (27.3%), including 3 neurofibromatosis associated with brachial plexus neurofibromas, 1 MPNST and 2 granular cell tumor in one patient. Conclusion : Resection of tumor is the choice of tumor in the most of benign and malignant brachial plexus tumors. Postoperative outcomes are related to grade of resection at surgery and pathological features of tumor.
