국내 결핵환자 발생 현황 고찰(2010-2018) (A Study on the Current Status of Tuberculosis Patients in Korea(2010-2018))

  • 국내 결핵 환자 관리의 방안을 모색하고 결핵 정책의 기초자료를 제공하고자 2010년부터 2018년까지 통계청과 질병관리본부의 결핵 환자 통계 연보와 세계보건기구(WHO; World Health Organization)의 GLOBAL Tuberculosis Report 등의 자료를 수집하여, 국내 결핵 환자 현황을 분석하였다. 결과는 다음과 같다. 첫째, 국내 결핵 환자 발생 현황 결과는, 2018년 33,796명(65.9%), 신환자는 2018년 26,433명(51.5%)으로 약 21.3% 감소한 것으로 나타났다. 둘째, 국내 결핵 환자치료 현황은 과거 치료결과 불명확은 2018년 655명으로 2014년 대비 약 160% 증가한 것으로 나타났다. 셋째, 청소년 결핵환자 발생률은 10에서 14세는 2018년 1.8%, 15세에서 19세는 15.0%로 고등학생의 결핵 발생률이 높았다. 넷째, 나이별 결핵 환자 발생률을 보면 60대 이상의 대상자가 60% 이상의 결핵 유병률을 보였으며 특히 80세 이상의 노인결핵 환자가 증가하였다. 다섯째, 성별 결핵 환자발생률 현황은 2010년 대비 2018년에 남성이 여성보다 1.4배가 많은 것으로 나타났다. 여섯째, 전국적으로 결핵 유병률은 전남 1,419명(75.6%)이 가장 높았고, 세종 99명(33.4%)이 가장 낮았다. 일곱째, 국내 외국인 환자 수는 2012년 1,510명으로 지속적으로 늘어나 2016년에는 2,569명으로 증가한 것으로 나타났다. 이상의 결과에 따라 결핵 재발자, 고등학생, 60세 이상의 연령층, 80세 이상의 노인, 군대 등의 남성 집단, 외국인 대상의 신속하고 정확한 결핵 조기 검진정책과 관리가 필요하다.

Factors Associated with Health-Related Quality of Life in Tuberculosis Patients Referred to the National Research Institute of Tuberculosis and Lung Disease in Tehran

  • Background: In tuberculosis (TB) patients, health-related quality of life (HRQoL) is significant in self-management, which in turn can be effective in therapeutic acceptance and prevention of treatment failure due to multi-drug resistant TB. This study was conducted to evaluate HRQoL and associated factors in TB patients referred to the National Research Institute of Tuberculosis and Lung Disease (NRITLD). Methods: In this study, patients were selected from TB clinics of the NRITLD in Tehran. In addition to an Iranian version of the Short-Form Health Survey (SF-36), demographic and disease characteristic questionnaires were used for data collection. The data were then analyzed using SPSS software. Results: Two hundred five TB patients, with the average age of $42.33{\pm}17.64$ years, participated in this study. The HRQoL scores in different domains ranged from $14.68{\pm}11.60$ for role limitations due to emotional problems to $46.99{\pm}13.25$ for general health perceptions. The variables of sex, marital status, education, job status, place of residence, and cigarette smoking, influenced the HRQoL scores in different dimensions. Conclusion: According to the study findings are the important variables that influenced the HRQoL of TB patients. The consideration of its can improve the HRQoL of TB patients.

Tuberculosis Relief Belt Supporting Project (Tuberculosis Patient Management Project for Poverty Group)

  • Background: The "Tuberculosis Relief Belt Supporting Project (Tuberculosis Patient Management Project for Poverty Groups)" is a national program for socioeconomically vulnerable tuberculosis (TB) patients. We sought to evaluate the clinical and socioeconomic characteristics of poverty-stricken TB patients, and determined the need for relief. Methods: We examined in-patients with TB, who were supported by this project at the National Medical Center from 2014 to 2015. We retrospectively investigated the patients' socioeconomic status, clinical characteristics, and project expenditures. Results: Fifty-eight patients were enrolled. Among 55 patients with known income status, 24 (43.6%) had no income. Most patients (80%) lived alone. A total of 48 patients (82.8%) had more than one underlying disease. More than half of the enrolled patients (30 patients, 51.7%) had smear-positive TB. Cavitary disease was found in 38 patients (65.5%). Among the 38 patients with known resistance status, 19 (50%) had drug-resistant TB. In terms of disease severity, 96.6% of the cases had moderate-to-severe disease. A total of 14 patients (26.4%) died during treatment. Nursing expenses were supported for 12 patients (20.7%), with patient transportation costs reimbursed for 35 patients (60%). In terms of treatment expenses for 31 people (53.4%), 93.5% of them were supported by uninsured benefits. Conclusion: Underlying disease, infectivity, drug resistance, severity, and death occurred frequently in socioeconomically vulnerable patients with TB. Many uninsured treatment costs were not supported by the current government TB programs, and the "Tuberculosis Relief Belt Supporting Project" compensated for these limitations.

북한이탈청소년의 결핵에 대한 지식 및 태도가 결핵예방행위에 미치는 영향 (The Effects of Knowledge and Attitudes related to Tuberculosis on Tuberculosis Prevention Behaviors in North Korea Refugee Adolescents)

  • Purpose: The purpose of this study was to identify the factors affecting tuberculosis prevention behaviors in North Korean adolescent refugees. Methods: From September 16 to October 8, 2019, a survey was conducted to collect data from 143 North Korean adolescent refugees aged 13 to <25 at alternative and general schools in Seoul and Gyeonggi Province. The scale developed by Park (2008) and adapted by Cha (2012) was used to rate tuberculosis prevention behaviors in daily life with the aim of preventing high school students from getting infected with tuberculosis. The collected data were processed through frequency analysis, descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and multiple linear regression, using SPSS 25.0. Results: The mean score was 13.65 for knowledge of tuberculosis, 44.23 for attitudes towards tuberculosis, and 41.33 for tuberculosis prevention behaviors. Significant differences were found according to gender, country of origin, duration of stay in South Korea, tuberculosis examination, pre-entry chest x-ray, education about tuberculosis, alcohol intake, and subjective health status. A positive correlation was found between knowledge, attitudes, and prevention behaviors. The factors affecting tuberculosis prevention behaviors were attitudes (β=.38, p<001) and subjective health status (β=.26, p<.001). Conclusion: It is necessary to provide attitude-based education that can reinforce positive attitudes in order to improve North Korean adolescent refugees' tuberculosis prevention behaviors. This is expected to help them pay more attention to health care and become better at prevention behaviors. It is, therefore, necessary to develop a tuberculosis prevention education program with the aim of making them more knowledgeable about tuberculosis and providing them with systematic and sustainable education.

Current Status of Fluoroquinolone Use for Treatment of Tuberculosis in a Tertiary Care Hospital in Korea

  • Background: Fluoroquinolones are considered important substitutes for the treatment of tuberculosis. This study investigates the current status of fluoroquinolone for the treatment of tuberculosis. Methods: In 2009, a retrospective analysis was performed at one tertiary referral center for 953 patients diagnosed with tuberculosis. Results: A total of 226 patients (23.6%), who received fluoroquinolone at any time during treatment for tuberculosis, were enrolled in this study. The most common reasons for fluoroquinolone use were adverse events due to other anti-tuberculosis drugs (52.7%), drug resistance (23.5%), and underlying diseases (16.8%). Moxifloxacin (54.0%, 122/226) was the most commonly administered fluoroquinolone, followed by levofloxacin (36.3%, 82/226) and ofloxacin (9.7%, 22/226). The frequency of total adverse events from fluoroquinolone-containing anti-tuberculosis medication was 22.6%, whereas fluoroquinolone-related adverse events were estimated to be 2.2% (5/226). The most common fluoroquinolone-related adverse events were gastrointestinal problems (3.5%, 8/226). There were no significant differences in the treatment success rate between the fluoroquinolone and fluoroquinolone-$na{\ddot{i}}ve$ groups (78.3% vs. 78.4%, respectively). Conclusion: At our institution, fluoroquinolones are commonly used for the treatment of both multidrug-resistant tuberculosis and susceptible tuberculosis, especially as a substitute for adverse event-related drugs. Considering the low adverse event rates and the comparable treatment success rates, fluoroquinolones seem to be an invaluable drug for the treatment of tuberculosis.

초등교사의 결핵 예방행위와 영향요인 (Influencing Factors in Preventive Behavior of Tuberculosis among Elementary School Teachers)

  • Objectives: The purpose of this study was to explore and describe the influencing factors in preventive behavior of tuberculosis among elementary school teachers. Methods: Participants were 151 teachers working at 3 elementary schools in Kimhae City, 3 in Changwon City and 2 in Miryang City. Data were collected with structured questionnaires from May. 8. to Jun. 1. in 2018. Data were analyzed by SPSS/WIN 21.0 program for descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and stepwise multiple regression. Results: The preventive behavior of tuberculosis among elementary school teachers were significantly positive correlated with knowledge(r=.275, p=.001), attitude(r=.493, p<.001) and self-efficacy(r=.280, p=.001). Moreover, The significant factors of the preventive behavior of tuberculosis were attitude(${\beta}=.341$, p<.001), gender(${\beta}=.407$, p<.001), smoking status(${\beta}=.210$, p=.004), self-efficacy(${\beta}=.187$, p=.005) and knowledge(${\beta}=.145$, p=.026). Conclusions: In this study, in order to promote the preventive behavior of tuberculosis among elementary school teachers, it is necessary to find ways to increase knowledge, attitude, and self-efficacy of tuberculosis, which are significant influencing factors. In addition it would be possible to develop more effective and efficient nursing intervention program if the individual characteristics of the subjects such as gender and smoking status are considered.

결핵 환자의 지식, 스트레스 및 자가간호수행의 관계 (A Correlational Study on the Knowledge, Stress and Self-care Performance among Tuberculosis Patients)

  • Purpose: This study was conducted to analyze the correlations among the knowledge, stress, and self-care performance in tuberculosis patients. Methods: The participants consisted of 140 outpatients who had been diagnosed with tuberculosis in S University Hospital. Data were derived from self-report questionnaires and analyzed by SPSS 22.0. Statistical analysis included descriptive statistics and Pearson's correlation coefficients analysis. Results: Means of stress and self-care performance were 1.76 and 3.21, respectively. Percentage of correct answer of knowledge about tuberculosis was 71.8. Knowledge about tuberculosis was significantly different according to age (F=21.81, p<.001), marital status (F=13.79, p<.001), education (F=36.63, p<.001), and monthly income (F=16.23, p<.001). Stress was significantly different according to gender (t=3.58, p<.001). Self-care performance was significantly different according to age (F=5.74, p=.004), marital status (F=8.79, p<.001), education (F=9.15, p<.001), monthly income (F=7.87, p=.001), and attendance of tuberculosis education (t=2.34, p=.020). Self-care performance had significant correlation with knowledge about tuberculosis and stress. Conclusion: This study suggests that knowledge about tuberculosis and stress had significant correlations with self-care performance. Therefore, the nursing strategies of increasing knowledge about tuberculosis and decreasing stress which improve self-care performance should be developed for tuberculosis patients.

Incidence of Active Tuberculosis within One Year after Tumor Necrosis Factor Inhibitor Treatment according to Latent Tuberculosis Infection Status in Patients with Inflammatory Bowel Disease

  • Background: We investigated the incidence of active tuberculosis among patients with inflammatory bowel disease (IBD) treated with tumor necrosis factor (TNF) inhibitors, with or without latent tuberculosis infection (LTBI). Methods: The study was performed at a Korean tertiary referral center between January 2011 and June 2017. In total, 740 patients with IBD who underwent LTBI screening tests and were followed-up for ${\geq}1$ year after TNF inhibitor treatment initiation were enrolled. LTBI was detected on the basis of tuberculin skin test results, interferon-gamma release assay results, chest X-ray findings, and previous tuberculosis treatment history. The patients were classified into LTBI (n = 84) or non-LTBI (n = 656) group. The risk of developing tuberculosis in each group was assessed on the basis of standardized incidence ratio (SIR) and 95% confidence interval (CI) for active tuberculosis. Results: Mean patient age was 33.1 years, and patients with Crohn's disease were predominant (80.7%). Within 1 year after the initiation of TNF inhibitor treatment, 1 patient in the LTBI group (1/84; 1.2%) and 7 patients in the non-LTBI group (7/656; 1.1%) developed active tuberculosis. The overall 1-year incidence of tuberculosis among the patients was significantly higher than that among the general population (SIR, 14.0; 95% CI, 7.0-28.0), and SIR was not affected by LTBI status (LTBI group: 14.5, 95% CI, 2.0-102.6; non-LTBI group: 14.0, 95% CI, 6.7-29.4). Conclusion: Patients with IBD undergoing TNF inhibitor treatment showed a higher 1-year incidence of tuberculosis than the general population irrespective of LTBI status.

결핵 발병과 CD44 유전자 다형성사이의 연관성 연구 (Association between Tuberculosis Case and CD44 Gene Polymorphism)

  • 결핵균에 의한 만성 세균성 감염인 결핵은 결핵균의 특성, 숙주의 면역상태와 유전적 감수성의 차이에 의해 잠복성과 활동성으로의 진행정도에 차이가 있다. 결핵균에 대한 숙주 방어 기전은 주로 대식세포, T 세포 및 수지상 세포 사이의 상호 작용에 기인한다. CD44는 결핵균에 감염되면 활성 T 세포에서 발현되며 림프구 이동을 조절한다. 또한 CD44는 ECM에 대한 백혈구의 부착을 매개하여 대식세포, CD4+ T cell 등을 폐로 불러모으는 역할을 한다. 따라서, CD44 유전자의 다형성은 결핵균에 대한 숙주세포의 면역기전 저하를 유발할 수 있다. 이 연구의 목적은 CD44 유전자의 유전자 다형성이 결핵의 감수성에 영향을 미치는지 조사하는 것이다. 결핵균과 CD44의 연관성에 대하여 대한 한국 협회 자원의 443명의 cases와 3228명의 control을 이용하여 CD44 유전자의 237개의 SNP를 분석하였다. 이 중 17개의 SNP가 결핵과 통계적으로 유의한 관련성을 보였다. 가장 유의성 있는 SNP는 rs75137824였다(OR=0.231, CI: 1.51~3.56, $P=1.3{\times}10^{-4}$). 또한 결핵 발병에 유의성이 있는 SNP중 rs10488809의 경우는 전사인자 JUND 및 FOS에 결합하는 부위로써 CD44 유전자 발현에 영향을 줄 수 있는 것으로 확인할 수 있었다. 이러한 결과는 결핵 발병이 CD44 발현 차이에 의한 숙주 면역반응에 차이에 의해서 감수성의 차이가 있을 수 있음을 나타 낼 수 있다. 이번 연구 결과는 결핵균 감염에 대한 숙주면역의 유전적 차이가 결핵 진행정도의 차이를 유발할 수 있다는 유전적 배경에 대한 기반을 마련해 줄 수 있을 것이라고 기대한다.
