Nontuberculous Mycobacterial Lung Disease Caused by Mycobacterium simiae: The First Reported Case in South Korea

  • This is a report of the first South Korean case of a lung disease caused by Mycobacterium simiae. The patient was a previously healthy 52-year-old female. All serial isolates were identified as M. simiae by multi-locus sequencing analysis, based on hsp65, rpoB, 16S-23S rRNA internal transcribed spacer, and 16S rRNA fragments. A chest radiography revealed deterioration, and the follow-up sputum cultures were persistently positive, despite combination antibiotic treatment, including azithromycin, ethambutol, and rifampin. To the best of our knowledge, this is the first confirmed case of a lung disease caused by M. simiae in South Korea.

  • 규폐증은 유리규산에 노출될 수 있는 다양한 작업장에서 발생할 수 있는 질환으로 호흡성 분진의 축적에 의한 폐실질의 병리학적 변화를 가져오는 대부분 만성적인 경과를 보이는 질환이다. 하지만 드문 경우에 노출된 분진의 강도와 노출기간에 따라 임상양상이 급격하게 진행되는 경우가 있을 수 있다. 저자들은 진폐증으로 진단하였던 환자가 약 2개월 동안의 급격한 방사선학적 소견의 진행을 보여, 속립성 결핵과의 감별을 위해 시행한 경기관지 폐생검을 통하여 진단하였던 가속형 규폐증 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Hafnia alvei에 의한 균혈증 1예 (A Case of Bacteremia by Hafnia alvei)

  • Hafnia alvei is a gram-negative facultatively anaerobic bacillus that is rarely isolated from human specimens and is rarely considered to be pathogenic. It has been associated with gastroenteritis, pneumonia, meningitis, bacteremia, and nosocomial wound infections. But, only one case of extraintestinal H. alvei infection was documented in Korea to our knowledge. A 74-years old women with bronchial tuberculosis and cerebral infarct was admitted to out hospital via the internal medicine department due to the use of anti-tuberculotic agents. The two consecutive blood cultures yielded H. alvei. The organism was susceptible to all antimicrobial agents tested, except ampicillin and amoxacillin-clavulanic acid. The patients was treated with ceftriaxone and recovered effectively.

  • The textbook of insurance medicine is very bulky volume and it's revision time may be long. Nowadays medical knowledge and evidences are developing rapidly. It is necessary to revise current insurance risk of certain disease. Review of respiratory diseases in terms of insurance medicine may be valuable information for insurance doctors and life underwriters. Newly estimated mortality ratio and excess death rate of several respiratory diseases in this review are organizing pneumonia, 266%/44‰; multi-drug resistant tuberculosis, 1200%/110‰; idiopathic interstitial pneumonia, 869%/85‰; VATS lobectomy of stage I lung cancer, 550%/33‰; lymphangioleiomyomatosis 9826%/66‰; lung transplantation 2026%/92‰, respectively.

Intracranial Calcification Caused by a Brain Abscess : A Rare Cause of Intracranial Calcification

  • Intracranial calcifications are relatively common computed tomographic findings in the field of neurosurgery, and cysticercosis, tuberculosis, HIV, and cryptococcus are acquired intracranial infections typically associated with calcifications. However, intracranial calcification caused by a bacterial brain abscess is rare. Here, we present a rare case of intracranial calcification caused by a bacterial brain abscess, from which staphylococcus hominis was isolated. To the best of our knowledge, no previous report has been published on intracranial calcification caused by bacterial brain abscess after decompressive craniectomy for traumatic brain injury. In this article, the pathophysiological mechanism of this uncommon entity is discussed and relevant literature reviewed.

A Case of Typhlitis Developed after Chemotherapy with Irinotecan and Cisplatin in a Patient with Small Cell Lung Carcinoma

  • Typhlitis is a necrotizing colitis that usually occurs in neutropenic patients and develops most often in patients with hematologic malignancies such as leukemia and lymphoma. Typhlitis may proceed to bowel perforation, peritonitis and sepsis, which requires immediate treatment. Irinotecan is a semisynthetic analogue of the natural alkaloid camptothecin which prevents DNA from unwinding by inhibition of topoisomerase I. It is mainly used in colon cancer and small cell lung carcinoma (SCLC), of which the most common adverse effects are gastrointestinal toxicities. To the best of our knowledge, no case of typhlitis after chemotherapy with a standard dose of irinotecan in a solid tumor has been reported in the literature. We, herein, report the first case of typhlitis developed after chemotherapy combining irinotecan and cisplatin in a patient with SCLC.

  • The cellular change of the pulmonary tuberculous lesions may be divided into two groups,exudative and proliferative form by their course and fate. In the most cases, the patients usually have very much complex type of cellular changes. Therefore, the shadows of the chest films in pulmonary tuberculosis are also much variable in nature. And Daniel said that knowledge of the pathology of tuberculosis and an appreciation of the method of progression and healing are essential to proper interpretation of the films. Author, having reviewed 33 cases of resected tuberculous lung obtained in N.M.T.H. for one year from Oct. '75 to Sep. '76 by surgical managements, classified the Pathological findings such as: 1) caseation only, 2) tuberculoma, 3) atelectatic lung 4) cavitary lesion and 5) atelectasis with cavity, and examined the relationship between the roentgenological characteristics of the chest films and the pathological process of tuberculous lesions of the resected lungs, The result were obtained as follows. (1) Tuberculoma was commonly appeared in $S_2$ segment in right and $S_6$ segment in left. (2) Atelectasis and destroyed lung were more commonly appeared in left lung than right, and their containing rate of cavity was 82%. (3) Cavities were mostly appeared in $S_1$ and $S_2$ segments of both lung and the appearance-rate of cavity on $S_6$ segment was higher in left than right. And among the cavitary lesions of the resected lung, cavity was not seen in the preoperative chest films in 22%. (4) The configuration, thickness and sharpness of the walls of cavities, which revealed the cavitary shadows in the preoperative chest films, were mostly depended on the degree of increased collagenous fiber of the wall, existence of perifocalitis, and more or less of the caseous masses on the inner surface of the cavity wall.

  • Novel influenza A (H1N1) virus is a common pathogen of febrile respiratory infection recently. Here, we report the case of a 63-year-old male patient who presented with 3 days' ongoing cough and fever. He was diagnosed with novel influenza A (H1N1) pneumonia by real-time reverse-transcriptase-polymerase-chain-reaction (rRT-PCR). During treatment for novel influenza A (H1N1), his symptoms and radiologic findings improved initially, but multiple lung nodules developed subsequently and found on chest x-ray (on the 5th hospital day). Mycobacterium abscessus was isolated repeatedly from sputum and bronchoalveolar lavage fluid. To our knowledge, this is the first reported case of Mycobacterium abscessus lung disease in a patient with H1N1 influenza pneumonia.

  • 약제 유발성 BOOP의 경우는 약을 중단하여도 증상과 방사선학적 소견이 호전되지 않을 수 있고 부신피질스테로이드를 사용한 후 뚜렷한 호전을 보일 수 있기 때문에 BOOP를 일으킬 수 있는 약제에 대한 인지가 진단과 치료에 중요하다. 저자들은 carbamazepine에 의한 BOOP 1예를 경험하였기에 보고하는 바이다.
