Takotsubo Cardiomyopathy Caused by Pulmonary Tuberculosis: A Case Report

  • Takotsubo cardiomyopathy (TTC) is defined as a reversible, acute ventricular dysfunction without any evidence of coronary artery obstruction. There have been reports of TTC caused by emotional or physical stress, drug use, hormone imbalance, or medical conditions such as pulmonary disease, sepsis, and trauma, but a relationship between TTC and pulmonary tuberculosis has not previously been reported. From our knowledge, this is the first report of TTC caused by pulmonary tuberculosis.

  • Knowledge of molecular pathogenesis of non-small cell lung cancer has increased remarkably and changed the principles of treatment, especially during the past decade. These advancements have been limited mainly to adenocarcinoma of the lung. Recently, genetic alterations in squamous cell lung cancer (SQCLC) have been detailed and positive results of clinical trials using agents targeting these changes have indicated the potential for improved treatment outcomes for SQCLC.

  • Purpose: Tuberculosis is an infectious condition with a high disease burden, and the stigma in patients with tuberculosis causes negative health outcomes. The purpose of this study was to define and clarify the concept of self-stigma among patients with tuberculosis. Methods: The analysis was conducted using Walker and Avant's conceptual approach. Twenty-seven studies met the selection criteria. Results: Self-stigma in patients with tuberculosis can be defined by the following attributes: 1) self-esteem decrement; 2) fear; 3) negative emotions to oneself; 4) social withdrawal; and 5) discrimination. The antecedents identified were 1) inappropriate knowledge of tuberculosis, 2) spread of improper health information through media and social communications, 3) stereotypes and prejudices, 4) visibility due to symptoms appearing, 5) recognizing the risk of infection, and 6) low financial status. The consequences were 1) concealing the disease, 2) treatment delay, 3) poor treatment adherence, 4) poor quality of life, and 5) deterioration in or lack of social activities. Conclusion: The definition and attributes of self-stigma identified by this study can be applied to enhance the understanding of stigma in tuberculosis patients and to improve communications between healthcare providers and researchers. It can also be used to develop theories and measurements related to stigma in patients with tuberculosis.

간호대학생의 잠복결핵 감염관리에 대한 인식도와 수행도, 예방행위 (Awareness, Performance, and Preventive Behavior of Latent Tuberculosis Infection management among Nursing students)

  • 본 연구는 간호대학생의 잠복결핵 감염관리에 대한 지식, 인식도와 수행도, 예방행위와의 관계를 파악하고, 예방행위에 영향을 미치는 요인을 파악하고자 시도되었다. 연구대상자는 D광역시에 소재하는 2개 대학에 재학 중인 191명이었다. 자료수집을 위한 설문조사는 2020년 8월 31일부터 11월 30일까지 실시되었다. 자료분석은 SPSS/21.0 프로그램을 이용하여 t-test, ANOVA, Pearson's correlation coefficient, stepwise multiple regression을 이용하였다. 연구결과 대상자의 잠복결핵 감염관리 예방행위와 수행도(r=.21, p=.003), 인식도와 수행도(r=.88, p<.001)는 유의한 양의 상관관계가 있는 것으로 나타났다. 잠복결핵 감염관리에 대한 예방행위에 영향을 미치는 요인은 학년(β=.29, p<.001), 최종 교육시기(β=.19, p=.004)였으며, 예방행위에 설명력은 11.9%로 나타났다. 본 연구결과에 의하면 간호대학생의 잠복결핵 감염관리에 대한 예방행위를 높이기 위해서는 학년과 최종 교육시기에 따른 잠복결핵 감염관리 교육프로그램 개발이 요구된다.

응급실 간호사의 결핵감염관리에 대한 인식과 수행 (Emergency Nurses' Perception and Performance of Tuberculosis Infection Control Measures)

  • Purpose: The purpose of this study was to investigate the perception and performance of tuberculosis infection control measures among emergency department nurses. Methods: A structured questionnaire was administered to a convenience sample of 227 emergency department nurses from 20 hospitals in B City. Results: The mean perception score was 4.52 out of a possible 5, and the mean performance score was 3.31 out of a possible 5. There were big differences between the perception and performance score on the items regarding wearing a gown or mask, while there were small differences on the items regarding dealing with patients' articles or medication. Conclusion: Emergency nurses showed high perception and low performance score on the items related to facilities and regulation such as visitation restriction or patient isolation. There was a positive correlation between perception and performance score. Conclusion: For emergency nurses, perception is higher than performance of tuberculosis infection control measures. The findings indicate the importance of providing continuing education for emergency nurse professionals seeking to update and increase their knowledge and to develop evidence-based nursing protocols on tuberculosis infection control in emergency department.

시보건소 결핵등록관리환자들의 치료충실도에 미치는 요인에 관한 조사 -서울특별시 일개 구보건소 등록환자 중심- (A Study on the Factors which Influenced the Faithfulness to Treatment Among the Pulmonary Tuberculosis Patients Registered in a City Health Center)

  • Personal and social characteristics, disease status, reason of delay of receiving drug from City Health Center and knowledge and attitude on the pulmonary tuberculosis were compared between the faithful group in treatment and the unfaithful group among the patients for pulmonary tuberculosis registered in a City Health Center in order to analyze the factors which influenced the faithfulness to treatment. Record as well as interview survey was done toward all patients of 247 who were registered and being cared during over 6 months in a City Health Center from April 1, 1982 to March 31, 1983, and results were as follows; 1. There was no statistically significant difference in sex and age distribution between the faithful group in treatment and the unfaithful group. 2. There was no statistically significant difference in marital status distribution between the faithful group and unfaithful group in treatment. 3. The lower education group comprised higher proportion than the higher education group among the faithful group in treatment. 4. The proportion of the emlployed was higher than unemlpoyed among the unfaithful group in treatment. 5. The proportion of mild case was higher than severe case among the unfaithful group in treatment. 6. As for the onset of delay of receiving drug, 'less than 3 month after starting treatment' was 59.5% among the unfaithful group in treatment. 7. The reasons of delay of receiving drug were 'no time' (42.2%). 'being away from home' (25.0%), 'being sick' (13.8%), and 'forgot the appointed date' (12.0%). 8. There was no statistically significant difference in the knowledge on the communicability of the tuberculosis between the faithful groups in treatment and the unfaithful group. 9. There was no statistically significant difference in the knowlege on B.C.G. as vaccination drug of tuberculosis between the faithful group and the unfaithful group in treatment. 10. There was no statistically significant difference in the satisfaction on the treatment of health center between the group of faithful and unfaithful in treatment. 11. There was no statistically significant difference in the belief on the complete recovery of tuberculosis between the faithful group and the unfaithful group in treatment. 12. The rate of consulting on tuberculosis treatment with life partner was higher among the faithful group in treatment than the unfaithful group.

  • "Yu-ong" is similar breast abscess, tuberculosis of breast in western medical. It is exist an inappreciable difference. The incidence of Breast abscess has been gradually decreasing because of the development of antibiotics and improved life style, greater knowledge about health. Never the less breast abscessis occasionally found on clinics and effects the physical and psycholosical health of mother and baby. Tuberculosis of breast, is extremely rare and differential diagnosis from carcinoma, pyogenic breast abscess, in necessory. An inappreciable difference examine carefully.

  • Tuberculosis, which is caused by Mycobacterium tuberculosis (Mtb), is among the most pressing worldwide problems. Mtb uniquely interacts with innate immune cells through various pattern recognition receptors. These interactions initiate several inflammatory pathways that play essential roles in controlling Mtb pathogenesis. Although the TLR signaling pathways have essential roles in numerous host's immune defense responses, the role of TLR signaling in the response to Mtb infection is still unclear. This review presents discussions on host-Mtb interactions in terms of Mtb-mediated TLR signaling. In addition, we highlight recent discoveries pertaining to these pathways that may help in new immunotherapeutic opportunities.

Multidrug-Resistant Tuberculosis Presenting as Miliary Tuberculosis without Immune Suppression: A Case Diagnosed Rapidly with the Genotypic Line Probe Assay Method

  • Miliary tuberculosis (TB) is a rare extrapulmonary form of TB, and there have been only two reports of miliary TB associated with infection with multidrug-resistant (MDR)-TB pathogen in an immunocompetent host. A 32-year-old woman was referred to our hospital because of abnormal findings on chest X-ray. The patient was diagnosed with MDR-TB by a line probe assay and was administered proper antituberculous drugs. After eight weeks, a solid-media drug sensitivity test revealed that the pathogen was resistant to ethambutol and streptomycin in addition to isoniazid and rifampicin. The patient was then treated with effective antituberculous drugs without delay after diagnosis of MDR-TB. To the best of our knowledge, this is the first case of miliary TB caused by MDR-TB pathogen in Korea.

  • 저자들은 기침을 주소로 내원하여 우측 종격동 종괴로 인한 상대정맥증후군을 보였으며 비디오 흉강경을 이용한 종괴의 조직검사와 조직에서의 항산균 도말검사 양성으로 결핵성 림프절염으로 진단하고 치료한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
