복식호흡 운동이 요통환자의 체간근육 활성화에 미치는 영향 (Effect of diaphragmatic breathing exercise on Activation of trunk muscle of patients with low back pain)

  • The purpose of this study was to investigate the effects of diaphragmatic breathing on activation of trunk muscles of patients with low back pain. Diaphragmatic breathing may affect activation of trunk muscles. The assumptions are as follows: the crural diaphragm attatches to the lumbar vertebrae from L1 to L3, the voluntary downward pressurization of the diaphragm increases intra-abdominal pressure, and this increases the stiffness of the spine. Diaphragmatic breathing increases intra-abdominal pressure and the increased intra-abdominal pressure may contribute to the lumbar stability. Sixty patients with low back pain were randomly divided into two groups. Experimental group performed diaphragmatic breathing exercise with six breathing positions and control group performed only the breathing positions for five times per week during six weeks. % maximal voluntary contraction(% MVC) of trunk muscles on six breathing positions of experimental and control group was measured according to testing period of pre test, three weeks, and six weeks. The repeated measures of one-way ANOVA were used to analyze % MVC on trunk muscles of experimental and control group according to testing period. The results of this study were as follows: First, % MVC of right and left erector spinae in the right leg extension position indicated the statistically significant difference in experimental group which performed diaphragmatic breathing exercise rather than control group (p<0.05). Second, % MVC of right and left erector spinae in all-four positions indicated the statistically significant difference in experimental group which performed diaphragmatic breathing exercise rather than control group (p<0.05). Third, % MVC of right and left erector spinae, external oblique in the sitting position indicated the statistically significant difference in experimental group which performed diaphragmatic breathing exercise rather than control group (p<0.05). Fourth, % MVC of right and left erector spinae, external oblique in the standing position indicated the statistically significant difference in experimental group which performed diaphragmatic breathing exercise rather than control group (p<0.05). Fifth, % MVC of right and left erector spinae, external oblique in the supine position indicated the statistically significant difference in experimental group which performed diaphragmatic breathing exercise rather than control group (p<0.05). Sixth, % MVC of right and left erector spinae, external oblique in the lying on prone position indicated the statistically significant difference in experimental group which performed diaphragmatic breathing exercise rather than control group (p<0.05). In conclusion, as experimental group performed diaphragmatic breathing exercise according to the period of pre-test, post three weeks, and post six weeks, experimental group showed the greater significant effect on the activation of right, left erector spinae, and external oblique muscle. Diaphragmatic breathing exercise which resulted in activation of trunk muscles can be effective for managing the patients with back pain and should be utilized as the new therapeutic intervention.

치과위생사 스켈링 시술자세의 2D에 의한 인간공학적 분석 (The ergonomic analysis on dental hygienists' scaling treatment posture based on two dimensional motion)

  • This study was performed on 17 female dental hygienists to find an appropriate method to reduce the hygienists' body discomfort while scaling, and provide a foundation to educate them how 10 posture during the process. To assess the arm abduction, elbow flexion, neck flexion, trunk flexion and trunk lateral bending through Video 2D(two dimensional motion) analysis and assessing the risk through the Action level of RULA(rapid upper limb assessment) checklist, an ergonomic analysis method. Correlation analyses on the posture angles and on body discomfort were performed. ANOVA analysis on scaling treatment position and the scaling treatment region of patients was also performed. The results are as following. 1. 20 analysis while scaling, arm abduction was $40{\sim}79.9^{\circ}$, elbow flexion $20{\sim}110^{\circ}$, neck flexion $50{\sim}100^{\circ}$, trunk flexion $60{\sim}80^{\circ}$, and trunk lateral bending $5{\sim}19.9^{\circ}$. 2. The Action level of RULA was 2. 3 resulted from scores 4 and 5 of group A which includes upper arm, lower ann, wrist, and scores 2 and 4 of group B which includes neck, trunk, legs. It means that the scaling treatment posture causes a high incidence rate of musculoskeletal that an additional investigation and improvement should be followed without hesitation. 3. There were significant differences among the maxilla right, maxilla anterior, maxilla left, mandible left, mandible anterior, and mandible right of a patient of the right and left upper arm, lower arm, neck, trunk, group A, group B, final RULA score while scaling treatment. 4. There were significant differences among the time position of 8, 9, 10, 11, 12, 13 of the right and left upper arm, lower arm, neck, trunk, group A, group B, final RULA score while scaling treatment, 5. As for the body discomfort, neck, right shoulder, left shoulder, right back, right wrist etc. were listed on top. As a conclusion, performing the time position of 12 which shows low right and left final RULA scores is better than the time position of 8 and 10 which show high final RULA scores to reduce the body discomfort while scaling treatment.

동작관찰훈련이 만성 뇌졸중 환자의 앉은 자세에서 균형과 몸통조절능력에 미치는 영향 (The Effect of Action on the Balance and the Trunk Control Ability in the Sit Position of Chronic Stroke Patients)

  • Purpose : To find out how action observation training for chronic stroke patients affects their balance and body control abilities in the posture seated in the rehabilitation of stroke. Methods : This study was conducted on 30 subjects who were diagnosed with stroke. The group conducted motion observation training through video clips, while the control group only conducted physical training, and the general physical therapy was performed equally by both counties. The static balance was measured using Biorescue and the dynamic balance was measured using Modified Functional Reach Test (MFRT), Postural Assessment Scale for Stroke, and Trunk Impairment Scale. Results : Static balance showed statistically significant difference in foot pressure (p<.05) as a result of comparison between pre and post exercise training. Dynamic balance was statistically significant (p>.05) as a result of comparing pre and post differences using modified functional reach test. The trunk control ability was statistically significant (p>.001). Comparison between the results of before and after motion observation training showed a statistically significant difference. Conclusion: This study confirmed that exercise training in sitting position was effective for static, dynamic balance ability and trunk control ability of hemiplegic patients due to stroke. These results suggest that the use of motion monitoring in stroke patients may have a positive impact on the diversity and function of rehabilitation.

The effects of increased unilateral and bilateral calcaneal eversion on pelvic and trunk alignment in standing position

  • Objective: Generally, it is known that there is a correlation between excessive calcaneus eversion and a patient with low back pain and it also affects pelvic alignment. However, there are not enough studies that show calcaneal eversion having an effect on the alignment of the trunk. Design: Cross-sectional study. Methods: A 3-dimensional motion analysis system was used to assess the lower limbs, pelvic alignment, and trunk alignment with increased unilateral and bilateral calcaneal eversion in twenty-one subjects. All subjects were asked to maintain a static posture for seven seconds on a wedge three times per posture for measurement and analysis purposes. The wedge used in the process was a lateral wedge with a 10-degree tilt to the lateral direction. To unify all of the subjects' foot position, the front and inner side of the wedge were marked. The height of the tilted wedge's inner side and flat wedge were balanced equally in order to be able to maintain the lateral part of the foot to the same height when producing an increased calcaneal eversion. Results: Comparing the changes in trunk and pelvic alignment in accordance to calcaneal eversion for each posture, there was a significant different in the X and Y-axis for each posture, but not in the Z-axis (p<0.05). Thus, it can be confirmed that calcaneal eversion in the sagittal plane and frontal plane may have and effect on the pelvis and the trunk. Conclusions: Postures with increased bilateral and unilateral calcaneal eversion has an effect on pelvic alignment, but does not cause any changes in trunk alignment.

만성요통환자와 정상인의 체간 운동시 요추 신전근의 근활성도와 관절운동범위의 변화 (The Changes of Range of Motion of Lumbar Region and Muscle Activities of Lumbar Extensor During Trunk Motions Between Subjects With Low Back Pain and Healthy Subjects)

  • The purpose of this study was to compare the electromyography (EMG) activities of the lumbar extensor muscles during motion of trunk flexion-extension and compare range of motion (ROM) with a 3-dimensional motion analysis system of the lumbar region between subjects with chronic low back pain (CLBP) and healthy subjects during the trunk flexion-extension, trunk rotation and trunk lateral flexion cycle. Thirty CLBP subjects and thirty healthy subjects were included. We measured the root mean square (RMS) value of the lumbar extensor muscles from resting, standing, lumbar flexion and return position. The RMS ratio was normalized from maximal EMG activity of the lumbar extensor muscles during trunk motion. The results of this study showed that the RMS ratio of the lumbar extensor was significantly higher in CLBP subjects than healthy subjects during all of trunk motion (p<.05). The ratio of the highest RMS value during flexion and extension was higher in CLBP subjects than in healthy subjects (p<.05). The ROM of the lumbar region was significantly lower in CLBP subjects than healthy subjects during trunk flexion-extension, trunk rotation and lateral flexion cycle. The relationship between the RMS ratio for full lumbar flexion and the ROM of lumbar flexion was not correlated significantly. CLBP subjects have both decreased ROM of the lumbar region and higher muscle activities of the lumbar extensor muscle than healthy subjects.

전신 기울기 운동 시 시각적 바이오피드백이 올바른 자세 유지 시간과 체간 근육의 근활성도에 미치는 영향 (The Effect of Visual Biofeedback on EMG Activity of Trunk Muscles and Endurance Holding Time for Correct Position During Whole-Body Tilt Exercise)

  • The purpose of this study was to assess visual biofeedback's influence on trunk muscles' (EMG) activity and endurance holding time for correct position during whole-body tilt exercise. For the study, we recruited 14 volunteers who showed no symptom of lumbar disease during medical tests. We measured the EMG activity of their rectus abdominis, external abdominal oblique, internal abdominal oblique and erector spinae muscles, and their endurance holding time for correct position during $40^{\circ}$ anterior and posterior whole-body tilt under two conditions: whole-body tilt with and without visual biofeedback. Resistance with gravitational force on the trunk during whole-body tilt was applied by using a device that had a monitor on which the subjects could check their alignment and that sounded an alarm if a subject's alignment collapsed. The study showed an increase in the EMG activity of external abdominal oblique, internal abdominal oblique/rectus abdominis ratio and endurance holding time for correct position during both $40^{\circ}$ anterior and posterior whole-body tilt with visual biofeedback compared with without visual biofeedback (p<.05). We suggest that the whole-body tilt exercise with visual biofeedback could be a beneficial strategy for selectively strengthening the internal abdominal oblique muscle and minimizing the rectus abdominis muscle's activity while maintaining correct alignment during whole-body tilt exercise.

  • Automobiles should be light and practical to reduce the price of manufacturing and maintaining expense. Hatchback vehicles are far from the silence for its practicality. This paper introduced the effective method of the planning of the asphalt material which applies the asphalt damping material. This paper showed the experiment which restrains the vibration of the vehicle trunk using asphalt damping material and chose the position of damping material using the velocity map and strain energy map and planed the optimum position.

만성요통환자의 반복체간운동 시 요부신전근 피로 분석 (A Fatigue Analysis on Lumbar Extension Muscle during Repeated Trunk Extention Exercise for Chronic Lumbar Pain)

  • The purpose of this study were analysed fatigue character of lumbar extension Muscle during repeated trunk flexion-extension motion we used EMG multi-telemeter (WEB 5000, Nihon Koden, Japan), Medex lumbar extension machine(Ocala, USA). We evaluated 20 persons on chronic low back pain group and 20 persons on control group. We analysed the quantitative variables IMF, MF, decreased ratios of MF to investigate fatigue. As a result, the following conclusion was drawn : 1. Each lumbar IMF of CLBP was L5> L3> L1 in the order of their size. CON group was similar quantitative and same order. But the relationship of two groups were not significantly. 2 The IMF within lumbar position(L1-L3, L1-L5, L3-L5) of two groups was significantly all positions(p<.05). 3. The MF shifting at lumbar position of two groups during repeated trunk flexion-extension motion was decrease shifting pattern with increase repeating motion, especially CLBP group was suddenly dropping than CON at all lumbar positions(L1, L3, L5) on early stage motion(34 set). 4. The lumbar fatigue of CLBP was higher than CON during repeated trunk flexion-extention motion, so CLBP supposed more tired than CON. And it was significantly within two groups at lumbar positions(L1, L3 : p<.05, L5 : P<.01).

일반인과 뇌졸중 편마비 환자의 연령별 선 자세에서 눕기 시 동작유형의 비교 (Movement Patterns for Lying from Erect Stance to Supine Position of Healthy Adults and Hemiplegia Patients Aged from 50 to 70)

  • Purpose: The purpose of this study was to describe the movement patterns when lying from an erect stance to a supine position of healthy adults and hemiplegia patients in the age group from 50 to 70. Methods: The subjects used in this study were 230 patients (144 healthy adults and 86 hemiplegia patients). Movement patterns were classified using categorical descriptions of the action of three body regions: the upper extremity, lower extremity, and the head-trunk region. Results: In the most common supine lying position in healthy adults (29.7%), the female fifties age group performed a symmetrical pattern using the upper extremity region, a symmetrical squat pattern using the lower extremity region, and a symmetrical pattern using the head-trunk region (1-1-1). However, in the most supine lying position in hemiplegia patients (32.0%), the female sixties age group usually performed this pattern by using the upper extremity and lower extremity regions in a squat position by raising the leg, and performing rotation of the head-trunk region (3-4-3). Conclusion: Using this clinical therapy, and considering the patients ability to exercise when lying from an erect stance to a supine position, the proper care could be provided. Moreover, this therapy introduces action based on a variety of training methods and on the effects which might result from any changes.

배가로근 수축을 강조한 체간 안정화 훈련이 양궁선수들의 정적 균형 및 경기기록에 미치는 영향 (The Effects of Trunk Stabilization Training Emphasizing Transverse Abdominis Contraction on Static Balance and Game Records for Archers)

  • Purpose: The purpose of this study was to investigate the effects of trunk stability training on static balance and game records among archers. Methods: The subjects comprised 24 voluntary archers in middle and high school (aged 14-19 years). All the subjects received trunk stability training for an hour per day 3 days a week for 4 weeks. The Tetrax balance system was used to measure the stability index of the subjects' static balance. The subjects' game records were scored using a single FITA round system. All the subjects were measured before the intervention, 2 weeks into the intervention, and 4 weeks after the intervention, with a follow-up measurement 2 weeks later. Results: The results of this study showed that the stability index was significantly different across all the measuring positions after the intervention (p<0.05). Furthermore, the results of the measurements of the standing position with eyes open and closed were significantly different before the intervention compared to 4 weeks after the intervention (p<0.05). The archery position with the head turned to the left and eyes closed was also significantly different pre-intervention compared to 2 weeks after the start of the intervention (p<0.05). Additionally, the archery records were significantly different after the intervention (p<0.05), as well as before the intervention and 4 weeks after the intervention (p<0.05). Conclusion: Trunk stability training can improve static balance in archers. It may also be helpful in improving athletic performance and maintaining the life of the athlete. Accordingly, trunk stability training may prevent and resolve injuries through careful management when playing one-side sports.
