Diffusion Tensor-Derived Properties of Benign Oligemia, True "at Risk" Penumbra, and Infarct Core during the First Three Hours of Stroke Onset: A Rat Model

  • Objective: The aim of this study was to investigate diffusion tensor (DT) imaging-derived properties of benign oligemia, true "at risk" penumbra (TP), and the infarct core (IC) during the first 3 hours of stroke onset. Materials and Methods: The study was approved by the local animal care and use committee. DT imaging data were obtained from 14 rats after permanent middle cerebral artery occlusion (pMCAO) using a 7T magnetic resonance scanner (Bruker) in room air. Relative cerebral blood flow and apparent diffusion coefficient (ADC) maps were generated to define oligemia, TP, IC, and normal tissue (NT) every 30 minutes up to 3 hours. Relative fractional anisotropy (rFA), pure anisotropy (rq), diffusion magnitude (rL), ADC (rADC), axial diffusivity (rAD), and radial diffusivity (rRD) values were derived by comparison with the contralateral normal brain. Results: The mean volume of oligemia was $24.7{\pm}14.1mm^3$, that of TP was $81.3{\pm}62.6mm^3$, and that of IC was $123.0{\pm}85.2mm^3$ at 30 minutes after pMCAO. rFA showed an initial paradoxical 10% increase in IC and TP, and declined afterward. The rq, rL, rADC, rAD, and rRD showed an initial discrepant decrease in IC (from -24% to -36%) as compared with TP (from -7% to -13%). Significant differences (p < 0.05) in metrics, except rFA, were found between tissue subtypes in the first 2.5 hours. The rq demonstrated the best overall performance in discriminating TP from IC (accuracy = 92.6%, area under curve = 0.93) and the optimal cutoff value was -33.90%. The metric values for oligemia and NT remained similar at all time points. Conclusion: Benign oligemia is small and remains microstructurally normal under pMCAO. TP and IC show a distinct evolution of DT-derived properties within the first 3 hours of stroke onset, and are thus potentially useful in predicting the fate of ischemic brain.

  • The mordem linear accelerators are equipped with X-ray collimators that can be moved independently to allow asymmetric fields with field centers positioned away from the true central axis of the beam. We have studied the dosimetric of an independent collimator system using measurements performed quarter fields. The field size factors for asymmetric fields are compared to those for symmetric fields. Dosimetric measurements include portent depth don, beam penumbra and shape of isodose curves for symmetric and quarter fields.

Comparison of Noise Power Spectrum Methodologies in Measurements by Using Various Electronic Portal Imaging Devices in Radiation Therapy (방사선치료시 전자포털영상장치를 이용한 잡음전력스펙트럼 방법론 측정비교)

  • The noise power spectrum (NPS) is one of the most general methods for measuring the noise amplitude and the quality of an image acquired from a uniform radiation field. The purpose of this study was to compare different NPS methodologies by using megavoltage X-ray energies. The NPS evaluation methods in diagnostic radiation were applied to therapy using the International Electro-technical Commission standard (IEC 62220-1). Various radiation therapy (RT) devices such as TrueBeam$^{TM}$(Varian), BEAMVIEW$^{PLUS}$(Siemens), iViewGT(Elekta) and Clinac$^R$ iX (Varian) were used. In order to measure the region of interest (ROI) of the NPS, we used the following four factors: the overlapping impact, the non-overlapping impact, the flatness and penumbra. As for NPS results, iViewGT(Elekta) had the higher amplitude of noise, compared to BEAMVIEW$^{PLUS}$ (Siemens), TrueBeam$^{TM}$(Varian) flattening filter, Clinac$^{R}$iXaS1000(Varian) and TrueBeam$^{TM}$(Varian) flattening filter free. The present study revealed that various factors could be employed to produce megavoltage imaging (MVI) of the NPS and as a baseline standard for NPS methodologies control in MVI.

Feasibility of a Linear Diode Array Detector for Commissioning of a Radiotherapy Planning System

  • Purpose: Although ionization chambers are widely used to measure beam commissioning data, point-by-point measurements of all the profiles with various field size and depths are time-consuming tasks. As an alternative, we investigated the feasibility of a linear diode array for commissioning a treatment planning system. Methods: The beam data of a Varian TrueBeam® radiotherapy system at 6 and 10 MV with/without a flattening filter were measured for commissioning of an Eclipse Analytical Anisotropic Algorithm (AAA) ver.15.6. All of the necessary beam data were measured using an IBA CC13 ionization chamber and validated against Varian "Golden Beam" data. After validation, the measured CC13 profiles were used for commissioning the Eclipse AAA (AAACC13). In addition, an IBA LDA-99SC linear diode array detector was used to measure all of the beam profiles and for commissioning a separate model (AAALDA99). Finally, the AAACC13 and AAALDA99 dose calculations for each of the 10 clinical plans were compared. Results: The agreement of the CC13 profiles with the Varian Golden Beam data was confirmed within 1% except in the penumbral region, where ≤2% of a discrepancy related to machine-specific jaw calibration was observed. Since the volume was larger for the CC13 chamber than for the LDA-99SC chamber, the penumbra widths were larger in the CC13 profiles, resulting in ≤5% differences. However, after beam modeling, the penumbral widths agreed within 0.1 mm. Finally the AAALDA99 and AAACC13 dose distributions agreed within 1% for all voxels inside the body for the 10 clinical plans. Conclusions: In conclusion, the LDA-99SC diode array detector was found to be accurate and efficient for measuring photon beam profiles to commission treatment planning systems.
