Trigeminal neuralgia is one of the diseases which cause most chronic and intractable pain on the facial area. Several drugs includding analgegics, anticonvulsants, tranquilizers, vitamins or hormonal preparations have been expected to be effective but no drug could effectively relieve the patients from the pain. The pain could be relieved by surgical neurectomy or neurolysis of the Gasserian ganglion or the involved branches with absolute alcohol alternatively. Surgical microvascular decompression may be performed if the pain resulted from compression of the nerve by adjucent arterial loops. 4 cases of trigeminal neuralgia are presented. They were treated with alcohol neurolysis of the involved peripheral nerves combined with or without carbamazepine and/or amitriptyline with favorable result of pain relief.

Trigeminal neuralgia has a specific property which burning pain emerges suddenly and spasmodically. The following pain continuously emerges during 20 seconds or 30 seconds and then the pain disappears. This process of the pain emerges repeatedly. Trigeminal neuralgia was called Myuntong(麵痛). We experienced a patient who had Trigeminal neuralgia for five years. she was successfully treated by the herb-medication, accupunture and negative. The medication taken by the patient were Seunghyungsan(升荊散) and Jowyuseung chengtang(謂胃升淸湯) and so on. The accupunture was mainly done at mainly done G-14(陽白), S-2(瀉白), G-3(上關), S-36(足三里), LI-4(합閤穀) and so on. The negative was done sternocleidomastoid muscle, trapezius and Masseter muscle.

Objectives : The aim of this study is to review clinical studies on trigeminal neuralgia treatment through Korean Medicine. Methods : We used search engines such as KISS, RISS, KOREAMED and NDSL. We limited cases as idiopathic trigeminal neuralgia without comorbidities. We excluded dissertation. We considered papers published after year 2000. Results : Fourteen studies were searched for this study. Various treatments such as acupuncture therapy, herbal medicine, moxibustion therapy, manipulation, pharmocopuncture, vomiting therapy were practiced for trigeminal neuralgia. Conclusions : All study were case report and evidence level of the searched studies was not high. They reported meaningful improvement through Korean Medicine treatment and these studies implied effectiveness and safety of Korean Medicine for Trigeminal neuralgia.

Pharmacological management is the first choice for treatment of the trigeminal neuralgia patients; however, if this mode of treatment fails a minimally invasive procedure should be performed. One of the most commonly used procedures is conventional radiofrequency lesioning of the Gasserian ganglion. Despite its popularity and success rate, this technique has disadvantages such as diminished corneal reflex, masseter weakness, numbness and anesthesia dolorosa. As a result, many studies have been conducted in an attempt to find a better method of treating trigeminal neuralgia. We report here a case of a trigeminal neuralgia patient that was treated with pulsed radiofrequency lesioning of the supraorbital and supratrochlear nerve due to pain in the frontal head that was refractory to the pharmacological treatments. Following the procedure, the Visual Analogue Scale score for pain decreased to 1-2/10 and the pain relief persisted for 7 months. These results indicate that pulsed radiofrequency treatment of the peripheral nerve may be useful for trigeminal neuralgia patients that do not respond to pharmacological treatments.

Trigeminal neuralgia (TN) is neuropathic pain that affects the trigeminal nerve branches. Facial pain experienced by patients with TN is typically intense and excruciating. The second and third branches (maxillary and mandibular) are commonly affected. This case report focuses on the potential treatment options for acute TN attacks involving these branches. The proposed approach involves extra-oral peripheral blocks using local anesthetics. Pain levels were measured using a visual numeric scale (VNS) with potential side effects and other relevant documented information. The patients showed responses from high pain levels to almost complete remission (from 8 to 2 and from 10 to 2 on the final VNS), with no significant side effects. This technique provides immediate pain relief and complements oral medications by offering comfort and confidence until the desired drug effect is achieved.
Trigeminal neuralgia is defined as an episodic electrical shock-like sensation in a dermatomal distribution of the trigeminal nerve. When medications fail to control pain, various procedures are used to attempt to control refractory pain. Of available procedures, stereotactic radiosurgery is the least invasive procedure and has been demonstrated to produce significant pain relief with minimal side effects. Recently, linear accelerators were introduced as a tool for radiosurgery of trigeminal neuralgia beneath the already accepted gamma unit. Author have experienced one case with trigeminal neuralgia treated with linear accelerator. The patient was treated with 85 Gy by means of 5 mm collimator directed to trigeminal nerve root entry zone. The patient obtained pain free without medication at 20 days after the procedure and remain pain free at 6 months after the procedure. He didn't experience facial numbness or other side effects.
Chronic maxillary sinusitis is a common disease, with symptoms of dull, aching pain or pressure below the eyes and signs such as tenderness over the involved sinus, whereas trigeminal neuralgia is described as severe, paroxysmal, and lancinating brief pain limited in distributions of one or more divisions of trigeminal nerve. In cases where these two non-odontogenic toothache symptoms overlaps, the diagnostic process can be confusing. Here, we report a case of a 54-year-old male patient with chief complaints of intermittent, severe, and electric-like pain in the upper left premolar and first molar area, initially diagnosed with trigeminal neuralgia but finally with maxillary sinusitis after pain recurrence 2 years after that. Therefore, thorough history taking and precise imaging interpretation should be considered to make correct diagnose especially in case of a patient with newly developed or altered or atypical symptoms.

Objectives : This study was conducted to analyze the trends of international clinical research on acupuncture treatment for trigeminal neuralgia. Methods : A total of 14 clinical researches were selected by using PubMed, an international online database, to search for clinical researches written in English and Chinese. Selected papers were analyzed according to study characteristics, study subjects, acupuncture treatment methods, treatment results, evaluation index, safety, and adverse event reports. Results : Acupuncture was mostly used in combination with other treatment methods. The most commonly used acupuncture point was the 'Xiaguan(ST7)', and 11 acupuncture points belonging to the 'Stomach meridian' were used. In most researches, selection of local points and distant points were taken simultaneously, with some researches taking acupuncture points according to trigeminal nerve branch. There were significant improvements in most outcome measures after treatment compared to pre-treatment, and most adverse events disappeared without treatment or decreased or disappeared after grand therapy. Conclusion : According to this study, acupuncture treatment for trigeminal neuralgia was performed in various ways and was found to be effective and safe. However, it is necessary to confirm more meaningful results by supplementing the limitations of existing researches in the future.
We recently described a novel animal model of trigeminal neuropathic pain following compression of the trigeminal ganglion (Ahn et al., 2009). In our present study, we adapted this model using male Sprague-Dawley rats weighing between 250-260 g and then analyzed the behavioral responses of these animals following modified chronic compression of the trigeminal ganglion. Under anesthesia, the rats were mounted onto a stereotaxic frame and a 4% agar solution ($10{\mu}L$) was injected in each case on the dorsal surface of the trigeminal ganglion to achieve compression without causing injury. In the control group, the rats received a sham operation without agar injection. Air-puff, acetone, and heat tests were performed at 3 days before and at 3, 7, 10, 14, 17, 21, 24, 30, 40, 55, and 70 days after surgery. Compression of the trigeminal ganglion produced nociceptive behavior in the trigeminal territory. Mechanical allodynia was established within 3 days and recovered to preoperative levels at approximately 60 days following compression. Mechanical hyperalgesia was also observed at 7 days after compression and persisted until the postoperative day 40. Cold hypersensitivity was established within 3 days after compression and lasted beyond postoperative day 55. In contrast, compression of the trigeminal ganglion did not produce any significant thermal hypersensitivity when compared with the sham operated group. These findings suggest that compression of the trigeminal ganglion without any injury produces prolonged nociceptive behavior and that our rat model is a useful system for further analysis of trigeminal neuralgia.

Objective : To evaluate the role of Gamma Knife radiosurgery in essential and tumor-related(secondary) trigeminal neuralgia, outcomes of radiosurgery and microsurgery were compared each other. Methods : Five patients with essential trigeminal neuralgia underwent stereotactic radiosurgical treatment with Leksell Gamma Knife and twenty five patients were treated with microsurgery during the same period(1994. 1-1997. 6). A 4-mm collimator was used and REZ or proximal portion of trigeminal nerve was targeted with maximal dose of 60-72Gy. The mean follow-up after radiosurgery was 39.4 months and that after microsurgery was 47.9 months. Results : At the last follow-up, four patients(80%) had excellent(pain free) or good(50-90% pain relief) outcomes, one(20%) had poor control after radiosurgical treatment. Twenty-three patients(92%) had excellent or good outcomes and two(8%) had poor results after microsurgery. Postoperative complications occurred in ten(40%) with microsurgery, but there were no complications in patients with Gamma Knife radiosurgery. Six patients with secondary trigeminal neuralgia received radiosurgical treatment directed at their tumors, and three patients were surgically treated. Three of six(50%) patients treated with Gamma Knife had pain relief while two of three patients with surgical treatment showed immediate pain relief. Post-treatment complications were developed in two of six radiosurgical patients and in one of three surgical patients. Conclusion : Gamma Knife stereotactic radiosurgery may be considered as a useful and alternative option for the treatment of essential and secondary trigeminal neuralgia owing to of its safety and less complications. The preliminary results obtained in our series appear encouraging, although the outcome is not so good as that of surgery.
