삼첨판막 대치술의 장기간 임상성적 (Long-Term Clinical Results of Tricuspid Valve Replacement)

  • 삼첨판막대치술의 장기 결과에 대한 연구결과는 보고된 바가 많지 않다. 이에 저자들은 삼첨판막대치술의 위험인자를 분석해 보고, 삼첨판막대치술의 장기 결과를 알고자 연구를 시행하였다. 대상 및 방법: 대상 환자들은 1978년 10월부터 1996년 12월까지 삼첨판막대치술을 시행 받은 환자 70명을 대상으로 후향적인 연구를 진행하였는데, 7명의 환자들은 2차례의 삼첨판막대치술을 시행 받아 총 77예의 삼첨판막대치슬을 시행하였다. 환자들의 평균 나이는 38.8$\pm$15.9세였으며, 26예에서는 조직판막을, 51예에서는 기계판막을 이식하였다. 결과: 수술 사망률은 15.6%였고, 만기 사망률은 12.3%였다. 5년, 10년 그리고 13년에서의 생존율은 조직판막과 기계판막이 각각 81.3% vs. 100%, 66.1% vs. 100%, 60.6% vs. 100%였다(p=0.0175).판막과 관련된 재수술이 없을 확률은 5년, 10년 그리고 13년에서 조직판막과 기계판막이 각각 100% vs. 93.9, 100% vs. 93.9% 그리고 58.3% vs. 93.9%였다(p=0,3274). 판막과 관련된 재수술을 시행할 확률은 조직판막이 2.27%/환자-년이었고, 기계판막이 1.10%/환자-년이었다. 수술 사망과 관련된 위험인자 분석상, 수술 전 복수, 간비대, NYHA class가 나쁠수록, 그리고 삼첨판막대치술을 여러 번 받는 경우가 유의한 위험인자로 분석되었고, 조직판막의 사용과 심장수술을 여러 번 받는 경우가 만기 사망과 관련된 유의한 위험인자로 분석되었다. 걸론: 기계판막을 이용하여 삼첨판막대치술을 시행한 환자들의 장기간 생존율이 조직판막을 이용한 환자들보다 우수하였다. 따라서 수술 후에 적절한 추적관찰이 가능하다면, 기계판막을 이용하여 삼첨판막대치술을 시행하는 것이 필요하리라 생각한다.

1978년도 상반기 개심술 : 112례 보고 (Open Heart Surgery in the First Half of 1978: A Report of 112 Cases)

  • Last year in this department 100 cases of open heart surgery were done annually. This year 200 cases of open heart surgery were scheduled. During the first 6 months of this year 112 open heart surgery cases were done with 13 deaths [11.6%]. There were 72 cases of cougenital malformation with 9 operative deaths [12.5%], consisting of 23 acyanotic cases with one death [4.5%] and 49 cases of cyanotic cases with 8 deaths [16.3%]. Out of 40 tetralogy of Fallot, 6 cases expired [15%]. For 39 cases of acquired valvular heart disease and one Ebstein anomaly valves were replaced with 4 operative deaths [10%]. Single valve replacement in 33 with 3 operative deaths and double valve replacement in 7 cases with one death were noted. Two patients expired among 28 mitral valve replacement cases [7.1%]. Among 7 double valve replacement patients, consisting of 3 mitral and aortic and 4 mitral and tricuspid valve replacement one case expired. In a case of Ebstein anomaly, tricuspid valve was replaced with plication of atrialized right ventricle successfully. The operative result was excellent.

  • A 75-year-old woman who had previously undergone a double valve replacement was admitted to Asan Medical Center because of severe bioprosthetic mitral valve dysfunction and tricuspid regurgitation. Under hypothermic fibrillatory arrest without aortic cross-clamping, minimally invasive mitral and tricuspid valve surgery was performed via a right minithoracotomy.

  • Traumatic tricuspid regurgitation is a rare complication of blunt chest trauma caused by chordal rupture, anterior papillary muscle rupture and anterior leaflet tear. Since clinical symptoms are vague, early diagnosis is difficult and some patient exhibit symptoms of right heart failure. Right heart failure has been the traditional indication for surgical treatment, such as tricuspid valve replacement. Recently, early detection using transthoracic echocardiography and surgical treatment, like valve repair, prior to overt right heart failure have been shown to better prognosis. We report a case of traumatic tricuspid regurgitation with chordal rupture in patient due to traffic accident.

Long-Term Outcomes of Modified Cone Reconstruction for Ebstein's Anomaly in Pediatric Patients in a Single Center

  • Background and Objective: We aimed to investigate long-term clinical and echocardiographic outcomes, including tricuspid valve durability, annular growth, and left ventricular reverse remodeling, after modified cone reconstruction in patients with Ebstein's anomaly. Methods: This was a retrospective analysis of all pediatric patients who underwent modified cone reconstruction for Ebstein's anomaly at a single tertiary center between January 2005 and June 2021. Results: A total of 14 pediatric patients underwent modified cone reconstruction for Ebstein's anomaly; the median age was 5.8 years (range, 0.01-16.6). There were three patients (21.4%) with Carpentier type B, ten patients with Carpentier type C (71.4%), and one patient with Carpentier type D (7.1%). There was no early or late mortality, arrhythmia, or readmission for heart failure at a 10-year follow-up. There were no cases of more than mild tricuspid stenosis or more than moderate tricuspid regurgitation during the study period, except for one patient with severe tricuspid regurgitation who underwent reoperation. The z value for tricuspid valve annular size significantly decreased immediately after the operation (2.46 vs. -1.15, p<0.001). However, from 1 year to 7 years after surgery, the z values were maintained between -1 and +1. Left ventricular end-systolic volume, end-diastolic volume, and stroke volume increased after surgery and remained elevated until seven years postoperatively. Conclusions: Ebstein's anomaly in children can be repaired by modified cone reconstruction with low mortality and morbidity, good tricuspid valve durability, and annular growth relative to somatic growth.

삼첨판막 이식: 57례 보고 (Tricuspid valve replacement with bioprosthesis)

  • Fifty-Seven Cases of tricuspid valve replacement were done from April 1976 to January 1983. Fourteen congenital and 43 acquired cases were found. In 13 cases tricuspid valve alone was replaced with 2 operative deaths and one late deaths. In 35 cases TVR and MVR were done with 6 operative deaths and 6 late deaths. In 9 cases TVR, MVR, and AVR were done with one operative deaths. Over all operative mortality was 15.8% and late mortality 12.3% among the 48 survivors. Over all Survival rate was 71.9% during follow-up period ranging from 8 months to 6 years and 9 months. In every case TVR was done with bioprosthetic xenograft valves.

삼첨판막 심내막염 (Tricuspid Valve Endocarditis)

  • 삼첨 판막 심내막염은 감수성이 있는 항생제에 효과적으로 치료될 수 있다. 그러나 패혈증이 지속되거 나, 중증의 심부전, 다발성 폐 색전증 그리고 실 초음파상 증식증 (vegetation)이 있는 경우는 수술이 필요 하다. . 19세횝 남자환자가 감염성심내막염으로 입원하였다. 환자는 9년전에 perimembranous type의 VSD로 수술받은 병 력 이 있다. 심초음파상 삼첨판의 전엽부에 커다란 증식증의 소견이 보였고,과거에 VSD를 봉합했던 부위에 누 출 (leakage)을 통한 좌우단락이 관찰되었다. 항생제치료와 함께 인공판막치 환술을 시행하였다. St. Jude Medical 양엽판막 (size 33 mm으로 삼첨 판막 대 치술을 시 행하였고 잔여 VSD는 단순봉합으 로 폐쇄하였다. 술후 시행한 심초음파검사에서 치환된 삼첨 판막의 기능은 좋았고, 증식증과 좌우단락의 소견은 보이지 않았다. 환자는 합병증없이 술후 25일째에 퇴원하였다. 저자들이 경험한 본 증례는 잔여 VSD로 인해 생긴 감염성심내막염을 초기에 적극적인 수술을 시행 하여 좋은결과를 얻었음을 보여준다. 이 에 문헌고찰과 더불어 보고하는 바이다.

Chordae Tendineae Approximation Technique for Severe Tricuspid Regurgitation with Severe Leaflet Tethering Using a Totally Endoscopic Beating-Heart Strategy: A Case Report

  • Untreated severe tricuspid regurgitation (TR) is associated with poor outcomes. Functional TR occurs secondary to dilatation of the annulus and tethering of the leaflets. Ring annuloplasty alone can correct most cases, but is insufficient in cases of severe annular dilatation due to severe leaflet tethering. In such cases, a tricuspid edge-to-edge technique may be an option. However, stitching of the leaflet tips alone is likely to result in tearing of the leaflets. Approximation of the durable chordae tendineae is considered helpful for this problem. Herein, we present the case of a 39-year-old man who had undergone open-heart surgery for acute type A aortic dissection 13 months earlier. A right mini-thoracotomy approach with a beating-heart strategy was used, which did not require unnecessary pericardial adhesiolysis and dissection. This technique had the advantage of reducing the operation time and the risk of bleeding. To summarize, we present a case of tricuspid valve repair in a high-risk patient with severe leaflet tethering that was successfully managed using these methods.

기능적 삼첨판 폐쇄부전증 환자에서 판막륜 성형술의 효과 (The Effects of Tricuspid Annuloplasty on Funcional Tricuspid Regurgitation)

  • Ninety-one adult patients underwent three different methods of annuloplasty and compared them by the amount of tricuspid regurgitation. Group I [n=17 is Kay method, Group II [n=46 is modified Kay method and Group III [n=28 is De Vega and modified De Vega method. Preoperative and postopeative size of the liver and its function, the cardiothoracic ratio, EKG and echocardiogram were analyzed. The follow up was done for all the patients [mean 20.0$\pm$ 8.5 months . The postoperative size of the liver, the postoperative cardiothoracic ratio and the postoperative systolic pressure of the right ventricle decreased significantly compared to preoperative size, ratio and pressure [p=0.0001, p=0.0001, p=0.0001 . But there were no differences between the groups. The results of annuloplasty revealed that tricuspid regurgitation improved postoperatively [p=0.0001 even though there was no statistically significant differences in relation to the methods of annuloplasty. The right ventricular systolic pressure and the amount of regurgitation decreased significantly during the postoperative period by performing 3 different methods of annuloplasty, although we could not find the differences between the three different methods.

연간 개심술 416례 보고 [1980 년도] (Annual Open Heart Surgery: Report of 416 Cases in 1980)

  • In 1980, 416 cases of open heart surgery were done in this Department with over all operative mortality of 12.3%. 1. There were 288 congenital anomalies consisting of 174 acyanotic and 114 cyanotic varieties, which showed operative mortality of 6.9% and 25.4% respectively. 2. There were 128 cases of acquired lesions, 124 valvular disease and 3 myxoma being the main lesions. 3. There were 128 cases of valve replacement with operative mortality of 7.8%. 4. The most frequently operated anomaly was VSD, 90 pure VSD and 21 cases were associated with one or 2 cardiac anomalies. Over all operative mortality in 111 VSD cases was 8.1% but in 90 pure VSD cases it was 6.7%. 5. Tetralogy of Fallot showed the highest incidence in cyanotic group with 88 cases, consisting of 68 pure and 20 with other cardiac anomalies. Over all mortality in 88 cases was 19.3% but in pure form 16.2%. 6. In 128 valve replacement cases over all mortality was 9.4%. There were 85 mitral, 11 aortic, 2 tricuspid, 21 mitral with aortic, 6 mitral with tricuspid, 3 mitral, aortic, and tricuspid valve replacement cases. For mitral valve replacement operative mortality was 5.9%. 7. Twenty-one cases of babies under 10kg body weight were operated on with over all operative mortality of 28.6%. Sixteen cases of VSD were found with operative mortality of 25%. 8. Among 128 cases of valve replacement 7 were under the age of 15 years and 12 were between 15 and 20 years old. Five pediatric cases underwent mitral valve replacement without mortality, 9 year old boy was the youngest among them. In this Department open heart surgery for infancy and complex anomalies showed still hip operative risk which should be improved in the coming years. For open heart surgery Shiley oxygenators and 2 sets of A-O de-lux 5 head roller pump were utilized exclusively. For valve replacement Ionescu-Shiley bovine pericardial xenografts were mainly used. In pediatric and rural patients Persantin with aspirin regimen was satisfactorily administered for anticoagulation after valve replacement. Routinely Coumadin was administered for one year after valve replacement* In patients who had thrombus on valve sites, chronic atrial fibrillation, and giant left atrium Persantin-Aspirin regimen was used when one year coumadin administration was discontinued.
