A Study on the Prediction of Mortality Rate after Lung Cancer Diagnosis for Men and Women in 80s, 90s, and 100s Based on Deep Learning (딥러닝 기반 80대·90대·100대 남녀 대상 폐암 진단 후 사망률 예측에 관한 연구)

  • Recently, research on predicting the treatment results of diseases using deep learning technology is also active in the medical community. However, small patient data and specific deep learning algorithms were selected and utilized, and research was conducted to show meaningful results under specific conditions. In this study, in order to generalize the research results, patients were further expanded and subdivided to derive the results of a study predicting mortality after lung cancer diagnosis for men and women in their 80s, 90s, and 100s. Using AutoML, which provides large-scale medical information and various deep learning algorithms from the Health Insurance Review and Assessment Service, five algorithms such as Decision Tree, Random Forest, Gradient Boosting, XGBoost, and Logistic Registration were created to predict mortality rates for 84 months after lung cancer diagnosis. As a result of the study, men in their 80s and 90s had a higher mortality prediction rate than women, and women in their 100s had a higher mortality prediction rate than men. And the factor that has the greatest influence on the mortality rate was analyzed as the treatment period.

  • In this paper, a study was conducted to find a self-diagnosis method to prevent the spread of COVID-19 based on machine learning. COVID-19 is an infectious disease caused by a newly discovered coronavirus. According to WHO(World Health Organization)'s situation report published on May 18th, 2020, COVID-19 has already affected 4,600,000 cases and 310,000 deaths globally and still increasing. The most severe problem of COVID-19 virus is that it spreads primarily through droplets of saliva or discharge from the nose when an infected person coughs or sneezes, which occurs in everyday life. And also, at this time, there are no specific vaccines or treatments for COVID-19. Because of the secure diffusion method and the absence of a vaccine, it is essential to self-diagnose or do a self-diagnosis questionnaire whenever possible. But self-diagnosing has too many questions, and ambiguous standards also take time. Therefore, in this study, using SVM(Support Vector Machine), Decision Tree and correlation analysis found two vital factors to predict the infection of the COVID-19 virus with an accuracy of 80%. Applying the result proposed in this paper, people can self-diagnose quickly to prevent COVID-19 and further prevent the spread of COVID-19.

  • In the current field of Medical Informatics, the information increases, and changes fast, so we can access the various data types which are ranged from text to image type. A small number of technician digitizes these data to establish database, but it is needed a lot of money and time. Therefore digitization by many end-users confronting data and establishment of searching database is needed to manage increasing information effectively. New data and information are taken fast to provide the quality of care, diagnosis which is the basic work in the medicine. And also It is needed the medical database for purpose of private study and novice education, which is tool to make various data become knowledge. However, current medical database is used and developed only for the purpose of hospital work management. In this study, using text input, file import and object images are digitized to establish database by people who are worked at the medicine field but can not expertise to program. Data are hierarchically constructed and then knowledge is established using a tree type database establishment method. Consequently, we can get data fast and exactly through search, apply it to study as subject-oriented classification, apply it to diagnosis as time-depended reflection of data, and apply it to education and precaution through function of publishing questions and reusability of data.

Diagnostic Laparoscopy in Infantile Cholestatic Jaundice (영아 정체성 황달에 대한 진단적 복강경 의의)

  • When jaundice persists for more than 14 days postnatally, the early diagnosis of surgical jaundice is important for the prognosis in extrahepatic biliary atresia after draining procedure. The role of diagnostic laparoscopy to differenctiate medical causes of jaundice from biliary atresia is evaluated in this report. Four patients with prolonged jaundice have been included in this study. When the gallbladder was not visualized we proceeded to laparotomy. In patients with enlarged gallbladder visualized at laparoscopy, laparoscopic guided cholangiogram was performed, and laparoscopic liver biopsy was done for those who had a patent biliary tree. Two patients had small atretic gallbladder and underwent a Kasai hepato-portoenterostomy. One patients showed a patent gallbladder and common bile duct with atresia of the common hepatic and intrahepatic ducts, and they underwent a Kasai hepatic-portoenterostomy. One patient showed an enlarged gallbladder and laparoscopic-guided cholangiogram were normal. Laparoscopic liver biopsy was performed. There were no complications. Laparoscopy with laparoscopic-guided cholangiogram may be a valuable method in accurate and earlier diagnosis in an infant with prolonged jaundice.

  • Pulmonary sequestration is the part of a spectrum of bronchopulmonary foregut anomalies in which a portion of lung parenchyma does not communicate with the tracheobronchial tree and usually receives its arterial supply from a systemic vessel. The sequestrated portion of the lung is susceptible to infection. The patient with this entity will have a paucity of symptoms and will present himself for treatment because of a persistent pneumonia. The associated aberrant systemic artery makes the preoperative diagnosis of the lesion imperative because of the life-threatening technical hazards posed by this artery. We experienced a case of intralobar pulmonary sequestration. Initially, the diagnosis of sequestration was unsuspected and open thoracotomy was done for management of homogenous cystic mass on left lower lobe, but one anomalous systemic artery from thoracic descending aorta to sequestrated lung was incidentally revealed. Then we underwent lower lobectomy and ligation of anomalous artery.

New Approaches to Xerostomia with Salivary Flow Rate Based on Machine Learning Algorithm

  • Purpose: We aimed to investigate the objective cutoff values of unstimulated flow rates (UFR) and stimulated salivary flow rates (SFR) in patients with xerostomia and to present an optimal machine learning model with a classification and regression tree (CART) for all ages. Materials and Methods: A total of 829 patients with oral diseases were enrolled (591 females; mean age, 59.29±16.40 years; 8~95 years old), 199 patients with xerostomia and 630 patients without xerostomia. Salivary and clinical characteristics were collected and analyzed. Result: Patients with xerostomia had significantly lower levels of UFR (0.29±0.22 vs. 0.41±0.24 ml/min) and SFR (1.12±0.55 vs. 1.39±0.94 ml/min) (P<0.001), respectively, compared to those with non-xerostomia. The presence of xerostomia had a significantly negative correlation with UFR (r=-0.603, P=0.002) and SFR (r=-0.301, P=0.017). In the diagnosis of xerostomia based on the CART algorithm, the presence of stomatitis, candidiasis, halitosis, psychiatric disorder, and hyperlipidemia were significant predictors for xerostomia, and the cutoff ranges for xerostomia for UFR and SFR were 0.03~0.18 ml/min and 0.85~1.6 ml/min, respectively. Conclusion: Xerostomia was correlated with decreases in UFR and SFR, and their cutoff values varied depending on the patient's underlying oral and systemic conditions.

  • In this study, theory of school consulting for founding the development strategy of industrial high school was explored, consulting model and performing technique model was suggested, case of consulting of industrial high school performed by the model and performing technique suggested. Up to now, research field of school consulting was instruction and supervision consulting mostly, there was not much school management consulting. In this paper, a school consulting model for industrial high school was developed, which was five step such as entry, diagnosis, action planning, action, and termination. A case of consulting for C technical high school applied by the consulting model suggested was analyzed, it will be used any other consulting project of industrial high school. According to the 5 step school consulting model, analysis of the present state of C technical high school, survey research, SWOT analysis, Issue Tree, school assesment were performed. In order to set up a development strategy for C technical high school, a table of problem and improvement based on issue tree was made, and then a Pay-Off Matrix was made out. Finally, a development plan of short and mid- and long-term for C technical high school was constructed.

  • Symbolic data analysis extends the data mining and exploratory data analysis to the knowledge mining, we can suggest the SDA tree model on clinical and genomic data with new knowledge mining SDA approach. Using SDA application for huge genomic SNP data, we can get the correlation the availability of understanding of hidden structure of HCC data could be proved. We can confirm validity of application of SDA to the tree structured progression model and to quantify the clinical lab data and SNP data for early diagnosis of HCC. Our proposed model constructs the representative model for HCC survival time and causal association with their SNP gene data. To fit the simple and easy interpretation tree structured survival model which could reduced from huge clinical and genomic data under the new statistical theory of knowledge mining with SDA.

Diagnosis of the growth status and actual condition of the remaining old trees in the village - Focused on Hyeongok-myeon in Gyeongju-si - (마을 내 잔존 노거수의 생육현황 및 실태진단 - 경주시 현곡면을 중심으로 -)

  • The purpose of this study is to provide basic data for the establishment of future countermeasures by diagnosing the growth status and actual condition of undesignated old trees distributed in Hyeongok-myeon, Gyeongju, Gyeongsangbuk-do. The number of old trees surveyed was 2 weeks in Gajeong-ri, 2 weeks in Haguri, 3 weeks in Nae-Tae-ri, 1 week in Nawon-ri, 6 weeks in Oryu-ri, 3 weeks in Sangguri, and 2 weeks in Sohyeon-ri, The trees species composition was 6 trees Celtis sinensis Pers., 1 Diospyros lotus L. trees, 4 trees Salix chaenomeloides Kimura trees, 2 Styphnolobium japonicum L. trees, and 7 Zelkova serrata (Thunb.) Makino trees. Growth status is 7.1~22.0m in height, 14.6~25.1m in long axis, 10.2~19.2m in short axis, root diameter is 76.0~236.4cm, diamter at breast height is 67.0~220.0cm, soil acidity is pH4.9~7.0, soil The hardness was measured to be 4.0-27.0mm. The result grade of the scoring data of health information is represented by monitoring generally, monitoring critically, and absolute monitoring, and it was confirmed that out of the 20 trees population in Hyeongok-myeon, the general monitoring grade was 7 weeks, the major monitoring grade was 13 weeks, and there was no absolute monitoring grade. Accordingly, the number of old trees of the general surveillance level was maintained at the current level, and ecological surgical operations were introduced for the major surveillance level, but the case of village forests should be different, and sequential treatments were the old tree urgently needed. The level and bark of the target tree, the state of the crown, the root exposure, the presence of pests and pests, the vitality and the ground condition, the degree of pollution are normal, poor, or very poor, operation and protection management, soil improvement, removal of cover, and disinfection were urgently needed for the old trees with the surveyed data. In order to compensate for these matters, continuous monitoring and management measures for the old number should be sought.

Thoracic Actinomycosis - A Case Report - (흉부 방선균증-1례 보고-)

  • Actinomycosis of the lung is a chronic, suppurative granulomatous infection which is caused by Actinomyces israelii. It is believed to enter the thorax by way of the bronchial tree, by aspiration of contaminated aerosol particles in the upper digestive tract. Symptoms of chronic cough, sputum, hemoptysis, low grade fever, chest pain, and weight loss are common. Chest X-ray shows mass like lesion, pulmonary infiltration, abscess, and tuberculosis like lesion, which makes differential diagnosis from lung cancer very difficult. Surgical intervention is needed for the diagnosis and treatment, and diagnosis of actinomycosis is achieved when histologic examination reveals sulfur granules containing filamentous organisms. Penicillin is the drug of choice. Two or three months of penicillin treatment is recommended to treat the oropharyngeal or dental abscess to avoid recurrences. We present a case of actinomycosis which is suspected to malignant with review of literatures.
