사진측량을 이용한 초구장 기본 계획에 관한 연구 (A Study on the Basic Planning of Country Club Using Photogrammetry)

  • 본 연구는 사친 측량을 이용한 효율적인 초구장 기본 계획을 수립하고자 연구 대상지를 선정하여 종래 방법과 사진측량 방법을 비교 분석하였다. 또한, 대상지 항공사진과 지형도를 이용하여 사친 판독에 의해 식생, 수계도를 작성하고, 대상지를 격자로 구성하여 수치지형모델에 의한 지형 변화 및 경관을 예측하므로서 계획에 필요한 여러 요소를 종래방법과 비교 고찰하였다. 본 연구 결과 사진 판독에 식생도 및 수계도를 작성하므로서 기존 조사방법 보다 세밀한 결과를 얻을 수 있었고, 수치지형모델을 이용하여 지형, 경관 및 수계 등의 변화를 표현, 예측하므로서 우수처리, 경관, 조경 계획을 효율적으로 할 수 있었다. 또한, 항공사진과 지형도를 병용하므로서 토지 이용 계획과 기타 계획에 관하여 종래 방법보다 효율적임을 제시할 수 있었다.

  • The purpose of this study was to spatial organization feature and area composition of the over 500beds general hospital that will be expected construct in Seoul, Gyeonggi-do area after 2000year. From this study, we can suggest architectural planning for the efficiency operating of emergency medical facilities that can be used as preliminary data. The Spatial organization of emergency medical facilities are categorize into 4 session as medical examination and treatment, diagonosticradiology, staff, public and the others. We can find spatial organization system and area composition that compare area composition of 6 hospital the subject of research in a emergency medical facilities. The result of this study were as follows : spatial distribution of its field, grasping of spatial organization factor and recognize its merits and demerits should be researched in advance. Through this kind of recognizing emergency system, realistic medical local and condition architectural planning for spatial organization will be operate.

Development of PC-based Radiation Therapy Planning System

  • The main principle of radiation therapy is to deliver optimum dose to tumor to increase tumor cure probability while minimizing dose to critical normal structure to reduce complications. RTP system is required for proper dose plan in radiation therapy treatment. The main goal of this research is to develop dose model for photon, electron, and brachytherapy, and to display dose distribution on patient images with optimum process. The main items developed in this research includes: (l) user requirements and quality control; analysis of user requirement in RTP, networking between RTP and relevant equipment, quality control using phantom for clinical application (2) dose model in RTP; photon, electron, brachytherapy, modifying dose model (3) image processing and 3D visualization; 2D image processing, auto contouring, image reconstruction, 3D visualization (4) object modeling and graphic user interface; development of total software structure, step-by-step planning procedure, window design and user-interface. Our final product show strong capability for routine and advance RTP planning.

장애인 구강진료 접근성 제약 - Part 1. 장애인 치과 의료진 측면에서의 고찰 (TREATMENT BARRIERS OF SPECIAL NEEDS PATIENTS - PART 1. CONSIDERATION FROM THE PERSPECTIVES OF SPECIAL CARE DENTISTS)

  • The aim of this study was to clarify the barriers of dental treatment for special needs patients felt by dentists and to determine the dentist-related factors contributing to the obstacles in treatment planning and decision making. Questionnaires were distributed and responded by dentists working at five public-based special care clinics in South Korea. Factors divided into three parts (dentist demographics, clinical factors, and educational and administrational factors) were assessed and analyzed for correlations between dentist-related factors and dentist-felt burdens for special care treatment. Fisher's exact test and Chi-square test were used at the level of 0.05. A total of 34 dentists responded to the questionnaires. Almost all dentists had obstacles in the treatment of special needs patients in terms of the patients' lack of cooperation (94.1%), proxy communication with caregivers (94.1%), payment reward system (63.6%), deficient workforce (67.7%), and others. The longer dentists had been practicing for special needs patients, the more they were dissatisfied with the reward system and a longer time was spent for communication with patients and their caregivers (p<0.05). For specialists, more obstacles were experienced in treatment planning due to a deficiency in the clinical information obtained from their patients compared to general practitioners (p<0.05). A total of 82.4% of the respondents approved of mandatory educational programs for special care dentists. There were practitioner-based factors related to the amount of obstacles felt by special care dentists. To overcome the treatment barrier of special needs patients, in-depth education and training are required in special care dentistry.

자궁경부암 근접방사선치료 시 2차원, 3차원 치료계획 비교평가 (Comparison of 2D and 3D Brachytherapy Planning for Cervical Cancer)

  • 본 연구에서는 자궁경부암의 치료를 위한 전산화단층촬영(computer tomography)영상을 이용한 3차원 근접치료계획의 유용성을 알고자 하였다. 10명의 자궁경부암 환자에서 2차원 근접치료계획을 시행하였고, 동일 자세로 전산화단층촬영을 시행하였다. 3차원 근접치료계획 프로그램(ECLIPSE treatment planning system v6.5, Varian Medical System, USA)를 이용하였으며, 고위험임상표적체적(High risk CTV, HR CTV)에 5 Gy를 처방하였다. 3차원 근접치료계획의 제한은 적어도 고위험임상표적체적의 90%에 처방선량인 5 Gy가 조사되도록, 방광의 $2cm^3$에 7.5 Gy 미만으로 들어가도록 그리고 직장의 $2cm^3$에 5 Gy 미만이 들어가도록 하였다. 계획의 평가는 선량체적표(dose-volume histograms; DVHs)를 이용하여 육안적종양체적(Gross tumor volume for brachytherapy; $GTV_B$), 고위험임상표적체적, 직장과 방광에 조사되는 선량을 구하였다. 크기가 큰 종양이나 자궁의 위치이상이 있는 환자에서 Point A에 처방한 2차원 근접치료계획을 하였을 경우에는 고위험임상표적체적이 충분히 포함되지 않았다. 그러나 3차원 근접치료계획은 이러한 환자들에서 직장이나 방광의 선량을 증가시키지 않고 고위험임상표적체적을 잘 포함할 수 있었다. 2차원 근접치료계획에서 높은 선량부터 $2cm^3$에 들어가는 직장선량은 10명 중 1명에서, $2cm^3$에 들어가는 방광선량은 6명에서 5 Gy를 넘었으므로 ICRU (International Commission on Radiation Units) 방광선량이 직장선량에 비해 과소평가됨을 확인할 수 있었다. 자궁경부암 환자에서 전산화단층촬영 이용한 3차원 근접치료계획은 종양과 위험장기에 대한 선량평가가 가능하므로, 직장과 방광의 부작용을 증가시키지 않으면서 크기가 큰 종양이나 자궁의 위치이상이 있는 환자에서 위치 설정의 오류를 줄일 수 있을 것으로 생각된다.

THE MANAGEMENT OF A COMPLEX IMPLANT CASE USING CAD-CAM TECHNOLOGY: A CLINICAL REPORT

  • PURPOSE: The application of computer-aided technology to implant dentistry has created new opportunities for treatment planning, surgery and prosthodontic treatment, but the correct selection and combination of available methods may be challenging in times. Hence, the purpose of this case report is to present a combination of several computer-aided tools as approaches to manage complicated implant case. MATERIAL AND METHODS: A 47 year-old female patient with severe dental anxiety, high expectations, financial restrictions and poor compliance presented for a fixed rehabilitation. A CT scan with a radiographic template obtained with software (SimPlant, Materialize, Leuven, Belgium) was used for treatment planning. The surgical plan was created and converted into a stereolithographic model of the maxilla with bone-supported surgical templates (SurgiGuide, Materialise, Leuven, Belgium), that allowed for the precise placement of 7 implants in a severely resorbed edentulous maxilla. After successful osseointegration, an accurate scan model served as the basis for the fabrication of a one-piece milled titanium framework using the Procera (Nobel Biocare, Gothenburg, Sweden) technology. The final rehabilitation of the edentulous maxilla was rendered in the form of a screw-retained maxillary metal-reinforced resin-based complete prosthesis. RESULTS: Despite challenging circumstances, 7 implants could be placed without bone augmentation in a severely resorbed maxilla using the SimPlant software for pre-implant analysis and the SurgiGuide-system as the surgical template. The patient was successfully restored with a fixed full arch restoration, utilizing the Procera system for the fabrication of a milled titanium framework.

공간구문론을 이용한 국내권역외상센터 공간구성에 관한 연구 (A Study on the Spatial Configuration for Regional Trauma Center in Korea by Using Space Syntax)

  • The regional trauma center should be a trauma treatment center equipped with facilities, equipments, and manpower capable of providing optimal treatment from emergency surgery to a severely traumatized patient upon arrival at the hospital. In order to establish a medical system for effective severe diseases, it is necessary to prepare architectural planning guidelines for the regional trauma centers. This study analyzes the connectivity, control, integration, and mean depth of current trauma centers using the convex map of space syntax, And to provide basic data for building for more efficient regional trauma center. The major areas that must be included in the regional trauma center are trauma resuscitation room, trauma operating room, trauma intensive care unit, and trauma general ward. It is necessary to carry out the architectural planning to increase the interconnection of the four areas. Also, the elevator plan for trauma patients should be emphasized. In addition, a regional trauma center should be separated from the existing facility for independent operation. According to the case analysis of the space configuration of the regional trauma center, the location of the operating room is most important considering the connection with each department of the hospital and the treatment flow of the severe trauma patients.

종합병원 외래진료부 진로인지계획 모형에 관한 연구 (A Study on the Wayfinding Model of Outpatient Department in General Hospital)

  • Recently, hospital patients experience anxiety, confusion, and stress about wayfinding as the spacial layout and treatment circulatory system of hospitals have become complicated due to their oversized and complex structure. As part of finding a solution to the problem, this study seeks to examine what are the essential elements of the wayfinding planning of O.P.D. in general hospitals, to develop the model of wayfinding, and to suggest the methods of improving the wayfinding system. The research methods of this study adopted were literature review in wayfinding cognition, plan analysis of ten general hospitals, space analysis of these hospitals through space syntax, analysis of the system of visual-perceptual information through a field study, and analysis of surveys and follow-up surveys conducted to support the results. Based on these results, the proposals for finding decision points, providing the information, and developing a model planning are listed as follows. 1) The comprehensive understanding of O.P.D. spacial layout and the visual-perceptual information system is necessary to find the essential elements of wayfinding. 2) The decision points are found through the full understanding of spacial functions, circulation systems, and facility configuration, considering the spacial layout, the bound of the visual-perceptual information system, and the circulatory system. Furthermore, the information decision points could be confined by space syntax. 3) The checklist and color compound & color codes, developed through the planning of signage system and color system could be applied to the methods of providing the information. 4) The planning of wayfinding system according to the whole process of practices for outpatients was mentioned above. The system of visual-perceptual information developed through the process of this study should be integrated in the spacial layout of the whole O.P.D.

  • This paper describes interactive computer procedures for design the forming sequences in cold forging. This system is implemented on the personal computer and its environment is a commercial AutoCAD system. The programming language. AutoLISP, was used for the configuration of the system. Since the process of metal forming can be considered as a transformation of geometry, treatment of the geometry of the part is a key in process planning. To recognize the part section geometry, the section entity representation, the section coordinate-redius representation and the section primitive geometru were adopted. This system includes six major modules such as input module, forging design module, forming sequence design module, die design module, FEM verification module and output module which are used independently or in all. The sequence drawing wigh all dimensions, which includes the dimensional tolerances and the proper sequence of operations, can generate under the environment of AutoCAD. The acceptable forming sequences can be verified further, using the FE simulation.

방사선치료시 최적의 빔 위치와 크기 결정 (The Determination of Optimum Beam Position and Size in Radiation Treatment)

  • 방사선 치료에 따른 방사선량 최적화 문제를 풀기 위한 새로운 방법이 제시되었다. 기존의 2차원 치료계획과는 달리 3차원 문제에서는 모든 조건이 훨씬 복잡하고 관련된 변수도 많아지기 때문에 문제를 해결하기가 쉽지 않다. 본 연구에서는 3차원 선량최적화 문제를 접근하는데 있어서, 해가 존재할 수 있는 범위를 줄여주고, 중요한 파라미터들을 미리 구해주어서 치료계획에 관련된 변수를 줄이는 방법을 연구하였다. 먼저 선형가속기와 환자좌표계사이의 좌표변환을 이용하여 두부 내의 중요기관을 피하는 빔 위치를 찾았다 그리고 임의의 빔 위치에 대해 병소를 완전히 감싸는 빔 크기와 콜리메이터 회전각을 구하였다. 그 결과 가능한 빔 위치를 줄여줄 수 있었고, 빔 크기와 회전각에 대한 의존성을 없앨 수 있었다. 따라서 고려해야할 변수의 조합이 크게 줄어들게 되었고, 목적함수를 이용한 선량최적화에 있어서 최소한의 변수로만 계산이 가능하게 되었다. 위의 결과를 이용하여 임상에 널리 쓰이는 2차원 방사선치료계획의 선량최적화 문제를 해결하였다. 선량기울기, 중요기관의 선량, 선량분포 균일도를 조합한 목적함수를 최소화하는 최적해를 step search 방법을 이용하여 구하였다. 그리고 이 최적해를 이용한 선량분포로부터 새로운 방법에 의한 선량최적화의 가능성을 확인할 수 있었고, 후속 연구를 통하여 상용 방사선 치료계획 시스템에 적용함으로써 임상에 쓰일 수 있을 것으로 사료된다.
