Treatment and Management of Conversion Disorder (전환장애의 치료와 관리)

  • Conversion disorder is a psychologically produced alteration or loss of physical functioning suggestive of a physical disorder. Conversion symptoms are often superimposed on organic disease and can be overlooked. Psychological techniques are central to the management include the following: avoiding confrontation with the patients; avoiding reinforcement or trivializing the symptoms; reviewing results of tests and exams and creating an expectation of recovery; educating the patient before a treatment is begun; evaluating the patient's emotional adjustment and considering it at a treatment; using caution in labeling the condition; considering referral for psychotherapy; establishing particularly a treatment plan and making a definite treatment program; adjusting patient' s environment; letting participate a family at appropriate time; developing a reinforcement program for a treatment of chronic symptoms; developing a home program for outpatients. Use behavior therapy reinforcement may be helpful with more chronic or resistant symptoms, especially when there is a history of vague or excessive somatic complaints or significant secondary gain.

An Analysis of Effects on Vision Care Program Including Traditional Korean Treatment for Elementary School Students (초등학생에서 한의학적 치료를 접목한 시력증진 프로그램의 효과 분석)

  • Purpose: To evaluate the effects on vision care program including traditional Korean treatment for elementary school students. Methods: 172 eyes of eighty-six older elementary students underwent a vision care program, including traditional Korean treatment, for 3 months between March 2010 and June 2010. The vision care program included health education, such as lifestyle modification, vision strengthening exercises and a course of treatment with traditional Korean methods. Traditional Korean treatment covered periocular acupuncture point stimulation, periauricular acupuncture point stimulation and herbal drug therapy. All patients were examined for visual acuity and auto-refraction and filled out a satisfactory questionnaire about ocular discomforts and quality of life before and after treatments. Results: Patients with mild myopia (-0.75~-2.00) were 61 (35.5%), and 46 (26.7%) were emetropia (${\pm}0.50$ or less). After treatment, uncorrected visual acuity in both eyes was $0.77{\pm}0.46$, which was lower than that before treatment ($0.82{\pm}0.51$), but corrected visual acuity was $0.98{\pm}0.21$, which was higher than that before treatment ($0.93{\pm}0.28 $). About the refractive change, the emetropia group showed a slightly myopic shift while the mild myopia group intended to reduce the myopic error. However, this was not statistically significant. The scales of eyeball pain or discomfort decreased significantly from $17.59{\pm}22.5$ before treatment to $10.28{\pm}17.80$ after treatment, and the scales of the quality of life increased from $1.85{\pm}1.25$ before treatment to $2.25{\pm}1.30$ after treatment. Most patients (90.7%) were satisfied with their treatment results. Conclusions: The vision care program including traditional Korean treatment for elementary school students was effective to improve corrected visual acuity in both eyes and delay the progression of school myopia. In addition, it helped to reduce eyeball pain and discomfort and ultimately to enhance quality of life. Long-term follow-up studies will be needed.

Efficacy of PNF Group Exercise Program in Chronic Stroke (만성 뇌졸중 환자에 대한 PNF 집단 운동프로그램의 효과)

  • Objective : The purpose of this study was to evaluate the influence of a community based group exercise intervention on motor functional capacity. To evaluate the immediate(post-treatment) effects after 6-weeks exercise program on the group exercise intervention(PNF and Circuit exercise). Methods : Subjects-Included persons with stroke who were living in the community. Thirty-seven subjects were randomly assigned to the PNF, Circuit exercise and control group participated in a repeated measures design that evaluated the subjects with pre-treatment, post-treatment(6 weeks). Functional ability outcome measures assessed the motor assessment scale(MAS) and EMG. Both treatment groups participated in exercise cJass three times a week for 6 weeks. Group programs focused on balance, functional motor capacity and walking ability. The PNF program was modified PNF pattem and techniques with emphasis on functional tasks when possible, as well as stretching of the more affected limb particularly in the more affected shoulder. The Circuit program with subjects completing practice at a selies of work station as well as participating in walking races and relay with other members of the group. Results : Compared with the control group, the treatment group had larger improvements in the motor function ability after 6 weeks treatment and Post-treatment test scores were more significant than the pre-treatment score. 1. Motor function were assessed by using MAS, sit to stand, walking and upper function were assessed pre-treatment versus post-treatment measures revealed a statically significant(p<.05). There were significant differences between the groups. Compared with the control group, the treatment group had larger improvements. 2. In the treatment groups, demonstrated difference in the electromyographic activation of biceps, triceps, quadriceps and tibialis anterior muscles on the paretic side in the response to the reaching arm movement and stepping motion in stance. The difference in muscle activation improvement were not statically significant. Conclusion : The results of this study showed that the PNF and Circuit group exercise intervention can improve motor functional ability. This study suggests that the PNF and Circuit exercise programs is appropriate for community-based group exercise principles. It leads to gain and maintain potential function for disabled persons after stroke in the community.

  • Intervertebral disk disease is a common neurological disease in dogs. In veterinary neurology clinics, the incidence of intervertebral disk disease and the spinal cord compression rate of the disk are well documented. However, the direction of herniation of the intervertebral disk is relatively less known on Magnetic resonance imaging (MRI). Therefore, this study systematically investigated and analyzed the intervertebral disk disease in dogs. Especially, classification according to the direction of herniation of the disk, the degree of pre - treatment neurological manifestation, and the compression rate was investigated. In addition, the treatment methods of patients who received treatment after visiting the Chonbuk animal medical center (CAMC) were divided into pharmacological treatment and surgical treatment. The location of intervertebral disc in spinal canal was mainly ventral (69.9%), and when the prognosis was evaluated according to the compression rate of the spinal cord, the prognosis of the surgical treatment (72.7%) was more effective than the non - surgical medication (40.6%). Surgical treatment (83.3%) was more successful than nonsurgical medication (60%), especially in cases of severe spinal cord compression (> 50%). In this study, surgical treatment was more effective when the neurological symptoms were severe or the spinal cord compression rate was high.

Effects of a Maternal Education Program Based on the Temperament Theory on the Temperamental Goodness-of-Fit between Mother and Child (기질이론에 근거한 어머니교육 프로그램이 모아기질조화에 미치는 효과)

  • Purpose: This study was aimed to evaluate the effects of a maternal education program based on the temperament theory on the temperamental goodness-of-fit between mother and child. Method: The research method was a nonequivalent, control group, non-synchronized design and the experimental period was from May 10, 2004 to July 24, 2004. The subjects were mothers who had children 3-4 years old who registered in eight kindergartens or infant schools in M city. Among the total 94 subjects, 47 were allocated as an experimental group and 47 were a control group. Using the SPSS/WIN 10.0 program, the data was analyzed by a Chi-square test, and t-test. Result: 1. After treatment with a maternal education program based on the temperament theory, the experimental group significantly decreased in scores for perceived children's behavior problems in comparison to the control group(t=-4.01, p<.001). 2. After treatment with a maternal education program based on the temperament theory, the experimental group did not significantly decrease in scores for parenting stress in comparison to the control group(t=-.85, p=.40). 3. After treatment with a maternal education program based on the temperament theory, the experimental group did not significantly decrease in scores for rejective parenting behaviors in comparison to the control group(t=-1.32, p=.19). Conclusion: A maternal education program based on the temperament theory is a useful intervention to decrease perceived children's behavior problems.

  • We demonstrated micro-scale conducting polymer patterning based on a selective surface treatment. A substrate with a patterned photoresist was immersed into OTS (Octadecyltrichlosilnae) solution. The protected substrate areas were hydrophilic after removing the PR resist, where a conducting polymer solution was coated selectively by spin-coating method.

Implementation of A Hospital Information System in Ubiquitous and Mobile Environment

  • In this paper, we developed a Hospital Information System in which the business process is formalized and a wire/wireless integrated solution is used. This system consists of the administration office program, the medical office program, the ward management program and the rounds management program. The administration office program can enroll and accept patients, issue and reissue the RFID card. The medical office program inputs a medical examination and treatment, outputs a diagnosis, requests a hospitalization, retrieves the record of a medical examination and treatment, assigns the corresponding examination room to the accepted patients, and updates the number of an waiting patient and a patient number according to the examination room on real time. The ward management program handles hospitalizations and leaving hospital, a nurse's note, and an isolation ward monitoring. The rounds management program handles a medical examination and treatment, and a leaving hospital using PDA. This developed system can be built at low cost and increase the quality of the medical services highly by making it automated the medical administration automation. Also the small number of the medical staffs can manage the inpatients efficiently by using the monitoring functions.

Development of Teacher Training Programs for Game Addiction Treatment (게임 중독 치료를 위한 교사 연수 프로그램 개발)

  • This study aimed to develop the teacher training programs to prevent and treat of game addiction. First, we analyzed the existing training programs concerning game addiction and searched for various strategies applicable to game addiction prevention and treatment education programs. On this ground, training programs were divided into three courses that were the prevention program for normal students, the treatment program I for students in the potential-risk group, and the treatment program II for those in the high-risk group. For the prevention program, commonalities were drawn and developed from the existing studies and training programs; and for treatment programs, game addiction clinic centers were analyzed to draw various useful strategies and contents. We also developed the details of those programs in consultation with experts. As a result of content validity about the developed teacher training program to prevent and treat of game addiction, this program was generally appropriate and acceptable. We expect that the developed programs help to treat a student who has game addiction effectively.

Clinical Effect of Korean Medical Treatments on Stroke in a Public Health Center (일개 보건소에서 시행한 중풍에 대한 한의 진료의 효과분석)

  • Objectives: In 2019, we conducted a 12-week public health promotion program on the use of Korean medical treatments for elderly people who are at potential risk for stroke. A significant improvement effect was obtained through the program. The objective of this study was to report the results of this public health promotion program. Methods: Fifty people participated in the program. The participants were received herbal medicine and acupuncture treatments for stroke prevention for 12 weeks. Systolic blood pressure, diastolic blood pressure, blood sugar, weight, and body fat were evaluated twice. A questionnaire was used to assess the European Quality of Life-5 Dimensions (EQ-5D) score, the stroke danger signs, the recognition of stroke signs, and treatment satisfaction. Results: During the 12-week Korean medical treatment program, systolic blood pressure was significantly decreased by 119.40±19.32 mmHg, diastolic blood pressure was decreased by 79.80±8.92 mmHg, and weight was decreased by 58.24±9.70 Kg. The EQ-5D score was increased by 13.82±1.2 and the stroke danger signs score was increased by 26.76±3.59. Sex and EQ-5D score, stroke danger signs score, weight, and body fat were correlated. Age and weight were correlated. Conclusions: For elderly people with the potential risk for stroke, the 12-week Korean medical treatment program was effective and their satisfaction with the treatment was high. This study has significance because this is an unusual report in Korea on the effect of a Korean medical treatment program for stroke using a Korean medicine public health promotion.

The Development of Game Addiction Treatment Program using Digital Storytelling (디지털스토리텔링을 적용한 게임중독치료교육 프로그램의 개발)

  • The study proposed new approach for the developing a game addiction treatment program using digital storytelling. We chose 5 types of multimedia elements as educational contents for digital storytelling learning and prepared the teaching strategy for game addiction treatment. We applied the proposed program to 5 students who were included the high risk-user group of game addiction. We also conducted the game addiction tendency test, game usage time test, interview and the effectiveness test of proposed program. As a result of tests, the proposed program showed a positive effect that reduced the game addiction tendency. In the effectiveness test of proposed program, we found the result that 5 experimenters were improved the immersions, challenges, and social interaction, sense of accomplishment in digital storytelling program. We expect that this study will provide positive role and new education strategy for game addiction treatment.
