A Dose Volume Histogram Analyzer Program for External Beam Radiotherapy (방사선치료 관련 연구를 위한 선량 체적 히스토그램 분석 프로그램 개발)

  • Purpose: To provide a simple research tool that may be used to analyze a dose volume histogram from different radiation therapy planning systems for NTCP (Normal Tissue Complication Probability), OED (Organ Equivalent Dose) and so on. Materials and Metohds: A high-level computing language was chosen to implement Niemierko's EUD, Lyman-Kutcher-Burman model's NTCP, and OED. The requirements for treatment planning analysis were defined and the procedure, using a developed GUI based program, was described with figures. The calculated data, including volume at a dose, dose at a volume, EUD, and NTCP were evaluated by a commercial radiation therapy planning system, Pinnacle (Philips, Madison, WI, USA) for comparison. Results: The volume at a special dose and a dose absorbed in a volume on a dose volume histogram were successfully extracted using DVH data of several radiation planning systems. EUD, NTCP and OED were successfully calculated using DVH data and some required parameters in the literature. Conclusion: A simple DVH analyzer program was developed and has proven to be a useful research tool for radiation therapy.

Analysis of Community-based Health Promotion Program Targeting School-aged Children and Program Using Forest: Based on National Community Health Plan (학령기아동 대상 지역사회 건강증진 프로그램 및 숲을 활용한 프로그램 분석: 전국 지역보건의료계획을 기반으로)

  • Purpose: The purpose of this study is to analyze community-based health promotion program for school-aged children and program using forest. Methods: Seventeen health promotion programs focused on school-aged children from Community Health Plan were selected to analyze after assembling 227 of the 5th National Community Health Plans. The analysis duration was from 2012 July to November. Results: Among 17 programs, the health promotion program targeting school-aged children were included in 16 programs except one program focusing on community- orientated rehabilitation program. Eight health promotion programs using forest in 7 different areas were found. The majority of the community-based health promotion programs were focused mainly on smoking cessation, obesity, physical activity, nutrition, mental health programs. Furthermore, there was a limitation of programs utilizing forest as a health promotion resource and most of the programs using forest were located in Jeollanamdo and focusing mainly on atopy prevention and treatment. Conclusion: The importance of this study is that it analyzed nation-wide community health plan systematically, and analyze community-based health promotion program targeting school-aged and the program using forest. The results of the analysis can be used as baseline data for developing physical and mental health promotion programs using forest targeting school-aged children.

Increased Prevalence of Chronic Disease in Back Pain Patients Living in Car-dependent Neighbourhoods in Canada: A Cross-sectional Analysis

  • Objectives: Chronic diseases, including back pain, result in significant patient morbidity and societal burden. Overall improvement in physical fitness is recommended for prevention and treatment. Walking is a convenient modality for achieving initial gains. Our objective was to determine whether neighbourhood walkability, acting as a surrogate measure of physical fitness, was associated with the presence of chronic disease. Methods: We conducted a cross-sectional study of prospectively collected data from a prior randomized cohort study of 227 patients referred for tertiary assessment of chronic back pain in Ottawa, ON, Canada. The Charlson Comorbidity Index (CCI) was calculated from patient-completed questionnaires and medical record review. Using patients' postal codes, neighbourhood walkability was determined using the Walk Score, which awards points based on the distance to the closest amenities, yielding a score from 0 to 100 (0-50: car-dependent; 50-100: walkable). Results: Based on the Walk Score, 134 patients lived in car-dependent neighborhoods and 93 lived in walkable neighborhoods. A multivariate logistic regression model, adjusted for age, gender, rural postal code, body mass index, smoking, median household income, percent employment, pain, and disability, demonstrated an adjusted odds ratio of 2.75 (95% confidence interval, 1.16 to 6.53) times higher prevalence for having a chronic disease for patients living in a car-dependent neighborhood. There was also a significant dose-related association (p=0.01; Mantel-Haenszel chi-square=6.4) between living in car-dependent neighbourhoods and more severe CCI scores. Conclusions: Our findings suggest that advocating for improved neighbourhood planning to permit greater walkability may help offset the burden of chronic disease.

  • The needs to develop high value-added foods are expanding due to the trends of food market, such as importance of food security by enlarged international free trades and expanded market size for processed foods. However, our home country exhibits limit transport to development of new products and high-value added area compared to other advanced countries. For this reason Ministry of Agriculture, Food and Rural Affairs and Institute of Planning and Evaluation for Technology have launched "High Value-Added Food Technology Development Program" for improvement of value of food resources for practical use since 2013. The program contains research to increase the values of agricultural and animal resources and the construction of database. Main research topics are studies in pretreatment techniques to improve the values of agricultural and animal resources, improvement of acceptability, and processing qualities that would be engrafted on materializaton and commercialization technologies. In addition the construction of a consolidated database regarding the research achievements is included in the program. Currently, studies in processing qualities for about 30 food materials re under progress, being suitable for materialization, for examples drying, fermentation, grinding, heat-treatment, and so on. The research results are provided in public through the consolidated database website after reconstruction in the form of united database format. To date the database containing the about 300 of research contents for process qualities has been constructed.

Improvement of Total Floor Area Planning for the Feasibility Study on the Establishment of a General Hospital - Focused on Pre-feasibility Study of KDI Case Analysis (종합병원 건립 타당성 조사의 연면적 계획 개선방안 - 공공의료기관 재정사업 예비타당성조사 사례를 중심으로)

  • Purpose: The planning of medical facilities involves formulating a comprehensive medical basic plan, translating it into spatial dimensions through a space program. Feasibility assessment often relies on empirical methods like floor area per bed. However, with the shift towards specialized medical concepts, proportional scaling to bed numbers is challenging. This study proposes scale planning improvements during the feasibility assessment stage for comprehensive hospitals, analyzing cases using area determination factors and standard areas based on medical resources. Methods: The Korean Development Institute's Public Investment Management Center (KDI) identified issues in the scale determination of medical facilities in the Preliminary Feasibility Study Guidelines and investigated alternative approaches for determining the scale of a case that passed the preliminary feasibility study in 2019. The study assessed the feasibility of applying individual factors to determine not only the number of beds but also the scale at the sector and department levels. Additionally, a statistical analysis was conducted to examine the correlation between the total number of beds and various area determination factors. Results: Results suggest a strong correlation between total beds and major equipment needs, but in hospitals with <500 beds, this correlation weakens. Ward section scale is better calculated per ward type, not just total beds. Outpatient department scale depends on specialists, influencing treatment room numbers. Medical personnel play a crucial role in determining the scale of sections like rehabilitation therapy rooms, operating rooms, dialysis rooms, and overall facility scale.

ANALYSIS BY SYNTHESIS FOR ESTIMATION OF DOSE CALCULATION WITH gMOCREN AND GEANT4 IN MEDICAL IMAGE

  • The use of GEANT4 simulation toolkit has increased in the radiation medical field for the design of treatment system and the calibration or validation of treatment plans. Moreover, it is used especially on calculating dose simulation using medical data for radiation therapy. However, using internal visualization tool of GEANT4 detector constructions on expressing dose result has deficiencies because it cannot display isodose line. No one has attempted to use this code to a real patient's data. Therefore, to complement this problem, using the result of gMocren that is a three-dimensional volume-visualizing tool, we tried to display a simulated dose distribution and isodose line on medical image. In addition, we have compared cross-validation on the result of gMocren and GEANT4 simulation with commercial radiation treatment planning system. We have extracted the analyzed data of dose distribution, using real patient's medical image data with a program based on Monte Carlo simulation and visualization tool for radiation isodose mapping.

A Research on Actual Conditions of Visiting Nursing Program on Busan (부산지역 방문간호사업 실태에 관한 연구)

  • Purpose: The purpose of this research is to give basic data about a way of connection to individual centers and vitalization of visiting nursing program on Busan. Method: The research is done with survey for eight hospitals. sixteen public heath centers, forty-six social welfare centers to be practised visiting nursing program on Busan for actual condition of that. Results: 1. The average nursing career is below 5-10 years. But visiting nursing career is below 3 years(70%) in hospital. public, public heath center, and heath center. 2. Hospitals coverage of visiting nursing service is city as a whole. Whereas public heath center and social welfare center covered some local area. 3. Client of visiting nursing service possess in order of alone lived elderly, person in uncomplete movement, a disabled person, and a chronic disease person. 4. The main service in visiting is patient assessment, basic nursing activity, and treatment nursing activity with medication care, B.P check, dressing, bedsore care, catheter care and exchange, fluid therapy. Particularly, the hospital runs parallel to basic care and treat care in 100%. The social service center has 65.0% in a patient assessment and basic nursing activity. 5. The concern about services connecting with other center is very high. Conclusion: Actual conditions of visiting nursing program on Busan, which is presented in this study. The results of this study will become the pillar of visiting nursing program planning and application.

A Remote Medical Treatment System for Stroke Recovery using ZigBee-Based Wireless Brain Stimulator (ZigBee 기반의 무선 뇌자극기를 이용한 원격 뇌졸중 치료 시스템)

  • Stroke patients need regular medical treatments and rehabilitation training from their doctors. However, severe aftereffects caused by stroke allow them minimum activities, which make it difficult for them to visit doctor. Recently, electric brain stimulation treatment has been found to be better way compared to conventional ones and many are interested in using this method for the treatment of stroke. In this study, we have developed a remote medical treatment system using wireless electric brain stimulator that can help the stroke patients to get a treatment without visiting their doctors. The developed remote medical treatment system connects the doctors to the brain stimulator implanted in the patients via the internet and ZigBee communication built in the brain stimulator. Also, the system receives personal information of the connected patients and cumulates the total records of electric stimulation therapy in a database. Doctors can easily access the information for better treatment planning with the help of graphical visualization tools and management software. The developed remote medical treatment system can be applied to the electric stimulation treatments for other brain diseases with a minor change.

Independent Verification Program for High-Dose-Rate Brachytherapy Treatment Plans (고선량률 근접치료계획의 정도보증 프로그램)

  • Purpose: The Planning of High-Dose-Rate (HDR) brachytherapy treatments are becoming individualized and more dependent on the treatment planning system. Therefore, computer software has been developed to perform independent point dose calculations with the integration of an isodose distribution curve display into the patient anatomy images. Meterials and Methods: As primary input data, the program takes patients'planning data including the source dwell positions, dwell times and the doses at reference points, computed by an HDR treatment planning system (TPS). Dosimetric calculations were peformed in a $10\times12\times10\;Cm^3$ grid space using the Interstitial Collaborative Working Group (ICWG) formalism and an anisotropy table for the HDR Iridium-192 source. The computed doses at the reference points were automatically compared with the relevant results of the TPS. The MR and simulation film images were then imported and the isodose distributions on the axial, sagittal and coronal planes intersecting the point selected by a user were superimposed on the imported images and then displayed. The accuracy of the software was tested in three benchmark plans peformed by Gamma-Med 12i TPS (MDS Nordion, Germany). Nine patients'plans generated by Plato (Nucletron Corporation, The Netherlands) were verified by the developed software. Results: The absolute doses computed by the developed software agreed with the commercial TPS results within an accuracy of $2.8\%$ in the benchmark plans. The isodose distribution plots showed excellent agreements with the exception of the tip legion of the source's longitudinal axis where a slight deviation was observed. In clinical plans, the secondary dose calculations had, on average, about a $3.4\%$ deviation from the TPS plans. Conclusion: The accurate validation of complicate treatment plans is possible with the developed software and the qualify of the HDR treatment plan can be improved with the isodose display integrated into the patient anatomy information.

  • Ubiquitous healthcare (U-healthcare) system is one of potential applications of embedded system. Conventional U-healthcare systems are used in health monitoring or chronic disease care based on measuring and transmission of various vital signs. However, future U-healthcare system can be of benefit to more people such as stroke patients which have limited activity by providing them proper medical care as well as continuous monitoring. Recently, an electric brain stimulation treatments have been found to be a better way compared to conventional ones and many are interested in using the method toward the treatment of stroke. In this study, we proposed a remote medical treatment system using ZigBee-based wireless electric brain stimulator that can help them to get a treatment without visiting their doctors. The developed remote medical treatment system connects the doctors to the brain stimulator implanted in the patients via the internet and ZigBee communication built in the brain stimulator. Also, the system receive personal information of the connected patients and cumulate the total records of electric stimulation therapy in a database. Doctors can easily access the information for better treatment planning with the help of graphical visualization tools and management software. The developed remote medical treatment system can extend their coverage to outdoors being networked with hand-held devices through ZigBee.
