Clinical outcomes of synchronous head and neck and esophageal cancer

  • Purpose: To investigate clinical outcomes of synchronous head and neck and esophageal cancer (SHNEC). Materials and Methods: We retrospectively reviewed 27 SHNEC patients treated with curative intent at a single institution. The treatment modality for individual cases was usually determined on a case by case basis. Results: The median follow-up duration for the surviving patients was 28.2 months. The most common site of head and neck cancer was hypopharyngeal carcinoma (n = 21, 77.7%). The lower esophagus was the most common location of esophageal carcinoma (n = 16, 59.3%). The 2-year progression-free survival (PFS) and overall survival (OS) rates were 57.5% and 39.6%. Major pattern of failure was locoregional recurrence in the study patients. Esophageal cancer stage, the Eastern Cooperative Oncology Group (ECOG) performance status, and pretreatment weight loss were significant prognostic factors for OS in univariate analysis. Treatment-related death was observed in two patients, and one patient developed a grade 4 late treatment-related complication. Conclusion: Although the survival outcome for SHNEC is poor, long-term survival might be achievable with aggressive treatment with stage I-II esophageal cancer and good performance.

Research On General Attitude Of Parents Toward Their Cerevral Palsy Children (뇌성마비환자 부모들의 일반적인 개념조사)

  • This Research has been made through the questionnaire(enquete) sent to parents whose children are under cerebral palsy treatment in the hospitals of Daejon, Daegu and Pusan area and in other social welfare organizations such as Korea C.P. Welfare institution and Social Welfare Corporation during the period of Februay to August 1997. Form this research, we have obtained following results : 1. Mostly, the blood type of parents of cerebral palsy children is "0" type in father side (24 persons : 30%) and "A" type in mother side(28 persons : 35%), 2. For their(parents) baby-delivery, it shows that the nature delivery was adoped in most cases i. e. 50 persons(62.5%). 3. The birth weight of cerebral palsy children appears to be less than 2.4Kg in most cases(24 persons : 30%) 4. Most of parents positively want to take a medical treatment for their children with hope, but parents of 5 patients(6.3%) have a negative attitude against medical treatment and its cure. 5. Parents of 25 patients (31.7%) appear to have a sense of shame about the fact that their children are C.P. patients. 6. The 65 parents(81.3%) are depending on medical treatment and physical therapy as a means of cure.

Review on Conservative Treatment of Spinal Scoliosis (척추측만증의 보존적 치료에 대한 문헌적 고찰 - 국내 발표 논문을 중심으로 -)

  • Objective : The purpose of this study is to research the trend of the theses related to conservative treatment of spinal scoliosis and to establish the direction of futher studies into the spinal scoliosis Methods : We reviewed and analyzed all theses published by Korean research institution. And these theses were classified by field of study, theses type, symptoms and illnesses, evaluation methods, treatment type, clinical outcome. Result and conclusion : 1. Classified by the major field of study, oriental rehabilitation medicine and physical education accounted for 9 papers, followed by 6 in orthopedics medicine. 2. Upon classifying theses according to research method type, clinical trial theses were more than casuistic theses. 3. Upon classifying theses according to symptoms and illnesses in scoliosis, the most was idiopathic scoliosis. 4. Theses used mostly Cobb's angle for evaluating in scoliosis. 5. Some few of theses used chuna therapy for treating in scoliosis. 6. Chuna therapies side by side with correction exercises were the most efficient for treating in scoliosis.

  • Chinese medicine hospital enables the users to expect more efficient cure result in the psychological stability and the comfortable indoor environment by supplying the functional and aesthetic cure space with the medical action of good quality Medical activity is to treat the human life. Thus, hospital must be more human-centered-place than other space. Thus study is establishment of space to be able to lead more rational and active participation than the conservative and passive image of the department of diagnosis and treatment for outpatients is desirable. This study is the indoor schedule to suggest the direction about the department of diagnosis and treatment for outpatients of Chinese medicine hospital of native image with more comfortable and positive approach on the basis of above points at issues as the schedule to fulfill the performance of medical function and the emotional and psychological satisfaction of users as the human being-centered-medical institution on the subject of the department of diagnosis and treatment for outpatients in Chinese medicine hospital. And, this researcher progressed as follows by being premised on this 1. Description of Goai, Range and Method of Study and Suggestion of Study Direction 2. Concept introduction as the Basic Approach of Theory which is necessary for Study 3. The Problems were recognized by grasping the present condition in Korea through the questionaines 4. establishment of Concept and direction which are necessary for planning the indoor of the department of Diagnosis and Treatment for Chinese Medicine Hospital 5. Progression of Design Plan attendant upon Concept 6. Analysis of the Conents attendant upon this, and Conclusiov.

Predictors of Violent Behavior by Patient or Caregiver of Patient in the Emergency Department (응급실 환자 및 보호자의 폭력행위 예측요인)

  • Purpose: The purpose of this study was to investigate predictors of violent behavior by patient or caregiver of patient in the emergency department. Methods: Subjects of the study were 447 residents who have visited the emergency department in community P Metropolitan City during the past year. The data collecting period was from June 1, 2012 to August 31, 2012. Data was collected using self-reported questionnaires. Results: Predictors of violent behavior in the emergency department were divided into personal and institutional factors. Personal factors included relationship with patients, presence of alcohol, reports of discontent during and past treatment and responses to deterioration in patient's conditions, institution factors included perceived attitudes toward medical workers' explanations and proficiency of medical workers, and delay in medical treatment hours. Conclusion: Knowledge of personal and institutional factors may permit emergency staff to minimize or prevent potential violence in the emergency department.

  • In 2015, Ministry of Health and Welfare of Korea announced a research and development plan of using Korean healthcare data to support decision making, reduce cost and enhance a better treatment. This project relies on the adoption of BigData technology such as Apache Hadoop, Apache Spark to store and process HealthCare Data from various institution. Here we present an approach a design and implementation of OLAP server in Korean HealthCare BigData platform. This approach is used to establish a basis for promoting personalized healthcare research for decision making, forecasting disease and developing customized diagnosis and treatment.

Result of Surgical Resection for Pulmonary Metastasis from Urothelial Carcinoma

  • Background: Treatment of pulmonary metastasis from urothelial cell carcinoma has been mostly palliative chemotherapy and the role of pulmonary metastasectomy has not been investigated much. Materials and Methods: This study is a retrospective interim review of pulmonary metastasectomy from urothelial carcinoma at single institution between 1998 and 2010. Overall 16 patients underwent pulmonary metastasectomies. Results: There was no postoperative complication or hospital mortality. Mean hospital stay was 6 days. Overall and disease-free 5-year survival were 65.3% and 37.5%, respectively. Conclusion: In selected patients with pulmonary metastasis from urothelial carcinoma, surgical treatment is feasible and could contribute to long-term survival in selected patients.

Mucoepidermoid Carcinoma of the Soft Palate in a Child (소아에서 발생한 연구개 점액표피양 종양 1례)

  • The purpose of this paper is to review our experience with mucoepidermoid carcinoma(MEC), a rare tumour in minor salivary glands, in a paediatric patients. 15-year-old boy was noted to have a irregular round mass appearing atthe soft palate just to the right of the mid-line. A computed tomographic(CT) scan showed a palatal mass limited to soft palate with no bony erosion. The lesion was curetted and debulked. Pathology was reported as an intermediate-grade mucoepidermoid carcinoma, and the patient was considered to radiation therapy institution for further treatment. To date, patients remain free of disease. Wide local excision is the treatment of choice for low to intermediate grade MEC of the minor salivary glands in paediatric patients.

Thoracoplasty and Myoplasty for Operative Treatment of Postpneumonectomy Empyema - A Case Report - (전폐절제술후 발생한 농흉의 흉곽성형술과 근성형술을 이용한 수술치험 -1례 보고-)

  • Initial successful treatment of postpneumonectomy empyema depends to a large extent on adequate dependent drainage of the empyema sac and the use of antibiotics. But definite control of the infected space remains a disturbing and controversial area in the field of thoracic surgery. A 55-year-old man had a right pneumonectomy for tuberculosis with the development of postoperative thoracic empyema and in October 1973. Postoperatively, an empyema developed and the condition was managed with closed drainage and an open window thoracostomy. He was transferred to our institution in October 1988, and underwent thoracoplasty for the obliteration of the empyema space, resulting in a remaining space. The remaining space after thoracoplasty was obliterated by myoplasty using a rotation flap of splitted pectoralis major muscle three months later. He was discharged with uneventful course 12 days after operation, and continues to do well 3 months following operation. Our experience shows that thoracoplasty and myoplasty offer an effective alternative method of management of post-pneumonectomy empyema.

A Survey Examining Satisfaction with Korean Medical Treatment in Pediatric Patients Recovering from Traffic-Accident Injuries (소아 교통사고 환자의 한방치료 만족도 조사)

  • Objectives This study aims at using information about treatment satisfaction obtained from surveying pediatric traffic-accident patients to guide future improvement in the quality of traditional Korean medical services. Methods The survey included 95 pediatric patients treated for traffic-accident injuries at a traditional Korean medical institution. The questionnaire was developed from a similar satisfaction survey for adult traffic-accident patients and approved by a traditional Korean pediatrician. The year-long survey ran from April 2020 to April 2021. The collected data were analysed using t-tests, one-way analysis of variance, and cross-tabulation. Results Of the pediatric traffic-accident patients surveyed, the highest percentage of children were aged between 1 and 7 years (70.5%). Just over half the patients (55.8%) complained of sleeping difficulties, and 54.7% exhibited symptoms of anxiety and fear. Almost all the patients surveyed (97.9%) reported an improvement in their symptoms after undergoing traditional Korean medical treatment. The degree of satisfaction with the treatment correlated with the alleviation of symptoms. Approximately 83.2% of respondents reported 'difficult-to-administer treatment for children' and rated their satisfaction degree comparatively lower than those patients with no reported treatment difficulties. The most struggling treatment was herbal medicine (42.1%), highlighting the need for improvement in the adaptability of herbal medicine to the treatment of pediatric patients. Conclusions Treatment acceptance by pediatric traffic-accident patients affects reported treatment satisfaction. It is important to improve treatment experience and adaptability to pediatric patients to improve the perceived quality of traditional Korean medical services.
