Billet Treatment and Die Design for Net-Shape Forming of Gear by Cold Forging (정밀정형 냉간단조 기어성형을 위한 소재처리와 다이설계)

  • In this paper, net-shape forming of an automobile gear is investigated. Barrel, a component of automobile start motor, is adopted as a net-shape forming. In order to accomplish the goal of net-shape forming without cutting of tooth and cam after forming, forming ability is raised through billet treatment and die design. As a technique of billet treatment spheroidizing annealing of billet to get low hardness and molybdenum disulphide coating to get low contact friction between billet and die is carried out. One of critical points of die design, fillet radii variation of tooth of die is applied to get smooth surface of barrel after cold forging. As a measurement of tooth accuracy, distance between two pins and lead-tooth alignments are investigated. Cam profile accuracy is checked with a 3D measuring instrument. Results obtained from the tests revealed reasonable result with respect to design goal. By these results, the paper shows that reasonable results can be obtained by billet treatment and die design for net-shape forming.

The Retrospective Analysis of Obesity and Overweight Female Patients with Clinical Treatment including Herbal Medicine (비만 및 과체중 여성 환자에서 한약 처방의 치료 효과에 대한 후향적 분석)

  • Objectives: The aim of this study was to investigate the effect of herbal medicine on obesity and overweight female patients and to gather the information of a proper treatment goal for anti-obesity program in Korean medicine clinic.Methods: 230 subjects were recruited and those data were analyzed for body-mass index (BMI), treatment duration, weight, body fat, muscle volume and body fat/weight loss ratio. It was compared among age groups and severity of obesity.Results: It was achieved to loss 12-13 kg for 105.8 days thru obesity program of Korean medicine clinic. There were no significant differences for BMI loss, weight loss, body fat loss, muscle volume gain and treatment duration among age groups. It was found that there were significant differences among overweight, obesity and severe obesity group for BMI loss, weight loss, body fat loss and muscle volume gain except treatment duration. It was assumed the weight loss for 1 month was 3.2 kg, 4.2 kg, 4.5 kg for each severity group respectively.Conclusions: It was found herbal medicine treatment would be effective for female obesity and expected the patients could achieve 12-13 kg weight loss for 15 weeks treatment including 8-12 weeks herbal medicine administration and BMI loss per month could be 1.22 kg/m2, 1.60 kg/m2 and 1.70 kg/m2 for severity of obesity respectively. Hereafter, further controlled study with more numbers of patients should be needed to determine the goal of medical treatment for obesity.

An Effect of a Group Art Treatment Program on Isolated Children's Self-Concept and Adaptation to the School Life in Elementary School (집단미술치료프로그램이 초등학교 고립아동의 자아개념 및 학교생활적응에 미치는 효과)

  • This study aimed at examining an effect of a group art treatment program on isolated children's self-concept and adaptation to the school life in an elementary school. To achieve this goal, the two hypotheses were made. Hypothesis 1. Children in the experimental group that participated in the group art treatment program will show more improvement in self-concept than those in the control group. Hypothesis 2. Children in the experimental group that participated in the group art treatment program will show more improvement in the ability to adapt themselves to the school life than those in the control group. To test the hypotheses, the socio-emotional isolation test developed by Heo Seung-hi (1993) was carried out with 120 third-graders at K elementary school in Busan; then, 24 isolated children (12 boys and 12 girls) at the upper 25 percent level were randomly sampled and assigned to each of experimental and control groups. The five-stage art treatment program as a reconstruction of the prior study to meet the goal of this study was provided to children in the experimental group at the researcher's classroom after school for 13 sessions, two sessions per week. The self-concept test manufactured by Song In-seop (1996), the school life adaptation test developed by Lee Yeong-seon (1997), and the experimental group's school life drawing (KSD) were used to test an effect of the program. For data analysis, two methods, a quantitative analysis of two tests and a qualitative analysis of KSD, were used. The findings were as follows: After implementing the art treatment program with the experimental group, there was significant improvement in all areas. This study concluded that if a group art treatment program is applied to elementary school practice, it will help isolated children improve their self-concept and ability to adapt themselves to the school life.

  • The prevalence of lower extremity disease is increasing with age. With recent technological advancements, endovascular treatment is being performed more frequently. The treatment goal of intermittent claudication is to improve walking and reduce claudication. To achieve these goals, anatomical durability and patency are important. In patients with critical limb ischemia, the lesions are diffuse and particularly severe in below-the-knee arteries. The treatment goal of critical limb ischemia is to promote wound healing and to prevent major amputation, which is evaluated by the limb salvage rate. Primary stenting using covered or bare metal stents is a widely accepted endovascular treatment. While drug-eluting technologies with or without atherectomy are widely used in the treatment of femoropopliteal disease, balloon angioplasty is the mainstay treatment for below-the-knee intervention. CT angiography provides a road map for planning endovascular treatment in patients without absolute contraindications.

The Effects of Concept Mapping with Explanation Feedback in the Undergraduate General Chemistry Course (일반 화학 수업에서 설명적 피드백을 이용한 개념도 학습의 효과)

  • In this study, the effects of concept mapping with feedbacks providing explanatory comments on students' achievement, science learning anxiety, and science learning motivation were investigated in the undergraduate general chemistry course. The aptitude-treatment interactions between students' level of mastery goal orientation and the concept mapping with explanation feedback treatment were also examined. Sixty-seven freshmen from an university of education were assigned to a control group and a treatment group. The tests of mastery goal orientation, science anxiety, and science learning motivation were administered as pretests. For the treatment group, feedback providing students with explanatory comments through whole class discussion was presented after each concept mapping. Whereas the control group students were presented with opportunities solving excercise problems followed by explanation feedback. The intervention was lasted for 10 weeks (30 class periods). After the instructions, a researcher-made achievement test, the science learning anxiety test, and the science learning motivation test were administered. The results indicated that no statistically significant difference was found in students' achievement. In the science learning anxiety, however, the scores of the treatment group was significantly lower than those of the control group. The scores of the treatment group also tended to be higher, though not significant, than those of the control group in the science learning motivation. However, no significant aptitude-treatment interactions were found in all dependent variables.

  • Purpose: As emergency medical service fund is further expanded due to amendment of the law on emergency medical services in 2008, Korean government has prepared to intervene in a comprehensive manner to strengthen a trauma treatment system. As a result, it announced a master plan to establish a serious trauma treatment center in 16 areas across the nation. Therefore, this study has attempted to investigate the current status of the serious trauma treatment centers and suggest the goal and improvement plan of future serious trauma treatment centers. Methods: As of 2011, Korea operates 23 emergency cerebrovascular service centers, 23 emergency heart disease centers and 35 severe trauma treatment centers across the country. 12 emergency medical service centers have been chosen among the serious trauma treatment centers. Then, top six (6) centers chosen at Emergency Medical Institute Assessment 2011 by Ministry of Health and Welfare have been selected, and floor layout and spatial allocation by usage have been reviewed and analyzed. Results: Consequently, this study has investigated the spatial components, circulation layout and spatial allocation of a serious trauma treatment. For construction planning in consideration of the fundamental objectives and goal of emergency medical services, it is essential to allot spaces and select exact spatial components. It appears that it is necessary to design spaces for emergency medical services and come up with construction planning through appropriate spatial allocation.

Proposal of East-West Integrative Medicine Manual for Rehabilitation after Shoulder Surgery (어깨 수술 후 재활 치료에서의 한양방협진 매뉴얼 제안)

  • Objectives This is one of the manuals of East-west integrative medicine which was created by the Committee on integrative medicine of Chung-Yeon Korean Medicine Hospital. The purpose of this manual is to support clinical decision making and communication during the rehabilitation of patients after shoulder surgery. Methods The drafting was done by two rehabilitation specialists in Korean medicine. After a rehabilitation specialist in western medicine reviewed the draft and exchanged their ideas, a revised version that reflects the goal of consultation was made. Then the Committee agreed to adopt the manuals through the process of review and feedback in addition to face-to-face discussions. Results This manual describes clinical decision making for rehabilitation after arthroscopic rotator cuff repair, SLAP repair, and arthrolysis. Therefore it contains the schedule of rehabilitation treatment through the surgical technique, general goal of the rehabilitation by phase, guide for patients and postoperative infection management. Conclusions The proposal of this manual has a significance for it provides information about decision making process and contents of treatment in one medical institution for East-west integrative rehabilitation treatment after shoulder surgery.

Perioperative Hypertension Management during Facelift under Local Anesthesia with Intravenous Hypnotics

  • Perioperative hypertension is a phenomenon in which a surgical patient's blood pressure temporarily increases throughout the preoperative and postoperative periods and remains high until the patient's condition stabilizes. This phenomenon requires immediate treatment not only because it is observed in a majority of patients who are not diagnosed with high blood pressure, but also because occurs in patients with underlying essential hypertension who show a sharp increase in their blood pressure. The most common complication following facelift surgery is hematoma, and the most critical risk factor that causes hematoma is elevated systolic blood pressure. In general, a systolic blood pressure goal of <150 mm Hg and a diastolic blood pressure goal of >65 mm Hg are recommended. This article discusses the causes of increased blood pressure and the treatment methods for perioperative hypertension during the preoperative, intraoperative, and postoperative periods, in order to find ways to maintain normal blood pressure in patients during surgery. Further, in this paper, we review the causes of perioperative hypertension, such as anxiety, epinephrine, pain, and postoperative nausea and vomiting. The treatment methods for perioperative hypertension are analyzed according to the following 3 operative periods, with a review of the characteristics and interactions of each drug: preoperative antihypertensive medicine (atenolol, clonidine, and nifedipine), intraoperative intravenous (IV) hypnotics (propofol, midazolam, ketamine, and dexmedetomidine), and postoperative antiemetic medicine (metoclopramide and ondansetron). This article focuses on the knowledge necessary to safely apply local anesthesia with IV hypnotics during facelift surgery without the assistance of an anesthesiologist.

  • Objective : The purpose of this study was conducted to find treatment approach, functional test process with proprioceptive neuromuscular facilitation(PNF). Method : This is a literature study with books, seminar note and book for PNF international course. Result : Suggest of the functional test with functional level and structural level treatment approaching with hypotheses and problem list from the functional test and provide how, what, where for the treatment with the level. Conclusion : Treatment plan and approach has changed from disease to structure function, performance, participation the life, relationship with environment factors of human being. For the treatment approach need the functional test and functional goal.
