State of Orthodontic Treatment and Facial Plastic Surgery among Adult Patients (일부 성인의 교정치료 및 안면 성형 수술에 대한 실태)

  • This study was designed to investigate the status of orthodontic treatment and facial plastic surgery among 389 patients who visited dental clinics located in Seoul and Gyeonggi Province. After the collection of data, an analysis was carried out using a spss statistical program. The analysis results are as follows. the level of satisfaction with orthodontic treatment from patients with experience of the orthodontic treatment turned out to be high(p<0.01). Women considered 3.01 to 4 million won to be reasonable cost for orthodontic treatment (p<0.05), which is higher cost recognized by men, and patients with experience of orthodontic treatment recognized 19 to 24 months as the proper orthodontic treatment period(p<0.01). In addition, women showed higher figures than men in their awareness on the need for orthodontic treatment checkups(p<0.001), and facial plastic surgery experience (p<0.01). With respect to the part of the face on which they hoped to have plastic surgery, eye plastic surgery was higher in women (p<0.01), and nose surgery turned out to be higher in patients with experience of orthodontic treatment(p<0.05). Meanwhile, women considered the eye as the most important part among other facial areas (p<0.001), and they recognized that dentist should perform plastic surgery on both jaws among facial areas(p<0.01). In conclusion, the dental medical staff should provide consultations and operate treatment systems in consideration of gender differences and needs of dental patients. In the case of orthodontic treatment for the improvement of facial appearance, especially double jaw surgery which is closely related to the oral function, professional dental medical staff needs to perform the operation, considering the special oral environment. In addition, it is determined that provision of relevant medical information and promotion of dental service is required to provide patients with correct knowledge about orthodontic treatment and facial plastic surgery.

Physical Therapy of Male External Genitalia Lymphedema from Urological Cancer -Case Report- (비뇨기 암으로 인한 남성 외성기 림프부종의 물리치료 -사례연구-)

  • Purpose: Cases of male external genitalia lymphedema are relatively uncommon in the clinical physical therapy field. Of more than two thousand clinical cases treated by the author in the lymphedema treatment center, only 4~5 cases of male external genitalia lymphedema were encountered. The present case report presents the experience and treatment results of male external genitalia lymphedema from urological cancer. Method: This case was a 47-year-old man, who was treated with complex decongestive physical therapy consisting of manual lymph drainage, compression bandage (including the external genitalia), remedial exercise, skin care. Result: The results were relatively good. The clinical experience would suggest that early treatment and education can reduce the external genitalia lymphedema. However, the treatment efficacy remains limited. Therefore, early treatment is vital for more successful outcomes. Conclusion: The relief of symptoms and a decrease in male external genitalia lymphedema may be the best result from long-term conservative management.

Exploring the Factors Influencing Attitudes Toward Withdrawal of Life-sustaining Treatment in Adolescents (청소년의 연명치료중단에 대한 태도에 영향을 미치는 요인)

  • Purpose: This study aimed to explore the factors influencing attitudes toward withdrawal of life-sustaining treatment (WLST) in adolescents. Methods: This study used a descriptive research design. A structured questionnaire was administered to 185 adolescents. Data were analyzed based on t-tests, analysis of variance, Pearson's correlation coefficient, and multiple regression using the IBM SPSS 20.0. Statistical package. Results: There was a negative correlation between attitudes toward WLST and respect for life (r=-.188, p=.010). Knowing about life-sustaining treatment (β=-0.30, p<.001), experience of WLST education (β=0.21, p=.003), experience of respect for life education (β=-0.16, p=.021), and experience of death (family or acquaintance) (β=-0.13, p=.039) explained 16.7% (F=8.39, p<.001) of the variance in attitudes toward WLST. The respect for life did not affect attitudes toward WLST (β=-0.07, p=.347). Conclusion: The study findings suggest that WLST in adolescents is different from that of adults. Further research is needed to develop strategies for sound and positive attitudes toward WLST in adolescents.

Experience with Blunt Pancreatic Trauma at Eulji University Hospital (둔상에 의한 외상성 췌장 손상의 임상적 고찰)

  • Purpose: Traumatic pancreatic injury is not common in abdominal trauma injury. However, the morbidity and the mortality rates of patients with pancreatic injury, which are related with difficulties of initial assessment, establishment of diagnosis, and treatment are relatively high. The aim of this study is to review our institution's experience and suggest a diagnosis and therapeutic algorithm for use in cases involving traumatic pancreatic injury. Methods: Eighteen(18) patients with blunt pancreatic injury from January, 2004 to October 2012 were included in this study. We analyzed treatment and diagnosis method, other organ injury, treatment interval, hospital stay and complications retrospectively. Results: Nine patients were treated with conservative medication and another nine patients were treated surgically. Complications occurred in nine patients, and one patient died due to intraventricular hemorrhage and subdural hemorrhage with multiple organ failure. Delayed surgery was performed in three cases. The early and delayed surgery groups showed difference in hospital stay and intensive care unit stay. Delayed surgery was associated with a longer hospital stay (p=0.007) than immediate surgery. Conclusion: In blunt pancreatic trauma, proper early diagnosis and prompt treatment are recommended necessity. Based on this review of our experience, we also suggest the adoption of our institution's algorithm for cases involving traumatic pancreatic injury.

Nursing staffs' Experience in Treatment and Managing Medical Problem in Long Term Care Settings (노인요양 시설 간호 인력의 의료적 처치 및 관리 경험)

  • Purpose: The purpose of this study is to describe the experience of medical treatment and management of nursing staffs in long term care settings. Methods: This study is qualitative research. The participants were seven nursing staff (five nurses and two nursing assistants) who had experience over six months in long term care facilities. Data were collected through in-depth interviews using semi-structured research questions. The collected data were analysed using qualitative thematic analysis method. Results: Four main themes were emerged as 'various medical treatments', 'difficulties about managing medical problems', 'lack of regulatory support' and 'strategies for solving medical problems'. Conclusion: The findings of this study provide the nursing staffs' difficulties and opinion about medical treatment and management in long term care settings. To provide more effective health service for the aged, the regulatory complement about medical treatment in long term care facilities should be considered.

Decision factors of Orthodontic Treatment by orthodontic patients (교정환자의 교정치료 결정요인)

  • Purpose: The purpose of this study is to examine decision factors to start orthodontic treatment with male and female undergraduates having experience in orthodontic treatment at a dental clinic or hospital because of malocclusion. Methods: A survey was carried out to investigate decision factors to begin orthodontic treatment with 330 male and female undergraduates attending universities located in Daegu Metropolitan City or Gyeongsangbuk-do who had experience in orthodontic treatment at a dental clinic or hospital because of malocclusion, and out of 330 questionnaire sheets, total 294 (89.1%) sheets were employed for analysis after excluding ones unreliably or erroneously answered. Results: According to the results of analyzing decision factors for orthodontic treatment, such factors as 'a dentistry-related person's suggestion', 'feeling necessity on one's own' are found to be significantly higher in students majoring in health units than those having non-dental health majors, and students residing in small and medium cities are influenced more significantly by this decision factor, 'an acquaintance's suggestion', than those living in farming and fishing villages or big cities, and students who started orthodontic treatment at elementary school were more significantly influenced by the factor, 'feeling necessity through the public media'. Conclusion: Among the factors influencing their decision on orthodontic treatment, no influence was found in sexual difference, and each of the decision factors influenced them considerably according to the difference of their residence, economic power, majors, or time to start orthodontic treatment.

Advances in management of pediatric chronic immune thrombocytopenia: a narrative review

  • Immune thrombocytopenia (ITP) is a disease in which thrombocytopenia occurs because of immune-mediated platelet destruction and decreased platelet production. Although many pediatric patients with ITP experience spontaneous remission or reach remission within 12 months of first-line therapy, approximately 20% progress to chronic ITP. Patients who do not respond to first-line treatment or experience frequent relapses are of great concern to physicians. This review summarizes recent treatments for second-line treatment of pediatric chronic ITP.

A Study Assessing Support Project of Korean Medical Treatment in Infertility in Jecheon (제천시 한방 난임 치료 지원 사업에 관한 연구)

  • Objectives : This study aims to assess whether Korean medical treatment in infertile couple is effective on clinical pregnancy. Methods : Korean medical treatment using herbal medicine, acupuncture and moxibustion was performed to thirty-one women and one man of infertility from March 2013 to April 2015. Results : After the treatment, nine patients (eight women and 1 man) of infertility became pregnant (28.13%) in thirty-two patients. In nine patients with pregnancy, five patients became spontaneous pregnant. Two patients became pregnant with in vitro fertilization (IVF), and other two patients became pregnant with intrauterine insemination (IUI). Factors influenced pregnancy assessed a shorter duration of infertility. After the treatment, survey of satisfaction in Korean medical treatment in infertility was done. Out of thirty-two patients, convenience and reliability of Korean medical treatment was reported by who had an experience of sterilization surgery. After the assessment, seven patients who had an experience of sterilization surgery and beame pregnant were having more convenience than twelve patients who were not pregnant despite sterilization surgery. Conclusions : This study suggests Korean medical treatment is useful for infertile women and men, in reverse proportion to shorter duration of infertility.

The related factors with experience and intention of dental implant among some rural elderly (일부 농촌 지역 노인의 임플란트 치료 경험 및 의향 관련 요인)

  • Objectives: Dental implants are one treatment method for tooth loss. The purpose of this study is to investigate the status of experience and intention for dental implants and related factors among elderly. Methods: The participants were 244 elderly residing in rural areas with age over 65 in Samhoeup, Yeongamgun and Jeollanamdo, Korea. Chi-square tests, t-tests, and a multiple logistic regression analysis were performed to investigate the factors influencing the experience and intention of implants. Results: Many elderlies residing in rural areas rural elderly had experienced tooth loss (83.6%). The proportion of dental implant experience was 30.9% and intention to receive implants was 57.8%. The odds ratio (OR) for experience of implants was significantly higher for participants with a lower number of chronic diseases (0: OR=12.48, 95% CI=2.12-73.38, 1: OR=4.95, 95% CI=1.66-14.72), they have dental treatment experience during the past 1 year (OR=6.61, 95% CI=1.83-23.89), they have surrounding person who have experience of implant (OR=18.06, 95% CI=2.81-115.99), higher dental implant recognition (OR=3.97, 95% CI=1.92-8.23). The odds ratio (OR) for dental implant intention was significantly higher for participants with a lower age (65-69: OR=8.18, 95% CI=2.38-278.08, 70-79: OR=3.17 95% CI=1.04-9.68), lower number of chronic diseases (0: OR=4.15, 95% CI=1.00-17.29), they have drink alcohol (OR=5.03, Cl=1.31-19.34), they have surrounding person who has experience of implant (OR=3.22, 95% CI=1.30-8.02), they have not experience of tooth loss (OR=4.65, Cl=1.22-17.70), higher dental implant recognition (OR=2.69, 95% CI=1.55-4.67). Conclusions:In the future, it is necessary to utilize the results of this study to address loss of teeth and to support dental implant treatment selection through improved increased awareness of the advantages and disadvantages of dental implants.

DIFFICULTY IN MANAGING THE BEHAVIOR AND TIME REQUIRED FOR TREATING THE PEDODONTIC PATIENTS IN ASSOCIATION WITH THEIR AGE (치과치료시 소아의 나이에 따른 행동 조절의 난이도 및 소요시간에 관한 연구)

  • The present study was undertaken to examine the relationship between time required for each step in the treatment process for pedodontic patients. and age of the patients, and experience of special training for pedodontics and career of dental practitioners. Information of these matters was gained from 580 questionnaires collected from 69 practitioners (62 male and 7 female). The questionnaires included questions about the patients' sex(303 male and 207 female) and age. The patients were categorized into 5 different age groups : Group 1, age $1{\sim}3$-year ; Group 2, $4{\sim}6$-year ; Group 3, $7{\sim}9$-year ; Group 4, $10{\sim}12$-year ; Group 5, $13{\sim}15$-year. The questions about the dental practitioners were the years of experience in private dental practice(5 years or more than 5 years) and whether or not they took the special training for pedodontics in the authorized institutes. The practitioners were asked to answer the questions about Frankl scale of the patients' behavior during the treatment, and time spent for managing the patients to be subjected to local anesthetic injection (the first behavior control), time for the injection, and time for the subsequent treatment. The results obtained by analyzing the information collected from the questionnaires were as follows : 1. The younger the patients, the lower the Frankl scale was counted at the time of the first behavior control, injection, and the subsequent treatment(p<0.001). 2. The lowest Frankl scale was scored during the injection regardless of the age of the patients. 3. Time for management and treatment was decreased in the order of age Groups 1 and 2< Group 3 < Groups 4 and 5. 4. The patients showed a more positive frankl scale in response to the treatment performed by those who were more-experienced in dental practice as compared with those were less-experienced. 5. Pedodontic training experience of the practitioners did not appear to influence the patients with respect to the treatment time and Frankl scale.
