Fabrication of complete dentures using treatment dentures for a patient with erratic mandibular movements: A case report (불규칙한 하악 운동을 하는 환자의 treatment denture를 이용한 총의치 수복 증례)

  • Long-term use of ill-fitting dentures may cause irregular movements of the mandible. In such cases, satisfactory outcomes both in terms of function and esthetics could be achieved by accurate registration of mandibular movement and centric relation when fabricating new dentures. In this case, treatment dentures with flat occlusal tables were used to register centric relation and mandibular movements of a patient with erratic mandibular movements.

  • For patients with unstable mandibular movements and centric relation of the mandible due to long-term use of ill-fitting dentures, the successful fabrication of new dentures is challenging. Before fabrication new dentures, registration of centric relation is the critical point in such cases. In this case, treatment dentures with a flat occlusal table were used to stabilize mandibular movements and to register centric relation. In order to reflect the patient's jaw relation and vertical dimension obtained through the treatment dentures to the final denture, the treatment dentures were digitally duplicated, and impressions and jaw relations were obtained using the duplicated therapeutic dentures. The clinical results were satisfactory on the aspect of aesthetic and masticatory functioned.

  • Overlay complete dentures are simple, reversible and economical treatment modality for patients with congenital or acquired disorders that severely affect the tooth development. It satisfies both the esthetic and functional demands where the extraction of teeth is not generally indicated. In pediatric patients, the overlay dentures establish a relatively stable occlusion that improves patient's tolerance to the future treatment procedures for worn dentition. This clinical report highlights the imperative need of appropriate treatment strategy and application of maxillary and mandibular overlay dentures in a pediatric patient who suffered from congenitally mutilated and worn dentition.

FACTORS INFLUENCING PATIENT SATISFACTION WITH COMPLETE DENTURES (총의치 환자 만족도의 영향요인)

  • Statement of problem: In spite of the progress in techniques and materials in complete denture prosthodontics, patients still complain of discomfort after the insertion of complete dentures. For the last several decades many prosthodontists tried to find factors influencing patient complete denture satisfaction, however the reported results became a controversy. Purpose: The purpose of the present study was to verify the factors influencing patient satisfaction with complete dentures using multiple regression analysis. Materials and methods: 33 patients who visited the department of prosthodontics, dental hospital of Yonsei University, 4 to 6 weeks after the complete denture delivery, were asked to complete the questionnaires on complete denture satisfaction, social variables and psychological variables. The Prosthodontists who treated the patients with complete dentures were also asked to complete the questionnaires on evaluation of patients' oral condition and technical quality of dentures. The factors influencing patients' satisfaction with their complete dentures were analyzed using multiple regression analysis. Results: Among the patients' sociodemographic variables. the variables of relationship with children, economic status, housing condition, other people's opinions of dentures and gender were the influential factors on patients' satisfaction with complete dentures. Patients showing the symptoms of depression, one of the psychological variables, were dissatisfied with their complete dentures. In spite of the good oral condition, patients were dissatisfied with complete dentures, where-as the technical quality of dentures did not influence patients' complete denture satisfaction. Conclusion : According to the results above, patients' sociodemographic and psychological variables rather than clinical variables including oral condition and technical quality of dentures were the influential factors on complete denture satisfaction. The results of this study may not only enable prosthodontists to predict the success and failure of complete denture treatment, but also help both prosthodontists and patients be informed of the essentials of increasing satisfaction with complete dentures.

Study on Aged Men's Utilization Status and Satisfaction of Removable Dentures (일부 노인들의 가철성 틀니 사용 실태 및 만족도 조사)

  • Purpose: It is to survey old men's utilization status of dental clinic and temporary-binding dentures and then to provide basic data to make a program for manufacturing old men's dentures and their teeth health education. Methods: Random selection was made on old men living in Daejeon and Chungnam, and a survey was conducted from April 15 to May 20, 2011. 500 questionnaires were distributed and 420 questionnaires were collected. Using spss 18.0 program, final analysis was made on 363 questionnaires except for 57 questionnaires which had missing value or showed some doubts about the unreliability. Results: 1. With respect to number of natural teeth as per the age, the upper/lower jaws had meaningful differences (P<.001), which means that the older the person is, the smaller number of natural teeth is. In the meantime, there was no meaningful difference according to sex. 2. With respect to kind of prosthetics installed in a mouth as per the age, there was meaningful difference in the upper jaws (P<.05) but there was no meaningful difference in the lower jaws. It was found out that the older the persons are, the more they use removable dentures while the younger the persons are, the more they use fixed partial dentures. 3. With respect to mouth health condition recognized by oneself, it showed bad (44.4%) and good (10.3%),which means recognition as bad is greater, and the age when prosthetics were first used is dispersed variously from below 40 to over 70. 4. Dental care institutions which they mainly used are hospital and clinics (91.5%), and 66.4% of them visited dental clinics lately. Preferential treatment if they go to dentists is as follows in the order of prosthetics (33.9%), gum treatment (24.2%) and pain treatment (14.9%). 5. 68.4% of them revisited dentists after installation of dentures and the reason for revisit is as follows in the order of poorly fitted dentures (35.1%), painful gum 25.9%, regular check-up 25.4% and difficult chewing 9.6%. Conclusion: As our country enters into an aging society, mouth health problem of old men has come to a serious issue, and therefore, a survey was conducted on some old men with respect to their utilization status of dental clinic and removable dentures in order to provide basic data for improvement of quality of their life, maintenance and enhancement of their mouth health. The study result shows that the older they are, the higher the loss rate of teeth as well as needs for prosthetics are, and thus it is thought the study will help to develop mouth health education program which may improve quality of old men's life by recovery of mouth functions declined due to loss of teeth and by regular check-up, education for mouth health control and following-up control of dentures.

PSYCHOLOGICAL ASPECT OF PATIENT SATISFACTION AND ACCEPTANCE OF COMPLETE DENTURES (총의치 환자 만족도에 대한 정신의학적 측면)

  • In spite of the progress in techniques and materials of the prosthodontic rehabilitation of patients with complete edentulous arches, prosthodontists still face patients' complaints about dissatisfaction and discomfort from their dentures. In the past several decades, prosthodontists tried to find the factors that influence the patient's satisfaction with dentures. However the results are contraversial especially with the psychological factor. In this study using 'patient denture satisfaction questionnaire' and Hopkins Symptom Checklist, we tried to find the correlation between the patient's denture satisfaction and the patient's psychological aspects. 23 complete edentulous patients who have visited the Department of Prosthodontics, Yonsei University Dental Hospital from September 1998 to June 1999 for complete denture treatment were asked to complete the questionnaires 4 to 6 weeks after the upper and lower complete delivery. After the measurement of validity of the questionnaires, correlations between the patient's general satisfaction with their new upper and lower complete dentures and the other satisfaction questions including the satisfactions with esthetics, retention, mastication, speech, comfort and other people's opinion and between the patient's general satisfaction with their new upper and lower complete dentures and the 5 symptoms of Hopkins Symptoms Checklist including somatic, obsessive-compulsive, interpersonal sensitivity, depression and anxiety were analyzed. Among the several satisfaction questionnaire items, comfort with the lower denture showed highest relation to the patient's general satisfaction with dentures. However, only the anxiety scale of Hopkins Symptoms Checklist among the other symptom scales was related to the patient's general satisfaction with dentures. The two questionaires used in this study turned out to be valid means of analyzing patient's denture satisfaction and psychological status before and after the complete denture treatment.

Full mouth rehabilitation in edentulous patient with unstable mandibular position using flat table treatment dentures and CAD-CAM technology (치료의치와 CAD-CAM 기술을 이용한 불안정한 하악위를 가진 완전 무치악 환자의 치료 증례)

  • For edentulous patients with unstable mandibular movements and abnormal facial features due to condylar fractures and morphological abnormalities, it is important to find a stable mandibular position. In this case, the patient's facial improvement, mandibular movement stability, and denture stability were improved by using flat table treatment dentures. In addition, computer-aided design/manufacturing (CAD-CAM) technology was used in denture fabrication to maintain the vertical dimension and lip support of flat table treatment dentures, we report good results in improving the patient's discomfort.

Effect of Facial Treatment for Resin Teeth on Bonding Strength of Denture (레진치아의 표면처리가 의치상과의 결합강도에 미치는 영향)

  • Acrylic resin has been widely used for dental care since it requires relatively simple equipment for treatment and less time and cost are needed to make it and, furthermore, proper strength, dimensional stability as well as durability are ensured after treatment. A survey of denture users showed, however, that more than 60% of dentures installed need repair each year, $22{\sim}30%$ of which are due to falling out of teeth. This study is aimed at exploring the means to increase bonding strength of denture by reducing the causes for falling out of teeth during the processing of dentures. For this aim, the bonding strength of dentures was compares and analyzed before and after the glazed surface of teeth contacting denture was eliminated. From the analysis, it was revealed that there was a difference of 4.3MPa in average in bonding strength between 20MPa for the glazed surface not eliminated and 24.3MPa for the glazed surface eliminated.

  • Complete dentures are one of the most basic treatment methods for the treatment of edentulous patients. The manufacturing process of traditional complete dentures goes through the steps of taking primary impressions, secondary impressions, jaw relation record, trying in wax denture, and final denture insertion. Multiple visits and complex manufacturing procedures are required, and errors may occur in each step. With the development of digital technology, manufacturing steps have been reduced by introducing digital technology to the denture treatment process. In the process of manufacturing dentures by introducing a digital process, a more precise work is possible using Computer-Aided Design, and it is possible to shorten the period of labor and reduce the number of visits. In this case, the anterior teeth arrangement of the patient's existing dentures was transferred to the final dentures using a digital method. After taking impression, try-in dentures were digitally fabricated and tried in the oral cavity to evaluate their retention in the oral cavity. Final dentures were manufactured by milling process. The number of visits was reduced, satisfactory retention and stability of dentures were obtained, and aesthetic recovery was achieved.

  • Many factors should be considered for successful denture treatment at edentulous patients: support, retention, stability, occlusion, esthetics, etc. The patient who has severely absorbed residual ridges, however, treatments are challenging to satisfy those factors. The dentures that use anatomic artificial teeth show good mastication efficiency and esthetics but, can easily lose stability at absorbed ridges. On the contrary, the dentures that use non-anatomic artificial teeth perform better stability but, lower masticatory efficiency and esthetics at absorbed ridges. The lingualized occlusion, using both anatomic and non-anatomic teeth, introduced for compromise those of the pros and cons. At lingualized occlusion, buccal cusps of the teeth do not contact on centric relation. Therefore, direction of the occlusal force towards lingually, then stability of dentures increases. This case report shows the results of the treatment flat residual ridges using complete dentures with ligualized occlusion to increase dentures stability and satisfactory of the patient.
