일부지역 치과위생사와 치위생과 재학생의 진료자세와 통증과의 관계연구 (A Study on the Relation between Treatment posture and Musculoskeletal disorders Between Dental hygienist and Dental hygiene student some areas)

  • The objectives of this study was to suggest for prophylatic of Musculoskeletal system disorders in Dental Hyginists and Dental hygiene students to maintain good posture in the dental hygiene curriculum. This study was carried with 100 dental hygienists in "M" city located in Jolla Province and 82 college students majoring in dental hygiene. The results were as follows: 1. During the operators' medical treatment, 41.7% answered that the patients' head was located in the operators' chest during their treatment. Also, 58.3% answered that the patients' head was located in the operators' abdomen. For this reason, 81.3% said that the patients' mouth became close to the operators' eyes - less than 30cm. And 65.1% said that they performed an operation in various directions. 2. The most common posture -between the position of operating and the position of assisting- is the position of operationg 1. The position of operationg 1 and 5 have an effect on the waist, and the position of assisting 3 has an effect on pains of the waist, bottoms, knees, ankles and feet. 3. The common symptoms of musculoskeletal system disorders in dental hygienists and student was the neck ache and the shoulders, and was followed by the lower back. From the cases that they visited in health care or apothecary to the above pain, symptoms were highest in the shoulders(18.3%) and were followed by the lower back(13.4%) and neck(9.1%). 4. On The necessity of education related to, According to researches, 96.3% college students and 92.5% dental hygienists answered that education related to Musculoskeletal system disorders is necessary. 5. In this study, I have 3 limitations. First, the subject of survey was selected for specific people in the specific city, Second, the personal judgment was made in judging the relations of the position of operating, the position of assisting and the aches. Third, the relations of the position of operating, the position of assisting and the aches don't have supporting medical evidences. Therefore, this study will be carried on more people with more objectives and medical method.

  • Objectives To introduce a case of a traffic accident trauma in a patient with lumbar kyphosis and straightened lumbar curvature, where the Korean medicine treatment with Chuna manual therapy (CMT) improved lumbar lordotic angle(LLA) and lower back pain. Methods Chuna manual therapy and other Korean medicine treatments; were provided to patient with a flat back posture and low back pain due to the trauma from a traffic accident. Cobb's angle to evaluate LLA and numeric rating scale (NRS) to evaluate pain were compared before and after the treatment. Results There was a significant increase in LLA to reach the normal range and a decrease in low back pain NRS after Korean medicine treatment with CMT for 9 days of admission treatment and 3 weeks of outpatient treatment. Conclusions This study suggests that Korean medicine treatment, including CMT, may improve low back pain as well as LLA of straightened lumbar curvatures in patients.

  • PURPOSE: This study analyzed the impact of AI and biosensors on physical therapy, identifying the stage of customized technology development and future prospects. AI and biosensors improve the efficiency, establish customized treatment plans, and expand patient treatment opportunities. The study employed a literature review by searching databases and collecting research. METHODS: This study searched various databases related to the topic, collected existing research, papers, and reports, evaluated the literature, and summarize the results. RESULTS: Exercise therapy utilizing artificial intelligence can provide personalized and optimal exercise plans while monitoring rehabilitation progress. In addition, biosensors such as EMG sensors and accelerometers can monitor the individual progress in physical therapy, particularly in stroke patients, which can help improve physical therapy strategy and promote patient recovery. CONCLUSION: This study suggested that artificial intelligence can be applied in many areas of physical therapy, such as exercise therapy, customized treatment plans, rehabilitation and management, pain management, neuro rehabilitation, and auxiliary devices. Using AI technology, it is possible to analyze and improve exercise and posture, retrain the central nervous system, establish customized treatment plans for individual patients, predict and compare patient progress before and after treatment, and provide customized pain analysis and treatment methods. In addition, AI can provide neuro rehabilitation programs and customized auxiliary devices.

측두하악 장애환자에서 두부자세 변화가 초기 교합접촉에 미치는 영향 (The Effect of Head Posture Change on Initial Occlusal Contact in Temporomandibular Disorder Patient)

  • The purpose of this study was to evaluate an effect of change on head posture initial occlusal contacts with measuring the distances between initial occlusal contacts and maximum intercuspal position at different head posture in TMDs patient. For this study, 24 patients from age 13 to 36 were selected, they were examined health history taken, patients who have sign and symptoms of TMDs were examine before the study. For the normal group, 21 adults from age 23 to 25 were selected. They have normal or class I molar relationship, and have no other prosthetic restorations. Difference on distance between initial occlusal contact and maximum intercuspal position with mandibular kinesiograph$(MKG^R)$(K6 diagnostic system, Myo-tronic Inc, USA) in upright, supine, 45$^{\circ}$ extension, 30$^{\circ}$ flexion position of the head were measured. The Frankfort horizontal plane was used as a reference plane. The results were as follows : 1. There were significant differences between initial occlusal contacts of the normal and patient group on upright position and 30$^{\circ}$ flexion of the head(p<0.05, p<0.01) 2. The position of the initial occlusal contacts have a tendency to place anterior and inferior to maximal intercuspal position in upright position and 30( flexion of the head as well as posterior and inferior in supine position and 45$^{\circ}$ extension of the head in the normal and patient groups. 3. There were significant differences among the initial occlusal contacts between uptight and supine position; upright and 45$^{\circ}$ extension of the head(p<0.05); supine position and 30$^{\circ}$ flexion of the head, .and 30(flexion and 45$^{\circ}$ extension of the head in the patient group(p<0.01) The result have shown that after treatment on the supine position, it may be necessary to check occlusal contact on the upright position as well ass flexion of the head. It may need careful adjustment in occlusal condition on upright position of TMDs patient.

Evaluation of proximal contact strength by postural changes

  • STATEMENT OF PROBLEM. Proper proximal contact is important for maintaining and stabilizing the dental arch. However, the proximal contact strength (PCS) is not a constant value and can be affected by a variety of factors. PURPOSE. This study examined the influences of postural changes on the posterior PCS. MATERIAL AND METHODS. Twelve adults with a normal occlusion and had not undergone prosthetic treatment or proximal restoration were participated in this study. A metal strip was inserted into the proximal surface and removed at a constant velocity. The contact strength was measured in every contact point between canine to second molar in both arches. The PCSs were obtained initially in the upright position, secondly in the supine position and finally in the upright position again. All measurements were repeated after a 2 hour period. Statistical analysis was carried out using the Friedman test (P < .05). RESULTS. Generally, a decrease in PCS occurred when the posture was changed from the initial upright to supine position, while it increased when the posture was changed from the supine to upright position. A significant change was observed in all areas except for between the canine-first premolar in the maxilla and between the first molar-second molar in the mandible areas. CONCLUSION. The posterior PCS, which dentists generally believe to be a static feature of occlusion, is affected significantly by posture.

안굽이무릎을 가진 여성에게 볼을 이용한 스쿼트 운동이 무릎사이 간격과 Q각, 근 활성도에 미치는 효과 (Effects of Squat Exercise Using Balls on the Gap Interval between Knees, Q-angle, Muscle Activity in Women with Genu-Varum)

  • Purpose : The purpose of this study is to investigate the effect of squat exercise using a ball on the gap between knees and Q angle of a subject with a genu-varum, and to prove the effect, to provide a clinical basis for developing into a knee correction exercise program. Methods : As a result of posture measurement through GPS, 26 female with genu-varum with a knee length of 5 cm or more were studied. The group was randomly assigned to 13 squat exercise group using ball (experimental group) and 13 general squat exercise groups (control group). The experimental group placed the ball between both knees in a position where the distance between both feet was slightly wider than the shoulder width on a flat support surface, and fixed the ball, and then squat with the start signal. The control group squats without a ball. Each group performed three sets of exercise three times a week for six weeks. Before their exercise, after three and six weeks, EMG, GPS, digital goniometer measurement, the vastus medialis (VM), the vastus lateralis (VL), and the Q-angle were measured in the squat exercise posture. EMG was measured in squat exercise posture. Results : The distance between the knees was reduced. EMG is activated in group A, the group B experimental results showed the high activity of the VL. Q-angle had increased. But the experimental group increased more than the control group. Conclusion : We have confirmed through our experiments that the distance interval between the knees during squat exercises using a ball can be reduced. Furthermore, it would also be helpful to ensure the treatment of genu-varum.

Prediction of Cobb-angle for Monitoring System in Adolescent Girls with Idiopathic Scoliosis using Multiple Regression Analysis

  • Purpose: The purpose of this study was to select standing posture parameters that have a significant difference according to the severity of spinal deformity, and to develop a novel Cobb angle prediction model for adolescent girls with idiopathic scoliosis. Methods: Five normal adolescents girls with no history of musculoskeletal disorders, 13 mild scoliosis patients (Cobb angle: $10^{\circ}-25^{\circ}$), and 14 severe scoliosis patients (Cobb angle: $25^{\circ}-50^{\circ}$) participated in this study. Six infrared cameras (VICON) were used to acquire data and 35 standing parameters of scoliosis patients were extracted from previous studies. Using the ANOVA and post-hoc test, parameters that had significant differences were extracted. In addition, these standing posture parameters were utilized to develop a Cobb-angle prediction model through multiple regression analysis. Results: Twenty two of the parameters showed differences between at least two of the three groups and these parameters were used to develop the multi-linear regression model. This model showed a good agreement ($R^2$ = 0.92) between the predicted and the measured Cobb angle. Also, a blind study was performed using 5 random datasets that had not been used in the model and the errors were approximately $3.2{\pm}1.8$. Conclusions: In this study, we demonstrated the possibility of clinically predicting the Cobb angle using a non-invasive technique. Also, monitoring changes in patients with a progressive disease, such as scoliosis, will make possible to have determine the appropriate treatment and rehabilitation strategies without the need for radiation exposure.

Effect of the Type and Positon of Scoliosis on the Static Balance of Adolescents

  • Purpose: This paper provides basic clinical data on the treatment of scoliosis patients by analyzing the effects of the type and position of scoliosis on the static balance using Tetrax on adolescents who have balance disorders as a consequence of structural changes, such as scoliosis. Methods: A total of 110 adolescents were divided into 6 groups according to the radiographs and 60 adolescents were sampled, 10 each for each group. The static balance was measured and analyzed on the existence of sight using Tetrax. Results: The changes followed by existence of sight in static balance group showed a significant difference statistically in Stability index in all groups (p<0.001). The changes in the static balance in each group in terms of the stability index were significant in all groups, where the experiments were performed under the eyes opened and eyes-closed condition (p<0.01)(p<0.001). The post-hoc comparison revealed the stability index to be the highest in the eyes-opened condition in group III, but not in group IV. In the eyes-closed condition, group III showed the highest stability index of all the groups. Conclusion: Adolescents with lumbar scoliosis had a lower static balance in single scoliosis with the eyes-closed condition or had little movement compensation. In addition, unlike the other normal scoliosis, back scoliosis has negative effects on the posture because it causes an increase in the disturbance of posture. Therefore, future studied will be needed to examine the imbalance of posture in people suffering from back pain.

음악중재가 인공호흡기 유지 중환자의 통증에 미치는 효과 (The Effects of Music Intervention on Pain among Critically Ill Patients with Ventilatory Support)

  • Purpose: This study aimed to determine the effects of listening to music on pain and vital signs of critically ill patients with ventilatory support in intensive care units during nursing treatment (changes of posture and tracheal suction). Methods: The experimental treatment was to use an mp3 player and a speaker to let them listen to classical music by Mozart during nursing treatment. To determine the effects of music intervention, pain (Critical-Care Pain Observation Tool-K) was used. The data analysis was carried out by using PASW Statistics 20.0. Results: Hypothesis "The scores for pain would differ between the experimental group provided with music intervention during nursing treatment, and the control group" was supported. Conclusion: Application of music intervention during nursing treatment for critically ill patients with ventilatory support in intensive care units was found to be effective in reducing pain. Therefore, music intervention during nursing treatment for critically ill patients with ventilatory support can be used as non-pharmaceutical nursing intervention to reduce pain for the patients.

Thread Embedding Acupuncture and Complex Korean Medicine Treatment for Lumbar Spinal Stenosis with Degenerative Scoliosis: A Clinical Case Report

  • Lumbar spinal stenosis (LSS) is a common degenerative spinal condition that can have unpredictable improvement and worsening of symptoms which include low back pain, radiating pain, claudication, and degenerative lumbar scoliosis affecting quality of life. In this study, thread embedding acupuncture (TEA) was used as a conservative treatment for LSS in combination with complex Korean medicine treatments (acupuncture, herbal medicines, and physical therapy). The treatment was evaluated using the numerical rating scale, walking distance and duration, and inclination of radiological lumbar scoliosis according to antalgic posture. TEA was performed 27 times between June 8, 2020, and March 16, 2021. The patient showed improvement in numerical rating scale score from 7 to 2, pain-free walking distance from 10 m to 900 m, and scoliosis inclination following treatment. The findings of this study suggest that TEA may be helpful in the treatment of LSS.
