THE EFFECTIVENESS OF TWO SEDATIVE REGIMEN USED FOR CHILD PATIENT WHO FAILED TO ORAL CHLORAL HYDRATE (Chloral hydrate 경구 진정에 실패한 소아환자를 대상으로 사용한 두 가지 진정요법의 효과)

  • Chloral hydrate is one of the most widely used sedative agents to control the difficult-to-treat young age group in the dental clinic. We are often frustrated to see the patient still awake and cry with agitation even after far more than the normal onset time of Chloral hydrate. In such a case, the patient has to be rescheduled for another sedation visit with different agents and/or routes which greatly disappoints the guardians. This study was designed to test the efficacy of one sedative regimen that can possibly help the clinician complete scheduled treatment without postponement. We have tried sleep induction with mixed gas of Enflurane(2vol%) and $N_2O(50%)-O_2(50%)$ for $60\sim120$ seconds to 35 patients of those who failed to respond properly to the dose(70mg/kg)of oral Chloral hydrate. The Result of this regimen was compare to those of two oral regimen of Chloral hydrate/Hydroxyzine and Chloral hydrate only Analyses of result on vital signs and behavior pattern were performed. The outcome of the study suggest that sleep induction by a short inhalation of low dose of $Enflurane/N_2O-O_2$ provide dentist with suitable condition for the completion of scheduled treatment in the patient who failed to oral Chloral hydrate. Evidence of adverse effect was not detected or reported during and/or after the procedures.

  • Background: In recent reports, it has been reported that increased coagulation and decreased fibrinolytic activity has been responsible for abnormal fibrin turnover in exudative pleural effusion. In the cases of empyema, the fibrinopurulent stage is characterized by the fibrin deposition resulting in formation of limiting membranes in the visceral and parietal pleura. Recently attention has been focused on intrapleural fibrinolytic therapy capable of removing intrapleural fibrin deposits by urokinase (UK) in the treatment of empyema. However, these clinical trials have provided the clinical evidences for resolution of pleural loculation after intrapleural urokinase injection (UK-injection), the profiles of fibrinolytic activity following the treatment were still not investigated. Therefore in order to demonstrate the fibrinolytic evidences behind the clinical efficacy of intracavitary UK-injection, we examined intrapleural plasminogen activator activity (PA-activity) and D-dimer (D-Di) concentrations before and after each repeated UK-injection into the pleura in subjects with loculated empyema cavity. Methods: In a group of 14 patients with multiple loculated empyema cavity, PA-activity and D-Di concentrations were measured before and after repeated UK-injection. One hundred thousand IU of UK was injected at each time and all sujects had at least two times of UK injection accoring to clinical decisions. Nine out of 14 sujects had three times of UK-injection. Results: The mean (${\pm}SE$) PA-activity prior to treatemnt was $10.5{\pm}7.0$ and it was increased to $91.9{\pm}27.0,\;432.3{\pm}177.1,\;170.0{\pm}85.3$ IU tPA/ml after first, second and third time of UK-injection respectively (p<0.01). D-Di concentrations were also increased from $4.16{\pm}1.06{\times}10^5$ to $9.62{\pm}1.54{\times}10^5,\;12.31{\pm}1.89{\times}10^5,\;8.54{\pm}1.56{\times}10^5$ ng/ml in the same order as above (p<0.05). Conclusion: The suppressed fibrinolytic activity in the empyema cavity get removed sinificantly after inrracavitary injection of urokinase by generation of additional intrapleural plasmin.

  • Background: Irinotecan hydrochloride, a topoisomerase I inhibitor, is effective against small-cell lung cancer. Irinotecan also can act as a potential radiation sensitizer along with cisplatin. To evaluate efficacy and toxicity of irinotecan plus cisplatin (IP) with concurrent thoracic radiotherapy, we conducted a phase II study of IP followed by concurrent IP plus hyperfractionated thoracic radiotherapy in patients with previously untreated limited-stage small-cell lung cancer. Methods: Twenty-four patients with previously untreated small-cell lung cancer were enrolled onto the study since November 2004. Irinotecan $60mg/m^2$ was administered intravenously on days 1 and 8 in combination with cisplatin $60mg/m^2$ on day1 every 21 days. From the first day of third cycle, twice-daily thoracic irradiation (total 45 Gy) was given. Prophylactic cranial irradiation was given to the patients who showed complete remission after concurrent chemoradiotherapy. Restaging was done after second and sixth cycle with chest CT and/or bronchosocpy. Results: Up to November 2004, 19 patients were assessable. The median follow-up time was 12.5 months. A total of 99 cycles (median 5.2 cycles per patient) were administered. The actual dose intensity values were cisplatin $19.6mg/m^2$/week and irinotecan $38.2mg/m^2$/week. Among the 19 patients, the objective response rate was 95% (19 patients), with 9 patients (47%) having a complete response (CR). The major grade 3/4 hematological toxicities were neutropenia (35% of cycles), anemia (7% of cycles), thrombocytopenia (7% of cycles). Febrile neutropenia was 4% of cycles. The predominant grade 3/4 non-hematological toxicities was diarrhea (5% of cycles). Toxicities was not significantly different with concurrent administration of irinotecan and cisplatin with radiotherapy, except grade 3/4 radiation esophagitis (10% of patients). No treatment-related deaths were observed. The 1-year and 2-year survival rate of eligible patients was 89% (16/18) and 47% (9/18), respectively. Conclusion: Three-week schedule of irinotecan plus cisplatin followed by concurrent IP plus hyperfractionated thoracic radiotherapy is an effective treatment for limited disease small-cell lung cancer, with acceptable toxicity.

Control of Diatrype stigma Occurred on the Bed-log of Shiitake Using Wood Vinegar, Plant Extracts and Fungicides (목초액, 식물추출물 및 살균제를 이용한 표고골목해균인 주홍꼬리버섯 방제)

  • Attempts were made to control Diatrype stigma occurred on the bed-log of Shiitake using wood vinegar, Pinus koraiensis extract, Piper nigrum extract, and fungicides. Mycelial growth of D. stigma was inhibited completely at 35,000 ppm and no ascospore germinated at 25,000 ppm wood vinegar. Inhibition rates of Pinus koraiensis extract (200 ppm), and Piper nigrum extract (1,000 ppm) to ascospore germination were 98.9% and 95.9%, respectively. In fungicide selection, minimum inhibitory concentration (MIC) of benomyl, carbendazim, and thiabendazole ranged $0{\sim}0.4\;{\mu}g\;a.i/m{\ell}$. Difenoconazole at $0.08\;{\mu}g\;a.i/m{\ell}$ inhibited 98.9% of ascospore germination. Inhibition efficacy of fungicides was not highly variable among the low-, middle-, and high-temperature type strains of shiitake. Benomyl, carbendazim, thiabendazole and thiophanate-methyl could not suppress the mycelial growth of Shiitake. Tebuconazole at $0.4\;{\mu}g\;a.i/m{\ell}$ suppressed 80% of the mycelial growth and it was the highest inhibition rate among the fungicides. In field trials, wood vinegar, Pinus koraiensis extract, Piper nigrum extract, and fungicides were sprayed on the bed-logs before or after D. stigma produced pycnidia. Wood vinegar at 150,000 ppm concentration, showed control effect of 72.7% in the treatment before pycnidiospore formation. On the other hand, 70,000 ppm wood vinegar and 1,000 ppm of thiophanate-methyl showed control effects of 58.1% and 52.3% in the treatment after pycnidiospore formation.

Process development of a virally-safe dental xenograft material from porcine bones (바이러스 안전성이 보증된 돼지유래 골 이식재 제조 공정 개발)

  • A process for manufacturing virally-safe porcine bone hydroxyapatite (HA) has been developed to serve as advanced xenograft material for dental applications. Porcine bone pieces were defatted with successive treatments of 30% hydrogen peroxide and 80% ethyl alcohol. The defatted porcine bone pieces were heat-treated in an oxygen atmosphere box furnace at $1,300^{\circ}C$ to remove collagen and organic compounds. The bone pieces were ground with a grinder and then the bone powder was sterilized by gamma irradiation. Morphological characteristics such as SEM (Scanning Electron Microscopy) and TEM (Transmission Electron Microscopy) images of the resulting porcine bone HA (THE Graft$^{(R)}$) were similar to those of a commercial bovine bone HA (Bio-Oss$^{(R)}$). In order to evaluate the efficacy of $1,300^{\circ}C$ heat treatment and gamma irradiation at a dose of 25 kGy for the inactivation of porcine viruses during the manufacture of porcine bone HA, a variety of experimental porcine viruses including transmissible gastroenteritis virus (TGEV), pseudorabies virus (PRV), porcine rotavirus (PRoV), and porcine parvovirus (PPV) were chosen. TGEV, PRV, PRoV, and PPV were completely inactivated to undetectable levels during the $1,300^{\circ}C$ heat treatment. The mean log reduction factors achieved were $${\geq_-}4.65$$ for TGEV, $${\geq_-}5.81$$ for PRV, $${\geq_-}6.28$$ for PRoV, and $${\geq_-}5.21$$ for PPV. Gamma irradiation was also very effective at inactivating the viruses. TGEV, PRV, PRoV, and PPV were completely inactivated to undetectable levels during the gamma irradiation. The mean log reduction factors achieved were $${\geq_-}4.65$$ for TGEV, $${\geq_-}5.87$$ for PRV, $${\geq_-}6.05$$ for PRoV, and $${\geq_-}4.89$$ for PPV. The cumulative log reduction factors achieved using the two different virus inactivation processes were $${\geq_-}9.30$$ for TGEV, $${\geq_-}11.68$$ for PRV, $${\geq_-}12.33$$ for PRoV, and $${\geq_-}10.10$$ for PPV. These results indicate that the manufacturing process for porcine bone HA from porcine-bone material has sufficient virus-reducing capacity to achieve a high margin of virus safety.

Control of Powdery Mildew by Foliar Application of a Suspension of Cheonggukjang (청국장 현탁액 오이 엽면처리에 의한 흰가루병 방제효과)

  • This study was conducted to evaluate control efficacy of a fermented food 'Cheonggukjang' against cucumber powdery mildew caused by Sphaerotheca fuliginea in greenhouse. Sterilized Daepung beans were inoculated with the rice straw as natural inoculum and then incubated for 72 hrs at $42^{\circ}C$ in the household cheonggkjang maker. After 72 hrs of cheonggukjang fermentation, white zymogens were grown on the surface of a sterile Daepung beans. The pH of the 72 hrs fermented soybean was not significantly changed and electrical conductivity was found to increase by about 2 times than before fermentation. The population density of soybean zymogen showed a peak of growth at 60 hrs after fermentation and the concentration of zymogen was $8.2{\times}10^7cfu/ml$. Soybean zymogen form of the colony was divided into three kinds of bacteria and a white and a large colony (WL) was predominant bacteria among those up to 60 hrs of fermentation. To control the cucumber powdery mildew, diluted solutions of cheonggukjang was applied from 6.0% to 30.0% on cucumber leaves and they showed injury symptoms on cucumber leaves in more than 15% of them. However, more than 6.0% diluted cheonggukjang solutions showed more than 77.8% control effect of cucumber powdery mildew at 15 days after treatment. The fermented bacteria of Chenggukjang were well established in the cucumber leaf area at 15 days after treatment. The antifungal activity of 10% diluted cheonggukjang solutions was excellent for four species of plant fungal pathogens, Colletotrichum gloeosporioides, Sclerotinia cepivorum, Rhizoctonia sloani and Phytophthora capsici in the dual culture test. Results indicated that foliar application of Cheonggukjang solution could be used for the control of powdery mildews occurring on organically cultivated cucumber.

Locally Advanced, Unresectable Pancreatic Cancer Treated by Stereotactic Radiation Therapy (국소적으로 진행된, 절제 불가능한 췌장암에서 정위 방사선 치료)

  • Puroose: In order to find out whether stereotactic radiation therapy (RT) using CyberKnife (CK) could improve survival rate and lower acute toxicity compared to conventional RT. Materials and Methods: From April 2003 through April 2004, 19 patients with Eastern Cooperative Oncology Group (ECOG) performance status ${\leq}3$ and locally advanced pancreas cancer without distant metastasis, evaluated by CT or PET/CT, were included. We administered stereotactic RT consisting of either 33 Gy, 36 Gy or 39 Gy in 3 fractions to 6, 4 and 9 patients, respectively, in an effort to increase the radiation dose step by step, and analyzed the survival rate and gastrointestinal toxicities by the acute radiation morbidity criteria of Radiation Therapeutic Oncology Group (RTOG). Prognostic factors of age, sex, ECOG performance score, chemotherapy, bypass surgery, radiation dose, CA 19-9, planning target volume (PTV), and adjacent organ and vessel invasion on CT scan were evaluated by Log Rank test. Results: The median survival time was 11 months with 1-year survival rate of 36.8%. During follow-up period (range $3{\sim}20$ months, median 10 months), no significant gastrointestinal acute toxicity (RTOG grade 3) was observed. In univariate analysis, age, sex, ECOG performance score, chemotherapy, bypass surgery, radiation dose, CA 19-9 level, and adjacent organ and vessel invasion did not show any significant changes of survival rate, however, patients with PTV (80 cc showed more favorable survival rate than those with PTV>80 cc (p-value<0.05). In multivariate analysis, age younger than 65 years and PTV>80 cc showed better survival rate. Conclusion: In terms of survival, the efficacy of stereotactic radiation therapy using CK was found to be superior or similar to other recent studies achieved with conventional RT with intensive chemotherapy, high dose conformal RT, intraoperative RT (IORT), or intensity modulated RT (IMRT). Furthermore, severe toxicity was not observed. Short treatment time in relation to the short life expectancy gave patients more convenience and, finally, quality of life would be increased. Consequently, this could be regarded as an effective novel treatment modality for locally advanced, unresectable pancreas cancer. PTV would be a helpful prognostic factor for CK.

Induction Chemotherapy Followed by Radiotherapy for Stage IV Hypopharyngeal Cancer (4기 병기 하인두암에서 선행 항암화학요법후 방사선치료)

  • Purpose: Hypopharyngeal cancer is diagnosed at the advanced stage in most cases, which the prognosis known to be poor. Thus, the efficacy of induction chemotherapy followed by radiotherapy, with regards to the response and survival rate for stage IV hypopharyngeal cancer patients, was examined. Materials and Methods: From July 1998 to February 2000, 18 cases were diagnosedas AJCC stage IV hypopharyngeal cancer without distant metastasis. These patients were treated with induction chemotherapy followed by radiotherapy, and the results retrospectively analyzed. The regimen of the induction chemotherapy was the 5-FU and cisplatincombination, at 3-week intervals for, 2 cycles. The total radiation dose for the primary lesion and metastatic lymph nodes was $68.4\~72.0$Gy (median: 70.2 Gy). Results: The: The median follow up period was 28 months, ranging from 7 to 99 months. The 3-year overall survival and disease-free survival rate were 41.7 and $31.1\%$, respectively. In 6 cases ($33.3\%$), conservation of the larynx for over 3 years was possible. After the induction chemotherapy there were 16 partial responses ($88.8\%$), 1 complete response and 1 with no response ($5.6\%$ each), therefore, 17 of the 18 cases ($94.6\%$) showed responses. After the completion of the induction chemotherapy and radiotherapy, a complete response was noted in 13 cases ($72.2\%$), a partial response in 5 ($27.8\%$), with an overall response rate of $100\%$. In the analysis of the prognostic factors influencing the survival rate, the 3-year and disease-free survival rates for the complete and partial response groups were 43.1, and $20.0\%$, and 39.6, and $20.0\%$, respectively (p=0.0003, p=0.002). Only the final response after treatment completion was statistically significant. Conclusion: For stage IV hypopharyngeal cancer, induction chemotherapy followed by radiotherapy was an effective treatment, with no severe side effects.

  • The study was conducted to compare the interprete methods and examine the feasibility of mixture use of oxyfluorfen and bensulfuron in controlling principal Paddy weeds, annuals and perennials. Application ratio of both chemicals were obtained from the combinations of 5 levels(0, 5, 10, 15, 20 g ai/ha) of each chemicals, respectively. All the treatments were applied at 5 days after transplanting and water was maintained at 3.0cm in depth. Shoot fresh-weight of weeds was assessed at 35 days after treatments. Data obtained was analysed by Colby, Isobole, Calculus, Regression and EQM method, respectively. The results from the analysis of variance on the principal weeds treated with oxyfluorfen and bensulfuron showed significant interactions at 1% level on both Echinochloa crus-galli and Eleocharis Kuroguwai, and total species at 0.5% level on both Potamogeton distinctus and Cyperus serotinus, but non significant on Scirpus juncoides and Sagittaria pygmaea. Thereafter, the results of the models applied to Echinochloa crus-galli, Eleocharis kuroguwai and total species were as follows ; 1. The Colby method gave values nearly identical to regression estimate method (both multiplicative models) as provided by Akobundu et al. The Colby method and Regression method indicated synergistic toward Echinochloa curs-galli, and total species, but antagonistic toward Eleocharis kuroguwai. 2. The Isobole method shows synergism on Echinochloa crus-galli at $ID_{50}$, and total species at $ID_{60}$ on Eleochari kuroguwai. 3. The Calculus method gave positive signs for the first differentiation and negative signs for the second differentiation except for some rates on Echinochloa crus-galli and total species, but reverse on Eleocharis kuroguwai. These result does not agree with the observed values. 4. ${\theta}$ value from the EQM method was greater than one at all combinations. This result was quite different from those of other methods. 5. The various models did not show the same results, but mixture of oxyfluorfen and bensulfuron tend to have synergistic effect. Weeding effect also was high. Treatment in terms of two chemical combination was expected to reduce rates, and to enhence weeding efficacy compared with single treatment.

In vitro Multiplication through Single-Node Culture of Sea-Milkwort (Glaux maritima L.) (갯봄맞이(Glaux maritima L.) 실생의 단마디배양을 통한 기내증식)

  • This study was conducted to establish an in vitro propagation system for sea-milkwort (Glaux maritima L.), which is an endangered coastal plant species with high horticultural value. Two phenotypes, 'Red type (RT)' and 'Pistachio type (PT)' based on the colors of stem and flower, were obtained from a personal horticulturist in 2009 and used for this study as plant materials. The stock plants showed typical morphologies in flower, capsule, and seed appearances as previously reported. Low temperature treatment at $4^{\circ}C$ for four or more weeks after in vitro sowing maximized seed germination percentage, indicating that imbibition of seed and subsequent low temperature treatment are crucial for its germination. The in vitro seedlings had phenotypic variation, falling into 'RT' and 'PT' classes like the stock plants. Although slight differences depending on genotype and medium were recognized, the fourth or fifth nodes detached from the in vitro seedlings revealed the best multiplication efficacy when estimated on the basis of total number of nodes of newly developed axillary shoots. In addition, the nodes from 'RT' and 'PT' regenerated the most shoots on medium supplemented with $0.5mg{\cdot}L^{-1}$ BA alone and $0.5mg{\cdot}L^{-1}$ BA plus $0.5mg{\cdot}L^{-1}$ IAA, respectively. The node culture-derived plantlets were well acclimatized in a culture room ex vitro and completed the pseudo-annual life cycle coincident with that in the natural salt march habitat with the current cultivation method of applying fresh water-irrigation under an inland environment. This work represents the first report of in vitro propagation of sea-milkwort. Thus, our study will contribute to exo-habitat conservation and natural habitat restoration of this endangered species in addition to development of a horticultural product.
