Objective : Repeated computed tomography (CT) follow up for traumatic brain injury (TBI) patients is often performed. But there is debate the indication for repeated CT scans, especially in pediatric patients. Purpose of our study is to find risk factors of progression on repeated CT and delayed surgical intervention based on the repeated head CT. Methods : Between March, 2007 and December, 2013, 269 pediatric patients (age 0-18 years) had admitted to our hospital for head trauma. Patients were classified into 8 subgroups according to mechanisms of injury. Types, amount of hemorrhage and amount changes on repeated CT were analyzed as well as initial Glasgow Coma Scale (GCS) scores. Results : Within our cohort of 269 patients, 174 patients received repeat CT. There were progression in the amount of hemorrhage in 48 (27.6%) patients. Among various hemorrhage types, epidural hemorrhage (EDH) more than 10 cc measured in initial CT was found to be at risk of delayed surgical intervention significantly after routine repeated CT with or without neurological deterioration than other types of hemorrhage. Based on initial GCS, severe head trauma group (GCS 3-8) was at risk of delayed surgical intervention after routine repeated CT without change of clinical neurologic status. Conclusion : We suggest that the patients with EDH more than 10 cc or GCS below 9 should receive repeated head CT even though absence of significant clinical deterioration.
Objective: Falling is a common cause of head injury in preschool aged children. We investigated the characteristics of mild head injuries caused by falling and the association between body weight and occurrence of traumatic brain injuries (TBI). Methods: This retrospective observational study was conducted on head-injured preschool-aged children that visited the emergency department from January 2012 to December 2015. Characteristics such as age, sex, weight, free fall height, floor type, and presence of TBI, as defined as cerebral hemorrhage or skull fracture, were investigated. We calculated body weight percentiles by calibrating age and weight and categorized them into four quartile ranges. We grouped all included cases into two groups according to the presence of TBI. The characteristics of the two groups were compared by using chi-square test, and the association with TBI was investigated by using binomial logistic regression. Results: A total of 701 children were included, and TBI was observed in 95 children. Children with TBI were younger. The proportion of children with TBI was higher in the third and fourth quartiles of the body weight group as well as according to soft floor and fall from high height (${\geq}1m$). The odds of soft floor being associated with TBI was higher than the odds for hard floor (odds ratio, 2.734; 95% confidence interval [CI], 1.597-4.680). The odds of high height (${\geq}1m$) being associated with TBI was higher than that for low height (odds ratio, 2.306; 95% CI, 1.155-4.601), and the odds ratio for the weight percentile group was 1.228 (95% CI, 1.005-1.499). Conclusion: Prevalence of TBI after falling in preschool-aged children might be associated with high fall-height and body weight quartiles.
목적: TBI 환자의 SPECT상 혈류장애를 보이는 부위의 분포를 조사하였으며 대뇌피질과 동반된 시상의 혈류변화를 관찰하여 환자의 인지 및 행동 장애 정도를 예측 할 수 있는지 알아보고자 하였다. 대상 및 방법: TBI 후 $0.5{\sim}55$개월까지(평균 10.3개월) 뇌손상 평가를 위해 MRI영상과 Tc-99m ECD SPECT를 시행한 103명의 환자를 대상으로 하였고 남자 81명과 여자 22명이었으며, 평균연령은 $34.7{\pm}15.4$세였다. 대상환자들은 MRI영상에서 정상, 국소손상 또는 미만성 손상여부에 따라 3군으로 나누었다. 신경심리학적 검사는 정신과 의사에 의한 행동 및 성격변화의 관찰과 임상심리사에 의한 인지기능검사에 의해 총 11개의 항목이 평가되었다. SPECT에서 혈류감소를 보이는 대뇌피질의 분포를 비교하였으며 대뇌피질과 시상의 혈류감소를 같이 보인 경우와 시상의 혈류가 정상인 경우로 나누어 인지 및 행동 장애정도와 관계가 있는지 Mann-whitney의 U검정을 이용하여 유의성을 알아보았다 결과: 혈류감소를 보인 병변의 분포는 전두엽(1군 42.3%, 2군 34.5%, 3군 33.3%), 측두엽($24{\sim}26%$), 시상($21{\sim}22.4%$), 두정엽과 후두엽(10%이하)순이었다. 또한 대뇌피질과 시상의 혈류감소가 같이 있었던 경우와 시상이 정상 혈류를 보였던 경우 신경정신과적 증상을 보인 항목 수와 비교한 결과 1군에서는 전자와 후자의 경우 각각 $4.7{\pm}1.5$개와 $3.2{\pm}1.4$개였으며, 2군에서 $5.0{\pm}1.1$개와 $4.8{\pm}1.2$개, 3군에서는 $6.8{\pm}1.8$개와 $6.3{\pm}1.1$개로 MRI에서 손상의 정도가 많을수록 신경정신과적 후유증은 많았으나 SPECT에서 시상의 혈류감소 유무에 따른 증상의 심각성과 관계는 1군에서만 유의하였다(p=0.002). 결론: TBI 후 기질적 정신장애를 갖는 환자의 뇌혈류 SPECT에서 혈류감소를 보이는 병변은 주로 전두엽, 측두엽, 시상순으로 많았고 특히 전두엽은 MRI에서 정상을 보인 군에서 더욱 많은 비율을 차지하였다. MRI에서 정상일 때 SPECT에서 대뇌피질과 시상의 혈류감소가 같이 동반된 경우 시상이 정상혈류를 보인 경우보다 인지 및 행동장애의 심각성과 유의한 관계를 보였다.

Functional photoacoustic tomography is a new non-invasive imaging modality, and it is emerging as a very practical method for imaging biological tissue structures by means of laser-induced ultrasound. Structures with high optical absorption, such as blood vessels, can be imaged with the spatial resolution of ultrasound, which is not limited by the strong light scattering in biological tissues. By varying wavelengths of the laser light and acquiring photoacoustic images, optical absorption spectrum of each image pixel is found. Since the biochemical constituents of tissues determine the spectrum, useful functional information like oxygen saturation ($SO_2$) and total haemoglobin concentration (HbT) can be extracted. In this study, as a proof-of-principle experiment, hypoxic brain tumor vasculature and traumatic brain injury (TBI) of small animal brain are imaged with functional photoacoustic tomography. High resolution brain vasculature images of oxygen saturation and total hemoglobin concentration are provided to visualize hypoxic tumor vasculature, and hemorrhage on the cortex surface by the TBI.
Pituitary adenylate cyclase-activating polypeptide (PACAP) is a pleiotropic bioactive peptide that was first isolated from an ovine hypothalamus in 1989. PACAP belongs to the secretin/glucagon/vasoactive intestinal polypeptide (VIP) superfamily. PACAP is widely distributed in the central and peripheral nervous systems and acts as a neurotransmitter, neuromodulator, and neurotrophic factor via three major receptors (PAC1, VPAC1, and VPAC2). Recent studies have shown a neuroprotective role of PACAP using in vitro and in vivo models. In this review, we briefly summarize the current findings on the neurotrophic and neuroprotective effects of PACAP in different brain injury models, such as cerebral ischemia, Parkinson's disease (PD), and Alzheimer's disease (AD). This review will provide information for the future development of therapeutic strategies in treatment of these neurodegenerative diseases.
Objective : It is well known that changes in cerebral hemodynamics occur after traumatic brain injury (TBI). Osmo-regulation in the brain is important for maintaining a constant milieu in the central nervous system. Nevertheless, to our knowledge, early osmolarity changes after minor head injury have not been studied until now. Methods : In this study, serum osmolarity was measured in 99 patients with minor head trauma. As a control group, blood samples were drawn from 99 patients who had a minor trauma in an extremity. Serum osmolarity was estimated using a fully automatic biochemical autoanalyzer within the first 3 hours after the trauma. Results : The mean serum osmolarity levels were $286.08{\pm}10.1\;mOsm/L$ in the study group and $290.94{\pm}5.65\;mOsm/L$ in the control group (p<0.001). However, after age adjustment between the study and control groups, this statistical significance was found to be valid only for patients over 30 years of age. Conclusion : It was noted that serum osmolarity levels decrease in the first 3 hours following minor head trauma in patients over 30 years of age. Further studies into this area could provide guidance for the management/treatment of elderly patients.

Objective : Posttraumatic cerebral infarction (CI) is a well-known complication of traumatic brain injury (TBI). However, the causation and apportionment of trauma in patients with CI after TBI is not easy. There is a scoring method, so-called trauma apportionment score (TAS) for CI, consisted with the age, the interval, and the severity of the TBI. We evaluated the reliability of this score. Methods : We selected two typical cases of traumatic CI. We also selected consecutive 50 patients due to spontaneous CI. We calculated TAS in both patients with traumatic and spontaneous CI. To enhance the reliability, we revised TAS (rTAS) adding three more items, such as systemic illness, bad health habits, and doctor's opinion. We also calculated rTAS in the same patients. Results : Even in 50 patients with spontaneous CI, the TAS was 4 in 44 patients, and 5 in 6 patients. TAS could not assess the apportionment of trauma efficiently. We recalculated the rTAS in the same patients. The rTAS was not more than 11 in more than 70% of the spontaneous CI. Compared to TAS, rTAS definitely enhanced the discriminating ability. However, there were still significant overlapping areas. Conclusion : TAS alone is insufficient to differentiate the cause or apportionment of trauma in some obscure cases of CI. Although the rTAS may enhance the reliability, it also should be used with cautions.
Objective : Traumatic brain injury including diffuse axonal injury has been shown to result in a decrease in brainfree magnesium concentration, an endogenous inhibitor of calcium entry into neuron, that is associated with the development of neurological motor deficits. The goal of this study is to establish the therapeutic window during which the therapy with $MgSO_4$ and/or hypothermia improve damaged neurons by TUNEL stain. Method : Moderate brain injury was induced in 64 adult Sprague-Dawley rats, weighing 350 to 450gm each, by using a simple weight-drop device(Marmarou model). The animals were randomly assigned to four groups(sixteen rats each, a control group, a group treated with $MgSO_4$, a group treated with hypothermia, and a group treated with $MgSO_4$ and hypothermia) and the rats in each group were sacrificed and studied after 12 hrs, 24 hrs, 1 wk, and 2 wks after insult. In hypothermic group, these rats were subjected to hypothermia after injury, with their rectal temperatures maintained at $32^{\circ}C$ for 1 hour. After 1-hour period of hypothermia, rewarming to normothermic level was accomplished over 30-minute period. In the groups treated $MgSO_4$, hypothermia and $MgSO_4$ were subsequently treated with $MgSO_4$($750{\mu}moles/kg$) infused intra-muscularly at 30 minutes after trauma. Result : In all treated groups, a significant reduction in TUNEL positive cells was found in comparison with the control group each time(p<0.001). Between treatment groups, No differnce was seen 12hrs, 24hrs, and 1wk. However, hypothermic group treated with or without $MgSO_4$ showed more significant reduction in apoptotic cells than group treated with $MgSO_4$ 2 weeks after trauma(p<0.05). However, hypothermic group treated with $MgSO_4$ showed no significant reduction in apoptotic cells compared with hypothermic group(p>0.05). Conclusion : These findings suggest that both hypothermia and $MgSO_4$ significantly improve pathological changes. Otherwise simultaneously $MgSO_4$ and hypothermia treatment groups is failed to provide additional neuroprotection. These results may be relevant to the design of future clinical trials of therapeutic hypothermia and $MgSO_4$ for traumatic brain injury.

본 연구는 뇌손상 아동의 후유증을 조사하고, 이 증상에 손상요인, 치료요인, 환경요인이 관련되어 있는 지와, 후유증을 평가하는데 일반적인 심리검사가 적절한지를 규명함으로써 뇌손상 아동의 후유증 치료, 예방 및 평가에 관한 임상적 지침을 얻고자하였다. 연세대학교 원주기독병원 응급의학과와 신경외과에서 뇌손상에 관한 치료를 받고 최소한 6개월이 지난 후 정신과에 신경정신과적 평가가 의뢰된 47명의 아동(연령 $5{\sim}14$세)을 대상으로 후유증을 네 범주로 나누어 조사했다. 초기 GCS 점수, 동반손상, 치료방법, 입원일수를 조사하고, EEG, MRI, 지능검사, 심리검사를 시행한 후 통계처리하여 얻은 결과는 다음과 같다. 1) 인지증상이 행동증상, 정서증상, 신체증상보다 많았으며, 이들 증상의 빈도는 경도집단과 중등도-중증 집단간에 차이가 없었다. 2) 인지증상은 두부수술을 받지 않은 아동이 더 많이 호소하였다(p<0.01). 3) 행동증상은 뇌손상 당시의 연령이 낮은 아동과(p<0.05), 투약한 아동에서(p<0.05) 더 많이 호소되었다. 4) 정서증상은 동반손상이 있는 아동이 더 많이 호소하였다(p<0.05). 5) 신체증상은 약물치료를 받지 않은 아동에서(p<0.05), 지능지수가 높을수록(p<0.05) 많았다. 6) GCS 점수가 낮을수록 지능지수가 낮았다(p<0.05). 인지증상을 호소한 총 42명 중 25명에서 심리검사상 인지장애가 확인되었다. 이들은 다른 아동에 비해 입원일수가 길었다(p<0.05). 7) 정서증상을 호소한 총 25명 아동 중 심리검사를 통하여 정서장애가 확인된 아동은 22명이었다. 이들은 다른 아동에 비해 인지증상이 더 많았다(p<0.05).
목적: 두부외상을 받은 후 뇌실질의 해부학적인 병변이 없음에도 불구하고 인지 및 정신운동성 기능장애 등을 호소하는 환자가 많지만 아직 그 병태 생리가 밝혀지지 않고 있다. 연구자는 두부외상 후 해부학적 뇌 병변이 없으면서도 지능저하를 보인 환자에서 국소 뇌혈류(regional cerebral blood flow, CBF) 및 국소 혈류예비능(regional cerebrovascular reserve, CVR) 이상이 있는 지를 알아보고, 지능저하와 관련된 뇌영역을 분석하고자 하였다. 대상 및 방법: 두부외상을 받은 후 지능저하를 호소하고, 만성기에 시행한 뇌 MRI에서 유의한 대뇌피질의 이상소견이 없었던 30명으로 남자 16명과 여자 14명이었으며, 평균연령은 $34{\pm}12$세였다. 대조군을 19명으로 남자 9명과 여자 10명이었으며, 평균연령은 $44{\pm}17$세였다. 휴식기에 Tc-99m HMPAO 925 MHq를 주사하고 5분째 뇌혈류 SPECT를 촬영하였으며, 촬영 도중 아세타졸아미드 1 g을 3분간 주사하였다. 첫 촬영이 끝난 직후 다시 Tc-99m HMPAO 925 MBq를 주사하여 같은 방법으로 뇌부위를 촬영하였다. 지능검사는 한국판-웩슬러 성인용 지능검사(K-WAIS)를 사용하여 언어성과 동작성에 관련된 11가지 소검사를 시행하였다. 각 소검사 별 비정상 점수를 보인 환자군과 정상 대조군간 유의한 CBF 차이를 윈도우용 SPM'97로 비교분석하여 다음과 같은 결과를 얻었다. 결과: 1) 모든 언어성 소검사는 좌측 대뇌반구에서 CBF 감소를 보였고, Wernicke 영역과 모두 관련이 있었고 Broca 영역과 관련을 보인 어휘문제를 포함하여 기본지식문제와 숫자외우기는 전두엽, 측두엽 그리고 두정엽과도 관련이 있었다. 우측 대뇌반구에 추가로 CBF 감소를 보인 경우는 기본지식문제, 숫자외우기, 어휘문제와 공통성문제로 전두엽, 측두엽 그리고 두정엽에서 CBF가 감소를 보였다. 2) 동작성 검사에서 CBF 감소는 주로 우측 대뇌반구에서 관찰되었다. 우측 상측두회 후면과 상연회, 운동전 피질, 그리고 일차적 체운동피질과, 우측 전두전야 일부에서 관련이 있었고, 좌측 전두엽과 상연회 일부에서 CBF 감소가 있었다. 3) CVR 감소는 전체적으로 총 64개 뇌영역에서 관찰되었다. 언어성 검사에서 주로 양측 전두엽과 측두엽에서 관찰되었다. 기본지식문제만 양측 하두정회와 후두엽에서 추가로 CVR 감소가 관찰되었다. 동작성 검사에서 주로 양측 전두전야와 상, 중측두회와 관련이 있었다. 결론: 두부외상 후 대뇌피질의 형태학적 손상을 발견할 수 없었던 경미한 손상에서도 CBF 뿐만 아니라 CVR의 감소를 초래되며, 지능 저하와 관련이 있음을 알 수 있었다. 이러한 지능 저하는 특정한 뇌영역과 관련되어 있음을 밝혔다.

제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
